    Gene Expression Analyses in Non Muscle Invasive Bladder Cancer Reveals a Role for Alternative Splicing and Tp53 Status

    Non-muscle invasive bladder cancer (NMIBC) represents a crucial problem for the national health care systems due to its high rates of recurrence and the consequent need of frequent follow-ups. Here, gene expression analyses in patients diagnosed as NMIBC were performed to determine those molecular pathways involved in tumor initiation, finding that both MYC and E2F are up regulated and helps to tumor initiation and progression. Our results also support an important involvement of alternative splicing events, modifying key pathways to favour bladder tumor evolution. Finally, since MDM2 showed differential exon usage, mutations in TP53 and its protein expression have been also studied in the same patients. Our data support that recurrence is epigenetically mediated and favoured by an increase protein expression of TP53, which appears more frequently mutated in advanced stages and grades, being associated to a worse prognosis. Therefore, TP53 mutational status could be used as a potential biomarker in the first stages of NMIBC to predict recurrence and prognosis

    The direct and indirect calibration experiences made at the Cape of Begur area in 1999, 2000 and 2002, for Topex/Poseidon and at the Ibiza island in 2003 have contributed to the international campaigns made at Harvest (USA), Corsica (France) and Bass (Australia). The main objective of IBIZA 2003 campaign has been the determination of the instantaneous sea surface/marine geoid gradient along Jason-1 tracks using a GPS catamaran and a network of GPS located in Portinatx and Ibiza and San Antonio harbours. The marine geoid will be used to relate the tide gauge coastal data with the altimeter data.     [EN] The Spanish Antibiogram Committee (Comité Español del Antibiograma, COESANT) presents in this document a series of recommendations intending to unify how cumulative antibiogram reports must be made in Clinical Microbiology Spanish laboratories. This article is based on the information included in the Clinical Microbiology Procedure No. 51, «Preparation of cumulative reports on antimicrobial susceptibility» of the Spanish Society of Infectious Diseases and Clinical Microbiology (SEIMC), published in 2014. The recommendations also include the modifications in the definition of clinical interpretive categories recently published by the European Committee on Antimicrobial Susceptibility Testing (EUCAST) in 2019. Its final objective is to establish a homogeneous way of preparing these summaries to compare results from different centers or aggregate the information from these in order to carry out an adequate local or even national surveillance regarding the evolution of antimicrobial susceptibility.[ES] El Comité Español del Antibiograma (COESANT) presenta en este documento una serie de recomendaciones cuya finalidad es unificar la forma en la que los Servicios y Unidades de Microbiología Clínica españoles realizan los informes de sensibilidad acumulada de las bacterias, aisladas en muestras clínicas, frente a los antimicrobianos.     Recommendations of the Spanish Antibiogram Committee (COESANT) for selecting antimicrobial agents and concentrations for in vitro susceptibility studies using automated systems

    Automated antimicrobial susceptibility testing devices are widely implemented in clinical microbiology laboratories in Spain, mainly using EUCAST (European Committee on Antimicrobial Susceptibility Testing) breakpoints. In 2007, a group of experts published recommendations for including antimicrobial agents and selecting concentrations in these systems. Under the patronage of the Spanish Antibiogram Committee (Comité Español del Antibiograma, COESANT) and the Study Group on Mechanisms of Action and Resistance to Antimicrobial Agents (GEMARA) from the Spanish Society of Infectious Diseases and Clinical Microbiology (SEIMC), and aligned with the Spanish National Plan against Antimicrobial Resistance (PRAN), a group of experts have updated this document. The main modifications from the previous version comprise the inclusion of new antimicrobial agents, adaptation of the ranges of concentrations to cover the EUCAST breakpoints and epidemiological cut-off values (ECOFFs), and the inference of new resistance mechanisms. This proposal should be considered by different manufacturers and users when designing new panels or cards. In addition, recommendations for selective reporting are also included. With this approach, the implementation of EUCAST breakpoints will be easier, increasing the quality of antimicrobial susceptibility testing data and their microbiological interpretation. It will also benefit epidemiological surveillance studies as well as the clinical use of antimicrobials aligned with antimicrobial stewardship programs.Los sistemas automáticos utilizados en el estudio de la sensibilidad a los antimicrobianos están introducidos en la mayoría de los laboratorios de Microbiología Clínica en España, utilizando principalmente los puntos de corte del European Committee on Antimicrobial Susceptibility Testing (EUCAST). En 2007, un grupo de expertos publicó unas recomendaciones para incluir antimicrobianos y seleccionar concentraciones en estos sistemas. Bajo el auspicio del Comité Español del Antibiograma (COESANT) y del Grupo de Estudio de los Mecanismos de Acción y Resistencia a los Antimicrobianos (GEMARA) de la Sociedad Española de Enfermedades Infecciosas y Microbiología Clínica (SEIMC) y alineado con el Plan Nacional frente a la Resistencia a los Antibióticos (PRAN), un grupo de expertos ha actualizado dicho documento. Las principales modificaciones realizadas sobre la versión anterior comprenden la inclusión de nuevos agentes antimicrobianos, la adaptación de los rangos de concentraciones para cubrir los puntos de corte clínicos y los puntos de corte epidemiológicos (ECOFF) definidos por el EUCAST, y para la inferencia de nuevos mecanismos de resistencia. Esta propuesta debería ser considerada por los diferentes fabricantes y los usuarios cuando se diseñen nuevos paneles o tarjetas. Además, se incluyen recomendaciones para realizar informes selectivos. Con este enfoque, la implementación de los puntos de corte del EUCAST será más fácil, aumentando la calidad de los datos del antibiograma y su interpretación microbiológica.     Autoantibodies against the immunodominant sCha epitope discriminate the risk of sudden death in chronic Chagas cardiomyopathy

    In Chagas disease (ChD) caused by Trypanosoma cruzi, new biomarkers to predict chronic cardiac pathology are urgently needed. Previous studies in chagasic patients with mild symptomatology showed that antibodies against the immunodominant R3 epitope of sCha, a fragment of the human basic helix-loop-helix transcription factor like 5, correlated with cardiac pathology. To validate sCha as a biomarker and to understand the origin of anti-sCha antibodies, we conducted a multicenter study with several cohorts of chagasic patients with severe cardiac symptomatology. We found that levels of antibodies against sCha discriminated the high risk of sudden death, indicating they could be useful for ChD prognosis. We investigated the origin of the antibodies and performed an alanine scan of the R3 epitope. We identified a minimal epitope MRQLD, and a BLAST search retrieved several T. cruzi antigens. Five of the hits had known or putative functions, of which phosphonopyruvate decarboxylase showed the highest cross-reactivity with sCha, confirming the role of molecular mimicry in the development of anti-sCha antibodies.     Genomic transformation and social organization during the Copper Age-Bronze Age transition in southern Iberia

    [EN]The emerging Bronze Age (BA) of southeastern Iberia saw marked social changes. Late Copper Age (CA) settlements were abandoned in favor of hilltop sites, and collective graves were largely replaced by single or double burials with often distinctive grave goods indirectly reflecting a hierarchical social organization, as exemplified by the BA El Argar group. We explored this transition from a genomic viewpoint by tripling the amount of data available for this period. Concomitant with the rise of El Argar starting similar to 2200 cal BCE, we observe a complete turnover of Y-chromosome lineages along with the arrival of steppe-related ancestry. This pattern is consistent with a founder effect in male lineages, supported by our finding that males shared more relatives at sites than females.     Clinical factors associated with brain neoplasms de novo in patients with headache to the emergency department

    La cefalea constituye uno de los principales síntomas de consulta en el servicio de urgencias y constituye un desafío para el médico. Objetivo: Determinar los factores clínicos que están asociados con la presencia de masa cerebral en pacientes que debutan con cefalea. Metodología: Estudio retrospectivo de casos y controles. Se definió como caso cada paciente que consultó por cefalea con evidencia imagenológica por tomografía axial computarizada (TAC) o resonancia nuclear magnética (RMN)de masa cerebral. Los controles fueron pacientes con cefalea sin evidencia imagenológica de masa cerebral. El tamaño de muestra fue de 272 pacientes (62 casos y 210 controles). Para determinar los factores asociados al desenlace se realizó un análisis de regresión logística binaria, incluyendo variables plausibles con p<0,2 en el análisis bivariado y aquellas biológicamente plausibles. Se calcularon los odds ratio ajustados (ORA) y sus respectivos intervalos de confianza. Se consideró significativo un valor de p<0,5. Resultados: Las variables clínicas asociadas con la presencia de masa cerebral fueron: 1) antecedente de cáncer fuera del sistema nervioso central, 2) cefalea que empeora con las maniobras de Valsalva, 3) alteraciones en el examen motor y 4) cefalea que despierta al paciente en la noche. Conclusiones: Se encontró que las variables mencionadas constituyen signos de alarma predictivos de la presencia de cefalea secundaria a masa cerebral y son importantes en la evaluación de todo paciente que consulta por cefalea en el servicio de urgencias y pueden orientar para la toma de decisiones.Artículo original22-33Headache is one of the main symptoms of consultation in the emergency department, on average 1.2 - 4.5% of all queries. Objective: To determine the clinical factors associated with the presence of brain neoplasms in patients presenting with headache. Methodology: A retrospective case-control study was conducted. Case was defined as each patient with headache and radiographical evidence by computed tomography (CT) or magnetic resonance imaging (MRI) of brain neoplasms and controls were patients with headache without radiographical evidence (CT or MRI) of brain neoplasms and meeting diagnostic criteria for primary headache. A sample size of 272 patients (62 cases and 210 controls) was determined. To determine factors associated with outcome analysis binary logistic regression was performed, including the plausible variables with p<0.2 in the bivariate analysis and those biologically plausible. The adjusted odds ratio (AOR) and their respective confidence intervals were calculated. A p<0.5 was considered significant. Conclusions: Despite the limitations of the study, it was found that the variables mentioned above are signs of predictive alarm in the presence of secondary headache to brain neoplasms and are of great importance in the primary evaluation of any patient who complains of headache in service emergency as they can guide the physician for decision making
