    Unilateral primary adrenal B-cell lymphoma clinically mimicking chronic gastritis

    Background: Primary adrenal lymphomas are a very rare type of extranodal lymphoma, and they usually are found bilaterally. Symptoms of the disease are variable and depend on the type of lymphoma and/or presence of adrenal insufficiency. Magnetic resonance imaging is the best radiologic modality for differentiating lymphomas from other adrenal malignancies; however, histopathology is considered as the gold standard method for diagnosing extranodal lymphomas. Case Report: We present a case of unilateral adrenal lymphoma that was initially diagnosed as an infectious disease and chronic gastritis, based on clinical and laboratory findings. Ultrasonography detected an adrenal mass, and magnetic resonance imaging excluded common lesions of the adrenal gland and showed lymphadenopathy around the major vessels of the abdomen. A percutaneous biopsy of the mass and bone marrow biopsy confirmed the diagnosis of primary adrenal lymphoma withoutbone marrow metastasis. Conclusions: Extranodal lymphomas can occur in almost every organ, and if nonspecific clinical findings are combined with the presence of a solid organ mass, lymphoma should be included in the differential diagnosi

    Mutagenic and antimutagenic properties of some lichen species grown in the Eastern Anatolia Region of Turkey

    All the methanol extracts did not show mutagenic activity in Ames/Salmonella and Z. mays MI test systems. Furthermore, some extracts showed significant antimutagenic activity against 9-AA in Ames test system. Inhibition rates for 9-AA mutagenicity ranged from 25.51 % (P. furfuracea – 0.05 μg/plate) to 66.14 % (C. islandica – 0.05 μg/plate). In addition, all of the extracts showed significant antimutagenic activity against sodium azide (NaN₃) mutagenicity on MI values of Z. mays.Целью работы было изучить мутагенный и антимутагенный потенциал метанольных экстрактов Cetraria islandica (L.) Ach. (Parmeliaceae), Pseudevernia furfuracea (L.) Zopf (Parmeliaceae) и Xanthoparmelia somloënsis (Gyeln.) Hale (Parmeliaceae) – лишайников из восточной части Турции. Ни один из экстрактов не показал мутагенной активности в тестах Эймса и Z. mays MI. Более того, некоторые экстракты проявляли заметную антимутагенную активность против 9-амино-акридина в тесте Эймса. Уровень ингибирования варьировал от 25,51 % (P. furfuracea) до 66,14 % (C. islandica). Кроме того, все экстракты проявляли значительную антимутагенную активность против азида натрия в Z. mays MI тесте. Все экстракты могут считаться генотоксично безопасными в исследованных концентрациях

    Hemiplejik serebral palsili çocukların üst ekstremitelerinin sağlam tarafı da gelişim geriliği gösterir

    Amaç: Çalışmamızda çocuklarda hemiplejik serebral palsi’nin HSP neden olduğu üst ekstremite gelişim geriliğinin antropometrik yöntemlerle incelenmesi amaçlandı. Gereç ve yöntem: Çalışmamızda Etik Kurul onayı alınarak 5-12 yaşlarındaki 32 HSP’li çocuk ile 40 normal çocuğun üst ekstremiteleri antropometrik set ile ölçülerek karşılaştırıldı. Bulgular: HSP’li çocukların sağlam taraf omuz eklemi, kol, dirsek eklemi, ön kol ve el bilek eklemi çevresi; üst ekstremite, kol, ön kol, el ve el ayası uzunluğu; dirsek eklemi, el bilek eklemi ve el genişliği değerleri HSP’li çocukların plejik tarafına göre anlamlı derecede fazlaydı. Normal çocukların omuz eklemi, kol, dirsek eklemi, ön kol ve el bilek eklemi çevresi; üst ekstremite, kol, ön kol, el ve el ayası uzunluğu; dirsek eklemi, el bilek eklemi ve el genişliği değerleri HSP’li çocukların plejik taraf ölçümlerine göre anlamlı derecede yüksekti. Normal çocukların dirsek eklemi ve ön kol çevresi; el ve el ayası uzunluğu; dirsek eklemi, el bilek eklemi ve el metakarpallerden genişliği değerleri HSP’li çocukların sağlam tarafına göre anlamlı derecede yüksekti. Sonuç: Antropometrik ölçümler HSP’li çocukların plejik taraf üst ekstremitelerinin sağlam tarafa göre, normal çocuklarla karşılaştırdığımızda ise hem plejik hem de sağlam taraflarının daha az geliştiğini göstermektedir. HSP’li çocukların az gelişmiş plejik taraflarını kullanamamaları günlük yaşam aktivitelerini de kısıtlayarak sağlam tarafta da kas ve kemik gelişim geriliğine sebep olmaktadır.     SUCCESS OF ARTIFICIAL INTELLIGENCE SYSTEM IN DETERMINING ALVEOLAR BONE LOSS FROM DENTAL PANORAMIC RADIOGRAPHY IMAGES

    Objectives: The aim of this study was to detect alveolar bone loss from dental panoramic radiographic images using artificial intelligence systems. Material and Methods: A total of 2276 panoramic radiographic images were used in this study. While 1137 of them belong to cases with bone destruction, 1139 were periodontally healthy. The dataset is divided into three parts as training (n=1856) , validation (n=210) and testing set (n= 210). All images in the data set were resized to 1472x718 pixels before training. A random sequence was created using the open-source python programming language and OpenCV, NumPy, Pandas, and Matplotlib libraries effectively. A pre-trained Google Net Inception v3 CNN network was used for preprocessing and data sets were trained using transfer learning. Diagnostic performance was evaluated with the confusion matrix using sensivitiy, specificity, precision, accuracy and F1 score. Results: Of the 105 cases with bone loss, 99 were detected by the AI system. Sensitivity was 0.94, specificity 0.88, precision 0.89, accuracy 0.91 and F1 score 0.91. Conclusion: The convolutional neural network model is successful in determining periodontal bone losses. It can be used as a system to facilitate the work of physicians in diagnosis and treatment planning in the future

    Aorta abdominalis ve dallari: Otopsi olgularında morfometri ve varyasyonları]

    Aim: Knowing the morphology of abdominal aorta (AA) and its branches are important as regards to diagnosis and surgical treatment. The aims of this study were to a) make morphometric measurements of AA and its branches, b) to investigate sites of the origins of the branches and their relationships and variations, and c) to compare the results with literature.Method: Ninety-five AA which had been removed in autopsies were measured with caliper morphometrically to determine diameters of branches and distances between branches. Possible variation of the vessels were investigated and photographed.Result: It was found that diameters of celiac trunk (CT), superior mesenteric artery (SMA) and inferior mesenteric artery (IMA) were 6.43±1.59 mm, 7.38±1.67 mm and 3.61±0.72 mm respectively. The distances between CT and aortic bifurcation (AB), CT and SMA, SMA and IMA, IMA and AB were 107.21±11.46 mm, 14.34±2.67 mm, 57.76±8.04 mm, 35.20±7.41 mm respectively. Numerous variations were observed during the study. These variations involved inferior phrenic artery (single trunk arising from CT, 4.2%), renal artery-RA (duplicated right RA 9.5%, duplicated left RA 4.2%, bilaterally duplicated 3.1%, %16.8 total multiple RA), gonadal arteries-GA (single GA, 1%), lumbar arteries-LA (3 pairs of LA 11.5%, 3rd or 4th LA arising as single trunk 3.1%) and median sacral artery (agenesis 2.1%). Conclusion: Knowledge of morphology of AA and its branches is important in regards to the diagnosis, surgical treatment and endovascular interventions of these vessels. We think our study will contribute to the medical education and clinical medicine in our country

    İncisional endometrioma mimicking a malignant mass: report of two cases

    Endometriozis dogurganlık çagındaki kadınlarda sık görülen klinik bir problemdir. Insizyon yerlesimli endometriomaya ise nadir rastlanır. Hastalarda genellikle sezaryen veya histerektomi gibi ameliyat öyküsü vardır. Endometriomayı olusturan dokudan malignite gelisebildigi bildirilmistir. Jinekolojik bir patoloji olmasına ragmen insizyonel herni ya da karın duvarı tümörleri gibi bulgu verebilmekte ve bu nedenle hastalar genellikle genel cerrahi polikliniklerine basvurmaktadırlar. Biz bu çalısmada dogurganlık çagında olan ve daha önce sezaryen ameliyatı öyküsü bulunan, klinik ve laboratuar olarak malignite süphesi tasıyan iki olguyu sunduk. Tedavide sezaryen skarındaki kitlelere genis eksizyon yapıldı ve olusan fasya defekti primer onarıldı. Patolojik inceleme sonucu kitlelerin endometrium dokusuna sahip endometrioma oldugu saptandı.Endometriosis is a common clinical problem of the child-bearing women. Incisional endometrioma is very rare. Patients frequently presented with a history of previous gynecological surgery such as cesarean section or hysterectomy. Although being a gynecological disorder, it may seem like an incisional hernia or abdominal wall tumor. Therefore, it is frequently referred to general surgery outpatient clinics. Here, we report two cases with previous histories of cesarean sections and questinable clinical and laboratory findings of malignancy. The masses located on the previous incision scars were excised widely, and pathological examination of surgical specimens revealed the diagnosis of endometrioma in both cases

    A Comparison of Depression and Anxiety among University Students in Nine Countries during the COVID-19 Pandemic.

    The mental health of young adults, particularly students, is at high risk during the COVID-19 pandemic. The purpose of this studywas to examine differences inmental health between university students in nine countries during the pandemic. The study encompassed 2349 university students (69% female) from Colombia, the Czech Republic (Czechia), Germany, Israel, Poland, Russia, Slovenia, Turkey, and Ukraine. Participants underwent the following tests: Patient Health Questionnaire (PHQ-8), Generalized Anxiety Disorder (GAD-7), Exposure to COVID-19 (EC-19), Perceived Impact of Coronavirus (PIC) on students’ well-being, PhysicalActivity (PA), andGeneral Self-ReportedHealth (GSRH). The one-wayANOVAshowed significant differences between countries. The highest depression and anxiety risk occurred in Turkey, the lowest depression in the Czech Republic and the lowest anxiety in Germany. The 2 independence test showed that EC-19, PIC, and GSRHwere associatedwith anxiety and depression inmost of the countries, whereas PA was associated in less than half of the countries. Logistic regression showed distinct risk factors for each country. Gender and EC-19 were the most frequent predictors of depression and anxiety across the countries. The role of gender and PA for depression and anxiety is not universal and depends on cross-cultural differences. Students’mental health should be addressed froma cross-cultural perspective

    Around the Clock Observations of the Q0957+561 A,B Gravitationally Lensed Quasar II: Results for the second observing season

    We report on an observing campaign in March 2001 to monitor the brightness of the later arriving Q0957+561 B image in order to compare with the previously published brightness observations of the (first arriving) A image. The 12 participating observatories provided 3543 image frames which we have analyzed for brightness fluctuations. From our classical methods for time delay determination, we find a 417.09 +/- 0.07 day time delay which should be free of effects due to incomplete sampling. During the campaign period, the quasar brightness was relatively constant and only small fluctuations were found; we compare the structure function for the new data with structure function estimates for the 1995--6 epoch, and show that the structure function is statistically non-stationary. We also examine the data for any evidence of correlated fluctuations at zero lag. We discuss the limits to our ability to measure the cosmological time delay if the quasar's emitting surface is time resolved, as seems likely.Comment: AAS LaTeX, 5 PostScript figure

    Beta1-Adrenoceptor Polymorphism Predicts Flecainide Action in Patients with Atrial Fibrillation

    BACKGROUND: Antiarrhythmic action of flecainide is based on sodium channel blockade. Beta(1)-adrenoceptor (beta(1)AR) activation induces sodium channel inhibition, too. The aim of the present study was to evaluate the impact of different beta(1)AR genotypes on antiarrhythmic action of flecainide in patients with structural heart disease and atrial fibrillation. METHODOLOGY/PRINCIPAL FINDINGS: In 145 subjects, 87 with atrial fibrillation, genotyping was performed to identify the individual beta(1)AR Arg389Gly and Ser49Gly polymorphism. Resting heart rate during atrial fibrillation and success of flecainide-induced cardioversion were correlated with beta(1)AR genotype. The overall cardioversion rate with flecainide was 39%. The Arg389Arg genotype was associated with the highest cardioversion rate (55.5%; OR 3.30; 95% CI; 1.34-8.13; p = 0.003) compared to patients with Arg389Gly (29.5%; OR 0.44; 95% CI; 0.18-1.06; p = 0.066) and Gly389Gly (14%; OR 0.24; 95% CI 0.03-2.07; p = 0.17) variants. The single Ser49Gly polymorphism did not influence the conversion rate. In combination, patients with Arg389Gly-Ser49Gly genotype displayed the lowest conversion rate with 20.8% (OR 0.31; 95% CI; 0.10-0.93; p = 0.03). In patients with Arg389Arg variants the heart rate during atrial fibrillation was significantly higher (110+/-2.7 bpm; p = 0.03 vs. other variants) compared to Arg389Gly (104.8+/-2.4 bpm) and Gly389Gly (96.9+/-5.8 bpm) carriers. The Arg389Gly-Ser49Gly genotype was more common in patients with atrial fibrillation compared to patients without atrial fibrillation (27.6% vs. 5.2%; HR 6.98; 95% CI; 1.99-24.46; p<0.001). CONCLUSIONS: The beta(1)AR Arg389Arg genotype is associated with increased flecainide potency and higher heart rate during atrial fibrillation. The Arg389Gly-Ser49Gly genotype might be of predictive value for atrial fibrillation
