    Toxic epidermal necrolysis and Stevens-Johnson syndrome

    Toxic epidermal necrolysis (TEN) and Stevens Johnson Syndrome (SJS) are severe adverse cutaneous drug reactions that predominantly involve the skin and mucous membranes. Both are rare, with TEN and SJS affecting approximately 1or 2/1,000,000 annually, and are considered medical emergencies as they are potentially fatal. They are characterized by mucocutaneous tenderness and typically hemorrhagic erosions, erythema and more or less severe epidermal detachment presenting as blisters and areas of denuded skin. Currently, TEN and SJS are considered to be two ends of a spectrum of severe epidermolytic adverse cutaneous drug reactions, differing only by their extent of skin detachment. Drugs are assumed or identified as the main cause of SJS/TEN in most cases, but Mycoplasma pneumoniae and Herpes simplex virus infections are well documented causes alongside rare cases in which the aetiology remains unknown. Several drugs are at "high" risk of inducing TEN/SJS including: Allopurinol, Trimethoprim-sulfamethoxazole and other sulfonamide-antibiotics, aminopenicillins, cephalosporins, quinolones, carbamazepine, phenytoin, phenobarbital and NSAID's of the oxicam-type. Genetic susceptibility to SJS and TEN is likely as exemplified by the strong association observed in Han Chinese between a genetic marker, the human leukocyte antigen HLA-B*1502, and SJS induced by carbamazepine. Diagnosis relies mainly on clinical signs together with the histological analysis of a skin biopsy showing typical full-thickness epidermal necrolysis due to extensive keratinocyte apoptosis. Differential diagnosis includes linear IgA dermatosis and paraneoplastic pemphigus, pemphigus vulgaris and bullous pemphigoid, acute generalized exanthematous pustulosis (AGEP), disseminated fixed bullous drug eruption and staphyloccocal scalded skin syndrome (SSSS). Due to the high risk of mortality, management of patients with SJS/TEN requires rapid diagnosis, evaluation of the prognosis using SCORTEN, identification and interruption of the culprit drug, specialized supportive care ideally in an intensive care unit, and consideration of immunomodulating agents such as high-dose intravenous immunoglobulin therapy. SJS and TEN are severe and life-threatening. The average reported mortality rate of SJS is 1-5%, and of TEN is 25-35%; it can be even higher in elderly patients and those with a large surface area of epidermal detachment. More than 50% of patients surviving TEN suffer from long-term sequelae of the disease

    Cervical lymph node metastasis in adenoid cystic carcinoma of the larynx: a collective international review

    Adenoid cystic carcinoma (AdCC) of the head and neck is a well-recognized pathologic entity that rarely occurs in the larynx. Although the 5-year locoregional control rates are high, distant metastasis has a tendency to appear more than 5 years post treatment. Because AdCC of the larynx is uncommon, it is difficult to standardize a treatment protocol. One of the controversial points is the decision whether or not to perform an elective neck dissection on these patients. Because there is contradictory information about this issue, we have critically reviewed the literature from 1912 to 2015 on all reported cases of AdCC of the larynx in order to clarify this issue. During the most recent period of our review (1991-2015) with a more exact diagnosis of the tumor histology, 142 cases were observed of AdCC of the larynx, of which 91 patients had data pertaining to lymph node status. Eleven of the 91 patients (12.1%) had nodal metastasis and, based on this low proportion of patients, routine elective neck dissection is therefore not recommended

    Cílem této práce bylo vytvoření takové aplikace, která koncovému uživateli dovolí vygenerovat libovolnou oblast na zemi jako 3D mapu, kterou si bude uživatel moct prohlédnout a seznámit se s touto oblastí, než se tam případně vydá. Aplikace byla vyvíjena jako desktopová aplikace v jazyce C++ s využitím OpenGL na zobrazení výsledku a Qt frameworku na uživatelské rozhraní. Přínosem aplikace je, že dovoluje nastavit například hustotu vegetace, a uživateli se tak zobrazí i běžně skryté cesty, a i na méně známých místech poskytuje reálné cesty.The goal of this thesis was to create an application, which allows the end user to generate any area on earth as a 3D map, which they could look at and get to know, before they set out on a journey there.     The goal of this bachelor thesis is a description of the whole years work of the drama group at The Grammar School in Bechyne, Skolni Street. The thesis describes the staging process leading to the formation of a performance, that was based on the story ?The Old Woman And The Tossed Boy? from the book ?The Thorn Girl? written by Jachym Topol. The thesis includes informations about the group, text about the literary artwork, informations about the realization and the presentation of the theatre project. It plots the effect of this work on the group. It describes the educational and theatre aspects of the process and tries to name advantages and disadvantages of my work as the drama teacher of the group. The important aspect of my bachelor thesis is my attempt at an insight into the running of the ?usual? school activities, and at recording, how to make an theatre project in those specific conditions

The application was developed as a desktop application in the C++ language with the use of OpenGL to render the result and Qt framework for user interface. Added value of this application is, that it allows to set for example the density of vegetation, so that small paths, that are usually hidden are visible and it provides real paths even in lesser known areas

Představuje také nový souhrnný pohled na dějiny hradu, o nichž se v literatuře objevují vzájemně se odporující názory, a přináší lingvistický rozbor původu jména hradu.Paper presents set of pottery, fragment of brick and fragment of plastering, which were found on 3rd April 2012 in an illegal excavation at Levnov castle. All artifacts can be in general dated to the 14 century. In paper are also presented new historical and linguistic data, that proove existence of Levnov-castle in 2nd half of 13th century and also allow us to postulate hypothesis, that Levnov castle could be foundation of the last Premyslids
