How Do Interviewers and Children Discuss Individual Occurrences of Alleged Repeated Abuse in Forensic Interviews?
Police interviews (n = 97) with 5- to 13-year-olds alleging multiple incidents of sexual abuse were examined to determine how interviewers elicited and children recounted specific instances of abuse. Coders assessed the labels for individual occurrences that arose in interviews, recording who generated them, how they were used, and other devices to aid particularisation such as the use of episodic and generic language. Interviewers used significantly more temporal labels than did children. With age, children were more likely to generate labels themselves, but most children generated at least one label. In 66% of the cases, interviewers ignored or replaced children’s labels, and when they did so, children reported proportionately fewer episodic details. Children were highly responsive to the interviewers’ language style. Results indicate that appropriately trained interviewers can help children of all ages to provide the specific details often necessary to ensure successful prosecution
Clinical leadership in service redesign using Clinical Commissioning Groups: a mixed-methods study
Background: A core component of the Health and Social Care Act 2012 (Great Britain. Health and Social Care Act 2012. London: HMSO; 2012) was the idea of devolving to general practitioners (GPs) a health service leadership role for service redesign. For this purpose, new Clinical Commissioning Groups (CCGs) were formed in the English NHS.Objectives: This research examined the extent to which, and the methods by which, clinicians stepped
forward to take up a leadership role in service redesign using CCGs as a platform.Design: The project proceeded in five phases: (1) a scoping study across 15 CCGs, (2) the design and administration of a national survey of all members of CCG governing bodies in 2014, (3) six main in-depth case studies, (4) a second national survey of governing body members in 2016, which allowed longitudinal
comparisons, and (5) international comparisons.Participants: In addition to GPs serving in clinical lead roles for CCGs, the research included insights from accountable officers and other managers and perspectives from secondary care and other provider organisations (local authority councillors and staff, patients and the public, and other relevant bodies).Results: Instances of the exercise of clinical leadership utilising the mechanism of the CCGs were strikingly varied. Some CCG teams had made little of the opportunity. However, we found other examples of clinicians stepping forward to bring about meaningful improvements in services. The most notable cases
involved the design of integrated care for frail elderly patients and others with long-term conditions. The leadership of these service redesigns required cross-boundary working with primary care, secondary care, community care and social work. The processes enabling such breakthroughs required interlocking processes of leadership across three arenas: (1) strategy-level work at CCG board level, (2) mid-range
operational planning and negotiation at programme board level and (3) the arena of practical implementation leadership at the point of delivery. The arena of the CCG board provided the legitimacy for strategic change; the programme boards worked through the competing logics of markets, hierarchy and networks; and the practice arena allowed the exercise of clinical leadership in practical problemsolving, detailed learning and routinisation of new ways of working at a common-sense everyday level.Limitations: Although the research was conducted over a 3-year period, it could be argued that a much longer period is required for CCGs to mature and realise their potential.Conclusions: Despite the variation in practice, we found significant examples of clinical leaders forging
new modes of service design and delivery. A great deal of the service redesign effort was directed at compensating for the fragmented nature of the NHS – part of which had been created by the 2012 reforms. This is the first study to reveal details of such work in a systematic way
Outcomes from a pilot psychological therapies service for UK military veterans
Aim: To evaluate the outcomes of participants attending a psychological therapies service for military veterans. Background: The UK Military Veterans’ Improving Access to Psychological Therapies Service (North West) (MV IAPT) provided a clinical psychological therapies service for military veterans. Outcomes of depression, anxiety and social adjustment were assessed after treatment in the service's pilot phase. Design: An observational, prospective cohort study examined changes in depression, anxiety and social adjustment during receipt of the service. Methods: Changes in depression (PHQ-9), anxiety (GAD-7) and social adjustment (WSAS) were examined in 952 veterans referred over 20 months from September 2011. Data were collected using the IAPT clinical information system plus additional fields. Changes for patients who completed treatment, remained in treatment and dropped out were compared. Results: Seven hundred and seven veterans received an initial assessment, from which 505 received two or more appointments. Of these, 156 completed treatments, 179 remained in treatment and 170 dropped out. The majority of veterans had been operationally deployed and were similar in risk characteristics to those in other military cohort studies. There were highly significant improvements on all measures (p<.01), with completers improving more and having higher rates of recovery from depression and anxiety than those remaining and drop outs. Recovery rates compared favourably with evaluations of general IAPT services and also exceeded reported natural recovery rates
Microbial biopesticides for integrated crop management : an assessment of environmental and regulatory sustainability
Herbivorous insects and mites, plant diseases and weeds are major impediments to the production of food crops and are increasingly difficult to control with conventional chemicals. This paper focuses on microbial control agents with an emphasis on augmentation. There are marked differences in the availability of products in different countries which can be explained in terms of differences in their regulatory systems. Regulatory failure arises from the application of an inappropriate synthetic pesticides model. An understanding of regulatory innovation is necessary to overcome these problems. Two attempts at remedying regulatory failure in the UK and the Netherlands are assessed. Scientific advances can feed directly into the regulatory process and foster regulatory innovation
Defining and assessing spiritual health : a comparative study among 13- to 15-year-old pupils attending secular schools, Anglican schools, and private Christian schools in England and Wales
This article argues that the nation's commitment to young people involves proper concern for their physical health, their psychological health, and their spiritual health. In this context the notion of spiritual health is clarified by a critique of John Fisher's model of spiritual health. Fisher developed a relational model of spiritual health, which defines good spiritual health in terms of an individual's relationship to four domains: the personal, the communal, the environmental, and the transcendental. In the present analysis, we make comparisons between pupils educated in three types of schools: publicly funded schools without religious foundation, publicly funded schools with an Anglican foundation, and new independent Christian schools (not publicly funded). Our findings draw attention to significant differences in the levels of spiritual health experienced by pupils within these three types of schools

Spirituality and Leadership: An Empirical Review of Definitions, Distinctions, and Embedded Assumptions
Spirituality and its relationship to workplace leadership is a compelling issue for management practitioners and researchers. The field of study is still in its infancy and as such is marked by differences in definitions and other basic characteristics. Much of what has been written on this subject has appeared in general, rather than academic publications and consequently may lack rigor. The purpose of this study is to analyze known academic articles for how they characterize workplace spirituality, explore the nexus between spirituality and leadership, and discover essential factors and conditions for promoting a theory of spiritual leadership within the context of the workplace.
An emergent process was used to identify and validate eight areas of difference and/or distinction in the workplace spirituality literature: 1.) definition, 2.) connected to religion, 3.) marked by epiphany, 4.) teachable, 5.) individual development, 6.) measurable, 7.) profitable/productive, and 8.) nature of the phenomenon. Eighty-seven scholarly articles were coded for each of these areas. Findings conclude that most researchers couple spirituality and religion and that most either have found, or hypothesize a correlation between spirituality and productivity. The emergent categories offer provocative new avenues for the development of leadership theory
Modeling U.S. Manufacturing Competitiveness
Few issues are discussed as frequently by American politicians as jobs, and particularly the loss of jobs, ostensibly to cheaper labor markets. The underlying reasons why some firms depart American soil and others do not, however, are not completely understood. The purpose of this study is to propose a model for determining what successful manufacturers are doing to remain competitive and, consequently, not moving to other countries unnecessarily.
There is no extant model that effectively differentiates what successful manufacturers are doing compared with unsuccessful (i.e., job-cutting or factory-closing) ones. To develop this conceptual model, we have conducted interviews with manufacturers in our region (one of the poorest in the country and one that has lost over 10,000 manufacturing jobs and 52 of 141 manufacturing plants between 1993-2003). We have also completed an exploratory content analysis of the literature on this subject. From this analysis, we have developed a model that will be described here. The model consists of five primary differentiators: (1) the flow of vital information, (2) the human factor (leadership, teamwork, etc.), (3) issues of cost and efficiency, (4) integration of information technology, and (5) cooperative methodologies
The Senior Leader as Both a Strategist and Technical Specialist
In the past, it was not uncommon for a major league baseball manager to also be one of the players of the team. Today, these positions are so specialized that this practice no longer occurs. However, modern organizations are increasingly moving in the direction of the old baseball team model, having senior leaders also “playing” in operational roles. I refer to this phenomenon as the “John McGraw duality,” in honor of a player/manager, who for eight seasons, managed a team to a World Series championship and also played at a level where he led the league in on-base-percentage three times. John McGraw (1873-1934) had a skill set that allowed him to be a very successful leader. His team depended on him for his individual expertise on the field and at the plate. As we plunge further into the twenty-first century, the role of senior leader continues to change and evolve
