    Luxación aislada de la articulación radiocubital distal : Un nuevo caso

    Se presenta 1 caso de luxación dorsal aislada de la articulación radiocubital distal en un paciente varón de 18 años que sufrió una caída sobre la mano izquierda en posición de pronación del antebrazo. Fue tratado mediante reducción cerrada bajo anestesia local y posterior inmovilización en un yeso braquioantcbraquial con el codo en 90° de flexión y el antebrazo en supinación durante 6 semanas. El resultado tras el tratamiento rehabilitador posterior a la retirada del yeso fue satisfactorio, sin dolor ni alteraciones estéticas ni funcionales en la extremidad.A case of isolated dislocation of the distal radioulnar joint is presented. A 18-yearold man felt on his left hand with the forearm in pronation. The treatment consisted of closed reduction and cast inmobilization of the upper extremity with the elbow in 90° of flexion and the forearm in supination during 6 weeks.     Xmipp is an open-source software package consisting of multiple programs for processing data originating from electron microscopy and electron tomography, designed and managed by the Biocomputing Unit of the Spanish National Center for Biotechnology, although with contributions from many other developers over the world. During its 25 years of existence, Xmipp underwent multiple changes and updates. While there were many publications related to new programs and functionality added to Xmipp, there is no single publication on the Xmipp as a package since 2013. In this article, we give an overview of the changes and new work since 2013, describe technologies and techniques used during the development, and take a peek at the future of the package

    Se presenta una serie de 125 pacientes afectos de cuadro de lumbociatalgia atípica, diagnosticados de síndrome facetario y tratados con infiltraciones de solución fenolada en las articulaciones interapofisarias posteriores afectadas, seguidas de un programa de rehabilitación protocolizado tras las infiltraciones. A propósito de los mismos y tras una análisis estadístico de los datos obtenidos del estudio, se establecen algunos criterios para el tratamiento de pacientes aquejados de este tipo de patología.The authors present a series of 125 patients afflicted with an atypical sciatic low-back pain, who were diagnosed of "facet joint syndrome" and treated with injections of phenol solution in the affected lumbar zygapophysial joints. The patients followed a protocolized rehabilitation program. After an statistical analysis of the data obtained from the study, some criteria are stabilized for the treatment of patients suffering this pathology

    Advances in image processing for single-particle analysis by electron cryomicroscopy and challenges ahead

    Electron cryomicroscopy (cryo-EM) is essential for the study and functional understanding of non-crystalline macromolecules such as proteins. These molecules cannot be imaged using X-ray crystallography or other popular methods. CryoEM has been successfully used to visualize molecules such as ribosomes, viruses, and ion channels, for example. Obtaining structural models of these at various conformational states leads to insight on how these molecules function. Recent advances in imaging technology have given cryo-EM a scientific rebirth. Because of imaging improvements, image processing and analysis of the resultant images have increased the resolution such that molecular structures can be resolved at the atomic level. Cryo-EM is ripe with stimulating image processing challenges. In this article, we will touch on the most essential in order to build an accurate structural three-dimensional model from noisy projection images. Traditional approaches, such as k-means clustering for class averaging, will be provided as background. With this review, however, we will highlight fresh approaches from new and varied angles for each image processing sub-problem, including a 3D reconstruction method for asymmetric molecules using just two projection images and deep learning algorithms for automated particle picking.     Genetic Architecture of ADHD and Overlap With Other Psychiatric Disorders And Cognition-Related Phenotypes

    Attention-deficit/hyperactivity disorder (ADHD) co-occurs with many other psychiatric disorders and traits. In this review, we summarize and interpret the existing literature on the genetic architecture of these comorbidities based on hypothesis-generating approaches. Quantitative genetic studies indicate that genetic factors play a substantial role in the observed co-occurrence of ADHD with many different disorders and traits. Molecular genetic correlations derived from genome-wide association studies and results of studies based on polygenic risk scores confirm the general pattern but provide effect estimates that are smaller than those from twin studies. The identification of the specific genetic variants and biological pathways underlying co-occurrence using genome-wide approaches is still in its infancy. The first analyses of causal inference using genetic data support causal relationships between ADHD and comorbid disorders, although bidirectional effects identified in some instances point to complex relationships. While several issues in the methodology and inferences from the results are still to be overcome, this review shows that the co-occurrence of ADHD with many psychiatric disorders and traits is genetically interpretable

    A chelating sorbent was obtained by incorporating Alizarin S and Sodium Morpholyldithiocarbamate (MorDTC) respectively in Amberlite IRA 400 (Cl). Structural characteristics were studied by X-Ray Powder Diffraction (XRD), Optical Microscopy (OM), Differential Scanning Calorimetry (DSC) and Adsorption Techniques. The pH of optimal retention pH of MorDTC in Amberlite IRA 400 (Cl-)was obtained (pH 8-9). For Alizarin S an optimal retention is achieved even at pH 1-2.     Introduction: Some COVID-19 patients evolve to severe lung injury and systemic hyperinflammatory syndrome triggered by both the coronavirus infection and the subsequent host-immune response. Accordingly, the use of immunomodulatory agents has been suggested but still remains controversial. Our working hypothesis is that methylprednisolone pulses and tacrolimus may be an effective and safety drug combination for treating severe COVID-19 patients. Methods: and analysis: TACROVID is a randomized, open-label, single-center, phase II trial to evaluate the ef- ficacy and safety of methylprednisolone pulses and tacrolimus plus standard of care (SoC) versus SoC alone, in patients at advanced stage of COVID-19 disease with lung injury and systemic hyperinflammatory response. Patients are randomly assigned (1:1) to one of two arms (42 patients in each group). The primary aim is to assess the time to clinical stability after initiating randomization. Clinical stability is defined as body temperature≤37.5 ◦C, and PaO2/FiO2 > 400 and/or SatO2/FiO2 > 300, and respiratory rate ≤24 rpm; for 48 consecutive hours. Discussion: Methylprednisolone and tacrolimus might be beneficial to treat those COVID-19 patients progressing into severe pulmonary failure and systemic hyperinflammatory syndrome. The rationale for its use is the fast effect of methylprednisolone pulses and the ability of tacrolimus to inhibit both the CoV-2 replication and the secondary cytokine storm. Interestingly, both drugs are low-cost and can be manufactured on a large scale; thus, if effective and safe, a large number of patients could be treated in developed and developing countries

    Improvement in detecting cytomegalovirus drug resistance mutations in solid organ transplant recipients with suspected resistance using next generation sequencing

    Objetives The aim of this study was to identify CMV drug resistance mutations (DRM) in solid organ transplant (SOT) recipients with suspected resistance comparing next-generation sequencing (NGS) with Sanger sequencing and assessing risk factors and the clinical impact of resistance. Methods Using Sanger sequencing as the reference method, we prospectively assessed the ability of NGS to detect CMV DRM in the UL97 and UL54 genes in a nationwide observational study from September 2013 to August 2016. Results Among 44 patients recruited, 14 DRM were detected by Sanger in 12 patients (27%) and 20 DRM were detected by NGS, in 16 (36%). NGS confirmed all the DRM detected by Sanger. The additional six mutations detected by NGS were present in <20% of the sequenced population, being located in the UL97 gene and conferring high-level resistance to ganciclovir. The presence of DRM by NGS was associated with lung transplantation (p = 0.050), the administration of prophylaxis (p = 0.039), a higher mean time between transplantation and suspicion of resistance (p = 0.038) and longer antiviral treatment duration before suspicion (p = 0.024). However, the latter was the only factor independently associated with the presence of DRM by NGS in the multivariate analysis (OR 2.24, 95% CI 1.03 to 4.87). Conclusions NGS showed a higher yield than Sanger sequencing for detecting CMV resistance mutations in SOT recipients.     Combining schizophrenia and depression polygenic risk scores improves the genetic prediction of lithium response in bipolar disorder patients

    Lithium is the gold standard therapy for Bipolar Disorder (BD) but its effectiveness differs widely between individuals. The molecular mechanisms underlying treatment response heterogeneity are not well understood, and personalized treatment in BD remains elusive. Genetic analyses of the lithium treatment response phenotype may generate novel molecular insights into lithium's therapeutic mechanisms and lead to testable hypotheses to improve BD management and outcomes. We used fixed effect meta-analysis techniques to develop meta-analytic polygenic risk scores (MET-PRS) from combinations of highly correlated psychiatric traits, namely schizophrenia (SCZ), major depression (MD) and bipolar disorder (BD). We compared the effects of cross-disorder MET-PRS and single genetic trait PRS on lithium response. For the PRS analyses, we included clinical data on lithium treatment response and genetic information for n = 2283 BD cases from the International Consortium on Lithium Genetics (ConLi+Gen; www.ConLiGen.org). Higher SCZ and MD PRSs were associated with poorer lithium treatment response whereas BD-PRS had no association with treatment outcome. The combined MET2-PRS comprising of SCZ and MD variants (MET2-PRS) and a model using SCZ and MD-PRS sequentially improved response prediction, compared to single-disorder PRS or to a combined score using all three traits (MET3-PRS). Patients in the highest decile for MET2-PRS loading had 2.5 times higher odds of being classified as poor responders than patients with the lowest decile MET2-PRS scores. An exploratory functional pathway analysis of top MET2-PRS variants was conducted. Findings may inform the development of future testing strategies for personalized lithium prescribing in BD
