Differences Between Frequency Of Diagnosis, Diagnosis Extremity, And Global Assessment Of Functioning Score In A Euro-American And Alaskan Native Client
Thesis (Ph.D.) University of Alaska Fairbanks, 2011This research answers the question, given identical client information, history, and presenting issues, but variation in ethnicity, does diagnosis frequency, diagnosis extremity, or Global Assessment of Functioning score differ for an Euro-American male versus an Alaska Native male mental health client. Graduate counseling students, six males and six females, ranging in age from 22--59, currently enrolled at either the University of Alaska Fairbanks, the University of Alaska Anchorage, or Alaska Pacific University, volunteered to participate in the present study. Participants were randomly assigned to view either a Euro-American or Alaska Native client's mock intake session. The mock videos were identical in script and environment; the only difference in the videos is that one male actor is Euro-American and the other actor is Alaska Native. Completed mental health intake forms were compared and evaluated through both quantitative and qualitative methods. Qualitatively, Strauss and Corbin's (1990) three step analytic process, grounded theory, was used to analyze the descriptive part of the intake form. Axis I, II, III, IV and V, of the DSM-IV-TR (APA, 2000), multi-axial system, were quantitatively, assessed to determine diagnosis differences between the Euro-American and Alaska Native client. Results identify that counseling students in training view the Alaska Native client as overall more maladaptive versus the Euro-American client. Counselors-in-training expressed this tendency through more frequent diagnosis and lower Global Assessment of Functioning scores for the Alaska Native client. These results support the need for future research and counselor training programs to be aware of these tendencies of counselors-in-training
A randomized controlled pilot trial of classroom-based mindfulness meditation compared to an active control condition in sixth-grade children
The current study is a pilot trial to examine the effects of a nonelective, classroom-based, teacher-implemented, mindfulness meditation intervention on standard clinical measures of mental health and affect in middle school children. A total of 101 healthy sixth-grade students (55 boys, 46 girls) were randomized to either an Asian history course with daily mindfulness meditation practice (intervention group) or an African history course with a matched experiential activity (active control group). Self-reported measures included the Youth Self Report (YSR), a modified Spielberger State-Trait Anxiety Inventory, and the Cognitive and Affective Mindfulness Measure –Revised. Both groups decreased significantly on clinical syndrome subscales and affect but did not differ in the extent of their improvements. Meditators were significantly less likely to develop suicidal ideation or thoughts of self-harm than controls. These results suggest that mindfulness training may yield both unique and non-specific benefits that are shared by other novel activities
Gender Differences in Response to a School-Based Mindfulness Training Intervention for Early Adolescents
Mindfulness training has been used to improve emotional wellbeing in early adolescents. However, little is known about treatment outcome moderators, or individual differences that may differentially impact responses to treatment. The current study focused on gender as a potential moderator for affective outcomes in response to school-based mindfulness training. Sixth grade students (N = 100) were randomly assigned to either the six weeks of mindfulness meditation or the active control group as part of a history class curriculum. Participants in the mindfulness meditation group completed short mindfulness meditation sessions four to five times per week, in addition to didactic instruction (Asian history). The control group received matched experiential activity in addition to didactic instruction (African history) from the same teacher with no meditation component. Self-reported measures of emotional wellbeing/affect, mindfulness, and self-compassion were obtained at pre and post intervention. Meditators reported greater improvement in emotional wellbeing compared to those in the control group. Importantly, gender differences were detected, such that female meditators reported greater increases in positive affect compared to females in the control group, whereas male meditators and control males displayed equivalent gains. Uniquely among females but not males, increases in self-reported self-compassion were associated with improvements in affect. These findings support the efficacy of school-based mindfulness interventions, and interventions tailored to accommodate distinct developmental needs of female and male adolescents
Continuing Education Training Focused on the Development of Behavioral Telehealth Competencies in Behavioral Healthcare Providers
Introduction: Telehealth allows behavioral health care and specialty services to be extended to rural residents. Telehealth is an important resource for the Alaskan healthcare system, which is tasked with providing services to culturally diverse populations living in remote areas. Training competent providers to deliver telehealth services is vital for the implementation of successful telehealth programs. Yet, the literature is lacking in the area of provider behavioral telehealth competency training.
Methods: This study assessed the impact of a Behavioral Telehealth Ethical Competencies Training program on 16 behavioral health providers\u27 development of behavioral telehealth competency. A total of 14 competencies were developed, which required participants to understand the roles and responsibilities of a behavioral telehealth coordinator working at the distal site as well as the roles and responsibilities of the therapist. Video vignettes evaluating the 14 competencies, self-reported competence surveys and follow-up surveys of progress on telehealth goals were utilized to assess effects of the training.
Results: Results indicated participants\u27 behavioral telehealth competencies increased following training. Participants reported positive perceptions regarding their competency, and achieved progress on the majority of behavioral telehealth goals set during the training.
Conclusions: This study provides a baseline for developing a best practice model for behavioral telehealth service delivery by identifying specific provider competencies for administering effective behavioral telehealth services. A unique continuing education training model, led by content experts including university professors and Alaska Native Elders, incorporating behavioral telehealth, rural ethics, cultural competency and vicarious trauma training is described. Lastly, this study details the use of an innovative video vignette assessment instrument for evaluating the effectiveness of continuing education training
