    Ethnic Minorities and their Health Needs: Crisis of Perception and Behaviours

    There is considerable evidence to suggest that racial and ethnic disparities exist in the provision of emergency and wider healthcare. The importance of collecting patient ethnic data has received attention in literature across the world and eliminating ethnic and racial health equalities is one of the primary aims of healthcare providers internationally. The poor health status of certain racial and ethnic groups has been well documented. The improvement of racial and ethnic disparities in healthcare is at the forefront of many public health agendas. This article addresses important policy, practice, and cultural issues confronted by the pre-hospital emergency care setup. This aspect of care plays a unique role in the healthcare safety net in providing a service to a very diverse population, including members of ethnic and racial minorities. Competent decision making by the emergency care practitioners requires patient-specific information and the health provider's prior medical knowledge and clinical training. The article reviews the current ethnicity trends in the UK along with international evidence linking ethnicity and health inequalities. The study argues that serious difficulties will arise between the health provider and the patient if they come from different backgrounds and therefore experience difficulties in cross-cultural communication. This adversely impacts on the quality of diagnostic and clinical decision making for minority patients. The article offers few strategies to address health inequalities in emergency care and concludes by arguing that much more needs to be done to ensure that we are hearing the voices of more diverse groups, groups who are often excluded from engagement through barriers such as language or mobility difficulties

    Reclamation of reactive metal oxides from complex minerals using alkali roasting and leaching- an improved approach to process engineering

    In nature, the commonly occurring reactive metal oxides of titanium, chromium, aluminium, and vanadium often chemically combine with the transition metal oxides such as iron oxides and form complex minerals. Physico-chemical separation of transition metal oxides from the remaining reactive metal oxides is therefore an important step in the purification of reactive oxide constituents. Each purification step has quite a high energy requirement at present. Current practice in industry yields sulphate and neutralized chloride waste from titanium dioxide enrichment, red mud from bauxite refining, slag and leach residues from vanadium extraction and chromite ore process residue (COPR) from chromate processes. In this review article, a novel alkali-based oxidative roasting and aqueous leaching for the extraction of mineral oxides is explained in the context of the original work of Le Chatelier in 1850, which was unsuccessful in the industrialization of bauxite processing for alumina extraction. However, much later in the 19th century the alkali-based oxidative mineral roasting was successfully developed for industrial scale manufacturing of chromate chemicals, which yields COPR. The crystal chemistry of mineral oxides, namely alumina, titanium dioxide, and chromium oxide in naturally occurring minerals is briefly reviewed in the context of chemical extraction, which is then developed as a model for developing thermodynamic chemical equilibrium principles for analyzing the physical separation and enrichment of such reactive metal oxides by forming water-soluble and water-insoluble alkali complexes. The involvement of the alkali roasting chemistry of non-magnetic titaniferous mineral waste is also reported in the initial separation of rare-earth oxide mixtures for subsequent separation of individual oxides. The paper concludes with a generic approach to process chemistry which minimizes waste generation and therefore helps in reducing the overall process and energy costs. Examples of recovering alkali from high pH solution using carbon dioxide are also demonstrated

    Anticipated impact of the 2009 Four Corners raid and arrests

    Archaeological looting on United States federal land has been illegal for over a century. Regardless, the activity has continued in the Four Corners region. This paper discusses how the 1979 Archaeological Resources Protection Act (ARPA) can be viewed as sumptuary law, and within a sumptuary context, subversion can be anticipated. An analysis of 1986 and June 2009 federal raids in the Four Corners will exemplify this point by identifying local discourses found in newspapers both before and after each raid, which demonstrate a sumptuary effect. Ultimately, this paper concludes that looting just adapted, rather than halted, after each federal raid and that understanding this social context of continued local justification and validation of illegal digging is a potential asset for cultural resource protection

    Incremental prognostic value of echocardiography of left ventricular remodeling and diastolic function in STICH trial

    Aims: We sought to determine which echocardiographic markers of left ventricular (LV) remodeling and diastolic dysfunction can contribute as incremental and independent prognostic information in addition to current clinical risk markers of ischemic LV systolic dysfunction in the Surgical Treatment for Ischemic Heart Failure (STICH) trial. Methods and results: The cohort consisted of 1511 of 2136 patients in STICH for whom baseline transmitral Doppler (E/A ratio) could be measured by an echocardiographic core laboratory blinded to treatment and outcomes, and prognostic value of echocardiographic variables was determined by a Cox regression model. E/A ratio was the most significant predictor of mortality amongst diastolic variables with lowest mortality for E/A closest 0.8, although mortality was consistently low for E/A 0.6 to 1.0. Mortality increased for E/A < 0.6 and > 1.0 up to approximately 2.3, beyond which there was no further increase in risk. Larger LV end-systolic volume index (LVESVI) and E/A < 0.6 and > 1.0 had incremental negative effects on mortality when added to a clinical multivariable model, where creatinine, LVESVI, age, and E/A ratio accounted for 74% of the prognostic information for predicting risk. LVESVI and E/A ratio were stronger predictors of prognosis than New York Heart Association functional class, anemia, diabetes, history of atrial fibrillation, and stroke. Conclusions: Echocardiographic markers of advanced LV remodeling and diastolic dysfunction added incremental prognostic value to current clinical risk markers. LVESVI and E/A ratio outperformed other markers and should be considered as standard in assessing risks in ischemic heart failure. E/A closest to 0.8 was the most optimal filling pattern
