    Unmet needs for family planning in a municipal area in North Kerala, India

    Background: An important concept to describe effectiveness of family planning programme is unmet need for family planning. Most married women want to limit childbirth and space deliveries but many are unable use contraceptives for varying reasons leading to unmet need. According to national survey in India (NFHS 3), the total unmet need is 13.2% (unmet need for spacing- 6% and limiting- 7%). According to district survey (DLHS 3), in Malappuram district, Kerala, India, the total unmet need was 19.7% (11.4% for spacing and 8.3% for limiting) higher than national unmet need. The objective of this study was to find out unmet need for family planning and its causes and the contraceptive practices among married women of reproductive age group (15-49 years) in Perinthalmanna municipality in northern Kerala, India.Methods: A cross sectional study was conducted in 35 wards of Perinthalmanna municipality between 8 April to 6 June 2015. The sample size was calculated as 725. From each of the 35 wards, 21 houses were selected randomly and married women in the reproductive age group 15-49 were interviewed using a semi structured questionnaire. Data was analyzed using epi- info version 7.Results: Among the 753 married women, the total unmet need was 21.2% (spacing 16.9, limiting 4.3). Among 87 (11.6%) currently pregnant women, 66 wanted to conceive, 20 wanted to delay and one didn’t want at all. Among the 666 non-pregnant women, 69 were considered in fecund. Among 326 (48.94%) contraceptive users 26.9% had undergone sterilization; 47.5% were using copper T; 13.5% used condoms and 11.9% used oral pills. Out of 340 non pregnant women not currently using any contraceptives, 24% did not want to conceive ever; 27% wants to conceive but later and 38.8% were ready to conceive now. Fear of complications, considering it against religious faith, lack of knowledge and partner’s opposition were common reasons for not using contraception.Conclusions: The unmet need for Family Planning is high and in order to reduce the gap, the program should address the above reasons

    Using the Prevalence of Individual Species of Intestinal Nematode Worms to Estimate the Combined Prevalence of Any Species

    Mixed infections with roundworm, whipworm and hookworm are common, but survey reports often give only the separate prevalence of each type. However, the combined prevalence is important to estimate accurately the number of individuals who would benefit from control programmes and to make decisions about the frequency of treatment. Previous work suggests that mixed infections involving hookworm occur randomly, but that roundworm and whipworm infections are found together more frequently than would be expected by chance. We used 63 data sets from community surveys that reported both the proportions infected with individual types of worms and the combined proportion infected with any worm. We then calculated the proportion that would be infected with any type of worm if infections had occurred randomly and compared it with the observed combined proportion infected. We found a strong correlation between the observed and predicted combined proportions infected. A small downward correction of the predicted proportion infected by dividing by a factor of 1.06 brought it to a value that nearly equalled the observed proportion infected almost all the time. This simple model could be applied to published survey data to estimate accurately the number of individuals that would benefit from mass deworming

    Serviceability performance of steel-concrete composite beams

    YesFor composite beams with low degrees of shear connection, additional deflections occur due to slip in the shear connectors, which can be significant for beams with low degrees of shear connection. A design formula is presented for the effective stiffness of composite beams taking account of the stiffness of the shear connectors, which is compared to measured deflections of 6 symmetric beams and an 11m span composite beam of asymmetric profile. It is shown that the comparison is good when using a shear connector stiffness of 70 kN/mm for single shear connectors and 100 kN/mm for pairs of shear connectors per deck rib. Results of push tests on a range of deck profiles confirm these initial elastic stiffnesses. To ensure that the slip at the serviceability limit state does not lead to permanent deformations of the beam, it is proposed that the minimum degree of shear connection should not fall below 30% for un-propped beams and 40% for propped beams of symmetric cross-section.European Commissio

    Adaptive remodeling of the bacterial proteome by specific ribosomal modification regulates Pseudomonas infection and niche colonisation

    Post-transcriptional control of protein abundance is a highly important, underexplored regulatory process by which organisms respond to their environments. Here we describe an important and previously unidentified regulatory pathway involving the ribosomal modification protein RimK, its regulator proteins RimA and RimB, and the widespread bacterial second messenger cyclic-di-GMP (cdG). Disruption of rimK affects motility and surface attachment in pathogenic and commensal Pseudomonas species, with rimK deletion significantly compromising rhizosphere colonisation by the commensal soil bacterium P. fluorescens, and plant infection by the pathogens P. syringae and P. aeruginosa. RimK functions as an ATP-dependent glutamyl ligase, adding glutamate residues to the C-terminus of ribosomal protein RpsF and inducing specific effects on both ribosome protein complement and function. Deletion of rimK in P. fluorescens leads to markedly reduced levels of multiple ribosomal proteins, and also of the key translational regulator Hfq. In turn, reduced Hfq levels induce specific downstream proteomic changes, with significant increases in multiple ABC transporters, stress response proteins and non-ribosomal peptide synthetases seen for both ΔrimK and Δhfq mutants. The activity of RimK is itself controlled by interactions with RimA, RimB and cdG. We propose that control of RimK activity represents a novel regulatory mechanism that dynamically influences interactions between bacteria and their hosts; translating environmental pressures into dynamic ribosomal changes, and consequently to an adaptive remodeling of the bacterial proteome

    Raman study of the anharmonicity in YBa2_2Cu3_3Ox_x

    A systematic Raman study in the visible carried out on the YBa2Cu316,18Ox (x=6-7) compounds, with isotopic substitution of 18O for 16O, has detected a doping dependent deviation from harmonic behavior for the frequency shift of the in-phase mode, a smaller amount of anharmonicity for the apex mode, and almost no effect for the out-of-phase B1g-symmetry phonon. It appears that the amount of anharmonicity depends strongly on the oxygen concentration; it diminishes close to the tetragonal to orthorhombic structural phase transition and close to optimal doping, while it reaches its maximum value for the ortho-II and a tetragonal phase. The almost zero anharmonicity at optimal doping persists even at 77K. The data in the overdoped oxygen concentration, where a softening of the in-phase phonon frequency occurs, indicate that the anharmonicity is not enhanced by the sudden increase in the CuO2 buckling. The results fully agree with recent studies of the ortho-II phase but they do not comply with a static double-well potential of the apical oxygen atom at optimal doping.Comment: Dedicated to Prof. K.     A modified echocardiographic protocol with intrinsic plausibility control to determine intraventricular asynchrony based on TDI and TSI

    <p>Abstract</p> <p>Background</p> <p>Established methods to determine asynchrony suffer from high intra- and interobserver variability and failed to improve patient selection for cardiac resynchronization therapy (CRT). Thus, there is a need for easy and robust approaches to reliably assess cardiac asynchrony.</p> <p>Methods and Results</p> <p>We performed echocardiography in 100 healthy subjects and 33 patients with left bundle branch block (LBBB). To detect intraventricular asynchrony, we combined two established methods, i.e., tissue synchronization imaging (TSI) and tissue Doppler imaging (TDI). The time intervals from the onset of aortic valve opening (AVO) to the peak systolic velocity (S') were measured separately in six basal segments in the apical four-, two-, and three-chamber view. Color-coded TSI served as an intrinsic plausibility control and helped to identify the correct S' measuring point in the TDI curves. Next, we identified the segment with the shortest AVO-S' interval. Since this segment most likely represents vital and intact myocardium it served as a reference for other segments. Segments were considered asynchronous when the delay between the segment in question and the reference segment was above the upper limit of normal delays derived from the control population. Intra- and interobserver variability were 7.0% and 7.7%, respectively.</p> <p>Conclusion</p> <p>Our results suggest that combination of TDI and TSI with intrinsic plausibility control improves intra- and interobserver variability and allows easy and reliable assessment of cardiac asynchrony.</p

    Comparative Analysis of Dengue and Zika Outbreaks Reveals Differences by Setting and Virus.

    The pacific islands of Micronesia have experienced several outbreaks of mosquito-borne diseases over the past decade. In outbreaks on small islands, the susceptible population is usually well defined, and there is no co-circulation of pathogens. Because of this, analysing such outbreaks can be useful for understanding the transmission dynamics of the pathogens involved, and particularly so for yet understudied pathogens such as Zika virus. Here, we compared three outbreaks of dengue and Zika virus in two different island settings in Micronesia, the Yap Main Islands and Fais, using a mathematical model of transmission dynamics and making full use of commonalities in disease and setting between the outbreaks. We found that the estimated reproduction numbers for Zika and dengue were similar when considered in the same setting, but that, conversely, reproduction number for the same disease can vary considerably by setting. On the Yap Main Islands, we estimated a reproduction number of 8.0-16 (95% Credible Interval (CI)) for the dengue outbreak and 4.8-14 (95% CI) for the Zika outbreak, whereas for the dengue outbreak on Fais our estimate was 28-102 (95% CI). We further found that the proportion of cases of Zika reported was smaller (95% CI 1.4%-1.9%) than that of dengue (95% CI: 47%-61%). We confirmed these results in extensive sensitivity analysis. They suggest that models for dengue transmission can be useful for estimating the predicted dynamics of Zika transmission, but care must be taken when extrapolating findings from one setting to another

    Dual Function of the NK Cell Receptor 2B4 (CD244) in the Regulation of HCV-Specific CD8+ T Cells

    The outcome of viral infections is dependent on the function of CD8+ T cells which are tightly regulated by costimulatory molecules. The NK cell receptor 2B4 (CD244) is a transmembrane protein belonging to the Ig superfamily which can also be expressed by CD8+ T cells. The aim of this study was to analyze the role of 2B4 as an additional costimulatory receptor regulating CD8+ T cell function and in particular to investigate its implication for exhaustion of hepatitis C virus (HCV)-specific CD8+ T cells during persistent infection. We demonstrate that (i) 2B4 is expressed on virus-specific CD8+ T cells during acute and chronic hepatitis C, (ii) that 2B4 cross-linking can lead to both inhibition and activation of HCV-specific CD8+ T cell function, depending on expression levels of 2B4 and the intracellular adaptor molecule SAP and (iii) that 2B4 stimulation may counteract enhanced proliferation of HCV-specific CD8+ T cells induced by PD1 blockade. We suggest that 2B4 is another important molecule within the network of costimulatory/inhibitory receptors regulating CD8+ T cell function in acute and chronic hepatitis C and that 2B4 expression levels could also be a marker of CD8+ T cell dysfunction. Understanding in more detail how 2B4 exerts its differential effects could have implications for the development of novel immunotherapies of HCV infection aiming to achieve immune control

    The Temporal Signature of Memories: Identification of a General Mechanism for Dynamic Memory Replay in Humans

    Reinstatement of dynamic memories requires the replay of neural patterns that unfold over time in a similar manner as during perception. However, little is known about the mechanisms that guide such a temporally structured replay in humans, because previous studies used either unsuitable methods or paradigms to address this question. Here, we overcome these limitations by developing a new analysis method to detect the replay of temporal patterns in a paradigm that requires participants to mentally replay short sound or video clips. We show that memory reinstatement is accompanied by a decrease of low-frequency (8 Hz) power, which carries a temporal phase signature of the replayed stimulus. These replay effects were evident in the visual as well as in the auditory domain and were localized to sensory-specific regions. These results suggest low-frequency phase to be a domain-general mechanism that orchestrates dynamic memory replay in humans
