Het doel van deze studie is een vergelijking tussen de effecten van plantaardige of minerale grondstoffen met een Cd-gehalte dicht onder de toegestane norm op de concentratie in de nieren van vleesvarkens. Het Cd-gehalte in bloed geeft de recente blootstelling van de varkens weer, terwijl het Cd-gehalte in de nieren de cumulatieve blootstelling weergeeft.     Five-year follow-up of bilateral stimulation of the subthalamic nucleus in advanced Parkinson's disease

    Background: Although the short-term benefits of bilateral stimulation of the subthalamic nucleus in patients with advanced Parkinson's disease have been well documented, the long-term outcomes of the procedure are unknown. Methods: We conducted a five-year prospective study of the first 49 consecutive patients whom we treated with bilateral stimulation of the subthalamic nucleus. Patients were assessed at one, three, and five years with levodopa (on medication) and without levodopa (off medication), with use of the Unified Parkinson's Disease Rating Scale. Seven patients did not complete the study: three died, and four were lost to follow-up. Results: As compared with base line, the patients' scores at five years for motor function while off medication improved by 54 percent (P<0.001) and those for activities of daily living improved by 49 percent (P<0.001). Speech was the only motor function for which off-medication scores did not improve. The scores for motor function on medication did not improve one year after surgery, except for the dyskinesia scores. On-medication akinesia, speech, postural stability, and freezing of gait worsened between year 1 and year 5 (P<0.001 for all comparisons). At five years, the dose of dopaminergic treatment and the duration and severity of levodopa-induced dyskinesia were reduced, as compared with base line (P<0.001 for each comparison). The average scores for cognitive performance remained unchanged, but dementia developed in three patients after three years. Mean depression scores remained unchanged. Severe adverse events included a large intracerebral hemorrhage in one patient. One patient committed suicide. Conclusions: Patients with advanced Parkinson's disease who were treated with bilateral stimulation of the subthalamic nucleus had marked improvements over five years in motor function while off medication and in dyskinesia while on medication. There was no control group, but worsening of akinesia, speech, postural stability, freezing of gait, and cognitive function between the first and the fifth year is consistent with the natural history of Parkinson's disease

    The mortality associated with erythema nodosum leprosum in Ethiopia: a retrospective hospital-based study.

    BACKGROUND: Erythema nodosum leprosum (ENL) is a debilitating multisystem disorder which complicates leprosy. It is characterised by fever, malaise and painful erythematous cutaneous nodules. ENL is often recurrent or chronic in nature and frequently severe. Patients often require prolonged treatment with high doses of oral corticosteroids. There are no data on the mortality associated with treated ENL. METHODOLOGY: The notes of patients who were admitted, discharged, transferred to another facility or died with a diagnosis of leprosy or a leprosy-related complication for a five year period were reviewed. RESULT/DISCUSSION: 414 individuals were identified from the ward database. 312 (75.4%) patient records were located and reviewed. Ninety-nine individuals had ENL and 145 had a Type 1 reaction. The median age of individuals with ENLwas 25 years. Eight patients with erythema nodosum leprosum died compared with two diagnosed with Type 1 reaction. This difference is statistically significant (p = 0.0168, Fisher's Exact Test). There is a significant mortality and morbidity associated with ENL in this Ethiopian cohort. The adverse outcomes seen are largely attributable to the chronic administration of oral corticosteroids used to control the inflammatory and debilitating symptoms of the condition

    Effects of forcing differences and initial conditions on inter-model agreement in the VolMIP volc-pinatubo-full experiment

    This paper provides initial results from a multi-model ensemble analysis based on the volc-pinatubo-full experiment performed within the Model Intercomparison Project on the climatic response to Volcanic forcing (VolMIP) as part of the sixth phase of the Coupled Model Intercomparison Project (CMIP6). The volc-pinatubo-full experiment is based on an ensemble of volcanic forcing-only climate simulations with the same volcanic aerosol dataset across the participating models (the 1991-1993 Pinatubo period from the CMIP6-GloSSAC dataset). The simulations are conducted within an idealized experimental design where initial states are sampled consistently across models from the CMIP6-piControl simulation providing unperturbed preindustrial background conditions. The multi-model ensemble includes output from an initial set of six participating Earth system models (CanESM5, GISS-E2.1-G, IPSL-CM6A-LR, MIROC-E2SL, MPI-ESM1.2-LR and UKESM1). The results show overall good agreement between the different models on the global and hemispheric scales concerning the surface climate responses, thus demonstrating the overall effectiveness of VolMIP's experimental design. However, small yet significant inter-model discrepancies are found in radiative fluxes, especially in the tropics, that preliminary analyses link with minor differences in forcing implementation; model physics, notably aerosol-radiation interactions; the simulation and sampling of El Niño-Southern Oscillation (ENSO); and, possibly, the simulation of climate feedbacks operating in the tropics. We discuss the volc-pinatubo-full protocol and highlight the advantages of volcanic forcing experiments defined within a carefully designed protocol with respect to emerging modelling approaches based on large ensemble transient simulations. We identify how the VolMIP strategy could be improved in future phases of the initiative to ensure a cleaner sampling protocol with greater focus on the evolving state of ENSO in the pre-eruption period

    Effects of parity order and reproductive management on the efficiency of rabbit productive systems.

    The aim of this work was to study the effect of parity order and reproductive management systems on rabbit production performance. A total of 73 rabbit does (I group) were submitted to a 35-day intensive rhythm [artificial insemination (AI) on day 4 post-partum (pp) and weaning at 25 days of lactation], and 108 rabbit does (SI group) were submitted to a 42-day semi-intensive rhythm (AI on day 11 pp and weaning at 35 days of lactation) during 9 months. Primiparous does had the lowest live body weight at parturition (P < 0.0001) and at 21 days of lactation (P < 0.0001). They also had lower milk production (P < 0.0001) than does with later parities. I group animals needed a higher number of AI than SI group to become pregnant (1.70 ± 0.03 vs. 1.39 ± 0.03; P < 0.0001: especially after the third). Prolificacy was not affected by the management system. Parturition interval (PI) was longer than expected in both groups [56.0 ± 1.4 and 50.9 ± 1.38 days in I and SI groups, respectively (P < 0.05)]. Mean productivity, estimated as number of weaned rabbits per female and year, was 12 kits higher in rabbit does of the SI group (P < 0.05). From the third parturition onward, an increase in live body weight of kits at different ages was observed. At 21 (P < 0.05) and 25 days of age (P < 0.01), kits from the I group rabbit does weighed more than those from the SI group; however, the latter showed a higher weight at 35 (P < 0.05) and 60 days of age (P < 0.05). Rabbit does with two or three parturitions had higher litter size at 21 and 25 days of age (P < 0.0001 and P < 0.001, respectively). Kit mortality between 21 and 25 days of age and between 35 and 60 days of age was not affected by treatments but was higher in the I group between 25 and 35 days (18.2 vs. 5.03% in the I and SI groups, respectively; P < 0.0001) and as age of does increased (P < 0.05). In light of these results, we could conclude that long term doe reproductive performance is negatively affected and litter viability decreased when using intensive compared to a semi-intensive reproductive management

    Pleistocene Tectono-magmato-volcanic events recorded east of Mayotte - insights for the ongoing seismo-volcanic crisis

    The offshore eastern Mayotte area has been extensively studied since the outbreak of the seismo-volcanic crisis in May 2018. Several oceanographic surveys have been carried out by REVOSIMA – https://doi.org/10.18715/MAYOTTE.REVOSIMA (MAYOBS cruises -https://doi.org/10.18142/291) for monitoring purpose, or for academic research (SISMAORE - https://doi.org/10.17600/18001331 and SCRATCH cruises - https://doi.org/10.17600/18002274). They offer a complete coverage of the eastern slope and abyssal plain with multibeam bathymetry and backscatter imagery, sub-bottom profiler (SBP), and several sediment cores. This set of data offers the opportunity to describe the morphology of the area and details the first tens of meters of the sedimentary succession on the abyssal plain.We discover several features including faults, domes and massive chaotic deposits that developed or occurred during Quaternary in the vicinity of the new Fani Maore Volcano. Domes are interpreted as forced folds related to the intrusion of a large sill at depth. The main fault crossing the largest forced fold is a succession of en-echelon segments with vertical throws of up to 8 meters and a preferential N130° strike compatible with the present day regional dextral context. Analysis of the SBP profiles reveals that faulting and doming associated to sill intrusion occurred simultaneously, and together with the deposition of two massive (several km3) and chaotic lobes at the foot of the Mayotte slope. Sediment cores collected over or close to the massive deposits indicate a mixed bioclastic-volcanoclastic content with a large amount of pumices, whose aspect and chemical composition are identical to volcanic edifices located on the upper slope (Horseshoe Volcano) or onshore (Petite-Terre). Backscatter imagery also reveals streaks that cover the lobes and trace back to the same upper slope area. Analysis of bioclasts from a core catcher stopped in the upper part of the deposit shows a large variety of foraminifers, shells fragments, with a little proportion originating from the upper-slope and shelf. We propose that these massive lobe deposits might have occurred as the pumice-dominated material ejected at HV or PT flows downslope, thus reworking and incorporating a substantial amount of hemipelagic deposits along the slope and over the abyssal plain. These observations suggests that a tectono-magmato-volcanic event occurred during recent geological time (Late Pleistocene according to early estimations). At least one comparable set of similar and synchronous features appears on our dataset thus implying the occurrence of a similar event, earlier in the Pleistocene. The correlations between these events and the activity at HV and PT volcanic centers are critical, as it would provide a recurring scenario to compare with present day seismo-volcanic crisis at Mayotte.This work is funded by REVOSIMA, ANR funded COYOTES project, and internal BRGM Project PDEV MAYOTTE.COmores & maYotte : vOlcanisme, TEctonique et Sismicit
