    Falla cardíaca avanzada en pacientes tratados con dispositivos electrofisiológicos en una unidad de electrofisología, Pereira (Colombia)

    Introduction: advanced heart failure it's a disease with a high prevalence in the world wide,with an increasing mortality and hospitalization rates, due to his severity and clinical profile. Which costs to the health system is up to 20% of the resources devoted to the management of cardiovascular disease in North Amerca. A cross-sectional study was made with 70 patients on pharmacological treatment and Cardiac Resynchronization Therapy with or without Implantable Cardioverter (CDI) or Implantable Cardioverter (CDI) among 2010 and 2012.Methods: a cross-sectional study.Results: we found 71% with hypertension, 65,75% ischemic cardiopathy , 58.3% with resynchronizator with or without CDI, 90% had two or more drugs for heart failure, 77% had beta blocker, 90% ACE inhibitor or ARB, 85.7% diuretics.Conclusion: patients with advanced heart failure who underwent electrophysiological therapy, were clinically characterized in Pereira, Colombia.Resumen: la falla cardíaca avanzada es una enfermedad prevalente, con una tasa alta de mortalidad y hospitalización con relación a su estadio y perfil clínico, que cuesta en Norteamérica hasta el 20% de los recursos destinados para el manejo de la enfermedad cardiovascular. Se realizó un análisis transversal a una cohorte de 70 pacientes en tratamiento farmacológico y cardioresincronizador con o sin cardiodesfibirlador, o solo cardiodesfibrilador automático (ICD) implantado entre los años 2010 y 2012.Métodos: el presente es un estudio transversal.Resultados: 71% presentaron hipertensión arterial HTA; 65,75% cardiopatía isquémica, siendo el 58.3% portadores de resincronizador con o sin cardiodesfibrilador. El 90% de los pacientes tiene 2 o más medicamentos, 77% betabloqueador, 90% iECA o ARA II, 85.7% algún diurético.Conclusión: se caracterizaron los pacientes que recibieron terapia electrofisiológica para manejo avanzado de falla cardíaca.ardíaca

    Cistouretropexia laparoscópica tipo Burch para el tratamiento de la incontinencia urinaria de esfuerzos: seguimiento a largo plazo

    Objetivos: Conocer las complicaciones, las tasas de cura subjetiva y valoración con el índice de severidad de Sandvik a largo plazo de pacientes en quienes se realizó cistouretropexia laparoscópica tipo Burch para la corrección de su incontinencia urinaria de esfuerzo (IUE). Método: Análisis descriptivo, retrospectivo. Nivel de evidencia III. Se incluyeron las pacientes con diagnóstico de IUE tipo Blaivas IIB, a quienes se les practicó Burch laparoscópico en la Clínica del Prado y Profamilia Medellín, entre los años 2003 y 2007. Las variables principales a evaluar fueron edad, tiempo quirúrgico, cura subjetiva, índice de severidad de Sandvik y complicaciones. Resultados: Se analizaron 72 pacientes con promedio de edad de 46,57 ± 9,54 años. El tiempo promedio de seguimiento fue de 24,4 meses (rango: 9,0-55,8 meses). El tiempo quirúrgico promedio fue de 79,83 minutos (rango: 44-160 minutos). Refirieron cura subjetiva 56 pacientes (77,8%). El índice de severidad de Sandvik muestra que a largo plazo 56,9% pacientes estaban secas con remisión completa de los síntomas y el 13,8% pacientes tenían pérdidas leves. Conclusiones: El Burch laparoscópico es una opción efectiva para el manejo de la incontinencia urinaria de esfuerzo tipo Blaivas IIB. Provee tasas de curación subjetiva a largo plazo similares a otros tipos de tratamiento. Es una opción más a tener en cuenta en el manejo de la IUE, en especial aquellas pacientes con patologías asociadas que se beneficien del abordaje por laparoscopia

    Opposite cannabis-cognition associations in psychotic patients depending on family history

    The objective of this study is to investigate cognitive performance in a first-episode psychosis sample, when stratifying the interaction by cannabis use and familial or non-familial psychosis. Hierarchical-regression models were used to analyse this association in a sample of 268 first-episode psychosis patients and 237 controls. We found that cannabis use was associated with worse working memory, regardless of family history. However, cannabis use was clearly associated with worse cognitive performance in patients with no family history of psychosis, in cognitive domains including verbal memory, executive function and global cognitive index, whereas cannabis users with a family history of psychosis performed better in these domains. The main finding of the study is that there is an interaction between cannabis use and a family history of psychosis in the areas of verbal memory, executive function and global cognition: that is, cannabis use is associated with a better performance in patients with a family history of psychosis and a worse performance in those with no family history of psychosis. In order to confirm this hypothesis, future research should explore the actual expression of the endocannabinoid system in patients with and without a family history of psychosis

    The interplay between functioning problems and symptoms in first episode of psychosis: an approach from network analysis

    The relationship between psychotic symptoms and global measures of functioning has been widely studied. No previous study has assessed so far the interplay between specific clinical symptoms and particular areas of functioning in first-episode psychosis (FEP) using network analysis methods. A total of 191 patients with FEP (age 24.45 ± 6.28 years, 64.9% male) participating in an observational and longitudinal study (AGES-CM) comprised the study sample. Functioning problems were assessed with the WHO Disability Assessment Schedule (WHODAS), whereas the Positive and Negative Syndrome Scale (PANSS) was used to assess symptom severity. Network analysis were conducted with the aim of analysing the patterns of relationships between the different dimensions of functioning and PANSS symptoms and factors at baseline. According to our results, the most important nodes were “conceptual disorganization”, “emotional withdrawal”, “lack of spontaneity and flow of conversation”, “delusions”, “unusual thought content”, “dealing with strangers” and “poor rapport”. Our findings suggest that these symptoms and functioning dimensions should be prioritized in the clinical assessment and management of patients with FEP.     Rey–Osterrieth Complex Figure – copy and immediate recall (3 minutes): Normative data for Spanish-speaking pediatric populations

    OBJECTIVE: To generate normative data for the Rey–Osterrieth Complex Figure (ROCF) in Spanish-speaking pediatric populations. METHOD: The sample consisted of 4,373 healthy children from nine countries in Latin America (Chile, Cuba, Ecuador, Guatemala, Honduras, Mexico, Paraguay, Peru, and Puerto Rico) and Spain. Each participant was administered the ROCF as part of a larger neuropsychological battery. The ROCF copy and immediate recall (3 minutes) scores were normed using multiple linear regressions and standard deviations of residual values. Age, age2, sex, and mean level of parental education (MLPE) were included as predictors in the analyses. RESULTS: The final multiple linear regression models showed main effect for age on copy and immediate recall scores, such that scores increased linearly as a function of age. Age2 affected ROCF copy score for all countries, except Puerto Rico; and ROCF immediate recall scores for all countries, except Chile, Guatemala, Honduras, Paraguay, and Puerto Rico. Models indicated that children whose parent(s) had a MLPE >12 years obtained higher scores compared to children whose parent(s) had a MLPE≤12 years for Chile, Puerto Rico, and Spain in the ROCF copy, and Paraguay and Spain for the ROCF immediate recall. Sex affected ROCF copy and immediate recall score for Chile and Puerto Rico with girls scoring higher than boys. CONCLUSIONS: This is the largest Spanish-speaking pediatric normative study in the world, and it will allow neuropsychologists from these countries to have a more accurate approach to interpret the ROCF Test in pediatric populations
