Factors affecting 30-day mortality in poor-grade aneurysmal subarachnoid hemorrhage: a 10-year single-center experience
Background: The management of patients with poor-grade aneurysmal
subarachnoid hemorrhage (aSAH) is burdened by an unfavorable prognosis
even with aggressive treatment. The aim of the present study is to investigate
the risk factors affecting 30-day mortality in poor-grade aSAH patients.
Methods: We performed a retrospective analysis of a prospectively collected
database of poor-grade aSAH patients (World Federation of Neurosurgical
Societies, WFNS, grades IV and V) treated at our institution from December 2010
to December 2020. For all variables, percentages of frequency distributions
were analyzed. Contingency tables (Chi-squared test) were used to assess the
association between categorical variables and outcomes in the univariable
analysis. Multivariable analysis was performed by using the multiple logistic
regression method to estimate the odds ratio (OR) for 30-day mortality.
Results: A total of 149 patients were included of which 32% had WFNS grade
4 and 68% had WFNS grade 5. The overall 1-month mortality rate was 21%. On
univariable analysis, five variables were found to be associated with the likelihood
of death, including intraventricular hemorrhage (IVH ≥ 50 mL, p = 0.005),
the total amount of intraventricular and intraparenchymal hemorrhage
(IVH + ICH ≥ 90 mL, p = 0.019), the IVH Ratio (IVH Ratio ≥ 40%, p = 0.003),
posterior circulation aneurysms (p = 0.019), presence of spot sign on initial CT
scan angiography (p = 0.015).
Nonetheless, when the multivariable analysis was performed, only IVH Ratio
(p = 0.005; OR 3.97), posterior circulation aneurysms (p = 0.008; OR 4.05) and
spot sign (p = 0.022; OR 6.87) turned out to be independent predictors of 30-
day mortality.
Conclusion: The risk of mortality in poor-grade aSAH remains considerable
despite maximal treatment. Notwithstanding the limitations of a retrospective
study, our report highlights some neuroradiological features that in the
emergency setting, combined with leading clinical and anamnestic parameters,
may support the multidisciplinary team in the difficult decision-making process and communication with family members from the earliest stages of poor-grade aSAH. Further prospective studies are warranted
A chemical survey of exoplanets with ARIEL
Thousands of exoplanets have now been discovered with a huge range of masses, sizes and orbits: from rocky Earth-like planets to large gas giants grazing the surface of their host star. However, the essential nature of these exoplanets remains largely mysterious: there is no known, discernible pattern linking the presence, size, or orbital parameters of a planet to the nature of its parent star. We have little idea whether the chemistry of a planet is linked to its formation environment, or whether the type of host star drives the physics and chemistry of the planet’s birth, and evolution. ARIEL was conceived to observe a large number (~1000) of transiting planets for statistical understanding, including gas giants, Neptunes, super-Earths and Earth-size planets around a range of host star types using transit spectroscopy in the 1.25–7.8 μm spectral range and multiple narrow-band photometry in the optical. ARIEL will focus on warm and hot planets to take advantage of their well-mixed atmospheres which should show minimal condensation and sequestration of high-Z materials compared to their colder Solar System siblings. Said warm and hot atmospheres are expected to be more representative of the planetary bulk composition. Observations of these warm/hot exoplanets, and in particular of their elemental composition (especially C, O, N, S, Si), will allow the understanding of the early stages of planetary and atmospheric formation during the nebular phase and the following few million years. ARIEL will thus provide a representative picture of the chemical nature of the exoplanets and relate this directly to the type and chemical environment of the host star. ARIEL is designed as a dedicated survey mission for combined-light spectroscopy, capable of observing a large and well-defined planet sample within its 4-year mission lifetime. Transit, eclipse and phase-curve spectroscopy methods, whereby the signal from the star and planet are differentiated using knowledge of the planetary ephemerides, allow us to measure atmospheric signals from the planet at levels of 10–100 part per million (ppm) relative to the star and, given the bright nature of targets, also allows more sophisticated techniques, such as eclipse mapping, to give a deeper insight into the nature of the atmosphere. These types of observations require a stable payload and satellite platform with broad, instantaneous wavelength coverage to detect many molecular species, probe the thermal structure, identify clouds and monitor the stellar activity. The wavelength range proposed covers all the expected major atmospheric gases from e.g. H2O, CO2, CH4 NH3, HCN, H2S through to the more exotic metallic compounds, such as TiO, VO, and condensed species. Simulations of ARIEL performance in conducting exoplanet surveys have been performed – using conservative estimates of mission performance and a full model of all significant noise sources in the measurement – using a list of potential ARIEL targets that incorporates the latest available exoplanet statistics. The conclusion at the end of the Phase A study, is that ARIEL – in line with the stated mission objectives – will be able to observe about 1000 exoplanets depending on the details of the adopted survey strategy, thus confirming the feasibility of the main science objectives.Peer reviewedFinal Published versio

Ariel, the Atmospheric Remote-sensing Infrared Exoplanet Large-survey, was adopted as the fourth medium-class mission in ESA's Cosmic Vision programme to be launched in 2029. During its 4-year mission, Ariel will study what exoplanets are made of, how they formed and how they evolve, by surveying a diverse sample of about 1000 extrasolar planets, simultaneously in visible and infrared wavelengths. It is the first mission dedicated to measuring the chemical composition and thermal structures of hundreds of transiting exoplanets, enabling planetary science far beyond the boundaries of the Solar System. The payload consists of an off-axis Cassegrain telescope (primary mirror 1100 mm x 730 mm ellipse) and two separate instruments (FGS and AIRS) covering simultaneously 0.5-7.8 micron spectral range. The satellite is best placed into an L2 orbit to maximise the thermal stability and the field of regard. The payload module is passively cooled via a series of V-Groove radiators; the detectors for the AIRS are the only items that require active cooling via an active Ne JT cooler. The Ariel payload is developed by a consortium of more than 50 institutes from 16 ESA countries, which include the UK, France, Italy, Belgium, Poland, Spain, Austria, Denmark, Ireland, Portugal, Czech Republic, Hungary, the Netherlands, Sweden, Norway, Estonia, and a NASA contribution
Ground calibration of the Ariel space telescope: optical ground support equipment design and description
This paper describes the Optical Ground Support Equipment (OGSE) that is being developed for the payload level testing of the Ariel Space Telescope. Ariel has been adopted as ESA’s “M4” mission in its Cosmic Visions Programme and will launch in 2029 to the second Earth-Sun Lagrange point. During four years of operation the Ariel payload (PL – the cryogenic payload module plus warm units) will perform precise transit spectroscopy of approximately 1000 known exoplanetary atmospheres using a 1.1 m × 0.7 m telescope coupled to two instruments: the Fine Guidance Sensor (FGS) and the Ariel Infrared Spectrometer (AIRS). These instruments provide three spectrometric channels that cover 1.0 to 7.8 μm wavelength range and three photometric channels between 0.5 and 1.1 μm. The Ariel OGSE will verify the optical and radiometric performance of the integrated Ariel PL under vacuum and cryogenic (<40 K) test conditions within the limitations of operation under Earth’s gravity and vibration environments. To achieve these verification requirements the OGSE is integrated with the main Ariel ground test 5 m thermal vacuum chamber. The test chamber contains a cryogenic enclosure (the Cryogenic Test Rig) that surrounds the PL and the OGSE itself comprises of four subsystems. (1) A cryogenic vacuum chamber and integrating sphere illumination module that is fed by visible, near infrared and thermal infrared sources. The illumination module is mounted external to the Ariel test chamber and coupled via a vacuum feedthrough that relays a 22 mm diameter test beam into the Cryogenic Test Rig. The test beam is then relayed using (2) an injection module that steers the beam to maintain alignment during cool-down and scan the Ariel telescope field of view. The beam is then expanded to partially illuminate the Ariel telescope primary mirror using an (3) ~0.3 m diameter target projector collimating mirror. The final optical component of the OGSE is a (4) beam expander placed on the Ariel common optical bench to compensate for the sub-aperture illumination of the primary and to ensure that the spectrometer modules provide illumination with correct cone angles during ground testing. It is planned to use the OGSE in 2026 for a full range of calibration and verification tests of the end-to-end telescope and instrument performance, including detectors, field of view and alignment. These tests will then ensure that Ariel meets it challenging photometric and spectral performance requirements

Drug Prescription and Delirium in Older Inpatients: Results From the Nationwide Multicenter Italian Delirium Day 2015-2016
reserved1229noObjective: This study aimed to evaluate the association between polypharmacy and delirium, the association of specific drug categories with delirium, and the differences in drug-delirium association between medical and surgical units and according to dementia diagnosis. Methods: Data were collected during 2 waves of Delirium Day, a multicenter delirium prevalence study including patients (aged 65 years or older) admitted to acute and long-term care wards in Italy (2015-2016); in this study, only patients enrolled in acute hospital wards were selected (n = 4,133). Delirium was assessed according to score on the 4 "A's" Test. Prescriptions were classified by main drug categories; polypharmacy was defined as a prescription of drugs from 5 or more classes. Results: Of 4,133 participants, 969 (23.4%) had delirium. The general prevalence of polypharmacy was higher in patients with delirium (67.6% vs 63.0%, P =.009) but varied according to clinical settings. After adjustment for confounders, polypharmacy was associated with delirium only in patients admitted to surgical units (OR = 2.9; 95% CI, 1.4-6.1). Insulin, antibiotics, antiepileptics, antipsychotics, and atypical antidepressants were associated with delirium, whereas statins and angiotensin receptor blockers exhibited an inverse association. A stronger association was seen between typical and atypical antipsychotics and delirium in subjects free from dementia compared to individuals with dementia (typical: OR = 4.31; 95% CI, 2.94-6.31 without dementia vs OR = 1.64; 95% CI, 1.19-2.26 with dementia; atypical: OR = 5.32; 95% CI, 3.44-8.22 without dementia vs OR = 1.74; 95% CI, 1.26-2.40 with dementia). The absence of antipsychotics among the prescribed drugs was inversely associated with delirium in the whole sample and in both of the hospital settings, but only in patients without dementia. Conclusions: Polypharmacy is significantly associated with delirium only in surgical units, raising the issue of the relevance of medication review in different clinical settings. Drug Prescription and Delirium in Older Inpatients: Results From the Nationwide Multicenter Italian Delirium Day 2015-2016
Objective: This study aimed to evaluate the association between polypharmacy and delirium, the association of specific drug categories with delirium, and the differences in drug-delirium association between medical and surgical units and according to dementia diagnosis. Methods: Data were collected during 2 waves of Delirium Day, a multicenter delirium prevalence study including patients (aged 65 years or older) admitted to acute and long-term care wards in Italy (2015-2016); in this study, only patients enrolled in acute hospital wards were selected (n = 4,133). Delirium was assessed according to score on the 4 "A's" Test. Prescriptions were classified by main drug categories; polypharmacy was defined as a prescription of drugs from 5 or more classes. Results: Of 4,133 participants, 969 (23.4%) had delirium. The general prevalence of polypharmacy was higher in patients with delirium (67.6% vs 63.0%, P =.009) but varied according to clinical settings. After adjustment for confounders, polypharmacy was associated with delirium only in patients admitted to surgical units (OR = 2.9; 95% CI, 1.4-6.1). Insulin, antibiotics, antiepileptics, antipsychotics, and atypical antidepressants were associated with delirium, whereas statins and angiotensin receptor blockers exhibited an inverse association. A stronger association was seen between typical and atypical antipsychotics and delirium in subjects free from dementia compared to individuals with dementia (typical: OR = 4.31; 95% CI, 2.94-6.31 without dementia vs OR = 1.64; 95% CI, 1.19-2.26 with dementia; atypical: OR = 5.32; 95% CI, 3.44-8.22 without dementia vs OR = 1.74; 95% CI, 1.26-2.40 with dementia). The absence of antipsychotics among the prescribed drugs was inversely associated with delirium in the whole sample and in both of the hospital settings, but only in patients without dementia. Conclusions: Polypharmacy is significantly associated with delirium only in surgical units, raising the issue of the relevance of medication review in different clinical settings. Specific drug classes are associated with delirium depending on the clinical setting and dementia diagnosis, suggesting the need to further explore this relationship
