Study on human chondrocyte culture viability for autologous transplantation in clinical application
Objective: The limited regenerative capacity of the cartilage tissuemakes the treatment of chondral lesions difficult. The techniquescurrently available to treat cartilage lesions may relieve symptoms,but do not regenerate the injured tissue. Autologous chondrocytetransplantation uses cell biology and cell culture techniques toregenerate the hyaline cartilage. Methods: In this study, we analyzechondrocyte biopsy collection and culture for autologoustransplantation. Ultrastructural analyses of hyaline cartilage biopsieswere performed 0, 6, 24 and 48 hours after collection. The tissue evenafter 48 hours. Eleven cell culture assays were performed to evaluateisolation, viability, morphology, proliferation and absence ofcontaminants. Results: The cell culture techniques used allowedchondrocyte proliferation. Rates on cell viability were maintained abovethe acceptable patterns (above 90). Control of cell culture laboratoryconditions showed absence of contaminants, assuring safety of theprocess. The chondrocytes obtained presented the morphology typicalof cultured cell monolayers. Conclusion: The results indicate viabilityof chondrocyte culture technique for clinical application in autologoustransplantation
Synovial giant cell tumor of the knee
Tumor gigantocelular sinovial é uma neoplasia benigna, raramente sendo relatada na forma de metástase maligna. A localização mais comum de ocorrer um tumor gigantocelular sinovial é na mão e as mais infrequentes são tornozelo e joelho. No presente estudo os autores têm como objetivo descrever um caso raro de tumor gigantocelular sinovial localizado no joelho e o tratamento escolhido. A artroscopia demonstrou, nesse caso, ser o método ideal para o tratamento da lesão, uma vez que permitiu abordagem pouco agressiva e, ao mesmo tempo, boa visualização de todos os compartimentos da articulação do joelho e a completa ressecção do tumor.Synovial giant cell tumor is a benign neoplasm, rarely reported in the form of malignant metastasis. Synovial giant cell tumor most frequently occurs on the hand, and, most uncommon, on the ankle and knee. In the present study, the authors describe a rare case of synovial giant cell tumor on the knee as well as the treatment approach. Arthroscopy has been shown, in this case, to be the optimal method for treating this kind of lesion, once it allowed a less aggressive approach, while providing good visualization of all compartments of knee joint and full tumor resection

Prevalence of Pain on Palpation of the Inferior Pole of the Patella Among Patients with Complaints of knee Pain
CONTEXT AND OBJECTIVE: Patellar tendinopathy is a common condition in sports. It may occur at any location of the patellar tendon, but the most commonly affected area is the inferior pole of the patella. Among various diagnostic tests, the one most used is palpation of the inferior pole of the patella. The aim of this study was to investigate the prevalence of pain complaints among individuals with pathological knee conditions and to evaluate palpation of the inferior pole of the patella as a diagnostic test for patellar tendinopathy. METHODS: Palpation of the patellar tendon was performed on 318 individuals who presented with knee-related complaints. Palpation was performed with the individual in the supine position and the knee extended. The age, gender, physical activity and labor activity of each individual were recorded at the time the symptoms appeared; the diagnosis was also recorded. RESULTS: Of the total number of individuals evaluated, 124 (39%) felt pain on palpation of the inferior pole of the patella. Of these, only 40 (32.3%) received a diagnosis of patellar tendinopathy. We did not observe any difference with respect to gender and age distribution. When evaluating daily physical activity levels, however, we observed that individuals with pain on palpation of the inferior pole of the patella experienced more intense physical activity. CONCLUSIONS: Palpation of the inferior pole of the patella is a diagnostic procedure with high sensitivity and moderate specificity for diagnosing patellar tendinopathy, especially among individuals who perform activities with high functional demands
Early osteoarthritis and reduced quality of life after retirement in former professional soccer players
OBJECTIVES: This study aims to compare the prevalence of osteoarthritis in two groups: one comprising former professional soccer players and the other comprising non-professional-athlete participants. METHODS: Twenty-seven male former professional soccer players and 30 male volunteers from different non-sports professional areas participated in the study. All participants underwent bilateral knee radiography and magnetic resonance imaging. In addition, the quality of life, knee pain and joint function were evaluated and compared using questionnaires given to all participants in both groups. Specific knee evaluations, with regard to osteoarthritis and quality of life, were performed in both groups using the Knee Injury and Osteoarthritis Outcome Score subjective questionnaires and the Short-form 36. The chi-squared test, Fisher's exact test, the Mann-Whitney U test and Student's t-test were used for group comparisons. RESULTS: The between-groups comparison revealed significant differences in the following: pain, symptoms and quality of life related to the knee in the Knee Injury and Osteoarthritis Outcome Score subscales; the physical aspects subscale of the SF-36; total whole-organ magnetic resonance imaging scores with regard to the dominant and non-dominant knees. Former soccer players had worse scores than the controls in all comparisons. CONCLUSIONS: Both the clinical and magnetic resonance evaluations and the group comparisons performed in this study revealed that former soccer players have a worse quality of life than that of a control group with regard to physical aspects related to the knee; these aspects include greater pain, increased symptoms and substantial changes in radiographic and magnetic resonance images of the knee
Por que o paciente submetido a reconstrucao do Ligamento Cruzado Anterior no Brasil permanece internado por um periodo superior a outros paises? Avaliacao Prospectiva de 30 pacientes e apresentacao de possiveis criterios de alta hospitalar
OBJECTIVE:Evaluate a better moment by the medical team and patient to be discharged and relate to possible medical discharge criteria.METHODS:31 anterior cruciate ligament reconstructed patients under similar conditions prospectively evaluated about the possibility of discharge with 24 and 48 hours after surgery and possibles discharges criteria such as pain, range of motion and capacity quadriceps contraction, besides the use of a validated scale to measure the patient's functional independence.RESULTS:50% and 6.4% of patients prefer remain hospitalized after 24 and 48 hours of surgery, respectively. The average of the visual analogue scale of pain was 2.63 and 1.76 points, and the range of motion of 79º and 86,7º after 24 and 48 hours, respectively. 100% of patients were able to quadriceps contraction in every evaluated moments.CONCLUSION:In Brazil, possible discharged criteria as pain, range of motion, quad contraction and motor independence motor function scale show that anterior cruciate reconstruction reconstructed patients could be discharged after 24 hours of surgery. However, 50% of patients still prefer to remain hospitalized for longer periods.OBJETIVO:Avaliar o momento considerado ideal pela equipe médica e pelo paciente para receber alta e relacioná-lo com possÃveis critérios de alta médica.MÉTODOS:Foram submetidos à reconstrução do ligamento cruzado anterior sob condições semelhantes 31 pacientes avaliados prospectivamente sobre a possibilidade de alta médica com 24 e 48 horas de cirurgia e possÃveis critérios de alta, como dor, arco de movimento e capacidade de contração do quadrÃceps, além do uso de uma escala validada para medir a independência funcional motora do paciente.RESULTADOS:A permanência hospitalar após 24 horas de cirurgia é preferida por 50% dos pacientes, enquanto que 6,3% preferem permanecer por mais de 48 horas após a cirurgia. A média do valor da escala visual analógica de dor foi de 2,63 e 1,76 pontos; e o arco de movimento de 79º e 86,7º após 24 e 48 horas, respectivamente. Todos os pacientes foram capazes de contrair o quadrÃceps em todos os momentos avaliados. CONCLUSÃO:No Brasil, critérios possÃveis de alta, como avaliação da dor, do arco de movimento, do controle do quadrÃceps e da independência funcional motora, mostram que seria possÃvel o paciente submetido à reconstrução artroscópica do LCA receber alta com 24 horas. Synthesis of 3-substituted 2-cyclohexenones through umpoled functionalization
El trabajo se refiere a una metodologÃa sobre un estudio de reactividad invertidaA new protocol to obtain 3-substituted 2-cyclohexenones, was developed by reversing the chemical reactivity of 2-cyclohexenone. One-pot synthesis of 3-substituted 2-cyclohexenones can be achieved by treatment of 3-phenylthiosilyl enol ether with a mixture of t-BuLi/HMPA that allows hydrogen-selective exchange in presence of reactive electrophiles such as aldehydes, ketones and alkyl halides. This affords the corresponding product in moderate overall yield, after silyl enol ether cleavage and concomitant thiophenol elimination initiated with TBAF.Conacy
The RMS Survey: Resolving kinematic distance ambiguities towards a sample of compact HII regions using HI absorption
We present high-resolution HI data obtained using the Australia Telescope
Compact Array to resolve the near/far distance ambiguities towards a sample of
compact HII regions from the Red MSX Source (RMS) survey. The high resolution
data are complemented with lower resolution archival HI data extracted from the
Southern and VLA Galactic Plane surveys. We resolve the distance ambiguity for
nearly all of the 105 sources where the continuum was strong enough to allow
analysis of the HI absorption line structure. This represents another step in
the determination of distances to the total RMS sample, which with over 1,000
massive young stellar objects and compact HII regions, is the largest and most
complete sample of its kind. The full sample will allow the distribution of
