    Exploring current practices in pediatric ARV rollout and integration with early childhood programs in South Africa: A rapid situation analysis

    This Horizons program report describes the status of pediatric HIV treatment in selected sites in South Africa, identifies gaps in service delivery, and proposes recommendations for strengthening services and expanding children’s access to treatment. The study provides much needed information on critical issues of pediatric HIV care, especially regarding health service and contextual issues surrounding the expansion of access to treatment for HIV-infected children, and key factors that facilitate sustainability of treatment by young children. The aims of the study were to identify successful program strategies in pediatric HIV treatment in South Africa and to determine priority knowledge gaps to be addressed by operations research. The demonstrated model of collaboration between government and NGOs, though not an easy relationship to manage, can be very successful at scaling up and ensuring sustainability

    Expanding pediatric access to antiretroviral therapy in South Africa

    In sub-Saharan Africa, AIDS has become one of the leading causes of death among children under the age of five years. Yet, despite increased availability of antiretroviral therapy (ART), children have been largely ignored or excluded from treatment initiatives. While efforts to get more children on treatment are increasing, important information is lacking to guide program and policy implementation. To address these gaps, the Horizons Program and the University of Cape Town conducted a rapid situational analysis in 2005 of pediatric HIV treatment sites in South Africa. In 2003, the South African government approved a plan for a national HIV treatment program with the goal of at least one service delivery point in each district providing treatment. The government guidelines emphasized providing treatment for both adults and children and the initial effort resulted in a significant number of children initiating treatment. This research summary details what is happening on the ground to understand how children have been affected by the ART rollout and what can be done to reach the thousands more that should be on treatment

    Introduction: Exploring the Comparative in Socio-Legal Studies

    Among the diverse approaches to comparison in socio-legal studies those that employ qualitative research, richness of detail, and attention to context are the focus of this special issue. The Introduction draws on comparative law and social science literature to argue that comparison amongst studies of laws in contexts can follow different trajectories: the comparison may start from an assumption of similarity—in form, purposes, or context—in order to identify significant differences; or it may identify significant similarity across social and cultural divides. What unites many of the projects of comparison undertaken by qualitative empirical researchers is that the points of relevant comparison are identified within the complexity of the empirical studies at hand; and they are allowed to emerge, or change, as the researcher comes to understand the facts and issues more deeply

    Anti-PD-1 immunotherapy leads to tuberculosis reactivation via dysregulation of TNF-alpha

    Previously, we developed a 3-dimensional cell culture model of human tuberculosis (TB) and demonstrated its potential to interrogate the host-pathogen interaction (Tezera et al., 2017a). Here, we use the model to investigate mechanisms whereby immune checkpoint therapy for cancer paradoxically activates TB infection. In patients, PD-1 is expressed in Mycobacterium tuberculosis (Mtb)-infected lung tissue but is absent in areas of immunopathology. In the microsphere model, PD-1 ligands are up-regulated by infection, and the PD-1/PD-L1 axis is further induced by hypoxia. Inhibition of PD-1 signalling increases Mtb growth, and augments cytokine secretion. TNF-a is responsible for accelerated Mtb growth, and TNF-a neutralisation reverses augmented Mtb growth caused by anti-PD-1 treatment. In human TB, pulmonary TNF-a immunoreactivity is increased and circulating PD-1 expression negatively correlates with sputum TNF-a concentrations. Together, our findings demonstrate that PD-1 regulates the immune response in TB, and inhibition of PD-1 accelerates Mtb growth via excessive TNF-a secretion.</p

    Belongingness is a Mediating Factor Between Religious Service Attendance and Reduced Psychological Distress During the COVID-19 Pandemic

    This study aimed to test whether pre-pandemic religious service attendance relates to both lesser impact from the COVID-19 pandemic and lower levels of psychological distress among a sample of 645 American adults across nine US regions. A second aim was to test whether belongingness mediated these relationships. First, it was expected that more frequent pre-pandemic religious service attendance relates to belongingness, which mediates the religious service attendance and psychological distress association. Second, it was expected that people who felt greater belongingness also experienced less perceived impact from the pandemic. Results from a path model supported these hypotheses. This is among the first studies to provide empirical evidence for religion’s association with psychological distress during the COVID-19 pandemic

    Analysis Of Candidate Genes That Suppress Chromosome Loss In Saccharomyces Cerevisiae Mutants With Defects In Chromosome Transmission

    Mentor: Heidi SleisterErrors in the eukaryotic cell cycle and processes that maintain genome stability can result in abnormal chromosome number and structure (e.g., in humans- Down Syndrome, cancer). The common baker’s yeast S. cerevisiae is a great model system for the analysis of chromosome transmission as yeast artificial chromosomes (YACs) can be lost or rearranged without harming the cell. In an effort to identify proteins important for chromosome transmission, yeast mutants that display increased loss of a YAC during cell division were generated. Suppressor analysis resulted in the isolation of both single and high copy yeast genomic plasmids that improve the chromosome loss defect in these mutants. Subsequent analysis revealed multiple candidate genes present within the suppressor plasmids.
