Challenging Social Cognition Models of Adherence:Cycles of Discourse, Historical Bodies, and Interactional Order
Attempts to model individual beliefs as a means of predicting how people follow clinical advice have dominated adherence research, but with limited success. In this article, we challenge assumptions underlying this individualistic philosophy and propose an alternative formulation of context and its relationship with individual actions related to illness. Borrowing from Scollon and Scollon’s three elements of social action – “historical body,” “interaction order,” and “discourses in place” – we construct an alternative set of research methods and demonstrate their application with an example of a person talking about asthma management. We argue that talk- or illness-related behavior, both viewed as forms of social action, manifest themselves as an intersection of cycles of discourse, shifting as individuals move through these cycles across time and space. We finish by discussing how these dynamics of social action can be studied and how clinicians might use this understanding when negotiating treatment with patients

Optical Detection of the Hidden Nuclear Engine in NGC 4258
The subparsec masing disk recently found to be orbiting a central mass of ~3.6 × 107 M☉ in the Seyfert/LINER galaxy NGC 4258 (Miyoshi and coworkers) provides the most compelling evidence to date for the existence of a massive black hole in the nucleus of a galaxy. The disk is oriented nearly edge-on, and the X-ray spectrum is heavily absorbed. Therefore, in this galaxy, the optical emission-line spectrum generally exhibited by an active galactic nucleus is perhaps best sought using polarized light: probing for light scattered off material surrounding the central source. New polarimetry of NGC 4258 has uncovered a compact polarized nucleus whose spectrum consists of a faint blue continuum similar to those of unobscured quasars (Fν ∝ ν-1.1), plus broadened (~1000 km s-1) emission lines. The lines are strongly linearly polarized (5%-10%) at a position angle (85° ± 2°) coincident with the plane of the maser disk. This result provides substantiating evidence for a weakly active central engine in NGC 4258 and for the existence of obscuring, orbiting tori, which impart many of the perceived distinctions between various types of active galaxies.Astronom

© 2017 American Speech-Language-Hearing Association. Purpose: The aim was to evaluate the effectiveness of computer-assisted input-based intervention for children with speech sound disorders (SSD). Method: The Sound Start Study was a cluster-randomized controlled trial. Seventy-nine early childhood centers were invited to participate, 45 were recruited, and 1,205 parents and educators of 4- and 5-year-old children returned questionnaires. Children whose parents and educators had concerns about speech were assessed (n =275); 132 children who were identified with phonological patternbased errors underwent additional assessment. Children with SSD and no difficulties with receptive language or hearing, typical nonverbal intelligence, and English as their primary language were eligible; 123 were randomized into two groups (intervention n = 65; control n = 58), and 3 withdrew. The intervention group involved Phoneme Factory Sound Sorter software (Wren & Roulstone, 2013) administered by educators over 9 weeks; the control group involved typical classroom practices. Participants were reassessed twice by a speech-language pathologist who was unaware of the initial assessment and intervention conditions. Factors associated with smoking in pregnancy
The aim of this cross-sectional study was to identify factors related to smoking during pregnancy. The sample included 267 puerperae hospitalized in the maternity unit of a university hospital in Porto Alegre/RS. The data were collected through a self-applied instrument and statistically analyzed. The majority of the puerperae (51.3%) were between 18 and 25 years old, 55.4% were nonsmokers, 25.5% were smokers, 19.1% had recently ceased smoking (in abstinence). The nonsmokers had more consultations than the smokers and the abstinent smokers (p=0.025). The number of women who had more than one child was higher among smokers than among nonsmokers and abstinent smokers (p=0.002). Women were more likely to stop smoking before pregnancy when they had a partner who was a nonsmoker (p=0.007). Several factors influence smoking and smoking cessation and these are important in prenatal interventions aimed at pregnant women and their partners
Uncanny Objects and the Fear of the Familiar:Hiding from Akan Witches in New York City
This article examines the cosmology and secret practices of West African traditional priests in New York City in preventing the spread of witchcraft, an evil invisible spirit transmitted between female members of the Akan matrilineage. Explored is an uncanny dynamic as everyday habitus becomes increasingly strange in the world of a young Ghanaian woman in the Bronx, who has become petrified of insinuations of witchcraft from close family members. In trying to hide the young woman from infection by her fellow witches, Akan priests attempt to ‘capture’ her habits and everyday routines, calling upon the iconic magic of New York City in order to ‘misplace’ familiarity within the anonymity of Manhattan. In this process, the transmission of the witch’s spirit to the intended victim is disturbed as the victim’s life and things are moved. UBVRI Light Curves of 44 Type Ia Supernovae
We present UBVRI photometry of 44 type-Ia supernovae (SN Ia) observed from
1997 to 2001 as part of a continuing monitoring campaign at the Fred Lawrence
Whipple Observatory of the Harvard-Smithsonian Center for Astrophysics. The
data set comprises 2190 observations and is the largest homogeneously observed
and reduced sample of SN Ia to date, nearly doubling the number of
well-observed, nearby SN Ia with published multicolor CCD light curves. The
large sample of U-band photometry is a unique addition, with important
connections to SN Ia observed at high redshift. The decline rate of SN Ia
U-band light curves correlates well with the decline rate in other bands, as
does the U-B color at maximum light. However, the U-band peak magnitudes show
an increased dispersion relative to other bands even after accounting for
extinction and decline rate, amounting to an additional ~40% intrinsic scatter

Smoking among pregnant women in Cantabria (Spain): trend and determinants of smoking cessation
<p>Abstract</p> <p>Background</p> <p>Cantabria (Spain) has one of the highest prevalence of smoking among women of the European Union. The objectives are to assess the trend of smoking during pregnancy in a five-year period and the determinants of smoking cessation during pregnancy in Cantabria.</p> <p>Methods</p> <p>A 1/6 random sample of all women delivering at the reference hospital of the region for the period 1998–2002 was drawn, 1559 women. Information was obtained from personal interview, clinical chart, and prenatal care records. In the analysis relative risks and 95% confidence intervals were estimated. Multivariable analysis was carried out using stepwise logistic regression.</p> <p>Results</p> <p>Smoking prior to pregnancy decreased from 53.6% in 1998 to 39.4% in 2002. A decrease in smoking cessation among women smoking at the beginning of pregnancy was observed, from 37.3% in 1998 to 20.6% in 2002. The mean number of cigarettes/day (cig/d) before pregnancy remained constant, around 16 cig/d, whereas a slight trend to increase over time was seen, from 7.7 to 8.9 cig/d. In univariate analysis two variables favoured significantly smoking cessation, although they were not included in the stepwise logistic regression analysis, a higher education level and to be married. The logistic regression model included five significant predictors (also significant in univariate analysis): intensity of smoking, number of previous pregnancies, partner's smoking status, calendar year of study period (these four variables favoured smoking continuation), and adequate prenatal care (which increased smoking cessation).</p> <p>Conclusion</p> <p>The frequency of smoking among pregnant women is very high in Cantabria. As smoking cessation rate has decreased over time, a change in prenatal care programme on smoking counseling is needed. Several determinants of smoking cessation, such as smoking before pregnancy and partner's smoking, should be also addressed by community programmes.</p
