    Feline primary erythrocytosis: a multicentre case series of 18 cats

    A retrospective multicentre case series of feline primary erythrocytosis (PE) was evaluated. The aim was to gain better understanding of disease presentation and progression to guide management and prognostication. Case records were assessed for evidence of increased packed cell volume (PCV; >48%), sufficient investigation to rule out relative and secondary erythrocytosis, and follow-up data for at least 12 months or until death. Eighteen cats were included in the case series. No significant trends in signalment were noted. Seizures and mentation changes were the most common presenting signs (both n = 10). Median PCV was 70% (median total protein concentration of 76 g/l) with no other consistent haematological changes. Sixteen cats survived to discharge. Phlebotomy was performed initially in 15/16 surviving animals and performed after discharge in 10/16. Hydroxyurea was the most common adjunctive therapy, used in 10/16 cats. Of the 16 patients surviving to discharge, 14 patients were still alive at the conclusion of the study (survival time >17 months post-discharge), with the two non-survivors having lived for 5 years or more after diagnosis. PCV, when stabilised, did not correlate with resolution of clinical signs

    Monitoring the response of canine hyperadrenocorticism to trilostane treatment by assessment of acute phase protein concentrations

    <b>Background</b>: Acute phase proteins (APPS) include haptoglobin (Hp), C-reactive protein (CRP) and serum amyloid A (SAA). Increased Hp concentrations may be induced by endogenous or exogenous glucocorticoids in dogs. <b>Objectives</b>: To assess whether control of hyperadrenocorticism (HAC) affects the concentrations of Hp, CRP, SAA, alkaline phosphatase (ALKP) and cholesterol, to determine whether these analytes can be used to assess control of HAC following trilostane treatment, and whether a combination of these tests offers a valid method of assessing disease control. <b>Methods</b>: Hp, CRP, SAA, ALKP and cholesterol were assessed in 11 dogs with spontaneous HAC before and after treatment with trilostane. Adequate control of HAC was defined as post-ACTH cortisol less than 150 nmol/l. <b>Results</b>: Significant reductions in Hp, ALKP, cholesterol and SAA (P<0·05) but not of CRP were found after control of HAC. Only Hp, cholesterol and ALKP were moderately informative (Se & Sp>0·7) of disease control when compared to adrenocorticotropin or corticotropin (ACTH) stimulation test. SAA and CRP were unhelpful (Se & Sp<0·7). The analysis of the combination of the analytes did not improve the correlation with ACTH stimulation test. <b>Clinical Relevance</b>: Relying on these analytes does not provide additional information over ACTH stimulation test results when assessing control of HAC treated with trilostane

    Agarose gel serum protein electrophoresis in cats with and without lymphoma and preliminary results of tandem mass fingerprinting analysis

    <b>Background</b>: Serum electrophoretic profiles in cats are poorly characterized with respect to the protein components of the globulin fractions, and interpretation of the electrophoretograms has routinely been done in ignorance of the identity of the proteins found within each fraction. <b>Objectives</b>: To compare the protein fractions from serum protein electrophoresis (SPE) in healthy cats and those with lymphoma and to confirm some component proteins in the major fractions after feline SPE, using tandem mass fingerprinting analysis (TMFA). <b>Methods</b>: Total protein was measured and agarose gel SPE performed on blood collected from 14 healthy cats and 14 with lymphoma. The absolute protein concentration within each fraction was compared between the two groups. Bands corresponding to the SPE fractions were excised from two controls and a lymphoma cat and analysed by liquid chromatography coupled to mass spectrometry. Results were compared to sequences in the NCBI protein database. <b>Results</b>: Median albumin concentrations were significantly decreased in lymphoma cats and median beta globulin concentrations were elevated. Narrow electrophoretic spikes were present in the beta/gamma fraction in 3 lymphoma cats. Following TMFA, multiple proteins were identified from each fraction and their mobility agreed with results from previous studies generated using alternative techniques. Inter–alpha (globulin) inhibitor 4 was identified in feline serum for the first time. <b>Conclusions</b>: Cats with lymphoma had lower median albumin and higher beta globulin concentrations than healthy cats.     Pre-trilostane and 3-hour post-trilostane cortisol to monitor trilostane therapy in dogs

    It is recommended that trilostane therapy of canine hyperadrenocorticism is monitored using an ACTH stimulation test, however this has never been validated. Three cortisol concentrations (pre-trilostane, 3-hour posttrilostane and 1-hour post-ACTH stimulation) were compared to a clinical score obtained from an owner questionnaire. There were 110 sets of 3 cortisol measurements and questionnaires obtained from 67 trilostane treated dogs. Questionnaire results were used to classify each dog as well or unwell. Well dogs were then categorised as having excellent, moderate or poor hyperadrenocorticism control, using thresholds produced by 14 independent veterinarians. Correlation co-efficients were used to compare the three cortisol concentrations to the owner score and the Kruskal Wallis and Mann-Whitney U tests were used to compare the three cortisol concentrations between categories of control. Cortisol cut-off values between significantly different categories were determined using ROC curves. Pre-trilostane and 3-hour post-trilostane cortisol were better correlated to the owner score and had cut-offs to differentiate between categories of control that had superior sensitivity and specificity results, than the post-ACTH cortisol. Iatrogenic hypoadrenocorticism was not detected in any unwell dog. This study shows that the pre-trilostane and 3-hour post-trilostane cortisol are potentially better monitoring methods than the ACTH stimulation test

    Trilostane in dogs

    Over the last 10 years, trilostane, a competitive inhibitor of steroid synthesis, is being widely used for the treatment of canine hyperadrenocorticism. Trilostane causes a significant but reversible decrease in cortisol production and a concomitant improvement in clinical signs in most dogs with this common condition. Side effects, though infrequent, can be serious: dogs treated with this drug require regular monitoring. This review summarizes current knowledge of the use of this drug with particular emphasis on its efficacy, safety, adverse reactions, and effects on endocrine parameters. Brief mention is made of its other uses in dogs and other species
