    Evaluating the impact of policies recommending PrEP to subpopulations of men and transgender women who have sex with men based on demographic and behavioral risk factors.

    IntroductionDeveloping guidelines to inform the use of antiretroviral pre-exposure prophylaxis (PrEP) for HIV prevention in resource-limited settings must necessarily be informed by considering the resources and infrastructure needed for PrEP delivery. We describe an approach that identifies subpopulations of cisgender men who have sex with men (MSM) and transgender women (TGW) to prioritize for the rollout of PrEP in resource-limited settings.MethodsWe use data from the iPrEx study, a multi-national phase III study of PrEP for HIV prevention in MSM/TGW, to build statistical models that identify subpopulations at high risk of HIV acquisition without PrEP, and with high expected PrEP benefit. We then evaluate empirically the population impact of policies recommending PrEP to these subpopulations, and contrast these with existing policies.ResultsA policy recommending PrEP to a high risk subpopulation of MSM/TGW reporting condomless receptive anal intercourse over the last 3 months (estimated 3.3% 1-year HIV incidence) yields an estimated 1.95% absolute reduction in 1-year HIV incidence at the population level, and 3.83% reduction over 2 years. Importantly, such a policy requires rolling PrEP out to just 59.7% of MSM/TGW in the iPrEx population. We find that this policy is identical to that which prioritizes MSM/TGW with high expected PrEP benefit. It is estimated to achieve nearly the same reduction in HIV incidence as the PrEP guideline put forth by the US Centers for Disease Control, which relies on the measurement of more behavioral risk factors and which would recommend PrEP to a larger subset of the MSM/TGW population (86% vs. 60%).ConclusionsThese findings may be used to focus future mathematical modelling studies of PrEP in resource-limited settings on prioritizing PrEP for high-risk subpopulations of MSM/TGW. The statistical approach we took could be employed to develop PrEP policies for other at-risk populations and resource-limited settings

    The magic numbers in exotic nuclei are discussed, and their novel origin is shown to be the spin-isospin dependent part of the nucleon-nucleon interaction in nuclei. The importance and robustness of this mechanism is shown in terms of meson exchange, G-matrix and QCD theories. In neutron-rich exotic nuclei, magic numbers such as N = 8, 20, etc. can disappear, while N = 6, 16, etc. arise, affecting the structure of lightest exotic nuclei to nucleosynthesis of heavy elements.Comment: 4 pages, 3 figures, revte

    Hydrodynamical assessment of 200 AGeV collisions

    We are analyzing the hydrodynamics of 200 A GeV S+S collisions using a new approach which tries to quantify the uncertainties arising from the specific implementation of the hydrodynamical model. Based on a previous phenomenological analysis we use the global hydrodynamics model to show that the amount of initial flow, or initial energy density, cannot be determined from the hadronic momentum spectra. We additionally find that almost always a sizeable transverse flow deve- lops, which causes the system to freeze out, thereby limiting the flow velocity in itself. This freeze-out dominance in turn makes a distinction between a plasma and a hadron resonance gas equation of state very difficult, whereas a pure pion gas can easily be ruled out from present data. To complete the picture we also analyze particle multiplicity data, which suggest that chemical equilibrium is not reached with respect to the strange particles.     Source Dimensions in Ultrarelativistic Heavy Ion Collisions

    Recent experiments on pion correlations, interpreted as interferometric measurements of the collision zone, are compared with models that distinguish a prehadronic phase and a hadronic phase. The models include prehadronic longitudinal expansion, conversion to hadrons in local kinetic equilibrium, and rescattering of the produced hadrons. We find that the longitudinal and outward radii are surprisingly sensitive to the algorithm used for two-body collisions. The longitudinal radius measured in collisions of 200 GeV/u sulfur nuclei on a heavy target requires the existence of a prehadronic phase which converts to the hadronic phase at densities around 0.8-1.0 GeV/fm3^3.     Chemistry in a gravitationally unstable protoplanetary disc

    Until now, axisymmetric, alpha-disc models have been adopted for calculations of the chemical composition of protoplanetary discs. While this approach is reasonable for many discs, it is not appropriate when self-gravity is important. In this case, spiral waves and shocks cause temperature and density variations that affect the chemistry. We have adopted a dynamical model of a solar-mass star surrounded by a massive (0.39 Msun), self-gravitating disc, similar to those that may be found around Class 0 and early Class I protostars, in a study of disc chemistry. We find that for each of a number of species, e.g. H2O, adsorption and desorption dominate the changes in the gas-phase fractional abundance; because the desorption rates are very sensitive to temperature, maps of the emissions from such species should reveal the locations of shocks of varying strengths. The gas-phase fractional abundances of some other species, e.g. CS, are also affected by gas-phase reactions, particularly in warm shocked regions. We conclude that the dynamics of massive discs have a strong impact on how they appear when imaged in the emission lines of various molecular species.Comment: 10 figures and 3 tables, accepted for publication in MNRA

    The effect of S-substitution at the O6-guanine site on the structure and dynamics of a DNA oligomer containing a G:T mismatch

    The effect of S-substitution on the O6 guanine site of a 13-mer DNA duplex containing a G:T mismatch is studied using molecular dynamics. The structure, dynamic evolution and hydration of the S-substituted duplex are compared with those of a normal duplex, a duplex with Ssubstitution on guanine, but no mismatch and a duplex with just a G:T mismatch. The S-substituted mismatch leads to cell death rather than repair. One suggestion is that the G:T mismatch recognition protein recognises the S-substituted mismatch (GS:T) as G:T. This leads to a cycle of futile repair ending in DNA breakage and cell death. We find that some structural features of the helix are similar for the duplex with the G:T mismatch and that with the S-substituted mismatch, but differ from the normal duplex, notably the helical twist. These differences arise from the change in the hydrogen-bonding pattern of the base pair. However a marked feature of the S-substituted G:T mismatch duplex is a very large opening. This showed considerable variability. It is suggested that this enlarged opening would lend support to an alternative model of cell death in which the mismatch protein attaches to thioguanine and activates downstream damage-response pathways. Attack on the sulphur by reactive oxygen species, also leading to cell death, would also be aided by the large, variable opening

    Shell model description of normal parity bands in odd-mass heavy deformed nuclei

    The low-energy spectra and B(E2) electromagnetic transition strengths of 159Eu, 159Tb and 159Dy are described using the pseudo SU(3) model. Normal parity bands are built as linear combinations of SU(3) states, which are the direct product of SU(3) proton and neutron states with pseudo spin zero (for even number of nucleons) and pseudo spin 1/2 (for odd number of nucleons). Each of the many-particle states have a well-defined particle number and total angular momentum. The Hamiltonian includes spherical Nilsson single-particle energies, the quadrupole-quadrupole and pairing interactions, as well as three rotor terms which are diagonal in the SU(3) basis. The pseudo SU(3) model is shown to be a powerful tool to describe odd-mass heavy deformed nuclei.Comment: 11 pages, 2 figures, Accepted to be published in Phys. Rev.

    Near-field interactions between metal nanoparticle surface plasmons and molecular excitons in thin-films: part I: absorption

    In this and the following paper (parts I and II, respectively), we systematically study the interactions between surface plasmons of metal nanoparticles (NPs) with excitons in thin-films of organic media. In an effort to exclusively probe near-field interactions, we utilize spherical Ag NPs in a size-regime where far-field light scattering is negligibly small compared to absorption. In part I, we discuss the effect of the presence of these Ag NPs on the absorption of the embedding medium by means of experiment, numerical simulations, and analytical calculations, all shown to be in good agreement. We observe absorption enhancement in the embedding medium due to the Ag NPs with a strong dependence on the medium permittivity, the spectral position relative to the surface plasmon resonance frequency, and the thickness of the organic layer. By introducing a low index spacer layer between the NPs and the organic medium, this absorption enhancement is experimentally confirmed to be a near field effect In part II, we probe the impact of the Ag NPs on the emission of organic molecules by time-resolved and steady-state photoluminescence measurements

    Gamow-Teller strength distributions in fp-shell nuclei

    We use the shell model Monte Carlo method to calculate complete 0f1p-shell response functions for Gamow-Teller (GT) operators and obtain the corresponding strength distributions using a Maximum Entropy technique. The approach is validated against direct diagonalization for 48Ti. Calculated GT strength distributions agree well with data from (n,p) and (p,n) reactions for nuclei with A=48-64. We also calculate the temperature evolution of the GT+ distributions for representative nuclei and find that the GT+ distributions broaden and the centroids shift to lower energies with increasing temperature
