    A Galactic O-Star Catalog

    We have produced a catalog of 378 Galactic O stars with accurate spectral classifications which is complete for V<8 but includes many fainter stars. The catalog provides cross-identifications with other sources; coordinates (obtained in most cases from Tycho-2 data); astrometric distances for 24 of the nearest stars; optical (Tycho-2, Johnson, and Stromgren) and NIR photometry; group membership, runaway character, and multiplicity information; and a web-based version with links to online services.Comment: 76 pages, 13 tables, and 3 figures.     NGC 604, the Scaled OB Association (SOBA) Prototype. I: Spatial Distribution of the Different Gas Phases and Attenuation by Dust

    We have analyzed HST and ground-based data to characterize the different gas phases and their interaction with the MYC in NGC 604, a GHR in M33. The warm ionized gas is made out of two components: a high-excitation, high-surface brightness H II surface located at the faces of the molecular clouds directly exposed to the ionizing radiation of the central SOBA; and a low-excitation, low-surface brightness halo that extends to much larger distances from the ionizing stars. The cavities created by the winds and SN explosions are filled with X-ray-emitting coronal gas. The nebular lines emitted by the warm gas experience a variable attenuation as a consequence of the dust distribution, which is patchy in the plane of the sky and with clouds interspersed among emission-line sources in the same line of sight. The optical depth at H alpha as measured from the ratio of the thermal radio continuum to H alpha shows a very good correlation with the total CO (1-0) column, indicating that most of the dust resides in the cold molecular phase. The optical depth at H alpha as measured from the ratio of H alpha to H beta also correlates with the CO emission but not as strongly as in the previous case. We analyze the difference between those two measurements and we find that <=11% of the H II gas is hidden behind large-optical-depth molecular clouds. We detect two candidate compact H II regions embedded inside the molecular cloud; both are within short distance of WR/Of stars and one of them is located within 16 pc of a RSG. We estimate the age of the main stellar generation in NGC 604 to be approx. 3 Myr from the ionization structure of the H II region. The size of the main cavity is smaller than the one predicted by extrapolating from single-star wind-blown bubbles.Comment: 50 pages, 15 figures.     The Dwarf Irregular/Wolf-Rayet Galaxy NGC4214: I. A New Distance, Stellar Content, and Global Parameters

    We present the results of a detailed optical and near-IR study of the nearby star-forming dwarf galaxy NGC4214. We discuss the stellar content, drawing particular attention to the intermediate-age and/or old field stars, which are used as a distance indicator. On images obtained with the Hubble Space Telescope WFPC2 and NICMOS instruments in the equivalents of the V, R, I, J and H bands, the galaxy is well resolved into stars. We achieve limiting magnitudes of F814W ~27 in the WF chips and F110W ~25 in the NIC2. The optical and near-infrared color-magnitude diagrams confirm a core-halo galaxy morphology: an inner high surface-brightness young population within ~1.5' (~1 kpc) from the center of the galaxy, where the stars are concentrated in bright complexes along a bar-like structure; and a relatively low-surface-brightness, field-star population extending out to at least 8' (7 kpc). The color-magnitude diagrams of the core region show evidence of blue and red supergiants, main-sequence stars, asymptotic giant branch stars and blue loop stars. We identify some candidate carbon stars from their extreme near-IR color. The field-star population is dominated by the "red tangle", which contains the red giant branch. We use the I-band luminosity function to determine the distance based on the tip-of-the-red-giant-branch method: 2.7\pm0.3 Mpc. This is much closer than the values usually assumed in the literature, and we provide revised distance dependent parameters such as physical size, luminosity, HI mass and star-formation rate.Comment: 24 pages, 19 figures, accepted for publication in the July 2002 issue of AJ.     Allelic variants of melanocortin 3 receptor gene (MC3R) and weight loss in obesity: a randomised trial of hypo-energetic high- versus low-fat diets

    INTRODUCTION: The melanocortin system plays an important role in energy homeostasis. Mice genetically deficient in the melanocortin-3 receptor gene have a normal body weight with increased body fat, mild hypophagia compared to wild-type mice. In humans, Thr6Lys and Val81Ile variants of the melanocortin-3 receptor gene (MC3R) have been associated with childhood obesity, higher BMI Z-score and elevated body fat percentage compared to non-carriers. The aim of this study is to assess the association in adults between allelic variants of MC3R with weight loss induced by energy-restricted diets. SUBJECTS AND METHODS: This research is based on the NUGENOB study, a trial conducted to assess weight loss during a 10-week dietary intervention involving two different hypo-energetic (high-fat and low-fat) diets. A total of 760 obese patients were genotyped for 10 single nucleotide polymorphisms covering the single exon of MC3R gene and its flanking regions, including the missense variants Thr6Lys and Val81Ile. Linear mixed models and haplotype-based analysis were carried out to assess the potential association between genetic polymorphisms and differential weight loss, fat mass loss, waist change and resting energy expenditure changes. RESULTS: No differences in drop-out rate were found by MC3R genotypes. The rs6014646 polymorphism was significantly associated with weight loss using co-dominant (p = 0.04) and dominant models (p = 0.03). These p-values were not statistically significant after strict control for multiple testing. Haplotype-based multivariate analysis using permutations showed that rs3827103-rs1543873 (p = 0.06), rs6014646-rs6024730 (p = 0.05) and rs3746619-rs3827103 (p = 0.10) displayed near-statistical significant results in relation to weight loss. No other significant associations or gene*diet interactions were detected for weight loss, fat mass loss, waist change and resting energy expenditure changes. CONCLUSION: The study provided overall sufficient evidence to support that there is no major effect of genetic variants of MC3R and differential weight loss after a 10-week dietary intervention with hypo-energetic diets in obese Europeans

    Efficacy of Anakinra in Refractory Adult-Onset Still's Disease: Multicenter Study of 41 Patients and Literature Review

    Adult-onset Still's disease (AOSD) is often refractory to standard therapy. Anakinra (ANK), an interleukin-1 receptor antagonist, has demonstrated efficacy in single cases and small series of AOSD. We assessed the efficacy of ANK in a series of AOSD patients. Multicenter retrospective open-label study. ANK was used due to lack of efficacy to standard synthetic immunosuppressive drugs and in some cases also to at least 1 biologic agent. Forty-one patients (26 women/15 men) were recruited. They had a mean age of 34.4 ± 14 years and a median [interquartile range (IQR)] AOSD duration of 3.5 [2-6] years before ANK onset. At that time the most common clinical features were joint manifestations 87.8%, fever 78%, and cutaneous rash 58.5%. ANK yielded rapid and maintained clinical and laboratory improvement. After 1 year of therapy, the frequency of joint and cutaneous manifestations had decreased to 41.5% and to 7.3% respectively, fever from 78% to 14.6%, anemia from 56.1% to 9.8%, and lymphadenopathy from 26.8% to 4.9%. A dramatic improvement of laboratory parameters was also achieved. The median [IQR] prednisone dose was also reduced from 20 [11.3-47.5] mg/day at ANK onset to 5 [0-10] at 12 months. After a median [IQR] follow-up of 16 [5-50] months, the most important side effects were cutaneous manifestations (n = 8), mild leukopenia (n = 3), myopathy (n = 1), and infections (n = 5). ANK is associated with rapid and maintained clinical and laboratory improvement, even in nonresponders to other biologic agents. However, joint manifestations are more refractory than the systemic manifestations

    Bronchiectasis and asthma: Data from the European Bronchiectasis Registry (EMBARC)

    \ua9 2024 The AuthorsBackground: Asthma is commonly reported in patients with a diagnosis of bronchiectasis. Objective: The aim of this study was to evaluate whether patients with bronchiectasis and asthma (BE+A) had a different clinical phenotype and different outcomes compared with patients with bronchiectasis without concomitant asthma. Methods: A prospective observational pan-European registry (European Multicentre Bronchiectasis Audit and Research Collaboration) enrolled patients across 28 countries. Adult patients with computed tomography–confirmed bronchiectasis were reviewed at baseline and annual follow-up visits using an electronic case report form. Asthma was diagnosed by the local investigator. Follow-up data were used to explore differences in exacerbation frequency between groups using a negative binomial regression model. Survival analysis used Cox proportional hazards regression. Results: Of 16,963 patients with bronchiectasis included for analysis, 5,267 (31.0%) had investigator-reported asthma. Patients with BE+A were younger, were more likely to be female and never smokers, and had a higher body mass index than patients with bronchiectasis without asthma. BE+A was associated with a higher prevalence of rhinosinusitis and nasal polyps as well as eosinophilia and Aspergillus sensitization. BE+A had similar microbiology but significantly lower severity of disease using the bronchiectasis severity index. Patients with BE+A were at increased risk of exacerbation after adjustment for disease severity and multiple confounders. Inhaled corticosteroid (ICS) use was associated with reduced mortality in patients with BE+A (adjusted hazard ratio 0.78, 95% CI 0.63-0.95) and reduced risk of hospitalization (rate ratio 0.67, 95% CI 0.67-0.86) compared with control subjects without asthma and not receiving ICSs. Conclusions: BE+A was common and was associated with an increased risk of exacerbations and improved outcomes with ICS use.     The VLT-FLAMES Tarantula Survey: XXX. Red stragglers in the clusters Hodge 301 and SL 639

    Aims: We estimate physical parameters for the late-type massive stars observed as part of the VLT-FLAMES Tarantula Survey (VFTS) in the 30 Doradus region of the Large Magellanic Cloud (LMC). Methods: The observational sample comprises 20 candidate red supergiants (RSGs) which are the reddest ((B − V) > 1 mag) and brightest (V < 16 mag) objects in the VFTS. We use optical and near-infrared (near-IR) photometry to estimate their temperatures and luminosities, and introduce the luminosity–age diagram to estimate their ages. Results: We derive physical parameters for our targets, including temperatures from a new calibration of (J − Ks)0 colour for luminous cool stars in the LMC, luminosities from their J-band magnitudes (thence radii), and ages from comparisons with current evolutionary models. We show that interstellar extinction is a significant factor for our targets, highlighting the need to take it into account in the analysis of the physical parameters of RSGs. We find that some of the candidate RSGs could be massive AGB stars. The apparent ages of the RSGs in the Hodge 301 and SL 639 clusters show a significant spread (12–24 Myr). We also apply our approach to the RSG population of the relatively nearby NGC 2100 cluster, finding a similarly large spread. Conclusions We argue that the effects of mass transfer in binaries may lead to more massive and luminous RSGs (which we call “red stragglers”) than expected from single-star evolution, and that the true cluster ages correspond to the upper limit of the estimated RSG ages. In this way, the RSGs can serve as a new and potentially reliable age tracer in young star clusters. The corresponding analysis yields ages of 24-3+5 Myr for Hodge 301, 22-5+6 Myr for SL 639, and 23-2+4 Myr for NGC 2100

    Long-term follow-up of certolizumab pegol in uveitis due to immune-mediated inflammatory diseases: multicentre study of 80 patients

    ObjectivesTo evaluate effectiveness and safety of certolizumab pegol (CZP) in uveitis due to immune-mediated inflammatory diseases (IMID).MethodsMulticentre study of CZP-treated patients with IMID uveitis refractory to conventional immunosuppressant. Effectiveness was assessed through the following ocular parameters: best-corrected visual acuity, anterior chamber cells, vitritis, macular thickness and retinal vasculitis. These variables were compared between the baseline, and first week, first, third, sixth months, first and second year.ResultsWe studied 80 (33 men/47 women) patients (111 affected eyes) with a mean age of 41.6 +/- 11.7 years. The IMID included were: spondyloarthritis (n=43), Behcet's disease (n=10), psoriatic arthritis (n=8), Crohn's disease (n=4), sarcoidosis (n=2), juvenile idiopathic arthritis (n=1), reactive arthritis (n=1), rheumatoid arthritis (n=1), relapsing polychondritis (n=1),ConclusionsCZP seems to be effective and safe in uveitis related to different IMID, even in patients refractory to previous biological drugs

    Bright OB stars in the Galaxy - III. Constraints on the radial stratification of the clumping factor in hot star winds from a combined Halpha, IR and radio analysis

    Recent results strongly challenge the canonical picture of massive star winds: various evidence indicates that currently accepted mass-loss rates, Mdot, may need to be revised downwards significantly. This is because the most commonly used mass-loss diagnostics are affected by ``clumping'' (small-scale density inhomogeneities), influencing our interpretation of observed spectra and fluxes. Such downward revisions would have dramatic consequences for the evolution of, and feedback from, massive stars, and thus robust determinations of the clumping properties and mass-loss rates are urgently needed. Here, we present a first attempt to constrain the radial stratification of the so-called clumping factor. To this end, we have analyzed a sample of 19 Galactic O-type supergiants/giants, by combining data for Halpha, IR, mm and radio fluxes, and using appropriate analysis methods. Clumping has been included into our analysis in the ``conventional'' way, by assuming the inter-clump matter to be void. Because (almost) all our diagnostics depends on the square of density, we cannot derive absolute clumping factors, but only factors normalized to a certain minimum. This minimum was usually found to be located in the outermost, radio-emitting region, i.e., the radio mass-loss rates are the lowest ones, compared to Mdot derived from Halpha and the IR. The radio rates agree well with those predicted by theory, but are only upper limits, due to unknown clumping in the outer wind. Our most important result concerns a (physical) difference between denser and thinner winds: for denser winds, the innermost region is more strongly clumped than the outermost one (with a normalized clumping factor of 4.1+/-1.4), whereas thinner winds have similar clumping properties in the inner and outer regions.Comment: 40 pages, 17 figures, accepted by A&

    Geographic variation in the aetiology, epidemiology and microbiology of bronchiectasis

    Bronchiectasis is a disease associated with chronic progressive and irreversible dilatation of the bronchi and is characterised by chronic infection and associated inflammation. The prevalence of bronchiectasis is age-related and there is some geographical variation in incidence, prevalence and clinical features. Most bronchiectasis is reported to be idiopathic however post-infectious aetiologies dominate across Asia especially secondary to tuberculosis. Most focus to date has been on the study of airway bacteria, both as colonisers and causes of exacerbations. Modern molecular technologies including next generation sequencing (NGS) have become invaluable tools to identify microorganisms directly from sputum and which are difficult to culture using traditional agar based methods. These have provided important insight into our understanding of emerging pathogens in the airways of people with bronchiectasis and the geographical differences that occur. The contribution of the lung microbiome, its ethnic variation, and subsequent roles in disease progression and response to therapy across geographic regions warrant further investigation. This review summarises the known geographical differences in the aetiology, epidemiology and microbiology of bronchiectasis.
