    Sistematizar la experiencia sobre la propuesta de construcción de la fundación Haclapa para fomentar el desarrollo de la inteligencia emocional a través del arte en la población vulnerable de la primera infancia del Barrio El Rincón del Lago de la comuna 4 del Municipio de Soacha.Esta sistematización de experiencias corresponde a un trabajo realizado para optar por el título en Especialista en Gerencia de Proyectos, de la Corporación Universitaria Minuto de Dios (Centro Regional Soacha). Básicamente, lo que se ha propuesto es la creación de la fundación Haclapa para fomentar el desarrollo de la inteligencia emocional a través del arte en la población vulnerable de primera infancia de 0 a 6 años (barrio El Rincón del Lago de la comuna 4 del Municipio de Soacha). A partir de lo anterior se toma como estrategia la creación de espacios artísticos y diseño de estrategias pedagógicas basadas en el arte que se impartirán a la población nombrada anteriormente; por medio de la fundación denominada Haclapa donde se busca el desarrollo de la inteligencia emocional a través del arte con la participación de miembros del barrio el rincón del lago de la comuna 4 de Soacha. La población que participa de dicho proceso corresponde a grupos de mujeres gestantes, madres lactantes, niños, niñas y sus cuidadores, para lo cual; a partir de los elementos teórico- prácticos propuestos mediante talleres y actividades de intervención con temáticas inclusivas y de interés, resaltando la importancia del desarrollo de la inteligencia emocional durante estos tiempos de pandemia, ya que las nuevas condiciones de vida han generado cambios donde cada individuo a tratado de adaptarse limitando su libertad de expresar sus emociones, afectando así su salud mental.This systematization of experiences corresponds to a work carried out to qualify for the title of specialist in project management, from the Minuto de Dios University Corporation (Soacha Regional Center). Basically, it is the creation of the Haclapa Foundation to promote the development of emotional intelligence through art in the vulnerable population of the El Rincón del Lago neighborhood of commune 4 of the Municipality of Soacha.     Embarazo ectópico cervical. Presentación de caso clínico

    Cervical pregnancy is the rarest form of ectopic pregnancy, in which the embryo implants and grows within the endocervical canal, like all ectopic pregnancies, can rupture acutely and are the main cause of maternal death in the first trimester of pregnancy, so early recognition and accurate location have significant clinical implications on morbidity and mortality in pregnant women. The aim of this study was to describe a clinical case of cervical ectopic pregnancy, successfully managed with conservative surgical treatment. Relevant aspects about this pathological clinical condition and the importance of timely diagnosis and treatment are described. Informed consent was requested from the patient. Authorization was obtained from the Department of Teaching and Research, allowing the review of clinical history and images for the publication of the case. The present study concludes that cervical ectopic pregnancy is a rare pathological condition with a very high mortality rate if not diagnosed and treated in time. Its management will depend on the hemodynamic state of the patient and her desire to preserve fertilityEl embarazo cervical, es la forma más rara de embarazo ectópico, en la cual el embrión se implanta y crece dentro del canal endocervical, al igual que todos los embarazos ectópicos, pueden romperse de forma aguda y son la principal causa de muerte materna en el primer trimestre del embarazo, por lo que, el reconocimiento precoz y la localización precisa tienen implicaciones clínicas significativas sobre la morbimortalidad en la gestante. El objetivo de este trabajo fue describir un caso clínico de embarazo ectópico cervical, manejado exitosamente con tratamiento quirúrgico conservador. Se describen aspectos relevantes sobre esta condición clínica patológica y la importancia del diagnóstico y tratamiento oportuno.  Se solicitó el consentimiento informado a la paciente. Se obtuvo la autorización del Departamento de docencia e investigación, permitiendo la revisión de historia clínica e imágenes para la publicación del caso. En el presente trabajo se concluye que el embarazo ectópico cervical, es una condición patológica rara y con una tasa de mortalidad muy alta si no se diagnostica y trata a tiempo.     The evolution of the ventilatory ratio is a prognostic factor in mechanically ventilated COVID-19 ARDS patients

    Background: Mortality due to COVID-19 is high, especially in patients requiring mechanical ventilation. The purpose of the study is to investigate associations between mortality and variables measured during the first three days of mechanical ventilation in patients with COVID-19 intubated at ICU admission. Methods: Multicenter, observational, cohort study includes consecutive patients with COVID-19 admitted to 44 Spanish ICUs between February 25 and July 31, 2020, who required intubation at ICU admission and mechanical ventilation for more than three days. We collected demographic and clinical data prior to admission; information about clinical evolution at days 1 and 3 of mechanical ventilation; and outcomes. Results: Of the 2,095 patients with COVID-19 admitted to the ICU, 1,118 (53.3%) were intubated at day 1 and remained under mechanical ventilation at day three. From days 1 to 3, PaO2/FiO2 increased from 115.6 [80.0-171.2] to 180.0 [135.4-227.9] mmHg and the ventilatory ratio from 1.73 [1.33-2.25] to 1.96 [1.61-2.40]. In-hospital mortality was 38.7%. A higher increase between ICU admission and day 3 in the ventilatory ratio (OR 1.04 [CI 1.01-1.07], p = 0.030) and creatinine levels (OR 1.05 [CI 1.01-1.09], p = 0.005) and a lower increase in platelet counts (OR 0.96 [CI 0.93-1.00], p = 0.037) were independently associated with a higher risk of death. No association between mortality and the PaO2/FiO2 variation was observed (OR 0.99 [CI 0.95 to 1.02], p = 0.47). Conclusions: Higher ventilatory ratio and its increase at day 3 is associated with mortality in patients with COVID-19 receiving mechanical ventilation at ICU admission. No association was found in the PaO2/FiO2 variation

    Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study

Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat

    COVID-19 symptoms at hospital admission vary with age and sex: results from the ISARIC prospective multinational observational study

    Background: The ISARIC prospective multinational observational study is the largest cohort of hospitalized patients with COVID-19. We present relationships of age, sex, and nationality to presenting symptoms. Methods: International, prospective observational study of 60 109 hospitalized symptomatic patients with laboratory-confirmed COVID-19 recruited from 43 countries between 30 January and 3 August 2020. Logistic regression was performed to evaluate relationships of age and sex to published COVID-19 case definitions and the most commonly reported symptoms. Results: ‘Typical’ symptoms of fever (69%), cough (68%) and shortness of breath (66%) were the most commonly reported. 92% of patients experienced at least one of these. Prevalence of typical symptoms was greatest in 30- to 60-year-olds (respectively 80, 79, 69%; at least one 95%). They were reported less frequently in children (≤ 18 years: 69, 48, 23; 85%), older adults (≥ 70 years: 61, 62, 65; 90%), and women (66, 66, 64; 90%; vs. men 71, 70, 67; 93%, each P < 0.001). The most common atypical presentations under 60 years of age were nausea and vomiting and abdominal pain, and over 60 years was confusion. Regression models showed significant differences in symptoms with sex, age and country. Interpretation: This international collaboration has allowed us to report reliable symptom data from the largest cohort of patients admitted to hospital with COVID-19. Adults over 60 and children admitted to hospital with COVID-19 are less likely to present with typical symptoms. Nausea and vomiting are common atypical presentations under 30 years. Confusion is a frequent atypical presentation of COVID-19 in adults over 60 years. Women are less likely to experience typical symptoms than men

    Eight years after IYA2009: a successful project of Outreach and Astronomy Education on selected countryside towns in Mexico

    We present the main results of a multidisciplinary project on education and outreach in Guanajuato (Mexico). This collaboration was initiated in the International Year of Astronomy (IYA09, IAU-UNESCO, 2009) and is led by a team including astronomers, sociologists and environment biologists. The Astronomy activities had their origin at the Public Observatory of Universidad de Guanajuato, where an outreach program was dedicated to young students, teachers and the general public. Thanks to the many activities linked to IYA09, the astronomers joined with a group of sociologists working on the development of rural zones of the Mexican State of Guanajuato. This region undergoes a high rate of migration towards the USA. Working together, our goals were enlarged, having important results on different aspects like social development, environment recovering and education. Astronomy plays a central role within this project, mainly through activities like stargazing, science games for young public, Astronomy courses for teachers, and talks for a wide audience
