    6-Year Experience of Outpatient Total and Completion Thyroidectomy

    Outpatient thyroidectomy has become slowly accepted with various published reports predominantly examining partial or subtotal thyroidectomy. Concerns regarding the safety of outpatient total and completion thyroidectomy remain, especially in regards to vocal fold paralysis, hypocalcemia, and catastrophic hematoma. We aimed to evaluate the safety of outpatient thyroid surgery in a large cohort. We conducted a retrospective review comparing outcomes in those who underwent outpatient (n=251) versus inpatient (n= 291) completion or total thyroidectomy between February 2009 and February 2015. Outpatient completion and total thyroidectomy had lower rates of temporary hypocalcemia (6% vs. 24.4%; p\u3c 0.001) and no significant difference in rates of return to emergency department (1.2% vs. 1.4%), hematoma formation (0.8% vs. 0.7%), temporary (2% vs. 4.1%) or permanent (0.4% vs. 0.7%) vocal fold paralysis, or permanent hypocalcemia (0.4% vs. 0%) compared to the inpatient group. Outpatients requiring calcium replacement had shorter duration of postoperative calcium supplementation (44.4±59.3 days vs. 63.3 ±94.4 days; p \u3c 0.001).     Cardiac tamponade is defined as abnormal fluid accumulation in the pericardial sacwhich causes compression of the heart and decreased cardiac output.Although cardiac tamponade can occur whit penetrating chest injury is relativelyrare (<i%) (1,3,5), when it occurs usually due to the weapon or sharp object piercesthe pericardium and lacerations of the cavities heart causing bleeding which accumulates in the pericardial cavity. The ventricles are the most frequently involved,with a slight predominance of the right ventricle(43%) due to its anatomical positionin the chest.     High modulus hydrogels for ophthalmic and related biomedical applications

    This paper presents three families of semi‐interpenetrating polymer network (SIPN) hydrogels based on an ester‐based polyurethane (EBPU) and hydrophilic monomers: N,N‐dimethylacrylamide (NNDMA), N‐vinyl pyrrolidone (NVP) and acryloylmorpholine (AMO) as potential materials for keratoprosthesis, orthokeratology and mini‐scleral lens application. Hydrogels sheets were synthesized via free‐radical polymerization with methods developed in‐house. SIPN hydrogels were characterized for their equilibrium water content, mechanical and surface properties. Three families of optically clear SIPN‐based hydrogels have been synthesized in the presence of water with >10% of composition attributable to EBPU. Water contents of SIPN materials ranged from 30% to 70%. SIPNs with ≤15% EBPU of total composition showed little influence to mechanical properties, whereas >15% EBPU contributed significantly to an increase in material stiffness. In the hydrated state, SIPNs with ≤15% EBPU of total composition show little difference in polar component (γp) of surface free energy, whereas for >15% EBPU there is a decrease in γp. The EBPU SIPN hydrogels display complementary material properties for keratoprosthesis, orthokeratology, and mini‐scleral applications

    Dolosigranulum pigrum modulates immunity against SARS-CoV-2 in respiratory epithelial cells

    In a previous work, we demonstrated that nasally administered Dolosigranulum pigrum 040417 beneficially modulated the respiratory innate immune response triggered by the activation of Toll-like receptor 3 (TLR3) and improved protection against Respiratory Syncytial Virus (RSV) in mice. In this work, we aimed to evaluate the immunomodulatory effects of D. pigrum 040417 in human respiratory epithelial cells and the potential ability of this immunobiotic bacterium to in-crease the protection against Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2). The respiratory commensal bacterium D. pigrum 040417 differentially modulated the production of IFN-β, IL-6, CXCL8, CCL5 and CXCL10 in the culture supernatants of Calu-3 cells stimulated with poly(I:C) or challenged with SARS-CoV-2. The differential cytokine profile induced by the 040417 strain was associated with a significant reduction in viral replication and cellular damage after coronavirus infection. Of note, D. pigrum 030918 was not able to modify the resistance of Calu-3 cells to SARS-CoV-2 infection, indicating a strain-specific immunomodulatory effect for respiratory commensal bacteria. The findings of this work improve our understanding of the immunological mechanisms involved in the modulation of respiratory immunity induced by respiratory commensal bacteria, by demonstrating their specific effect on respiratory epithelial cells. In addition, the results suggest that particular strains such as D. pigrum 040417 could be used as a promising alternative for combating SARS-CoV-2 and reducing the severity of COVID-19.Fil: Islam, Md Aminul.     Contamination profile for staphylococci and its enterotoxins and monitorization of the conditions of hygiene in a 'coalho' cheese production line

    This research aimed to evaluate the contamination by staphylococci and its enterotoxins as well as to monitor the Conditions of hygiene from a coalho cheese production like, using ATP bioluminescence assay. Staphylococcus sp. population varied from < ICFU mL(-1), in pasteurized milk to 1.5 x 10(7)CFU mL(-1) in raw milk, whereas coagulase-positive staphylococci count ranged from < ICFU mL(-1), in pasteurized milk to 5.0 x 10(6)CFU mL(-1) in raw milk. Coagulase-positive staphylococci were detected in 100% (25/25) of the row milk samples and in 8% (2/25) of cheese samples. Twelve Staphylococcus species were identified within the selected 68 isolates, being nine negative and three positive for coagulase. Raw milk samples showed a high rate of coagulase-positive, being S. aureus the most common, whereas other product samples and equipment sulfates, pieces of furniture, utensils and manipulator gloves samples presented a high frequency of coagulase-negative and low frequency of coagulase-positive. Staphylococcal enterotoxin was detected in 20% of the raw milk samples and therefore in pasteurized milk, curd and cheese. ATP measurement permitted to assess the effectiveness. of the surfaces cleaning, being considered adequate in 62.1% (36/95), "alert state" in 23.2% (22/95) and inadequate in 14.7% (14/95) of surfaces evaluated. Detection of staphylococci species with enterotoxigenic potential as well as enterotoxin presence reveal dissemination of contamination at the "coalho" Cheese production title, possibly due inappropriate Good Manufacturing Practices (GMP) from the initial milking step until the final cheese production.3851431143

    Immunobiotic lactobacilli improve resistance of respiratory epithelial cells to sars-cov-2 infection

    Previously, we reported that immunomodulatory lactobacilli, nasally administered, benefi-cially regulated the lung antiviral innate immune response induced by Toll-like receptor 3 (TLR3) activation and improved protection against the respiratory pathogens, influenza virus and respiratory syncytial virus in mice. Here, we assessed the immunomodulatory effects of viable and non-viable Lactiplantibacillus plantarum strains in human respiratory epithelial cells (Calu-3 cells) and the capacity of these immunobiotic lactobacilli to reduce their susceptibility to the acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection. Immunobiotic L. plantarum MPL16 and CRL1506 differentially modulated IFN-β, IL-6, CXCL8, CCL5 and CXCL10 production and IFNAR2, DDX58, Mx1 and OAS1 expression in Calu-3 cells stimulated with the TLR3 agonist poly(I:C). Furthermore, the MPL16 and CRL1506 strains increased the resistance of Calu-3 cells to the challenge with SARS-CoV-2. L. plantarum MPL16 induced these beneficial effects more efficiently than the CRL1506 strain. Of note, neither non-viable MPL16 and CRL1506 strains nor the non-immunomodulatory strains L. plantarum CRL1905 and MPL18 could modify the resistance of Calu-3 cells to SARS-CoV-2 infection or the immune response to poly(I:C) challenge. To date, the potential beneficial effects of immunomodulatory probiotics on SARS-CoV-2 infection and COVID-19 outcome have been extrapolated from studies carried out in the context of other viral pathogens. To the best of our knowledge, this is the first demonstration of the ability of immunomodulatory lactobacilli to positively influence the replication of the new coronavirus. Further mechanistic studies and in vivo experiments in animal models of SARS-CoV-2 infection are necessary to identify specific strains of beneficial immunobiotic lactobacilli like L. plantarum MPL16 or CRL1506 for the prevention or treatment of the COVID-19.Fil: Islam, Md Aminul.
