    Fk506 “rescue” for resistant rejection of renal allografts under primary cyclosporine immunosuppression

    Seventy-seven patients with ongoing acute rejection on initial CsA therapy were converted to FK506 to attempt graft salvage. Fifty-nine patients had undergone primary transplantation and 18 had been retransplanted; there were 52 cadaveric and 25 living-donor transplants. The indications for conversion to FK506 were ongoing, biopsy-confirmed rejection in all patients, including vascular rejection in 20. The median interval to rescue was 2 months (range 2 weeks to 36 months) after transplantation. Sixty-one of the 77 patients (79%) had already received one or more courses of an antilymphocyte preparation (OKT3: n=33; ALG or ATG: n=1; OKT3+ALG/ATG: n=27). Of the 77 patients, 57 (74%) have been successfully rescued and still have functioning grafts with a mean follow-up of 14 months, with a mean serum creatinine of 2.35±0.97 mg/dl. Eighteen patients were already dialysis-dependent at the time of conversion to FK506; of these, 9 (50%) were successfully salvaged and have a mean serum creatinine of 2.3 mg/dl. Of the 61 patients previously treated with antilymphocyte preparations, 48 (79%) were rescued. In those salvaged, prednisone doses have been lowered from 22.2±7.2 mg/day preconversion to 7.5±5.6 mg/day postconversion, and 12 patients are on FK506 monotherapy. In nondiabetics, mean serum glucose was 101.4±20.5 mg/dl preconversion and 93.2±22 postconversion (P=0.07), uric acid 7.3±2.3 and 7.1±1.5 mg/dl (P=0.53), and triglycerides 199.2±101.6 and 167.2±106.4 mg/dl (P=0.06). Cholesterol levels were significantly lower following FK conversion (207.7±46.5 mg/dl pre. vs. 188.3±39.7 post, P=0.007). FK506 is capable of salvaging renal allografts with ongoing acute rejection on CsA therapy, even when antilymphocyte preparations have been ineffective. © 1994 by Williams and Wilkins

    By applying an inverse Landau-Khalatnikov transformation, connecting (resummed) Schwinger-Dyson treatments in non-local and Landau gauges of QED3QED_3, we derive the infrared behaviour of the wave-function renormalization in the Landau gauge, and the associated critical exponents in the normal phase of the theory (no mass generation). The result agrees with the one conjectured in earlier treatments. The analysis involves an approximation, namely an expansion of the non-local gauge in powers of momenta in the infrared.     We review an scenario for the non-equilibrium dynamics of glassy systems that has been motivated by the exact solution of simple models. This approach allows one to set on firmer grounds well-known phenomenological theories. The old ideas of entropy crisis, fictive temperatures, free-volume... have clear definitions within these models. Aging effects in the glass phase are also captured. One of the salient features of the analytic solution, the breakdown of the fluctuation-dissipation relations, provides a definition of a bonafide {\it effective temperature} that is measurable by a thermometer, controls heat flows, partial equilibrations, and the reaction to the external injection of heat. The effective temperature is an extremely robust concept that appears in non-equilibrium systems in the limit of small entropy production as, for instance, sheared fluids, glasses at low temperatures when quantum fluctuations are relevant, tapped or vibrated granular matter, etc. The emerging scenario is one of partial equilibrations, in which glassy systems arrange their internal degrees of freedom so that the slow ones select their own effective temperatures. It has been proven to be consistent within any perturbative resummation scheme (mode coupling, etc) and it can be challenged by experimental and numerical tests, some of which it has already passed.Comment: 15 pages, 8 figure

    In recent years, models for co-creating value in a business-to-business context have often been examined with the aim of studying the strategies implemented by and among organisations for competitive and co-operative purposes. The traditional concepts of competition and co-operation between businesses have now evolved, both in terms of the sector in which the businesses operate and in terms of the type of goods they produce. Many researchers have, in recent times, investigated the determinants that can influence the way in which the model of co-opetition can be applied to the football world. Research interest lies in the particular features of what makes a good football. In this paper, the aim is to conduct an analysis of the rules governing the “football system”, while also looking at the determinants of the demand function within football entertainment. This entails applying to football match management the co-opetition model, a recognised model that combines competition and co-operation with the view of creating and distributing value. It can, therefore, be said that, for a spectator, watching sport is an experience of high suspense, and this suspense, in turn, depends upon the degree of uncertainty in the outcome. It follows that the rules ensuring that both these elements can be satisfied are a fertile ground for co-operation between clubs, as it is in the interest of all stakeholders to offer increasingly more attractive football, in comparison with other competing products.
