La Intercomprensió d'interrogatives absolutes amb tonemes descendents del català, de l'espanyol de les canàries i del gallec
L'estudi se centra en les oracions interrogatives del català parlat a Barcelona, de l'espanyol de Canàries parlat a Tenerife i el gallec parlat a Santiago de Compostel.la. Després d'una breu descripció acústica de la melodia de cada llengua, les frases que constitueixen el corpus (frases de tres accents tonals cadascuna) se sotmeten a tests perceptius després de ser sintetitzades i de ser eliminat el seu contingut lèxic que es presenta a oients de tots tres punts d'enquesta. Els resultats indiquen que, tot i que cada grup d'oients reconeix amb més facilitat i interpreta com a més «normals» les frases que corresponen a la seva varietat lingüística, la intercomprensió entre els parlants de les llengües romàniques estudiades és prou bona.This paper presents a study of interrogative sentences in the Catalan of Barcelona, the Spanish of Tenerife in the Canary Islands, and the Galician spoken in Santiago de Compostela. After a brief acoustic description of the melody used in each language variety, the results of perception tests carried out with subjects from the three areas of study on the sentences in the corpus (each containing three pitch accents) are given, the sentences first having been synthesized and stripped of their lexical content. The results show that, although each group of subjects recognize more easily and consider more normal the sentences from their own linguistic variety, mutual intelligibilty among speakers of the Romance varieties studied is good
Design of a case management model for people with chronic disease (Heart Failure and COPD). Phase I: modeling and identification of the main components of the intervention through their actors: patients and professionals (DELTA-ICE-PRO Study
Background
Chronic diseases account for nearly 60% of deaths around the world. The extent of this silent epidemic has not met determined responses in governments, policies or professionals in order to transform old Health Care Systems, configured for acute diseases. There is a large list of research about alternative models for people with chronic conditions, many of them with an advanced practice nurse as a key provider, as case management. But some methodological concerns raise, above all, the design of the intervention (intensity, frequency, components, etc).
Methods/Design
Objectives: General: To develop the first and second phases (theorization and modeling) for designing a multifaceted case-management intervention in people with chronic conditions (COPD and heart failure) and their caregivers. Specific aims: 1) To identify key events in people living with chronic disease and their relation with the Health Care System, from their point of view. 2) To know the coping mechanisms developed by patients and their caregivers along the story with the disease. 3) To know the information processing and its utilization in their interactions with health care providers. 4) To detect potential unmet needs and the ways deployed by patients and their caregivers to resolve them. 5) To obtain a description from patients and caregivers, about their itineraries along the Health Care System, in terms of continuity, accessibility and comprehensiveness of care. 6) To build up a list of promising case-management interventions in patients with Heart Failure and COPD with this information in order to frame it into theoretical models for its reproducibility and conceptualization. 7) To undergo this list to expert judgment to assess its feasibility and pertinence in the Andalusian Health Care. Design: Qualitative research with two phases: For the first five objectives, a qualitative technique with biographic stories will be developed and, for the remaining objectives, an expert consensus through Delphi technique, on the possible interventions yielded from the first phase. The study will be developed in the provinces of Almería, Málaga and Granada in the Southern Spain, from patients included in the Andalusian Health Care Service database with the diagnosis of COPD or Heart Failure, with the collaboration of case manager nurses and general practitioners for the assessment of their suitability to inclusion criteria. Patients and caregivers will be interviewed in their homes or their Health Centers, with their family or their case manager nurse as mediator.
Discussion
First of a series of studies intended to design a case-management service for people with heart failure and COPD, in the Andalusian Health Care System, where case management has been implemented since 2002. Accordingly with the steps of a theoretical model for complex interventions, in this study, theorization and intervention modeling phases will be developed.This research was carried out with the support of one research grant, awarded by the Regional Health Ministry of Andalusia (Exp. 0222/2008
A Supervised ML Biometric Continuous Authentication System for Industry 4.0
Continuous authentication (CA) is a promising approach to authenticate workers and avoid security breaches in the industry, especially in Industry 4.0, where most interaction between workers and devices takes place. However, introducing CA in industries raises the following unsolved questions regarding machine learning (ML) models: its precision and performance; its robustness; and the issue about if or when to retrain the models. To answer these questions, this article explores these issues with a proposed supervised versus nonsupervised ML-based CA system that uses sensors, applications statistics, or speaker data collected by the operator’s devices. Experiments show supervised models with equal error rates of 7.28% using sensors data, 9.29% with statistics, and 0.31% with voice, a significant improvement of 71.97, 62.14, and 97.08%, respectively, over unsupervised models. Voice is the most robust dimension when adding new workers, with less than 2% of false acceptance rate even if workforce size is doubled
Transit Performance Measures in California
This research is the result of a California Department of Transportation (Caltrans) request to assess the most commonly available transit performance measures in California. Caltrans wanted to understand performance measures and data used by Metropolitan Planning Organizations (MPOs) and transit agencies to help it develop statewide measures. This report serves as a summary reference guide to help Caltrans understand the numerous and diverse performance measures used by MPOs and transit agencies in California. First, investigators review the available literature to identify a complete transit performance framework for the purposes of organizing agency measures, metrics, and data sources. Next, they review the latest transit performance measures documented in planning reports for the four largest MPOs in California (San Francisco Bay Area, Los Angeles, San Diego, and Sacramento). Researchers pay special attention to the transit performance measures used by these MPOs, because these measures are available for the majority of California’s population. Finally, investigators summarize 231 performance measures used by a total 26 local transit agencies in the State of California, based on transit planning documents available on the internet
Placental-site trophoblastic tumor with bone metastasis: A diagnostic and therapeutic challenge
Placental-site trophoblastic tumor (PSTT) is a rare pathological entity included in the spectrum of gestational
trophoblastic neoplasia (GTN). It is a neoplasia with metastatic potential that, once metastasized, has poor
prognosis because the tumor tends to be less sensitive to chemotherapy. We present a rare case of gestational
trophoblastic neoplasia, in which hysterectomy for persistent gestational trophoblastic disease after hydatidiform
mole, revealed a primary PSTT in the uterus. Subsequently, a slight persistent elevation of the beta fraction of
human chorionic gonadotropin hormone (B-hCG) during follow-up revealed the presence of bone metastases.
This location is not usual from this tumor, being even more rare the case of PSTT with isolated bone metastases.
Metastasic foci were only identified with PET-CT since the usual diagnostic resources were not able to do it.
Finally, it is also remarkable in our case that the treatment required the confluence of chemotherapy together
with immunotherapy to achieve a favorable response
Illusory vowels in Spanish-English sequential bilinguals: Evidence that accurate L2 perception is neither necessary nor sufficient for accurate L2 production
Spanish native speakers are known to pronounce onset /sC/ clusters in English with a prothetic vowel, as in esport for sport, due to their native language phonotactic constraints. We assessed whether accurate production of e.g. spi instead of espi was related to accurate perceptual discrimination of this contrast in second language (L2) speech of Spanish–English sequential bilinguals. A same–different discrimination task in stimulus pairs such as spi–espi assessed speech perception and a phonemic verbal fluency task elicited speech production. Logistic mixed model regressions revealed significant differences in accuracy between the bilinguals and the English monolinguals, although some bilinguals performed within the monolingual range. For the production task, but not for the perception task, bilinguals with more exposure to English and greater grammatical knowledge of English performed significantly more accurately than those with less exposure and lower grammatical knowledge. There was no significant correlation between production accuracy and perception accuracy. Through examining phonotactic constraints, these results expand a growing body of research into single sounds which suggests dissociations between L2 perception and production. In contrast to predictions made by L2 speech models, the findings indicate that accurate L2 perception is neither necessary nor sufficient for accurate L2 production, and instead are interpreted to indicate that the two capacities recruit different executive control mechanisms and are acquired – at least to a certain extent – independently in L2 acquisition.Peer Reviewe
Innovación sanitaria en la gestión del riesgo de caídas de personas mayores en Atención Primaria
[ES] Las caídas son actualmente uno de los problemas de salud más frecuentes y graves en las personas mayores. La gestión sociosanitaria eficiente desde el punto de vista preventivo se destaca como uno de los objetivos principales para afrontar esta problemática. Para dar respuesta a ello, el IBV y el Hospital Universitario de la Ribera, gestionado por Ribera Salud II UTE Ley 18/82, llevan a cabo de forma conjunta una línea de investigación para desarrollar y validar nuevos avances tecnológicos. El proyecto que se presenta a continuación ha servido para conocer la aplicabilidad y utilidad de la herramienta FallSkip en el contexto de la consulta del médico de Atención Primaria. Experiences and coping with the altered body image in digestive stoma patients
Objetivo: descrever o enfrentamento de pessoas ostomizadas diante da notícia da realização do estoma, assim como analisar o significado e a vivência diante de sua nova realizada corporal. Método: estudo qualitativo fenomenológico mediante entrevistas semiestruturadas com 21 personas ostomizadas. Foi desenvolvido através da comparação constante de dados, incorporação progressiva de sujeitos e triangulação entre investigadores e enfermeiras especialistas em estomaterapia. Foi utilizado o software Atlas.ti. Resultados: emergiram duas categorias centrais: “Enfrentamento diante da notícia de que serão ostomizados” e “Significado e vivência da nova realidade corporal”. A resposta dos informantes é variável, revelando situações que vão desde a aceitação natural do seu processo até a resignação e a rejeição. As experiências prévias de outros familiares, a possibilidade de reconstrução do estoma ou o tipo de doença são fatores condicionantes. Conclusões: o enfrentamento diante da notícia do estoma está condicionado pelo tipo de doença. Apesar disso, observa-se que a maioria dos informantes tende a normalizar o processo. Genomic transformation and social organization during the Copper Age-Bronze Age transition in southern Iberia
The emerging Bronze Age (BA) of southeastern Iberia saw marked social changes. Late Copper Age (CA) settlements were abandoned in favor of hilltop sites, and collective graves were largely replaced by single or double burials with often distinctive grave goods indirectly reflecting a hierarchical social organization, as exemplified by the BA El Argar group. We explored this transition from a genomic viewpoint by tripling the amount of data available for this period. Concomitant with the rise of El Argar starting ~2200 cal BCE, we observe a complete turnover of Y-chromosome lineages along with the arrival of steppe-related ancestry. This pattern is consistent with a founder effect in male lineages, supported by our finding that males shared more relatives at sites than females. However, simple two-source models do not find support in some El Argar groups, suggesting additional genetic contributions from the Mediterranean that could predate the BA
