    Microstructural damage of the posterior corpus callosum contributes to the clinical severity of neglect

    One theory to account for neglect symptoms in patients with right focal damage invokes a release of inhibition of the right parietal cortex over the left parieto-frontal circuits, by disconnection mechanism. This theory is supported by transcranial magnetic stimulation studies showing the existence of asymmetric inhibitory interactions between the left and right posterior parietal cortex, with a right hemispheric advantage. These inhibitory mechanisms are mediated by direct transcallosal projections located in the posterior portions of the corpus callosum. The current study, using diffusion imaging and tract-based spatial statistics (TBSS), aims at assessing, in a data-driven fashion, the contribution of structural disconnection between hemispheres in determining the presence and severity of neglect. Eleven patients with right acute stroke and 11 healthy matched controls underwent MRI at 3T, including diffusion imaging, and T1-weighted volumes. TBSS was modified to account for the presence of the lesion and used to assess the presence and extension of changes in diffusion indices of microscopic white matter integrity in the left hemisphere of patients compared to controls, and to investigate, by correlation analysis, whether this damage might account for the presence and severity of patients' neglect, as assessed by the Behavioural Inattention Test (BIT). None of the patients had any macroscopic abnormality in the left hemisphere; however, 3 cases were discarded due to image artefacts in the MRI data. Conversely, TBSS analysis revealed widespread changes in diffusion indices in most of their left hemisphere tracts, with a predominant involvement of the corpus callosum and its projections on the parietal white matter. A region of association between patients' scores at BIT and brain FA values was found in the posterior part of the corpus callosum. This study strongly supports the hypothesis of a major role of structural disconnection between the right and left parietal cortex in determining 'neglect'

    "I know that you know that I know": neural substrates associated with social cognition deficits in DM1 patients

    Myotonic dystrophy type-1 (DM1) is a genetic multi-systemic disorder involving several organs including the brain. Despite the heterogeneity of this condition, some patients with non-congenital DM1 can present with minimal cognitive impairment on formal testing but with severe difficulties in daily-living activities including social interactions. One explanation for this paradoxical mismatch can be found in patients' dysfunctional social cognition, which can be assessed in the framework of the Theory of Mind (ToM). We hypothesize here that specific disease driven abnormalities in DM1 brains may result in ToM impairments. We recruited 20 DM1 patients who underwent the "Reading the Mind in the Eyes" and the ToM-story tests. These patients, together with 18 healthy controls, also underwent resting-state functional MRI. A composite Theory of Mind score was computed for all recruited patients and correlated with their brain functional connectivity. This analysis provided the patients' "Theory of Mind-network", which was compared, for its topological properties, with that of healthy controls. We found that DM1 patients showed deficits in both tests assessing ToM. These deficits were associated with specific patterns of abnormal connectivity between the left inferior temporal and fronto-cerebellar nodes in DM1 brains. The results confirm the previous suggestions of ToM dysfunctions in patients with DM1 and support the hypothesis that difficulties in social interactions and personal relationships are a direct consequence of brain abnormalities, and not a reaction symptom. This is relevant not only for a better pathophysiological comprehension of DM1, but also for non-pharmacological interventions to improve clinical aspects and impact on patients' success in life

    Grand Challenges in global eye health: a global prioritisation process using Delphi method

    Background: We undertook a Grand Challenges in Global Eye Health prioritisation exercise to identify the key issues that must be addressed to improve eye health in the context of an ageing population, to eliminate persistent inequities in health-care access, and to mitigate widespread resource limitations. Methods: Drawing on methods used in previous Grand Challenges studies, we used a multi-step recruitment strategy to assemble a diverse panel of individuals from a range of disciplines relevant to global eye health from all regions globally to participate in a three-round, online, Delphi-like, prioritisation process to nominate and rank challenges in global eye health. Through this process, we developed both global and regional priority lists. Findings: Between Sept 1 and Dec 12, 2019, 470 individuals complete round 1 of the process, of whom 336 completed all three rounds (round 2 between Feb 26 and March 18, 2020, and round 3 between April 2 and April 25, 2020) 156 (46%) of 336 were women, 180 (54%) were men. The proportion of participants who worked in each region ranged from 104 (31%) in sub-Saharan Africa to 21 (6%) in central Europe, eastern Europe, and in central Asia. Of 85 unique challenges identified after round 1, 16 challenges were prioritised at the global level; six focused on detection and treatment of conditions (cataract, refractive error, glaucoma, diabetic retinopathy, services for children and screening for early detection), two focused on addressing shortages in human resource capacity, five on other health service and policy factors (including strengthening policies, integration, health information systems, and budget allocation), and three on improving access to care and promoting equity. Interpretation: This list of Grand Challenges serves as a starting point for immediate action by funders to guide investment in research and innovation in eye health. It challenges researchers, clinicians, and policy makers to build collaborations to address specific challenges.     The study was carried out to evaluate wedge wire screen as a potential tool for collecting fish faeces from the tank outlet water. Apparent digestibility (AD) estimates of carbon (ADC), nitrogen (ADN), sulphur (ADS), organic material (ADORG) and individual amino acids obtained by the wire screen were compared with stripping. Three diets, with fishmeal, soybean meal (SBM) or rapeseed meal (RSM) as main protein source were extruded and fed to triplicate groups of 120-g rainbow trout (Oncorhynchus mykiss). Faeces were obtained by gentle stripping from the distal abdomen, and by collection from a wedge wire screen 15, 30, 60, 120 and 240 min post feeding. ADORG, ADN and ADS obtained by collection on the screen were significantly (P < .05) higher than those obtained by stripping, except for some values for ADC. Differences in AD between stripping and faeces collected with the wire screen were lower than reported for other methods of collection from water media. Most of the increase in AD of nutrients for wedge wire screen occurred during the first 15 min after defecation. The ADORG, ADC, ADN and ADS were lowest in the RSM diet. The AD of amino acids followed the same pattern as ADN. ADS in the rapeseed diet was particularly low (54.7%), compared to the fishmeal (72.4%) and SBM diet (70.1%). In addition to AD of cysteine, low digestibility of glucosinolates, its degradation products and other sulfonated components are likely reasons for the low ADS. Faeces from the RSM based diet had higher dry matter (DM) concentration than the SBM based diet. Faecal DM recovery for SBM was lowest among the diets for both collection methods. The low DM recovery for the SBM faeces did not affect AD estimates of faeces at the different time intervals, possibly due to simultaneous leakage of nutrients and indigestible marker. In conclusion, the use of wedge wire screen for collection of faeces represents an interesting supplement to stripping. The AD estimates had low random variation and nutrient leaching during faeces collection were dependent on dietary composition.publishedVersio
