This work reports the comparative study of heavy metal water concentrations from two coastal ecosystems of Cadiz Gulf (SWSpain): Bay of Cadiz and Ria ofHuelva estuary in samples taken on years 1999-2000. Sampling zones showed different heavymetal levels. Statistical analysis of dissolved trace metal concentrations (Cu, Zn, Cd and Pb) showed significant differences between Huelva and Cadiz zones based on the Cu level, with a marked seasonal factor and different metal sources (Industrial, geological, urban,...), observing a higher Cu concentration in Ria de Huelva respect to Bay of Cadiz. On the other hand, to evaluate the pollutant effects on aquatic biota, a histopathological analysis have been conducted in specimens of flat fish, Solea senegalensis from both zones. Lesions weremore severe in fish fromRia ofHuelva, being themost significant alterations: gill hyperplasia, fusion of secondary lamellae, disorganization of the hepatic parenchyma, cellular hypertrophy and vascular congestion in liver and tubular occlusion, loss of interstitial tissue and lipid-like vacuoles in kidney. Obtained results are useful as reference data for future environmental monitoring studies in these zones where to establish a heavy metal concentration temporal trend would be necessary

Using the implicit association test to assess risk propensity self-concept: analysis of its predictive validity on a risk-tak ing behaviour in a natural setting

The present work analyses the predictive validity of measures provided by several available self-report and indirect measurement instruments to assess risk propensity (RP) and proposes a measurement instrument using the Implicit Association Test: the IAT of Risk Propensity Self-Concept (IAT-RPSC), an adaptation of the prior IAT-RP of Dislich et al. Study 1 analysed the relationship between IAT-RPSC scores and several RP self-report measures. Participants’ risk-taking behaviour in a natural setting was also assessed, analyzing the predictive validity of the IAT-RPSC scores on risk-taking behaviour compared with the self-report measures. Study 2 analysed the predictive validity of the IAT-RPSC scores in comparison with other indirect measures. Results of these studies showed that the IAT-RPSC scores exhibited good reliability and were positively correlated to several self-report and indirect measures, providing evidence for convergent validity. Most importantly, the IAT-RPSC scores predicted risk-taking behaviour in a natural setting with real consequences above and beyond all other self-report and indirect measures analysed. This research has been funded by the Spanish Ministry of Science and Innovation, research grants PSI2008-02916 and PSI2011-2700

Introduction: Studies have shown significant associations between parenting practices, life satisfaction, and self-esteem, and the role of parenting practices in adolescent adjustment, emphasizing its influence on wellbeing. Objectives: To analyze the relationships between parenting practices, self-esteem, and life satisfaction, and test the mediating effect of self-esteem on the relationship between the different parenting practices and life satisfaction of adolescents. Method: The sample came to a total of 742 adolescents, with an average age of 15.63 (SD = 1.24; range 13–19). The Parenting Style Scale, the Rosenberg Self-Esteem Scale, and the Satisfaction with Life Scale were used. Results: Perception by adolescents of high levels of affect and communication, self-disclosure, and a sense of humor related to their parents, as well as low levels of psychological control, explained the life satisfaction of the adolescents. Self-esteem exerted a partial mediating effect on the relationship between parenting practices and satisfaction with the life of the adolescent. Finally, self-esteem also appeared to be a moderator variable, specifically in the effect of self-disclosure on the life satisfaction of the adolescent. Conclusions: The results reinforce the role of personal variables, especially self-esteem, in parent-child interaction and in the improved subjective wellbeing of the adolescent

Is hospital discharge administrative data an appropriate source of information for cancer registries purposes? Some insights from four Spanish registries

    Get PDF

Abstract

Background

The use of hospital discharge administrative data (HDAD) has been recommended for automating, improving, even substituting, population-based cancer registries. The frequency of false positive and false negative cases recommends local validation.

Methods

The aim of this study was to detect newly diagnosed, false positive and false negative cases of cancer from hospital discharge claims, using four Spanish population-based cancer registries as the gold standard. Prostate cancer was used as a case study.

Results

A total of 2286 incident cases of prostate cancer registered in 2000 were used for validation. In the most sensitive algorithm (that using five diagnostic codes), estimates for Sensitivity ranged from 14.5% (CI95% 10.3-19.6) to 45.7% (CI95% 41.4-50.1). In the most predictive algorithm (that using five diagnostic and five surgical codes) Positive Predictive Value estimates ranged from 55.9% (CI95% 42.4-68.8) to 74.3% (CI95% 67.0-80.6). The most frequent reason for false positive cases was the number of prevalent cases inadequately considered as newly diagnosed cancers, ranging from 61.1% to 82.3% of false positive cases. The most frequent reason for false negative cases was related to the number of cases not attended in hospital settings. In this case, figures ranged from 34.4% to 69.7% of false negative cases, in the most predictive algorithm.

Conclusions

HDAD might be a helpful tool for cancer registries to reach their goals. The findings suggest that, for automating cancer registries, algorithms combining diagnoses and procedures are the best option. However, for cancer surveillance purposes, in those cancers like prostate cancer in which care is not only hospital-based, combining inpatient and outpatient information will be required.</p

The Impact Of Rituximab Infusion Protocol On The Long-term Outcome In Anti-musk Myasthenia Gravis

    Get PDF
Objective: To evaluate whether the clinical benefit and relapse rates in anti-muscle-specific kinase (MuSK) myasthenia gravis (MG) differ depending on the protocol of rituximab followed. Methods: This retrospective multicentre study in patients with MuSK MG compared three rituximab protocols in terms of clinical status, relapse, changes in treatment, and adverse side effects. The primary effectiveness endpoint was clinical relapse requiring a further infusion of rituximab. Survival curves were estimated using Kaplan-Meier methods and survival analyses were undertaken using Cox proportional-hazards models. Results: Twenty-five patients were included: 11 treated with protocol 4 + 2 (375 mg/m(2)/4 weeks, then monthly for 2 months), five treated with protocol 1 + 1 (two 1 g doses 2 weeks apart), and nine treated with protocol 4 (375 mg/m(2)/4 weeks). Mean follow-up was 5.0 years (SD 3.3). Relapse occurred in 18.2%, 80%, and 33.3%, and mean time to relapse was 3.5 (SD 1.5), 1.1 (SD 0.4), and 2.5 (SD 1.4) years, respectively. Based on Kaplan-Meier estimates, patients treated with protocol 4 + 2 had fewer and later relapses than patients treated with the other two protocols (log-rank test P = 0.0001). Patients treated with protocol 1 + 1 had a higher risk of relapse than patients treated with protocol 4 + 2 (HR 112.8, 95% CI, 5.7-2250.4, P = 0.002). Patients treated with protocol 4 showed a trend to a higher risk of relapse than those treated with protocol 4 + 2 (HR 9.2, 95% CI 0.9-91.8, P = 0.059). InterpretationThis study provides class IV evidence that the 4 + 2 rituximab protocol has a lower clinical relapse rate and produces a more durable response than the 1 + 1 and 4 protocols in patients with MuSK MG
