    Use of Repeated Blood Pressure and Cholesterol Measurements to Improve Cardiovascular Disease Risk Prediction: An Individual-Participant-Data Meta-Analysis

    The added value of incorporating information from repeated blood pressure and cholesterol measurements to predict cardiovascular disease (CVD) risk has not been rigorously assessed. We used data on 191,445 adults from the Emerging Risk Factors Collaboration (38 cohorts from 17 countries with data encompassing 1962-2014) with more than 1 million measurements of systolic blood pressure, total cholesterol, and high-density lipoprotein cholesterol. Over a median 12 years of follow-up, 21,170 CVD events occurred. Risk prediction models using cumulative mean values of repeated measurements and summary measures from longitudinal modeling of the repeated measurements were compared with models using measurements from a single time point. Risk discrimination (Cindex) and net reclassification were calculated, and changes in C-indices were meta-analyzed across studies. Compared with the single-time-point model, the cumulative means and longitudinal models increased the C-index by 0.0040 (95% confidence interval (CI): 0.0023, 0.0057) and 0.0023 (95% CI: 0.0005, 0.0042), respectively. Reclassification was also improved in both models; compared with the single-time-point model, overall net reclassification improvements were 0.0369 (95% CI: 0.0303, 0.0436) for the cumulative-means model and 0.0177 (95% CI: 0.0110, 0.0243) for the longitudinal model.     Genome-wide association study identifies six new loci influencing pulse pressure and mean arterial pressure.

    Numerous genetic loci have been associated with systolic blood pressure (SBP) and diastolic blood pressure (DBP) in Europeans. We now report genome-wide association studies of pulse pressure (PP) and mean arterial pressure (MAP). In discovery (N = 74,064) and follow-up studies (N = 48,607), we identified at genome-wide significance (P = 2.7 × 10(-8) to P = 2.3 × 10(-13)) four new PP loci (at 4q12 near CHIC2, 7q22.3 near PIK3CG, 8q24.12 in NOV and 11q24.3 near ADAMTS8), two new MAP loci (3p21.31 in MAP4 and 10q25.3 near ADRB1) and one locus associated with both of these traits (2q24.3 near FIGN) that has also recently been associated with SBP in east Asians. For three of the new PP loci, the estimated effect for SBP was opposite of that for DBP, in contrast to the majority of common SBP- and DBP-associated variants, which show concordant effects on both traits. These findings suggest new genetic pathways underlying blood pressure variation, some of which may differentially influence SBP and DBP

    Finishing the euchromatic sequence of the human genome

    The sequence of the human genome encodes the genetic instructions for human physiology, as well as rich information about human evolution. In 2001, the International Human Genome Sequencing Consortium reported a draft sequence of the euchromatic portion of the human genome. Since then, the international collaboration has worked to convert this draft into a genome sequence with high accuracy and nearly complete coverage. Here, we report the result of this finishing process. The current genome sequence (Build 35) contains 2.85 billion nucleotides interrupted by only 341 gaps. It covers ∼99% of the euchromatic genome and is accurate to an error rate of ∼1 event per 100,000 bases. Many of the remaining euchromatic gaps are associated with segmental duplications and will require focused work with new methods. The near-complete sequence, the first for a vertebrate, greatly improves the precision of biological analyses of the human genome including studies of gene number, birth and death. Notably, the human enome seems to encode only 20,000-25,000 protein-coding genes. The genome sequence reported here should serve as a firm foundation for biomedical research in the decades ahead

    Force and form : Nietzsche and the concept of dance

    In The Birth of Tragedy Nietzsche says that in dance, the greatest strength is only potential and that strength is betrayed by the suppleness of movement (BT, 46). This thesis has found that dance, for Nietzsche, is not force itself; dance censors force through form. As opposed to aligning dance simply with Apollo, however, we have ultimately found Nietzsche’s concept of dance at the heart of Nietzsche’s dichotomy of art. Dance is joyful and free for Nietzsche in the sense that dance is the very experience of tragic intuition. Derrida, at the end of Force and Signification, appears to interpret the concept of dance otherwise, equating it with the triumph of force over form in the work of Nietzsche. In order to consolidate the weight of this thesis’ argument about dance, we will conclude by briefly highlighting how Derrida sees writing, as opposed to dance, as the representation of the victory of form over force: “we would have to choose then, between writing and dance” (Derrida, 34). Through this brief analysis of Derrida’s essay, we will seek to consolidate a pivotal argument in this thesis: Nietzsche’s concept of dance is in fact ultimately representative of the victory of form. This thesis has shown that dance is integral to Nietzsche’s philosophy due to its articulation of the force/form couple - the concept of dance is embedded in Nietzsche’s aesthetic theory for the way it enacts the event of representation in the aesthetic state. We have found that dance is at the core of Nietzsche’s theory of knowledge and that it participates in Nietzsche’s critique of language

    This essay seeks to dispel entrenched critical opinion regarding dance across Nietzsche's writings as representative of Dionysian intoxication alone. Taking as its prompt the riposte of Alain Badiou, 'Nietzsche is miles away from any doctrine of dance as a primitive ecstasy' and 'dance is in no way the liberated bodily impulse, the wild energy of the body', the essay uncovers the ties between dance and Apollo in the Nietzschean theory of art while qualifying dance's relation to Dionysus. Primarily through an analysis of The Dionysiac World View and The Birth of Tragedy, the essay seeks to illuminate enigmatic statements about dance in Nietzsche ('in dance the greatest strength is only potential, although it is betrayed by the suppleness of movement' and 'dance is the preservation of orderly measure'). It does this through an elucidation of the specific function of dance in Nietzsche's interpretation of classical Greece; via an assessment of the difficulties associated with the Nietzschean understanding of the bacchanal; and lastly through an analysis of Nietzsche's characterization of dance as a symbol. The essay culminates in a discussion of dance's ties to Nietzschean life affirmation; here the themes of physico-phenomenal existence, joy and illusion in Nietzsche are surveyed

    The United States is becoming more racially and ethnically diverse as a function of immigration, both legal and illegal, from Asia, Mexico, and Latin America. Latinos are the fastest growing population, and in 2000, Latinos replaced African Americans as the largest minority group in the United States. Although much of the media and scholarly attention has focused on demographic changes in traditional Latino immigrant destinations such as California, New Mexico, Texas, and Arizona, the rapid growth in Latino populations is occurring across the nation. The South has undergone a particularly dramatic alteration in terms of racial composition, with six of seven states tripling the size of their Latino populations between 1990 and 2000. This settlement of Latinos in the South is no more than ten to fifteen years old, and new immigrants from Mexico and Latin America are settling in states like North Carolina, Georgia, and Tennessee (Durand, Massey, and Carvet 2000). They bring ethnic and cultural diversity to areas previously defined exclusively as black and white. Not only have new Latino populations migrated to urban and suburban locations in the South, they also have settled in small towns and rural areas, reinforcing projections of the “Latinization” of the American South. Examples of these “New Latino Destinations” (Suro and Singer 2000) include cities such as Atlanta, Georgia; Charlotte, Greensboro-Winston Salem, and Raleigh-Durham, North Carolina; Nashville and Memphis, Tennessee; and Greenville, South Carolina

    Dramatic demographic changes are occurring in the United States, and some of the most dramatic changes are occurring in the South from Latino immigration. Latinos, by and large, are an entirely new population in the region. How are Black southerners reacting to this new population? Using survey data gathered from a southern location, this article explores several questions related to whether Blacks see these new residents as friendly neighbors or economic competitors. Results suggest that Blacks and non-Blacks perceive a potential economic threat from continued Latino immigration, but Blacks are more concerned about the effects of Latino immigration than are Whites

    The United States is undergoing dramatic demographic change, primarily from immigration, and many of the new Latino immigrants are settling in the South. This paper examines hypotheses related to attitudes of Latino immigrants toward black Americans in a Southern city. The analyses are based on a survey of black, white, and Latino residents (n = 500). The results show, for the most part, Latino immigrants hold negative stereotypical views of blacks and feel that they have more in common with whites than with blacks. Yet, whites do not reciprocate in their feelings toward Latinos. Latinos' negative attitudes toward blacks, however, are modulated by a sense of linked fate with other Latinos. This research is important because the South still contains the largest population of African Americans in the United States, and no section of the country has been more rigidly defined along a black-white racial divide. How these new Latino immigrants situate themselves vis-à-vis black Americans has profound implications for the social and political fabric of the South
