    This article is devoted to the study of consumer society reflection in V.O. Pelevin‘s works published in the twenty-first century. There is an attempt to determine the main features of the consumer society to which the characters belong: dominance of focus toward to the satisfaction of private needs, the acquisition of material wealth as a means of achieving prosperity and happiness, the quality of consumed products (goods, services, etc.) determines the status of the consumer in society. V.O. Pelevin defines and ridicules capitalist culture using sarcasm and grotesque, demonstrating the image of consumer society exclusively in negative connotations. V.O. Pelevin reconstructs modern society, endowing it with the features of the world we are accustomed to, but absurdly creating his world of sellers and consumers included in a repeating cycle and changing roles depending on status within the framework of artistic reality. Aimed at revealing the author’s attitude to the consumer society and the novel characters’ attitudes to the object of study, selective analysis of a work fragments demonstrates the gradual inclusion of characters in the consumption culture and a deeper study of the phenomenon by the author. The article considers the consumer society reconstruction in the art world of the following works by V.O. Pelevin: novels Generation P, The Werewolf’s Holy Book, Svyashennaya kniga oborotnya, Empire V, Secret Views of the Fuji Mount, [Sekretnye vidi na goru Fuji], and a story Wind search record [Zapis‘ o poiske vetra]. Such a selective analysis reflects the transformation (which, depending on the point of view, can be called both evolution and degradation) of the consumer culture image and the position of the author may be determined.     Geographic and temporal morphological stasis in the latest Cretaceous ammonoid Discoscaphites iris from the U.S. Gulf and Atlantic Coastal Plains

    We examine temporal and spatial variation in morphology of the ammonoid cephalopod Discoscaphites iris using a large dataset from multiple localities in the Late Cretaceous (Maastrichtian) of the United States Gulf and Atlantic Coastal Plains, spanning a distance of 2000 km along the paleoshoreline. Our results suggest that the fossil record of D. iris is consistent with no within species net accumulation of phyletic evolutionary change across morphological traits or the lifetime of this species. Correlations between some traits and paleoenvironmental conditions as well as changes in the coefficient of variation may support limited population-scale ecophenotypic plasticity, however where stratigraphic data are available, no directional changes in morphology occur prior to the Cretaceous/Paleogene (K/Pg) boundary. This is consistent with models of 'dynamic' evolutionary stasis. Combined with knowledge of life history traits and paleoecology of scaphitid ammonoids, specifically a short planktonic phase after hatching followed by transition to a nektobenthic adult stage, these data suggest that scaphitids had significant potential for rapid morphological change in conjunction with limited dispersal capacity. It is therefore likely that evolutionary mode in the Scaphitidae (and potentially across the broader ammonoid clade) follows a model of cladogenesis wherein a dynamic morphological stasis is periodically interrupted by more substantial evolutionary change at speciation events. Finally, the lack of temporal changes in our data suggest that global environmental changes (such as those possibly related to the emplacement of the Deccan Traps Large Igneous Province) had a limited effect on the morphology of North American ammonoid faunas during the latest Cretaceous prior to the K/Pg mass extinction event.Missing morphometric values are highlighted with NA in the dataset.Funding provided by: National Science FoundationCrossref Funder Registry ID: http://dx.doi.org/10.13039/100000001Award Number: 1924807Funding provided by: American Museum of Natural History and Richard Gilder Graduate School*Crossref Funder Registry ID: Award Number:We assembled a large morphometric dataset consisting of 328 individual fossil specimens of the scaphitid ammonoid cephalopod Discoscaphites iris collected from nine localities in Texas, Missouri, Mississippi, and New Jersey, representing a ~2000 km transect from SW to NE and encompassing the full geographic range of this species. Morphometric parameters were measured on well-preserved adult specimens of two dimorphs (Macroconchs - presumably the female, and microconch, presumably the male). We took up to seven morphometric measurements, and calculated ratios that captured the size, shape, and degree of compression of each of these ammonoid shells from each different locality. We evaluated the coefficient of variation (the standard deviation divided by the mean) for size and shape ratios as well as compression ratios at each locality. We used non-parametric statistical tests [Mann-Whitney U] to evaluate the significance of changes in mean morphological trait values between localities. To correct for multiple comparisons we applied a Bonferroni correction and also controlled for the false discovery rate. We also explored relationships between morphological traits and several environmental variables using linear modelling. All analyses were conducted in the R programming environment

    Prevalence, associated factors and outcomes of pressure injuries in adult intensive care unit patients: the DecubICUs study

Epidemiologic data is however unavailable. We aimed to provide an international picture of the extent of pressure injuries and factors associated with ICU-acquired pressure injuries in adult ICU patients. Methods: International 1-day point-prevalence study; follow-up for outcome assessment until hospital discharge (maximum 12 weeks). Factors associated with ICU-acquired pressure injury and hospital mortality were assessed by generalised linear mixed-effects regression analysis. Results: Data from 13,254 patients in 1117 ICUs (90 countries) revealed 6747 pressure injuries; 3997 (59.2%) were ICU-acquired. Overall prevalence was 26.6% (95% confidence interval [CI] 25.9–27.3). ICU-acquired prevalence was 16.2% (95% CI 15.6–16.8). Sacrum (37%) and heels (19.5%) were most affected. Factors independently associated with ICU-acquired pressure injuries were older age, male sex, being underweight, emergency surgery, higher Simplified Acute Physiology Score II, Braden score 3 days, comorbidities (chronic obstructive pulmonary disease, immunodeficiency), organ support (renal replacement, mechanical ventilation on ICU admission), and being in a low or lower-middle income-economy. Gradually increasing associations with mortality were identified for increasing severity of pressure injury: stage I (odds ratio [OR] 1.5; 95% CI 1.2–1.8), stage II (OR 1.6; 95% CI 1.4–1.9), and stage III or worse (OR 2.8; 95% CI 2.3–3.3). Conclusion: Pressure injuries are common in adult ICU patients. ICU-acquired pressure injuries are associated with mainly intrinsic factors and mortality. Optimal care standards, increased awareness, appropriate resource allocation, and further research into optimal prevention are pivotal to tackle this important patient safety threat

    Polyglandular autoimmune syndrome in adults: molecular genetic and clinical characteristics (lecture)

    Endocrine system disorders (diabetes mellitus, thyroid gland pathologies, polyglandular autoimmune syndromes, adrenal insufficiency, autoimmune ophthalmopathy and others) in development of which immune response disorders play an important role are among the most severe chronic human diseases prevention and early diagnostics of which are often hampered by disease clinical course latent phases. Polyglandular autoimmune syndromes (PAS) comprise damage of two or more endocrine glands characterized with multiple organ failure. In Russian classification the syndrome is divided into 2 main subtypes: PAS type 1 (the prevalence varies from 1:9 thousands to 1 per 200 thousands population) and PAS type 2 (the prevalence is 1.4–4.5 cases per 100 thousands population). Other classifications comprise hidden (latent and potential) forms of the disorder. Patients with PAS are a special cohort that requires careful monitoring and regular screening for prognosis and detection of new components of PAS. High prevalence and incidence rates, disabling complications, and reproductive losses – all of these are common in autoimmune endocrinopathies and are an important challenge at present. The lecture summarizes key clinical and diagnostic characteristics of PAS, proposes practical approach to these syndromes treatment, and specifies key laboratory markers for the disorder dynamics monitoring

    Problems of insufficient control of target blood pressure levels in the outpatient practice

    Aim. Assessment of the frequency of reaching the target level of blood pressure (BP) and the factors affecting it in outpatients with arterial hypertension (AH). Materials and methods. An open, one-stage, comparative study involving 64 patients with hypertension and 47 without hypertension at the age of 40 to 59 years. All patients underwent physical examination, assessment of cardiovascular risk (CVR), 24-hour blood pressure monitoring (ABPM), echocardiography (ECHOCG), color duplex scanning of brachiocephalic arteries. Results. Patients with hypertension and comparison groups were comparable in age, sex, smoking, history of myocardial infarction. 1st degree of hypertension was present in 26.6%, 2nd in 40.6%, 3rd in 12.5% of patients. Obesity was detected in 24.3% of patients and all patients with obesity had AH. The SCORE score in individuals with hypertension was 4.94.5; in the comparison group, 2.32.6 (p0.001). Non-stenosing atherosclerosis was present in 54.8% and 88%, p=0.020, and atherosclerotic plaque in the vascular lumen was present in 45.3% and 12% of patients with and without hypertension, respectively (p0.001). 68.8% were constantly treated, and the target BP was reached in 31.3% of patients with hypertension. Male gender (OR 1.68; 95% CI 1.6828.49; p=0.007), obesity (OR 4.78; 95% CI 1.1420.29; p=0.033), concomitant pathology (OR 3.09; 95% CI 1.029.37; p=0.046) were negative, and dyslipidemia (OR 0.10; 95% CI 0.010.84; p=0.033) was positive, affecting the achievement of the target level of blood pressure. Conclusion. The target level of blood pressure was achieved in 31.3% of outpatients with hypertension, mainly in women. Among patients who did not reach the target level of blood pressure, men, individuals with high SSR and obesity predominated. Concomitant pathology and obesity are negative, and dyslipidemia was positively associated with the achievement of the target level of blood pressure, which must be taken into account when developing measures for prevention and treatment
