    Luxatio Erecta Complicated By Anterior Shoulder Dislocation During Reduction

    Luxatio erecta humeri is an uncommon form of glenohumeral dislocation, resulting in the inferior displacement of the humeral head. Treatment with traction-counter traction techniques is usually successful in reducing most cases. We describe an unusual complication of this condition where initial reduction attempts of a luxatio erecta humeri repositioned the shoulder to an anterior dislocation position. After a thorough search of the literature, we were unable to find a similar case report of this type of complication during the reduction of a luxatio erecta shoulder dislocation

    Looking back and moving forward: A scoping review of research on preschool autism interventions in the field of speech-language pathology

    Background & Aims: Speech-language pathology services are frequently accessed by families of children who have suspected or diagnosed autism. This is expected given that social communication differences are a core feature of autism. This review looked broadly at the state of research in the field of speech-language pathology and preschool autism interventions in order to identify the types of studies that could be used to inform the practices of speech-language pathologists (SLPs), and to identify gaps in the field so they can be addressed in future research. Specifically, we examined the extent of research conducted on interventions delivered (at least in part) by SLPs to preschool children with suspected or diagnosed autism, identified the range of skill development areas targeted within the studies, and explored the characteristics of the interventions (i.e., theoretical models underlying the programs, service delivery models, treatment dosage). Methods: A scoping review of articles published between 1980 and 2019 was conducted using the five phases outlined by the Arksey and O’Malley framework: (a) articulating the research question; (b) identifying relevant studies; (c) selecting studies; (d) charting the data; and (e) collating, summarizing, and reporting the results. Main Contribution/Results: A total of 114 studies met inclusion criteria with most published since 2010 and conducted within North America. Case study or single-subject study designs were the most frequently used. Interventions delivered solely by SLPs and by multiprofessional teams that included SLPs were relatively equally represented. Across the included studies, nine skill development areas were targeted, but interventions targeting social communication, language, and augmentative communication skills made up the vast majority of studies. There was relatively even distribution of interventions informed by child-centered, clinician-directed, and hybrid models. Explicit information detailing intervention characteristics (e.g., treatment dosage, professional training of clinicians delivering the intervention) was poorly reported in many studies. For those studies providing details, there was a great deal of variability in the nature of interventions (e.g., service delivery models, SLPs’ role, dosage). Conclusions: This review revealed that research in the area of autism interventions delivered, at least in part, by SLPs has markedly increased over the past 10 years. Still, there remains a need for more research, and greater transparency detailing the nature of the interventions being investigated. The research conducted to date captures the versatility of the SLP’s role within preschool autism intervention. Improved reporting and studies with strong methodological rigor focused on capturing the complex and individualized nature of interventions are needed, as are intervention studies aligned with real-world community practice

    Building Strong Partnerships to Improve Clinically Oriented Teacher Preparation

    Educators and policymakers across the United States recognize a growing urgency to improve the nation's systems of teacher preparation. Ensuring that teachers stay and thrive in the profession depends largely on having system-wide policies and practices in place that address teacher shortages, promote equity and excellence, and cultivate expertise, diversity, and more.The California State University (CSU) system partnered with the S.D. Bechtel, Jr. Foundation to launch the New Generation of Educators Initiative (NGEI), in an effort to transform the nature and quality of teacher preparation at both individual CSU campuses and across the CSU system as a whole. To answer the question, "What does it take to transform teacher education?" WestEd and SRI International conducted an evaluation to examine and share learnings about the CSU-led effort to implement large-scale clinically oriented teacher preparation reform.As part of a series of new evaluation reports that explore key transformational elements of effective teacher preparation programs, this paper describes how participating CSU campuses and their partner school districts strengthened their relationships and developed strategic partnerships to establish the necessary foundations for high-quality, clinically oriented programming.This paper identifies four levers that can be operationalized in order to sustain strong partnerships between stakeholders:Lever 1: Create and operationalize a shared visionLever 2: Identify key rolesLever 3: Ensure space and time to collaborateLever 4: Share data to identify needs and monitor progres

    Structurable algebras and groups of type E_6 and E_7

    It is well-known that every algebraic group of type F_4 is the automorphism group of an exceptional Jordan algebra, and that up to isogeny all groups of type ^1E_6 with trivial Tits algebras arise as the isometry groups of norm forms of such Jordan algebras. We describe a similar relationship between groups of type E_6 and groups of type E_7 and use it to give explicit descriptions of the homogeneous projective varieties associated to groups of type E_7 with trivial Tits algebras. The underlying algebraic structure for the relationship considered here are a sort of 56-dimensional structurable algebra which are forms of an algebra constructed from an exceptional Jordan algebra.Comment: 35 pages, AMSLaTeX -- error in final section correcte

    RALFL34 regulates formative cell divisions in Arabidopsis pericycle during lateral root initiation

    We describe the role of RALFL34 during early events in lateral root development, and demonstrate its specific importance in orchestrating formative cell divisions in the pericycle.In plants, many signalling molecules, such as phytohormones, miRNAs, transcription factors, and small signalling peptides, drive growth and development. However, very few small signalling peptides have been shown to be necessary for lateral root development. Here, we describe the role of the peptide RALFL34 during early events in lateral root development, and demonstrate its specific importance in orchestrating formative cell divisions in the pericycle. Our results further suggest that this small signalling peptide acts on the transcriptional cascade leading to a new lateral root upstream of GATA23, an important player in lateral root formation. In addition, we describe a role for ETHYLENE RESPONSE FACTORs (ERFs) in regulatingRALFL34 expression. Taken together, we put forward RALFL34 as a new, important player in lateral root initiation

    Advance care planning in chronic kidney disease: a survey of current practice in Australia

    Aim: Advance care planning (ACP) in nephrology is widely advocated but not always implemented. The aims of this study were to describe current ACP practice and identify barriers/facilitators and perceived need for health professional education and chronic kidney disease (CKD)-specific approaches. Methods: An anonymous cross-sectional survey was administered online. Nephrology health professionals in Australia and New Zealandwere recruited via professional societies, email lists and nephrology conferences. Multiple regression explored the influence of respondents’ attributes on extent of involvement in ACP and willingness to engage in future. Results: A total of 375 respondents included nephrologists (23%), nurses (65%), social workers (4%) and others (8%) with 54% indicated that ACP at their workplace was performed ad hoc and 61% poorly. Perceived barriers included patient/family discomfort (84%), difficulty engaging families (83%), lack of clinician expertise (83%) and time (82%), health professional discomfort (72%), cultural/language barriers (65%), lack of private space (61%) and lack of formal policy/procedures (60%). Respondents overwhelmingly endorsed the need for more dialysis-specific ACP programs (96%) and education (95%). Whilst 85% thought ACP would be optimally performed by specially trained staff, comments emphasized that all clinicians should have a working proficiency. Respondents who were more willing to engage in future ACP tended to be non-physicians (odds ratio (OR) 4.96, 95% confidence intervals (CI) 1.74–14.07) and reported a greater need for CKD-specific ACP materials (OR 10.88, 95% CI 2.38–49.79). Conclusion: Advance care planning in nephrology needs support through education and CKD-specific resources. Endorsement by nephrologists is important. A multidisciplinary approach with a gradient of ACP expertise is also recommended

    We argue that the accumulated neutrino data, including recent results from KamLAND and K2K, point to a neutrino mixing matrix with (V_{11}, V_{21}, V_{31}; V_{21}, V_{22}, V_{32}; V_{13}, V_{23}, V_{33}) = (-2/\sqrt{6}, 1/\sqrt{6}, 1/\sqrt{6}; 1/\sqrt{3}, 1/\sqrt{3}, 1/\sqrt{3}; 0, 1/\sqrt{2}, -1/\sqrt{2}).     Submonolayer InGaAs/GaAs quantum dot solar cells

    Optical and structural properties of submonolayer InGaAs/GaAs quantum dot solar cells (SML-QDSCs) are investigated and compared with quantum well solar cells (QWSCs). Compared with InGaAs/GaAs QWSCs with a similar structure, the material quality for SML QDSCs is significantly improved with a reduced density of both crosshatch patterns and defects. This coincides with a much higher photoluminescence intensity obtained for SML QDSCs. SML QDSCs thus exhibit an increase in open circuit voltage of 70 meV and an improvement in short circuit current from 15.9 mA/cm2 to 17.7 mA/cm2 in comparison with QWSCs. These findings present a promising alternative to quantum wells in photovoltaic applications
