    An early chondrichthyan and the evolutionary assembly of a shark body plan

    Although relationships among the major groups of living gnathostomes are well established, the relatedness of early jawed vertebrates to modern clades is intensely debated. Here, we provide a new description of Gladbachus , a Middle Devonian (Givetian approx. 385-million-year-old) stem chondrichthyan from Germany, and one of the very few early chondrichthyans in which substantial portions of the endoskeleton are preserved. Tomographic and histological techniques reveal new details of the gill skeleton, hyoid arch and jaws, neurocranium, cartilage, scales and teeth. Despite many features resembling placoderm or osteichthyan conditions, phylogenetic analysis confirms Gladbachus as a stem chondrichthyan and corroborates hypotheses that all acanthodians are stem chondrichthyans. The unfamiliar character combination displayed by Gladbachus , alongside conditions observed in acanthodians, implies that pre-Devonian stem chondrichthyans are severely under-sampled and strongly supports indications from isolated scales that the gnathostome crown group originated at the latest by the early Silurian (approx. 440 Ma). Moreover, phylogenetic results highlight the likely convergent evolution of conventional chondrichthyan conditions among earliest members of this primary gnathostome division, while skeletal morphology points towards the likely suspension feeding habits of Gladbachus , suggesting a functional origin of the gill slit condition characteristic of the vast majority of living and fossil chondrichthyans. </jats:p

    Suzaku X-ray Spectra and Pulse Profile Variations during the Superorbital Cycle of LMC X-4

    We present results from spectral and temporal analyses of Suzaku and RXTE observations of the high mass X-ray binary LMC X-4. Using the full 13 years of available RXTE/ASM data, we apply the ANOVA and Lomb normalized Periodogram methods to obtain an improved superorbital period measurement of 30.32 +/- 0.04 days. The phase-averaged X-ray spectra from Suzaku observations during the high state of the superorbital period can be modeled in the 0.6--50 keV band as the combination of a power-law with Gamma ~ 0.6 and a high-energy cutoff at ~ 25 keV, a blackbody with kT_BB ~ 0.18 keV, and emission lines from Fe K_alpha, O VIII, and Ne IX (X Lyalpha). Assuming a distance of 50 kpc, The source has luminosity L_X ~ 3 x 10^38 ergs s^-1 in the 2--50 keV band, and the luminosity of the soft (blackbody) component is L_BB ~ 1.5 x 10^37 ergs s^-1. The energy resolved pulse profiles show single-peaked soft (0.5-1 keV) and hard (6-10 keV) pulses but a more complex pattern of medium (2-10 keV) pulses; cross-correlation of the hard with the soft pulses shows a phase shift that varies between observations. We interpret these results in terms of a picture in which a precessing disk reprocesses the hard X-rays and produces the observed soft spectral component, as has been suggested for the similar sources Her X-1 and SMC X-1.Comment: 13 emulateapj pages, 11 figures, 4 tables; accepted for publication in Ap

    The hepatoadrenal syndrome: A common yet unrecognized clinical condition

    Objective: Adrenal failure is common in critically ill patients, particularly those with sepsis. As liver failure and sepsis are both associated with increased circulating levels of endotoxin and proinflammatory mediators and reduced levels of apoprotein-1/ high-density lipoprotein, we postulated that adrenal failure may be common in patients with liver disease. Design: Clinical study. Setting: Liver transplant intensive care unit. Patients: The study cohort included 340 patients with liver disease. Interventions: Based on preliminary observational data, all patients admitted to our 28-bed liver transplant intensive care unit (LTICU) undergo adrenal function testing. An honest broker system was used to extract clinical, hemodynamic, medication, and laboratory data on patients admitted to the LTICU from March 2002 to March 2004. A random (stress) cortisol level <20 μg/dL in a highly stressed patient (respiratory failure, hypotension) was used to diagnose adrenal insufficiency. In all other patients, a random cortisol level <15 μg/dL or a 30-min level <20 μg/dL post-low-dose (1 μg) cosyntropin was considered diagnostic of adrenal insufficiency. Patients were grouped as follows: a) chronic liver failure; b) fulminant hepatic failure; c) patients immediately status post-orthotopic liver transplantation receiving a steroid-free protocol of immunosuppression; and d) patients status post-remote liver transplant (≥6 months). The decision to treat patients with stress doses of hydrocortisone was at the discretion of the treating intensivist and transplant surgeon. Measurements and Main Results: Two-hundred and forty-five (72%) patients met our criteria for adrenal insufficiency (the hepatoadrenal syndrome). Eight (33%) patients with fulminant hepatic failure, 97 (66%) patients with chronic liver disease, 31(61%) patients with a remote history of liver transplantation, and 109 (92%) patients who had undergone liver transplantation under steroid-free immunosuppression were diagnosed with adrenal insufficiency. The high-density lipoprotein level at the time of adrenal testing was the only variable predictive of adrenal insufficiency (p < .0001). In vasopressor-dependent patients with adrenal insufficiency, treatment with hydrocortisone was associated with a significant reduction (p = .02) in the dose of norepinephrine at 24 hrs, whereas the dose of norepinephrine was significantly higher (p = .04) in those patients with adrenal failure not treated with hydrocortisone. In vasopressor-dependent patients without adrenal insufficiency, treatment with hydrocortisone did not affect vasopressor dose at 24 hrs. One hundred and forty-one patients (26.4%) died during their hospitalization. The baseline serum cortisol was 18.8 ± 16.2 μg/dL in the nonsurvivors compared with 13.0 ± 11.8 μg/dL in the survivors (p < .001). Of those patients with adrenal failure who were treated with glucocorticoids, the mortality rate was 26% compared with 46% (p = .002) in those who were not treated. In those patients receiving vasopressor agents at the time of adrenal testing, the baseline cortisol was 10.0 ± 4.8 μg/dL in those with adrenal insufficiency compared with 35.6 ± 21.2 μg/dL in those with normal adrenal function. Vasopressor-dependent patients who did not have adrenal failure had a mortality rate of 75%. Conclusions: Patients with liver failure and patients post-liver transplantation have an exceedingly high incidence of adrenal failure, which may be pathophysiologically related to low levels of high-density lipoprotein. Treatment of patients with adrenal failure may improve outcome. High baseline serum cortisol levels may be a maker of disease severity and portend a poor prognosis.     A Variable-Density Absorption Event in NGC 3227 mapped with Suzaku and Swift

    The morphology of the circumnuclear gas accreting onto supermassive black holes in Seyfert galaxies remains a topic of much debate. As the innermost regions of Active Galactic Nuclei (AGN) are spatially unresolved, X-ray spectroscopy, and in particular line-of-sight absorption variability, is a key diagnostic to map out the distribution of gas. Observations of variable X-ray absorption in multiple Seyferts and over a wide range of timescales indicate the presence of clumps/clouds of gas within the circumnuclear material. Eclipse events by clumps transiting the line of sight allow us to explore the properties of the clumps over a wide range of radial distances from the optical/UV Broad Line Region (BLR) to beyond the dust sublimation radius. Time-resolved absorption events have been extremely rare so far, but suggest a range of density profiles across Seyferts. We resolve a weeks-long absorption event in the Seyfert NGC 3227. We examine six Suzaku and twelve Swift observations from a 2008 campaign spanning 5 weeks. We use a model accounting for the complex spectral interplay of three differently-ionized absorbers. We perform time-resolved spectroscopy to discern the absorption variability behavior. We also examine the IR-to-X-ray spectral energy distribution (SED) to test for reddening by dust. The 2008 absorption event is due to moderately-ionized (logξ1.21.4\log \xi\sim 1.2-1.4) gas covering 90% of the line of sight. We resolve the density profile to be highly irregular, in contrast to a previous symmetric and centrally-peaked event mapped with RXTE in the same object. The UV data do not show significant reddening, suggesting that the cloud is dust-free. The 2008 campaign has revealed a transit by a filamentary, moderately-ionized cloud of variable density that is likely located in the BLR, and possibly part of a disk wind.Comment: Accepted for publication by A&

    The evolution of the Compton thick fraction and the nature of obscuration for AGN in the Chandra Deep Field South

    (Abridged) We present the results from the X-ray spectral analysis of high-z AGN in the CDFS, making use of the new 4Ms data set and new X-ray spectral models from Brightman & Nandra, which account for Compton scattering and the geometry of the circumnuclear material. Our goals are to ascertain to what extent the torus paradigm of local AGN is applicable at earlier epochs and to evaluate the evolution of the Compton thick fraction (f_CT) with z, important for XRB synthesis models and understanding the accretion history of the universe. In addition to the torus models, we measure the fraction of scattered nuclear light, f_scatt known to be dependant on covering factor of the circumnuclear materal, and use this to aid in our understanding of its geometry. We find that the covering factor of the circumnuclear material is correlated with NH, and as such the most heavily obscured AGN are in fact also the most geometrically buried. We come to these conclusions from the result that f_scatt decreases as NH increases and from the prevalence of the torus model with the smallest opening angle as best fit model in the fits to the most obscured AGN. We find that a significant fraction of sources (~ 20%) in the CDFS are likely to be buried in material with close to 4 pi coverage having been best fit by the torus model with a 0\degree opening angle. Furthermore, we find 41 CTAGN in the CDFS using the new torus models, 29 of which we report here for the first time. We bin our sample by z in order to investigate the evolution of f_CT. Once we have accounted for biases and incompleteness we find a significant increase in the intrinsic f_CT, normalised to LX= 10^43.5 erg/s, from \approx 20% in the local universe to \approx 40% at z=1-4.Comment: Accepted for publication in MNRA

    Sibling relationships and family functioning in siblings of early adolescents, adolescents and young adults with autism spectrum disorder

    The purpose of the study was to investigate how family functioning (defined as the ability that family members hold to manage stressful events, and intimate and social relationships), the degree to which family members feel happy and fulfilled with each other (called family satisfaction), and the demographical characteristics of siblings (age and gender) impacted on sibling relationships. The Circumplex Model of Marital and Family Systems and Behavioral Systems constituted the theoretical frameworks that guided our study. Eighty-six typically developing adolescents and young adults having a sister or a brother with autism spectrum disorder were enrolled. Results indicated that the youngest age group (early adolescents) reported to engage more frequently in negative behaviors with their siblings with ASD than the two older age groups (middle adolescents and young adults). No significant differences were found among the three age groups regarding behaviors derived from attachment, caregiving and affiliative systems. Family satisfaction and age significantly predicted behaviors during sibling interactions. Suggestions on prevention and intervention programs were discussed in order to prevent parentification among typically developing siblings and decrease episodes of quarrels and overt conflicts between brothers and sisters with and without AS

    Noninvasive cardiac output and blood pressure monitoring cannot replace an invasive monitoring system in critically ill patients

    Background: Monitoring of cardiac output and blood pressure are standard procedures in critical care medicine. Traditionally, invasive techniques like pulmonary artery catheter (PAC) and arterial catheters are widely used. Invasiveness bears many risks of deleterious complications. Therefore, a noninvasive reliable cardiac output (CO) and blood pressure monitoring system could improve the safety of cardiac monitoring. The aim of the present study was to compare a noninvasive versus a standard invasive cardiovascular monitoring system. Methods: Nexfin HD is a continuous noninvasive blood pressure and cardiac output monitor system and is based on the development of the pulsatile unloading of the finger arterial walls using an inflatable finger cuff. During continuous BP measurement CO is calculated. We included 10 patients with standard invasive cardiac monitoring system (pulmonary artery catheter and arterial catheter) comparing invasively obtained data to the data collected noninvasively using the Nexfin HD. Results: Correlation between mean arterial pressure measured with the standard arterial monitoring system and the Nexfin HD was r2 = 0.67 with a bias of -2 mmHg and two standard deviations of ± 16 mmHg. Correlation between CO derived from PAC and the Nexfin HD was r2 = 0.83 with a bias of 0.23 l/min and two standard deviations of ± 2.1 l/min; the percentage error was 29%. Conclusion: Although the noninvasive CO measurement appears promising, the noninvasive blood pressure assessment is clearly less reliable than the invasively measured blood pressure. Therefore, according to the present data application of the Nexfin HD monitoring system in the ICU cannot be recommended generally. Whether such a tool might be reliable in certain critically ill patients remains to be determined
