    Food safety in solid-organ transplant recipients: a single-center qualitative study

    Infections represent an important source of morbidity and mortality in solid organ transplant recipients due to the state of immunosuppression associated with the use of anti-rejection treatments. Infection control and eviction of exposure risks is an important strategy for post-transplant management in these patients. Food-borne pathogens are one source of disease which is commonly targeted by strict guidelines on food avoidance and behaviors to adopt in order to minimize the risk of infection. However, these guidelines are not evidence-based, but are rather based on common sense, expert opinion, or knowledge of mechanisms of transmission of specific pathogens. Moreover, adherence of transplant patients to these guidelines is largely unexplored. Thus, it remains unknown if protection strategies are at all effective, or even if patients apply them in their daily lives. In this study, we used a self-report survey approach to ask whether 197 transplant patients treated the University Hospital of Lausanne (CHUV) applied the foodassociated behaviors prescribed by physicians, and whether sub-populations of patients could be differentiated by such behaviors. In particular, we asked if guideline adherence differed between recipients of different organs, or between patients at different time-points after transplantation, notably in the first year post-transplant or later. We found that over 90% of patients had consumed risk-associated food (RAF) on at least one occasion since the transplant, and that 74% reported occasional or frequent consumption of one or several RAFs. In contrast, hygiene recommendations were followed in a more reliable manner, with almost 70% reporting following them all of or most of the time. Finally, a series of hypothetical situations where participants were again tested on their general knowledge of risks associated with foods and behaviors to adopt showed that 71% of patients were unable to identify all situations where risk may be present. In general, higher levels of adherence to guidelines seemed to be associated with early post transplant period. In conclusion, transplant recipients frequently consume risk associated food, particularly late after transplantation and they follow appropriate measures of food hygiene. Whether this behavior is associated with increase infection rates needs to be further investigated

    Long-term pulse wave velocity outcomes with aerobic and resistance training in kidney transplant recipients - A pilot randomised controlled trial

    Background This pilot study examined long-term pulse wave velocity (PWV) and peak oxygen uptake (VO2peak) outcomes following a 12-week moderate-intensity aerobic or resistance training programme in kidney transplant recipients. Method Single-blind, bi-centre randomised controlled parallel trial. 42 out of 60 participants completed a 9-month follow-up assessment (Aerobic training = 12, Resistance training = 10 and usual care = 20). Participants completed 12 weeks of twice-weekly supervised aerobic or resistance training. Following the 12-week exercise intervention, participants were transitioned to self-managed community exercise activity using motivational interviewing techniques. Usual care participants received usual encouragement for physical activity during routine clinical appointments in the transplant clinic. PWV, VO2peak, blood pressure and body weight were assessed at 12 weeks and 12 months, and compared to baseline. Results ANCOVA analysis, covarying for baseline values, age, and length of time on dialysis pretransplantation, revealed a significant mean between-group difference in PWV of -1.30 m/ sec (95%CI -2.44 to -0.17, p = 0.03) between resistance training and usual care groups. When comparing the aerobic training and usual care groups at 9-month follow-up, there was a mean difference of -1.05 m/sec (95%CI -2.11 to 0.017, p = 0.05). A significant mean between-group difference in relative VO2peak values of 2.2 ml/kg/min (95% CI 0.37 to 4.03, p = 0.02) when comparing aerobic training with usual care was revealed. There was no significant between group differences in body weight or blood pressure. There were no significant adverse effects associated with the interventions. Conclusions Significant between-group differences in 9-month follow-up PWV existed when comparing resistance exercise intervention with usual care. A long-term between-group difference in VO2peak was only evident when comparing aerobic intervention with usual care. This pilot study, with a small sample size, did not aim to elucidate mechanistic mediators related to the exercise interventions. It is however suggested that a motivational interviewing approach, combined with appropriate transition to community training programmes, could maintain the improvements gained from the 12-week exercise interventions and further research in this area is therefore warranted. 2017 O'Connor et al.     Opportunities for marker-assisted selection (MAS) to improve the feed quality of crop residues in pearl millet and sorghum

    Cereal crop residues (straw, chaff, etc.) are important components of maintenance rations for ruminant livestock in many parts of the world. They are especially important in small-holder crop-livestock production systems in the sub-humid, semi-arid, and arid tropics and subtropics where most of the world's poorest livestock producers and consumers are found. Taking as examples tropically adapted cereals in the crop improvement mandate of the International Crops Research Institute for the Semi-Arid Tropics (ICRISAT), namely pearl millet [Pennisetum glaucum (L.) R. Br.] and sorghum [Sorghum bicolor (L.) Moench], this paper explores opportunities for using marker-assisted crop breeding methods to improve the quality and quantity of cereal crop residues for use as ruminant livestock feedstuffs. In the case of pearl millet, ICRISAT has been heavily involved with several UK-based collaborating research institutes, in development and initial application of the molecular genetic tools for marker-assisted breeding. We have obtained some useful experience in quantitative trait loci (QTL) mapping and marker-assisted selection (MAS) for stover yield, foliar disease resistance, and in vitro estimates of the nutritive value of various stover fractions for ruminants. In sorghum, ICRISAT has focused on initiating a large-scale high-throughput marker-assisted backcrossing program for the stay-green component of terminal drought tolerance - a trait that is likely to be associated not only with more stable grain and stover yield, but which is also expected to contribute to maintenance of ruminant nutritional value of stover produced under drought stress conditions. Conventional and marker-assisted breeding for foliar disease resistance is recommended for dual-purpose cereal improvement, or indeed for improvement of the nutritional value of residues for any crop in which these are used as feedstuffs for ruminant livestock.     1,4-addition of TMSCCl3to nitroalkenes: efficient reaction conditions and mechanistic understanding

    Improved synthetic conditions allow preparation of TMSCCl3 in good yield (70 %) and excellent purity. Compounds of the type NBu4X [X=Ph3SiF2 (TBAT), F (tetrabutylammonium fluoride, TBAF), OAc, Cl and Br] act as catalytic promoters for 1,4-additions to a range of cyclic and acyclic nitroalkenes, in THF at 0–25 °C, typically in moderate to excellent yields (37–95 %). TBAT is the most effective promoter and bromide the least effective. Multinuclear NMR studies (1H, 19F, 13C and 29Si) under anaerobic conditions indicate that addition of TMSCCl3 to TBAT (both 0.13 M) at −20 °C, in the absence of nitroalkene, leads immediately to mixtures of Me3SiF, Ph3SiF and NBu4CCl3. The latter is stable to at least 0 °C and does not add nitroalkene from −20 to 0 °C, even after extended periods. Nitroalkene, in the presence of TMSCCl3 (both 0.13 M at −20 °C), when treated with TBAT, leads to immediate formation of the 1,4-addition product, suggesting the reaction proceeds via a transient [Me3Si(alkene)CCl3] species, in which (alkene) indicates an Si⋅⋅⋅O coordinated nitroalkene. The anaerobic catalytic chain is propagated through the kinetic nitronate anion resulting from 1,4 CCl3− addition to the nitroalkene. This is demonstrated by the fact that isolated NBu4[CH2[DOUBLE BOND]NO2] is an efficient promoter.     An integrated genetic map and a new set of simple sequence repeat markers for pearl millet, Pennisetum glaucum

    Over the past 10 years, resources have been established for the genetic analysis of pearl millet, Pennisetum glaucum (L.) R. Br., an important staple crop of the semi-arid regions of India and Africa. Among these resources are detailed genetic maps containing both homologous and heterologous restriction fragment length polymorphism (RFLP) markers, and simple sequence repeats (SSRs). Genetic maps produced in four different crosses have been integrated to develop a consensus map of 353 RFLP and 65 SSR markers. Some 85% of the markers are clustered and occupy less than a third of the total map length. This phenomenon is independent of the cross. Our data suggest that extreme localization of recombination toward the chromosome ends, resulting in gaps on the genetic map of 30 cM or more in the distal regions, is typical for pearl millet. The unequal distribution of recombination has consequences for the transfer of genes controlling important agronomic traits from donor to elite pearl millet germplasm.     Structural properties governing drug-plasma protein binding determined by high-performance liquid chromatography method

    YesThe high-performance liquid chromatography (HPLC) method employing stationary phases immobilized with plasma proteins was used for this study to investigate the structural properties governing drug-plasma protein binding. A set of 65 compounds with a broad range of structural diversity (in terms of volume, hydrogen-bonding, polarity and electrostatic force) were selected for this purpose. The Abraham linear free energy relationship (LFER) analyses of the retention factors on the immobilized HSA (human serum albumin) and AGP (α1-acid glycoprotein) stationary phases showed that McGowan’s characteristic molecular volume (V), dipolarity/polarizability (S) and hydrogen bond basicity (B) are the three significant molecular descriptors of solutes determining the interaction with immobilized plasma proteins, whereas excess molar refraction (E) is less important and hydrogen bond acidity (A) is not of statistical significance in both systems, for electrically neutral compounds. It was shown that ionised acids, as carboxylate anions, bind very strongly to the immobilized HSA stationary phase and that ionised bases, as cations bind strongly to the AGP stationary phase. This is the first time that the effect of ionised species on plasma protein binding has been determined quantitatively; the increased binding of acids to HSA is due almost entirely to acids in their ionised form
