    Confirmation of symmetrical distributions of clinical attachment loss and tooth loss in a homogeneous Mexican adult male population

    Background/purpose To ascertain whether or not clinical attachment loss and tooth loss are present with similar severity and prevalence across the two sides of the mouth in a homogeneous sample of urban male adults. Materials and methods A cross-sectional study was carried out on 161 policemen (a largely homogeneous group in terms of ethnic background, socioeconomic status, sex, occupation, and medical/dental insurance) in Campeche, Mexico. Periodontal examinations were undertaken using the Florida Probe System in a dental chair by one trained and standardized examiner (kappa ≥ 0.60) to determine clinical attachment loss and tooth loss. We examined six sites in all teeth present in the mouth (a maximum of 168 sites, no third molars). Because of correlated data between observations, McNemar (for tooth loss) and Wilcoxon (for attachment loss) signed-rank tests were used to compare right and left sites within the same patient. Results The mean age was 38.4 ± 11.0 years. The mean number of teeth present was 24.4 ± 4.6; the mean number of periodontal sites/person was 146.7 ± 27.8. All P values were ≥ 0.05 (except for attachment loss in the upper first premolars), suggesting that there were no statistically significant differences between the right and left sides for the frequency of presentation of these two conditions. Conclusion Tooth loss and attachment loss measurements largely resemble each other on both sides of the mouth

    Connexin43 phosphorylation in brain, cardiac, endothelial and epithelial tissues

    AbstractGap junctions, composed of proteins from the connexin family, allow for intercellular communication between cells in essentially all tissues. There are 21 connexin genes in the human genome and different tissues express different connexin genes. Most connexins are known to be phosphoproteins. Phosphorylation can regulate connexin assembly into gap junctions, gap junction turnover and channel gating. Given the importance of gap junctions in development, proliferation and carcinogenesis, regulation of gap junction phosphorylation in response to wounding, hypoxia and other tissue insults is proving to be critical for cellular response and return to homeostasis. Connexin43 (Cx43) is the most widely and highly expressed gap junction protein, both in cell culture models and in humans, thus more research has been done on it and more reagents to it are available. In particular, antibodies that can report Cx43 phosphorylation status have been created allowing temporal examination of specific phosphorylation events in vivo. This review is focused on the use of these antibodies in tissue in situ, predominantly looking at Cx43 phosphorylation in brain, heart, endothelium and epithelium with reference to other connexins where data is available. These data allow us to begin to correlate specific phosphorylation events with changes in cell and tissue function.     Frequency of Tooth Brushing and Associated Factors in Mexican Schoolchildren Six to Nine Years of Age

    Objective: To determine the prevalence of daily tooth brushing and evaluate some variables associated. Subjects and Methods: A cross-sectional study was carried out in 320 schoolchildren six to nine years old in Campeche, Mexico. Information on sociodemographic and socio-economic variables, oral hygiene practices and attitudes were collected through a questionnaire. The frequency of tooth brushing was categorized as “0” = fewer than seven times/week, “1” = at least once a day. In the analysis, nonparametric tests were used. Results: Mean age was 6.99 ± 1.00 years, 52.5% were boys. The prevalence of daily tooth brushing was 81.6%. In bivariate analysis, the prevalence of tooth brushing was higher (p < 0.05) among the children of mothers with higher schooling (9.80 years vs 8.47 years, p < 0.05), and in younger children (84.6% in 6−7-year olds vs 71.2% in 8−9-year olds, p < 0.05). A slight, non-significant association (p < 0.10) was noted between the current frequency of tooth brushing and an earlier age when the child first started brushing with toothpaste. There were no statistically significant differences (p > 0.05) in the frequency of tooth brushing by gender or by the mother’s attitude toward the oral health of her child. Conclusions: The prevalence of daily tooth brushing was high compared to other studies. Mother’s maximum level of schooling (as an indicator of socio-economic position) was associated with higher frequency of tooth brushing. Maternal characteristics are associated with the oral health behaviour of their children

    Role of age and comorbidities in mortality of patients with infective endocarditis

    [Purpose]: The aim of this study was to analyse the characteristics of patients with IE in three groups of age and to assess the ability of age and the Charlson Comorbidity Index (CCI) to predict mortality. [Methods]: Prospective cohort study of all patients with IE included in the GAMES Spanish database between 2008 and 2015.Patients were stratified into three age groups:<65 years,65 to 80 years,and ≥ 80 years.The area under the receiver-operating characteristic (AUROC) curve was calculated to quantify the diagnostic accuracy of the CCI to predict mortality risk. [Results]: A total of 3120 patients with IE (1327 < 65 years;1291 65-80 years;502 ≥ 80 years) were enrolled.Fever and heart failure were the most common presentations of IE, with no differences among age groups.Patients ≥80 years who underwent surgery were significantly lower compared with other age groups (14.3%,65 years; 20.5%,65-79 years; 31.3%,≥80 years). In-hospital mortality was lower in the <65-year group (20.3%,<65 years;30.1%,65-79 years;34.7%,≥80 years;p < 0.001) as well as 1-year mortality (3.2%, <65 years; 5.5%, 65-80 years;7.6%,≥80 years; p = 0.003).Independent predictors of mortality were age ≥ 80 years (hazard ratio [HR]:2.78;95% confidence interval [CI]:2.32–3.34), CCI ≥ 3 (HR:1.62; 95% CI:1.39–1.88),and non-performed surgery (HR:1.64;95% CI:11.16–1.58).When the three age groups were compared,the AUROC curve for CCI was significantly larger for patients aged <65 years(p < 0.001) for both in-hospital and 1-year mortality. [Conclusion]: There were no differences in the clinical presentation of IE between the groups. Age ≥ 80 years, high comorbidity (measured by CCI),and non-performance of surgery were independent predictors of mortality in patients with IE.CCI could help to identify those patients with IE and surgical indication who present a lower risk of in-hospital and 1-year mortality after surgery, especially in the <65-year group

    La propuesta de este artículo es discutir el proceso de reforma en los servicios de salud puesta en práctica en México a partir de 1982 y que todavía está en curso. Ese proceso de reforma será estudiado como un fenómeno socio-político cuya dinámica y naturaleza son resultados de conflictos pasados y presentes que se articulan alrededor de proyectos técnico-asistenciales, expresión de determinadas visiones del futuro. El trabajo empezó por considerar la propia estructura de poder históricamente constituida a través de la acción del Estado en el sector salud la cual promovió a determinados grupos sociales, estimuló su desarrollo y los incorporó en sus estructuras de poder, al mismo tiempo que marginó y reprimió a otros. En un segundo momento, el trabajo reconstituyó empíricamente la 'historia social' de la reforma, esto es, la dinamización de los intereses y de los conflictos en torno de la mejor configuración del sector salud.<br>The article discusses the health care reforms that have been underway in Mexico since 1982. The process is examined as a socio-political phenomenon whose dynamics and nature are the products of past and present conflicts regarding technical and social-work projects, which constitute reflections of certain views of the future. The text first looks at the government-engendered power structure that has dominated the health care sector and that has benefited certain social groups, encouraged their development, and incorporated them into its power structures while simultaneously marginalizing or repressing others. The article next offers an empirical reconstruction of the "social history" of this reform, that is, of the dynamics of interests and conflicts surrounding the question of how the health care sector might best be shaped

    Avaliação nutricional antropométrica de gestantes brasileiras: uma revisão sistemática Anthropometric nutritional evaluation in pregnant Brazilian women: a systematic review

    Trata-se de uma revisão sistemática objetivando a identificação de métodos de avaliação nutricional de gestantes adotados no Brasil e sua associação com desfechos obstétricos. A busca bibliográfica foi feita nas bases Medline, Lilacs e Scielo, no período de 1980 a 2006. Os descritores utilizados em combinação foram pregnant, pregnancy nutritional assessment, anthropometric state, weight gain. Foram identificados 26 estudos, sendo o método proposto por Rosso (1985) o mais adotado. Alguns autores usaram apenas a avaliação do estado nutricional pré-gestacional com base no método proposto pelo Institute of Medicine (1990) ou a avaliação do ganho de peso gestacional segundo a Organização Mundial da Saúde (1995). Os resultados indicam a inexistência de metodologia apropriada para avaliação nutricional de gestantes brasileiras, o que pode estar contribuindo para a desvalorização desse procedimento nos serviços de assistência pré-natal. A maior parte dos estudos considerou o peso ao nascer como o principal desfecho do peso gestacional na validação dos métodos de classificação antropométrica. Os resultados encontrados no estudo demonstram a escassez de informações, em quantidade e qualidade, que possam contribuir para analisar a efetividade dos métodos de avaliação nutricional antropométrica para as gestantes brasileiras. O desenvolvimento de estudos com rigor metodológico neste campo é premente, e deverá contemplar as diferenças etárias e os fatores biológicos, socioeconômicos e ambientais das gestantes.<br>A systematic review was carried out of national and international literature on the available knowledge regarding nutritional assessment methods for anthropometric evaluation during pregnancy and its association with obstetric outcomes among Brazilian women was confirmed. Tracking resulted in 26 studies, where the most commonly used weight-gain assessment method was that proposed by Rosso (1985). Some of these used only pregestational nutritional status Institute of Medicine (1990) or weight gain during pregnancy assessments of World Health Organization (1995). The results indicate the lack of appropriate methods for assessing nutrition during pregnancy, which reflects the low importance prenatal services give to the standardization of anthropomorphic evaluation during pregnancy. There is an urgent need for well-defined methodological studies involving pregnant women that address age differences and intervening factors. Most researchers consider birth weight to be the main outcome of nutritional assessment during gestation to validate anthropometric methods. Outcomes encountered in this study demonstrate a lack of information, both quantitative and qualitative, to evaluate the effectiveness of nutritional assessment methods among pregnant Brazilian women. The development of studies using a rigorous methodology in this field is essential and these should address differences in terms of age and biological, socioeconomic and environmental factors
