    How have changes in the media landscape have affected the practice of reputation management? Public Relations Executive Tim Burt says that the proliferation of social media has greatly limited the amount of editorial control that can be exercised over the dissemination of information. Burt argues that social media have eroded the system of ā€œchecks and balancesā€ that historically governed the reporting process, a development that has (unsurprisingly) led to high levels of anxiety among CEOs. It has made it more difficult for Burt to manage the reputations of his corporate clientele, but he is also cautiously optimistic regarding how a diffuse media environment might increase democratic accountability.     Trapped: Experiences of unpaid carers of clinically vulnerable people "shielding" during the coronavirus disease 2019 pandemic

Available from SAGE Publications via the DOI in this record.ā€ÆSummary: Unpaid carers were profoundly impacted by the coronavirus disease 2019 pandemic and public health responses. In the UK, in March 2020, people identified as clinically extremely vulnerable and their household members were advised to ā€œshieldā€ for an initial 12-week period, which meant minimizing all contacts from outside the household and not leaving the house at all, unless in an emergency. In a modified form, shielding guidance remained in place until August 2020 and was reinstituted from December 2020 until April 1, 2021. This article, reporting on qualitative interviews with 47 unpaid carers in Wales, thematically analyzed using a coding framework, explores the experiences of unpaid carers affected by this shielding guidance and their wider implications for social work with unpaid carers in the future. Findings: Participants in our study described ways in which their caring role expanded, due to the need to provide additional practical and emotional support for loved ones who were shielding, and who lost access to other avenues of support. Some also described their caring role as becoming more involved and complex due to the declining health or self-care capacity of the person cared-for as a direct consequence of shielding restrictions. Alongside the increase in their caring responsibilities, carers reported losing access to important avenues of support for their own well-being. Applications: We draw on ecological systems theory to highlight the importance during care planning and management of exploring the carer's mesosystem to identify and optimize sustaining forces, and of attending to the microsystem involving the carer and person cared-for.Public Health Wale

    Movement from the double object construction is not fully symmetrical

    A movement asymmetry arises in some languages that are otherwise symmetrical for both A- and A-bar movement in the double object construction (DOC), including Norwegian, North-West British English, and a range of Bantu languages including Zulu and Lubukusu: a Theme object can be A-bar-moved out of a Recipient (Goal) passive, but not vice versa. Our explanation of this asymmetry is based on phase theory, more specifically a stricter version of the Phase Interpretability Condition proposed by Chomsky (2001). The effect is that, in a Theme passive, a Recipient object destined for the C-domain gets trapped within the lower V-related phase by movement of the Theme. The same effect is observed in Italian, a language in which only Theme passives are possible. Moreover, a similar effect is also found in some Bantu languages in connection with object marking/agreement: object agreement with the Theme in a Recipient passive is possible, but not vice versa. We show that this, too, can be understood within the theory that we articulate

    CRL4 antagonizes SCFFbxo7-mediated turnover of cereblon and BK channel to regulate learning and memory

    Intellectual disability (ID), one of the most common human developmental disorders, can be caused by genetic mutations in Cullin 4B (Cul4B) and cereblon (CRBN). CRBN is a substrate receptor for the Cul4A/B-DDB1 ubiquitin ligase (CRL4) and can target voltage- and calcium-activated BK channel for ER retention. Here we report that ID-associated CRL4CRBNmutations abolish the interaction of the BK channel with CRL4, and redirect the BK channel to the SCFFbxo7ubiquitin ligase for proteasomal degradation. Glioma cell lines harbouring CRBN mutations record density-dependent decrease of BK currents, which can be restored by blocking Cullin ubiquitin ligase activity. Importantly, mice with neuron-specific deletion of DDB1 or CRBN express reduced BK protein levels in the brain, and exhibit similar impairment in learning and memory, a deficit that can be partially rescued by activating the BK channel. Our results reveal a competitive targeting of the BK channel by two ubiquitin ligases to achieve exquisite control of its stability, and support changes in neuronal excitability as a common pathogenic mechanism underlying CRL4CRBNā€“associated ID

    Bioluminescent Imaging Reveals Divergent Viral Pathogenesis in Two Strains of Stat1-Deficient Mice, and in Ī±ĆŸĪ³ Interferon Receptor-Deficient Mice

    Pivotal components of the IFN response to virus infection include the IFN receptors (IFNR), and the downstream factor signal transducer and activator of transcription 1 (Stat1). Mice deficient for Stat1 and IFNR (Stat1āˆ’/āˆ’ and IFNĪ±ĆŸĪ³Rāˆ’/āˆ’ mice) lack responsiveness to IFN and exhibit high sensitivity to various pathogens. Here we examined herpes simplex virus type 1 (HSV-1) pathogenesis in Stat1āˆ’/āˆ’ mice and in IFNĪ±ĆŸĪ³Rāˆ’/āˆ’ mice following corneal infection and bioluminescent imaging. Two divergent and paradoxical patterns of infection were observed. Mice with an N-terminal deletion in Stat1 (129Stat1āˆ’/āˆ’ (N-term)) had transient infection of the liver and spleen, but succumbed to encephalitis by day 10 post-infection. In stark contrast, infection of IFNĪ±ĆŸĪ³Rāˆ’/āˆ’ mice was rapidly fatal, with associated viremia and fulminant infection of the liver and spleen, with infected infiltrating cells being primarily of the monocyte/macrophage lineage. To resolve the surprising difference between Stat1āˆ’/āˆ’ and IFNĪ±ĆŸĪ³Rāˆ’/āˆ’ mice, we infected an additional Stat1āˆ’/āˆ’ strain deleted in the DNA-binding domain (129Stat1āˆ’/āˆ’ (DBD)). These 129Stat1āˆ’/āˆ’ (DBD) mice recapitulated the lethal pattern of liver and spleen infection seen following infection of IFNĪ±ĆŸĪ³Rāˆ’/āˆ’ mice. This lethal pattern was also observed when 129Stat1āˆ’/āˆ’ (N-term) mice were infected and treated with a Type I IFN-blocking antibody, and immune cells derived from 129Stat1āˆ’/āˆ’ (N-term) mice were shown to be responsive to Type I IFN. These data therefore show significant differences in viral pathogenesis between two commonly-used Stat1āˆ’/āˆ’ mouse strains. The data are consistent with the hypothesis that Stat1āˆ’/āˆ’ (N-term) mice have residual Type I IFN receptor-dependent IFN responses. Complete loss of IFN signaling pathways allows viremia and rapid viral spread with a fatal infection of the liver. This study underscores the importance of careful comparisons between knockout mouse strains in viral pathogenesis, and may also be relevant to the causation of HSV hepatitis in humans, a rare but frequently fatal infection

    Symptom Dimensions in OCD: Item-Level Factor Analysis and Heritability Estimates

    To reduce the phenotypic heterogeneity of obsessive-compulsive disorder (OCD) for genetic, clinical and translational studies, numerous factor analyses of the Yale-Brown Obsessive Compulsive Scale checklist (YBOCS-CL) have been conducted. Results of these analyses have been inconsistent, likely as a consequence of small sample sizes and variable methodologies. Furthermore, data concerning the heritability of the factors are limited. Item and category-level factor analyses of YBOCS-CL items from 1224 OCD subjects were followed by heritability analyses in 52 OCD-affected multigenerational families. Item-level analyses indicated that a five factor model: (1) taboo, (2) contamination/cleaning, (3) doubts, (4) superstitions/rituals, and (5) symmetry/hoarding provided the best fit, followed by a one-factor solution. All 5 factors as well as the one-factor solution were found to be heritable. Bivariate analyses indicated that the taboo and doubts factor, and the contamination and symmetry/hoarding factor share genetic influences. Contamination and symmetry/hoarding show shared genetic variance with symptom severity. Nearly all factors showed shared environmental variance with each other and with symptom severity. These results support the utility of both OCD diagnosis and symptom dimensions in genetic research and clinical contexts.     <p>Abstract</p> <p>Background</p> <p>There is a need for novel approaches to the treatment of stimulant abuse and dependence. Clinical data examining the use of exercise as a treatment for the abuse of nicotine, alcohol, and other substances suggest that exercise may be a beneficial treatment for stimulant abuse, with direct effects on decreased use and craving. In addition, exercise has the potential to improve other health domains that may be adversely affected by stimulant use or its treatment, such as sleep disturbance, cognitive function, mood, weight gain, quality of life, and anhedonia, since it has been shown to improve many of these domains in a number of other clinical disorders. Furthermore, neurobiological evidence provides plausible mechanisms by which exercise could positively affect treatment outcomes. The current manuscript presents the rationale, design considerations, and study design of the National Institute on Drug Abuse (NIDA) Clinical Trials Network (CTN) CTN-0037 Stimulant Reduction Intervention using Dosed Exercise (STRIDE) study.</p> <p>Methods/Design</p> <p>STRIDE is a multisite randomized clinical trial that compares exercise to health education as potential treatments for stimulant abuse or dependence. This study will evaluate individuals diagnosed with stimulant abuse or dependence who are receiving treatment in a residential setting. Three hundred and thirty eligible and interested participants who provide informed consent will be randomized to one of two treatment arms: Vigorous Intensity High Dose Exercise Augmentation (DEI) or Health Education Intervention Augmentation (HEI). Both groups will receive TAU (i.e., usual care). The treatment arms are structured such that the quantity of visits is similar to allow for equivalent contact between groups. In both arms, participants will begin with supervised sessions 3 times per week during the 12-week acute phase of the study. Supervised sessions will be conducted as one-on-one (i.e., individual) sessions, although other participants may be exercising at the same time.
