    Efectos de la crisis en la desigualdad y la pobreza.

    El trabajo desarrollado hace un estudio de la evolución de datos macroeconómicos desde el inicio de la crisis económica. A su vez, hace referencia a la repercusión de los datos anteriormente nombrados en la sociedad española y sus diferentes ámbitos, como son el mercado laboral, la educación o la sanidad. Además, a lo largo del documento pueden verse los diferentes efectos en la población. Por otro lado, se definen las causas que han propiciado este período de recesión y las distintas políticas que han adoptado los gobiernos para paliar los efectos y poder salir de la crisis. Se reparará en estas políticas y en las reformas por parte del gobierno que han desencadenado un deterioro en el Estado del Bienestar español y un aumento de las desigualdades: tasa de paro juvenil elevada, alta tasa de paro de paro de personas con estudios primarios, aumento del porcentaje de hogares sin ingresos… La salida de la crisis ha tenido un alto coste social, lo que supone que en los próximos años sea necesaria una mejora en este sentido. Tras este periodo de recesión, España se sitúa entre los países desarrollados con mayor desigualdad.     Self-Regulation and Rubrics Assessment in Structural Engineering Subjects

    Present-day university systems need to educate graduates who are confident and highly independent, attributes that are especially relevant to engineering. We need to develop active methods that can analyze the prior knowledge of students and that impart teaching based on self-regulation and self-assessment by the student. In this study, we work with a sample of 116 students of architecture following a Structural Engineering subject module (61 in the experimental group and 55 in the control group). The objectives of the investigation are (1) to test whether significant differences exist in the knowledge of students after a training program in self-regulation and (2) to test whether the use of rubrics will improve the perceptions of students with regard to their own knowledge. We found that students trained in self-regulation methodologies improved their procedural knowledge in the field of structural engineering. Likewise, student self-perceptions of their own knowledge increased in relation to the design and expert assessment of structural elements and the graphic representation of constructive elements

    Conocimientos y tendencias actuales sobre la aspergilosis broncopulmonar alérgica

    Introducción: La aspergilosis broncopulmonar alérgica es una enfermedad pulmonar inflamatoria, caracterizada por una reacción de hipersensibilidad a la colonización de la vía aérea por Aspergillus fumigatus en individuos susceptibles, generalmente pacientes con asma y fibrosis quística. Objetivo: Actualizar los conocimientos en el diagnóstico y manejo de la aspergilosis broncopulmonar alérgica. Desarrollo: Se realizó una revisión bibliográfica temática, observacional y retrospectiva, de diciembre de 2019 a julio de 2020. Se accedió a bases de datos como PubMed/MEDLINE, SciElo, ReserchGate, Scopus, ScienceDirect, Redalyc, Latindex, Wed of Science y LILACS, usando los descriptores en Ciencias de la Salud (Aspergillus fumigatus, aspergilosis broncopulmonar alérgica, hipersensibilidad, allergic bronchopulmonary aspergillosis, aspergilose broncopulmonar alérgica, pulmonary aspergillosis, aspergilose pulmonar, hypersensitivity, hipersensibilidade). Se incluyeron artículos escritos en español, inglés o portugués, que informaron sobre la temática relacionada con aspergilosis broncopulmonar alérgica; seleccionados por la lectura del título, el resumen, el texto completo y por la evaluación de las variables (tipos de diseño metodológico, aspectos metodológicos generales propios de cada uno de los diseños encontrados y calidad de la investigación). Con estos criterios, fueron seleccionados 36 artículos nacionales e internacionales que incluyeron artículos originales, revisiones bibliográficas, estudios de intervención, revistas y anuarios estadísticos. Conclusiones: La aspergilosis broncopulmonar alérgica es mucho más frecuente de lo que se suponía, pero con asiduidad se subdiagnostica y el diagnóstico se hace tardíamente, lo que empeora el pronóstico de estos pacientes. Establecer el diagnóstico y tratamiento oportuno ayudará prevenir el desarrollo de serias complicaciones

    Historical and ecological drivers of the spatial pattern of Chondrichthyes species richness in the Mediterranean Sea

    Chondrichthyes, which include Elasmobranchii (sharks and batoids) and Holocephali (chimaeras), are a relatively small group in the Mediterranean Sea (89 species) playing a key role in the ecosystems where they are found. At present, many species of this group are threatened as a result of anthropogenic effects, including fishing activity. Knowledge of the spatial distribution of these species is of great importance to understand their ecological role and for the efficient management of their populations, particularly if affected by fisheries. This study aims to analyze the spatial patterns of the distribution of Chondrichthyes species richness in the Mediterranean Sea. Information provided by the studied countries was used to model geographical and ecological variables affecting the Chondrichthyes species richness. The species were distributed in 16 Operational Geographical Units (OGUs), derived from the Geographical Sub-Areas (GSA) adopted by the General Fisheries Commission of the Mediterranean Sea (GFCM). Regression analyses with the species richness as a target variable were adjusted with a set of environmental and geographical variables, being the model that links richness of Chondrichthyes species with distance to the Strait of Gibraltar and number of taxonomic families of bony fishes the one that best explains it. This suggests that both historical and ecological factors affect the current distribution of Chondrichthyes within the Mediterranean Sea.Postprin

    Intrapericardial Administration of Secretomes from Menstrual Blood-Derived Mesenchymal Stromal Cells: Effects on Immune-Related Genes in a Porcine Model of Myocardial Infarction.

    Acute myocardial infarction (AMI) is a manifestation of ischemic heart disease where the immune system plays an important role in the re-establishment of homeostasis. We hypothesize that the anti-inflammatory activity of secretomes from menstrual blood-derived mesenchymal stromal cells (S-MenSCs) and IFNγ/TNFα-primed MenSCs (S-MenSCs*) may be considered a therapeutic option for the treatment of AMI. To assess this hypothesis, we have evaluated the effect of S-MenSCs and S-MenSCs* on cardiac function parameters and the involvement of immune-related genes using a porcine model of AMI. Twelve pigs were randomly divided into three biogroups: AMI/Placebo, AMI/S-MenSCs, and AMI/S-MenSCs*. AMI models were generated using a closed chest coronary occlusion-reperfusion procedure and, after 72 h, the different treatments were intrapericardially administered. Cardiac function parameters were monitored by magnetic resonance imaging before and 7 days post-therapy. Transcriptomic analyses in the infarcted tissue identified 571 transcripts associated with the Gene Ontology term Immune response, of which 57 were differentially expressed when different biogroups were compared. Moreover, a prediction of the interactions between differentially expressed genes (DEGs) and miRNAs from secretomes revealed that some DEGs in the infarction area, such as STAT3, IGFR1, or BCL6 could be targeted by previously identified miRNAs in secretomes from MenSCs. In conclusion, the intrapericardial administration of secretome early after infarction has a significant impact on the expression of immune-related genes in the infarcted myocardium.     El objetivo principal del proyecto “Physico-chemical, biological and geological study of an underwater volcano in a degassing stage: Island of El Hierro”, (VULCANO-II) es estudiar, desde un punto de vista totalmente interdisciplinar, la fase de desgasificación activa del único volcán submarino monitoreado desde su nacimiento en aguas españolas. De esta forma, se pretende además, dar continuidad a los estudios multidisciplinares realizados sobre el volcán submarino de la isla de El Hierro en el contexto del proyecto del Plan Nacional VULCANO-I, (CTM2012-36317) y VULCANA (Vulcanología Canaria Submarina, IEO).     The evolution of the ventilatory ratio is a prognostic factor in mechanically ventilated COVID-19 ARDS patients

    Background: Mortality due to COVID-19 is high, especially in patients requiring mechanical ventilation. The purpose of the study is to investigate associations between mortality and variables measured during the first three days of mechanical ventilation in patients with COVID-19 intubated at ICU admission. Methods: Multicenter, observational, cohort study includes consecutive patients with COVID-19 admitted to 44 Spanish ICUs between February 25 and July 31, 2020, who required intubation at ICU admission and mechanical ventilation for more than three days. We collected demographic and clinical data prior to admission; information about clinical evolution at days 1 and 3 of mechanical ventilation; and outcomes. Results: Of the 2,095 patients with COVID-19 admitted to the ICU, 1,118 (53.3%) were intubated at day 1 and remained under mechanical ventilation at day three. From days 1 to 3, PaO2/FiO2 increased from 115.6 [80.0-171.2] to 180.0 [135.4-227.9] mmHg and the ventilatory ratio from 1.73 [1.33-2.25] to 1.96 [1.61-2.40]. In-hospital mortality was 38.7%. A higher increase between ICU admission and day 3 in the ventilatory ratio (OR 1.04 [CI 1.01-1.07], p = 0.030) and creatinine levels (OR 1.05 [CI 1.01-1.09], p = 0.005) and a lower increase in platelet counts (OR 0.96 [CI 0.93-1.00], p = 0.037) were independently associated with a higher risk of death. No association between mortality and the PaO2/FiO2 variation was observed (OR 0.99 [CI 0.95 to 1.02], p = 0.47). Conclusions: Higher ventilatory ratio and its increase at day 3 is associated with mortality in patients with COVID-19 receiving mechanical ventilation at ICU admission. No association was found in the PaO2/FiO2 variation

    Treatment with tocilizumab or corticosteroids for COVID-19 patients with hyperinflammatory state: a multicentre cohort study (SAM-COVID-19)

    Objectives: The objective of this study was to estimate the association between tocilizumab or corticosteroids and the risk of intubation or death in patients with coronavirus disease 19 (COVID-19) with a hyperinflammatory state according to clinical and laboratory parameters. Methods: A cohort study was performed in 60 Spanish hospitals including 778 patients with COVID-19 and clinical and laboratory data indicative of a hyperinflammatory state. Treatment was mainly with tocilizumab, an intermediate-high dose of corticosteroids (IHDC), a pulse dose of corticosteroids (PDC), combination therapy, or no treatment. Primary outcome was intubation or death; follow-up was 21 days. Propensity score-adjusted estimations using Cox regression (logistic regression if needed) were calculated. Propensity scores were used as confounders, matching variables and for the inverse probability of treatment weights (IPTWs). Results: In all, 88, 117, 78 and 151 patients treated with tocilizumab, IHDC, PDC, and combination therapy, respectively, were compared with 344 untreated patients. The primary endpoint occurred in 10 (11.4%), 27 (23.1%), 12 (15.4%), 40 (25.6%) and 69 (21.1%), respectively. The IPTW-based hazard ratios (odds ratio for combination therapy) for the primary endpoint were 0.32 (95%CI 0.22-0.47; p < 0.001) for tocilizumab, 0.82 (0.71-1.30; p 0.82) for IHDC, 0.61 (0.43-0.86; p 0.006) for PDC, and 1.17 (0.86-1.58; p 0.30) for combination therapy. Other applications of the propensity score provided similar results, but were not significant for PDC. Tocilizumab was also associated with lower hazard of death alone in IPTW analysis (0.07; 0.02-0.17; p < 0.001). Conclusions: Tocilizumab might be useful in COVID-19 patients with a hyperinflammatory state and should be prioritized for randomized trials in this situatio
