    Cefalea relacionada con la actividad sexual en una unidad de cefaleas

    La cefalea relacionada con la actividad sexual es un tipo de cefalea clasificada como primaria por la International Headache Society. Se encuadra en el grupo de “miscelánea”, junto con la cefalea relacionada con el ejercicio físico y la relacionada con la tos. Se define como cefalea que se desencadena durante el coito o masturbación. Pretendemos estudiar la frecuencia y características de la Cefalea relacionada con la actividad sexual en una Unidad de Cefaleas. Se consideraron todos los pacientes incluidos en el Registro general de la Unidad de Cefaleas del Servicio de Neurología del Hospital Clínico de Valladolid. Se elaboró un cuaderno de recogida de datos para analizarlos de forma retrospectiva. Incluimos un total de 19 pacientes. Encontramos una distribución por sexos similar con ligero predominio de los varones. Persistencia del trastorno de 9,16 meses. La duración media de cada episodio de cefalea fue de 266 minutos, siendo su localización más frecuente la frontal. La cefalea se desencadenaba en un 26,3% de los casos por la excitación, y en un 78,9% por el orgasmo. El 31,5% de los pacientes tenía además cefalea relacionada con el esfuerzo físico y un 5,2% asociada a maniobras de Valsalva. El carácter de la cefalea era mayoritariamente pulsátil y generalmente bilateral. La intensidad media de cada episodio fue valorada por nuestros pacientes con un 8,4 en una escala analógica verbal. En el 76,3% de los casos la cefalea se manifestó sindrómicamente como una “cefalea en trueno”. Conclusión: La cefalea relacionada con la actividad sexual es infrecuente, aún en un registro de cefaleas de un hospital terciario. Las características de los pacientes de nuestra serie son comparables a las previamente descritas en la literatura.Grado en Medicin

    Mathematical modelling of the waning of anti-RBD IgG SARS-CoV-2 antibody titers after a two-dose BNT162b2 mRNA vaccination

    BackgroundAfter exposure to SARS-CoV-2 and/or vaccination there is an increase in serum antibody titers followed by a non-linear waning. Our aim was to find out if this waning of antibody titers would fit to a mathematical model.MethodsWe analyzed anti-RBD (receptor binding domain) IgG antibody titers and the breakthrough infections over a ten-month period following the second dose of the mRNA BNT162b2 (Pfizer-BioNtech.) vaccine, in a cohort of 54 health-care workers (HCWs) who were either never infected with SARS-CoV-2 (naïve, nHCW group, n=27) or previously infected with the virus (experienced, eHCW group, n=27). Two mathematical models, exponential and power law, were used to quantify antibody waning kinetics, and we compared the relative quality of the goodness of fit to the data between both models was compared using the Akaik Information Criterion.ResultsWe found that the waning slopes were significantly more pronounced for the naïve when compared to the experienced HCWs in exponential (p-value: 1.801E-9) and power law (p-value: 9.399E-13) models. The waning of anti-RBD IgG antibody levels fitted significantly to both exponential (average-R2: 0.957 for nHCW and 0.954 for eHCW) and power law (average-R2: 0.991 for nHCW and 0.988 for eHCW) models, with a better fit to the power law model. In the nHCW group, titers would descend below an arbitrary 1000-units threshold at a median of 210.6 days (IQ range: 74.2). For the eHCW group, the same risk threshold would be reached at 440.0 days (IQ range: 135.2) post-vaccination.ConclusionTwo parsimonious models can explain the anti-RBD IgG antibody titer waning after vaccination. Regardless of the model used, eHCWs have lower waning slopes and longer persistence of antibody titers than nHCWs. Consequently, personalized vaccination booster schedules should be implemented according to the individual persistence of antibody levels

    COVID-19 : Age, Interleukin-6, C-reactive protein, and lymphocytes as key clues from a multicentre retrospective study

    Background: The SARS-CoV-2 infection has widely spread to become the greatest public health challenge to date, the COVID-19 pandemic. Different fatality rates among countries are probably due to non-standardized records being carried out by local health authorities. The Spanish case-fatality rate is 11.22%, far higher than those reported in Asia or by other European countries. A multicentre retrospective study of demographic, clinical, laboratory and immunological features of 584 Spanish COVID-19 hospitalized patients and their outcomes was performed. The use of renin-angiotensin system blockers was also analysed as a risk factor. Results: In this study, 27.4% of cases presented a mild course, 42.1% a moderate one and for 30.5% of cases, the course was severe. Ages ranged from 18 to 98 (average 63). Almost 60 % (59.8%) of patients were male. Interleukin 6 was higher as severity increased. On the other hand, CD8 lymphocyte count was significantly lower as severity grew and subpopulations CD4, CD8, CD19, and NK showed concordant lowering trends. Severity-related natural killer percent descents were evidenced just within aged cases. A significant severity-related decrease of CD4 lymphocytes was found in males. The use of angiotensin-converting enzyme inhibitors was associated with a better prognosis. The angiotensin II receptor blocker use was associated with a more severe course. Conclusions: Age and age-related comorbidities, such as dyslipidaemia, hypertension or diabetes, determined more frequent severe forms of the disease in this study than in previous literature cohorts. Our cases are older than those so far reported and the clinical course of the disease is found to be impaired by age. Immunosenescence might be therefore a suitable explanation for the hampering of immune system effectors. The adaptive immunity would become exhausted and a strong but ineffective and almost deleterious innate response would account for COVID-19 severity. Angiotensin-converting enzyme inhibitors used by hypertensive patients have a protective effect in regards to COVID-19 severity in our series.     RESUMEN: El estudio tecnológico del material cerámico requiere de la aplicación de técnicas de análisis arqueométrico que permitan un acercamiento a su mineralogía y composición química, con la fi nalidad de identifi car las etapas de su secuencia de elaboración. Este artículo presenta el análisis tecnológico de la cerámica neolítica documentada en los niveles inferiores del yacimiento de Kobaederra (Cortézubi, Bizkaia), a partir de su análisis mineralógico (petrografía y difracción de rayos X, DRX) y geoquímico (microscopio electrónico de barrido-espectómetro de dispersión de energía, MEB-EDS). Su objetivo es discriminar las potenciales áreas de aprovisionamiento de las materias primas y su modifi cación mediante la adición de desgrasantes. Por último, se discuten las implicaciones de los resultados obtenidos en relación con el resto de las evidencias arqueológicas disponibles para los niveles IV y III de Kobaederra.ABSRTACT: The technological study of ceramic materials requires the application of archaeometric analytical techniques to approach both their mineralogy and chemical composition, with the aim of identifying steps in their production sequence. This paper presents the technological analysis of the Neolithic pottery documented in the lower levels of the Kobaederra site (Cortézubi, Bizkaia) on the basis of their mineralogical (petrography and X-ray diffraction, XRD) and geochemical (SEM-EDS) analysis. Its goal is to discriminate the possible areas of raw materials supply and their modifi cation through the addition of tempers. Finally, the implications of the results in relation to the rest of available archaeological evidences from the IV and the III levels of the Kobaederra site are discussed

    Estudio cycEVA: casos y controles para la estimación de la efectividad de la vacuna antigripal en España, 2008-2013

    Fundamentos: Desde 2008-09 la efectividad de la vacuna (EV) antigripal en España se estima con el estudio de casos y controles para la evaluación de la EV antigripal (cycEVA), componente español de la red europea (Influenza-Monitoring Vaccine Effectiveness (I-MOVE). El objetivo es describir la evolución del estudio cycEVA durante las cinco temporadas del período 2008/09– 2012/13. Métodos: Se analizaron los siguientes indicadores: 1) participación de los médicos/pediatras centinela (MP); 2) población y periodo de estudio, 3) calidad de los datos y 4) difusión de los resultados mediantes publicaciones. Se calculó el porcentaje anual de cambio constante de los indicadores analizándose su tendencia mediante el test de Cochran-Armitage. Resultados: El número de MP participantes aumentó de 164 en 2008-09 hasta 246 en ediciones posteriores. El porcentaje de médicos que reclutaron al menos un paciente experimentó un cambio anual significativo (PCA) del 15,33%. El porcentaje de pacientes reclutados incluidos en el análisis aumen- tó del 77% en 2008-09 a más del 95% en las siguientes ediciones (PCA=5,91%). El porcentaje de casos y controles participantes en cycEVA sobre el total de pacientes que contribuyeron al estudio europeo I-MOVE osciló entre el 23% en la edición piloto y 30% en la temporada 2011-12. Los resultados finales se difundieron en revistas científicas con un factor de impacto situado en el cuartil 2 y en 2010-11 y 2011-12 se publicaron resultados preliminares en revistas con un factor de impacto situado en el cuartil 1 (97 citas). Conclusiones: La experiencia del estudio cycEVA se reflejó en una mejora en la oportunidad e impacto de sus resultados, cruciales para orientar las recomendaciones anuales de vacunación antigripal

    Identification of myocilin as a blood plasma protein and analysis of its role in leukocyte adhesion to endothelial cell monolayers.

    Myocilin is an extracellular glycoprotein with a poorly understood biological function and typically known because of its association with glaucoma. In this study, we analyzed the expression and biological activity of human myocilin in some non-ocular tissues. Western immunoblot showed the presence of myocilin in blood plasma as well as in liver and lymphoid tissues (thymus and lymph node). Quantitative PCR confirmed the expression of MYOC in these lymphoid organs and revealed that its mRNA is also present in T-lymphocytes and leukocytes. In addition, detection of 30 kDa C-terminal myocilin fragments in thymus and liver suggested that myocilin undergoes an in vivo proteolytic processing that might regulate its biological activity. The presence of myocilin in blood was further corroborated by peptide mass fingerprinting of the HPLC-isolated protein, and gross estimation of its concentration by Western immunoblot indicated that it is a medium-abundance serum protein with an approximate concentration of 0.85 mg/ml (15.5 μM). Finally, in vitro analyses indicated that myocilin acts as an anti-adhesive protein for human circulating leukocytes incubated with endothelial cell monolayers. Altogether, these data provide insightful information on new biological properties of myocilin and suggest its putative role as a blood matricellular protein
