Observations of ozone and related species in the northeast Pacific during the PHOBEA campaigns 2. Airborne observations
During late March and April of 1999 the University of Wyoming's King Air research aircraft measured atmospheric concentrations of NO, O3, peroxyacetyl nitrate (PAN), CO, CH4, VOCs, aerosols, and J(NO2) off the west coast of the United States. During 14 flights, measurements were made between 39°-48° N latitude, 125°-129° W longitude, and at altitudes from 0-8 km. These flights were part of the Photochemical Ozone Budget of the Eastern North Pacific Atmosphere (PHOBEA) experiment, which included both ground-based and airborne measurements. Flights were scheduled when meteorological conditions minimized the impact of local pollution sources. The resulting measurements were segregated by air mass source region as indicated by back isentropic trajectory analysis. The chemical composition of marine air masses whose 5-day back isentropic trajectories originated north of 40° N latitude or west of 180° W longitude (WNW) differed significantly from marine air masses whose 5-day back isentropic trajectories originated south of 40° N latitude and east of 180° W longitude (SW). Trajectory and chemical analyses indicated that the majority of all encountered air masses, both WNW and SW, likely originated from the northwestern Pacific and have characteristics of emissions from the East Asian continental region. However, air masses with WNW back trajectories contained higher mixing ratios of NO, NOx, O3, PAN, CO, CH4, various VOC pollution tracers, and aerosol number concentration, compared to those air masses with SW back trajectories. Calculations of air mass age using two separate methods, photochemical and back trajectory, are consistent with transport from the northwestern Pacific in 8-10 days for air masses with WNW back trajectories and 16-20 days for air masses with SW back trajectories. Correlations, trajectory analysis, and comparisons with measurements made in the northwestern Pacific during NASA's Pacific Exploritory Mission-West Phase B (PEM-West B) experiment in 1994 are used to investigate the data. These analyses provide evidence that anthropogenically influenced air masses from the northwestern Pacific affect the overall chemical composition of the northeastern Pacific troposphere. Consolidation of pathology services in England: have savings been achieved?
Background: During the last decade, pathology services in England have undergone profound changes with an extensive consolidation of laboratories. This has been driven by some national reviews forecasting a national reduction of costs by £250–£500 million (630 million) a year as a result.
The main aim of this paper is to describe the financial impact of such consolidation, with a specific focus on the forecasted savings. A secondary aim is to describe the development of private sector involvement in laboratory services in a traditionally publicly funded healthcare system and the development of pathology staff size.
Methods: In the English scenario, the majority of hospitals and laboratories are publicly funded and a survey was sent as Freedom of Information request to all directors of pathology. A descriptive comparison of savings among consolidated and non-consolidated pathology services was made by using the pathology budgets in two different periods (2015 versus 2010), adjusted by inflation and increased activity.
Results: The hub-and-spoke model has been implemented as part of the consolidation process of pathology services in England. Consolidated pathology networks have achieved higher savings compared to non-consolidated single laboratories. There has been an increased role of private providers and savings were achieved with negligible personnel redundancies.
Conclusions: Consolidated units have on average achieved larger cost savings than non-consolidated units but further analysis with stronger research design is required to independently evaluate the impact of pathology consolidation on both savings and quality
Minimal renal toxicity after Rituximab DHAP with a modified cisplatin application scheme in patients with relapsed or refractory diffuse large B-cell lymphoma
Background: Rituximab (R) in combination with DHAP is a widely accepted salvage regimen for patients with relapsed or refractory diffuse large B-cell lymphoma (DLBCL). A common adverse effect of this protocol is renal toxicity which may result in treatment discontinuation. Assuming that a lower single dose of cisplatin over several days would reduce renal toxicity, our institution has chosen to administer cisplatin in a dosage of 25Â mg/m2 per day as a 3-h infusion over 4 consecutive days. Methods: In this study, we analyzed the renal function of 122 patients with relapsed or refractory DLBCL treated with R-DHAP at our institution. Overall, 256 R-DHAP cycles were administered. 31 (25Â %), 61 (50Â %), 14 (12Â %) and 16 (13Â %) patients received one, two, three or four R-DHAP courses, respectively. Results: A glomerular filtration rate (GFR) decrease was observed after each R-DHAP cycle. However, in none of the subgroups the median GFR was lower than 60Â ml/min/1.73Â m2. In most patients, only renal impairment stage I and II was observed. Renal impairment stage III was seen in 10Â % and stage IV only in 1Â % of patients. Conclusion: We conclude that a modified R-DHAP regimen with administration of cisplatin 25Â mg/m2 over 4 consecutive cycles leads only to minimal renal toxicity
Having a lot of a good thing: multiple important group memberships as a source of self-esteem.

We propose and test an identity resource model in which personal self-esteem is boosted by membership in additional important social groups. Belonging to multiple important group memberships predicts personal self-esteem in children (Study 1a), older adults (Study 1b), and former residents of a homeless shelter (Study 1c). Study 2 shows that the effects of multiple important group memberships on personal self-esteem are not reducible to number of interpersonal ties. Studies 3a and 3b provide longitudinal evidence that multiple important group memberships predict personal self-esteem over time. Studies 4 and 5 show that collective self-esteem mediates this effect, suggesting that membership in multiple important groups boosts personal self-esteem because people take pride in, and derive meaning from, important group memberships. Discussion focuses on when and why important group memberships act as a social resource that fuels personal self-esteem.This study was supported by 1. scattering techniques have been used to observe the
diffusive regime of double-diffusive convection in the
laboratory. Pulse compression signal processing techniques
allow 1) centimetre-scale interface thickness to
be rapidly, remotely, and continuously measured, 2) the
evolution, and ultimate merging, of multiple interfaces
to be observed at high-resolution, and 3) convection
cells within the surrounding mixed layers to be observed.
The acoustically measured interface thickness,
combined with knowledge of the slowly-varying temperatures
within the surrounding layers, in turn allows
the direct estimation of double-diffusive heat and buoyancy
fluxes. The acoustically derived interface thickness,
interfacial fluxes and migration rates are shown
to support established theory. Acoustic techniques complement
traditional laboratory sampling methods and
provide enhanced capabilities for observing the diffusive
regime of double-diffusion in the ocean.Funding for this project was provided by the

BIOKID: Randomized controlled trial comparing bicarbonate and lactate buffer in biocompatible peritoneal dialysis solutions in children [ISRCTN81137991]
BACKGROUND: Peritoneal dialysis (PD) is the preferred dialysis modality in children. Its major drawback is the limited technique survival due to infections and progressive ultrafiltration failure. Conventional PD solutions exert marked acute and chronic toxicity to local tissues. Prolonged exposure is associated with severe histopathological alterations including vasculopathy, neoangiogenesis, submesothelial fibrosis and a gradual loss of the mesothelial cell layer. Recently, more biocompatible PD solutions containing reduced amounts of toxic glucose degradation products (GDPs) and buffered at neutral pH have been introduced into clinical practice. These solutions contain lactate, bicarbonate or a combination of both as buffer substance. Increasing evidence from clinical trials in adults and children suggests that the new PD fluids may allow for better long-term preservation of peritoneal morphology and function. However, the relative importance of the buffer in neutral-pH, low-GDP fluids is still unclear. In vitro, lactate is cytotoxic and vasoactive at the concentrations used in PD fluids. The BIOKID trial is designed to clarify the clinical significance of the buffer choice in biocompatible PD fluids. METHODS/DESIGN: The objective of the study is to test the hypothesis that bicarbonate based PD solutions may allow for a better preservation of peritoneal transport characteristics in children than solutions containing lactate buffer. Secondary objectives are to assess any impact of the buffer system on acid-base status, peritoneal tissue integrity and the incidence and severity of peritonitis. After a run-in period of 2 months during which a targeted cohort of 60 patients is treated with a conventional, lactate buffered, acidic, GDP containing PD fluid, patients will be stratified according to residual renal function and type of phosphate binding medication and randomized to receive either the lactate-containing Balance solution or the bicarbonate-buffered Bicavera(® )solution for a period of 10 months. Patients will be monitored by monthly physical and laboratory examinations. Peritoneal equilibration tests, 24-h dialysate and urine collections will be performed 4 times. Peritoneal biopsies will be obtained on occasion of intraabdominal surgery. Changes in small solute transport rates, markers of peritoneal tissue turnover in the effluent, acid-base status and peritonitis rates and severity will be analyzed
Mate Value and Self-Esteem: Evidence from Eight Cultural Groups
This paper explores self-perceived mate value (SPMV), and its association with self-esteem, in eight cultures. 1066 participants, from 8 cultural groups in 7 countries, rated themselves on 24 SPMVs and completed a measure of self-esteem. Consistent with evolutionary theory, women were more likely to emphasise their caring and passionate romantic nature. In line with previous cross-cultural research, characteristics indicating passion and romance and social attractiveness were stressed more by respondents from individualistic cultures, and those higher on self-expression (rather than survival) values; characteristics indicative of maturity and confidence were more likely to be mentioned by those from Traditional, rather than Secular, cultures. Contrary to gender role theory, societal equality had only limited interactions with sex and SPMV, with honesty of greater significance for male self-esteem in societies with unequal gender roles. These results point to the importance of cultural and environmental factors in influencing self-perceived mate qualities, and are discussed in relation to broader debates about the impact of gender role equality on sex differences in personality and mating strategies
Factor structure and validity of the shoulder pain and disability index in a population-based study of people with shoulder symptoms
Background: The Shoulder Pain and Disability Index (SPADI) is a self-administered questionnaire that aims to measure pain and disability associated with shoulder disease. The aim of the present study was to investigate the construct validity and factor structure of the SPADI in a population-based study of patients with self-reported chronic shoulder symptoms. Methods: The North West Adelaide Health Study is a representative longitudinal cohort study of people aged 18 years and over. The original sample was randomly selected and recruited by telephone interview. Overall, 3 206 participants returned to the clinic during the second stage (2004-2006) and were asked to report whether they had pain, aching or stiffness on most days in either of their shoulders. Data was also collected on body mass index and shoulder range of motion (ROM) and demographic factors. The SPADI (numeric rating scale) was administered to participants with shoulder symptoms. Principal components factor analysis, with varimax rotation of factor loadings, was used to assess subscale structure of SPADI. Correlations between the SPADI, shoulder ROM and SF-36 were performed. Results: Overall, 22.3% of participants indicated that they had pain, aching or stiffness in either of their shoulders. SPADI results were available for 588 of participants with current shoulder symptoms. The internal consistency of the SPADI subscales were high (Cronbach's alpha > 0.92). Two factors, explaining 61.4% of the total variance were extracted by factor analysis. These were interpreted as disability and pain respectively. There was a strong negative correlation between SPADI disability subscale scores and shoulder range of motion. SPADI disability, but not pain, subscale scores were correlated with age. Conclusions: The SPADI is a valid measure to assess pain and disability in people with shoulder pain in a population-based study.
