The influence of placements on adult nursing graduatesâ choice of first post
Background: This article presents findings from a study that sought to explore the extent to which clinical placements have an impact on nursing studentsâ decisions regarding their first staff nurse post. Within the UK, nursing is facing a recruitment crisis with particular difficulty recruiting to areas such as primary and elderly care. Transitioning into a new role is challenging in any occupation, but it is a particular problem in nursing where the realities of professional practice often differ from studentsâ perception of the staff nurse role as shaped by their clinical placements. Aim: This pilot study aimed to explore the influence of practice placements on final year adult nursing studentsâ career decisions. Method: Qualitative and quantitative data were collected in a single phase using a questionnaire distributed to nursing students on the final day of their course. A total of 35 completed questionnaires were returned (response rate 57%). Results: Half of the participants entered the course with preconceived preferences for clinical specialisms. However, only five participants (14%) applied for first destination posts in that specialism. The overall importance of placements in career choice increased across the three years of the programme. Although placements in all three years are important, the experiences in year 3 are pivotal, with 74% rankingthese as âsignificantly influentialâ in their decision-making process. Analysis of the data obtained from the free-text responses from the questionnaire suggested that working environment; the level of support provided by mentors and clinical staff; the opportunity to make a difference to patientsâ lives and the variety of placements, were key influences on nursing studentsâ decision regarding their first staff nurse post. Conclusions: This study highlights the key role of practice placements in the career choices of student nurses, particularly during the final year of their programme. It shows that students are likely to apply for posts in the placement area they found to be most supportive and developmental
The protease inhibitor JO146 demonstrates a critical role for CtHtrA for Chlamydia trachomatis reversion from penicillin persistence
The Chlamydia trachomatis serine protease HtrA (CtHtrA) has recently been demonstrated to be essential during the replicative phase of the chlamydial developmental cycle. A chemical inhibition strategy (serine protease inhibitor JO146) was used to demonstrate this essential role and it was found that the chlamydial inclusions diminish in size and are lost from the cell after CtHtrA inhibition without formation of viable elementary bodies. The inhibitor (JO146) was used in this study to investigate the role of CtHtrA for penicillin persistence and heat stress conditions for Chlamydia trachomatis. JO146 addition during penicillin persistence resulted in only minor reductions (~1 log) in the final viable infectious yield after persistent Chlamydia were reverted from persistence. However, JO146 treatment during the reversion and recovery from penicillin persistence was completely lethal for Chlamydia trachomatis. JO146 was completely lethal when added either during heat stress conditions, or during the recovery from heat stress conditions. These data together indicate that CtHtrA has essential roles during some stress environments (heat shock), recovery from stress environments (heat shock and penicillin persistence), as well as the previously characterized essential role during the replicative phase of the chlamydial developmental cycle. Thus, CtHtrA is an essential protease with both replicative phase and stress condition functions for Chlamydia trachomatis. © 2013 Ong, Marsh, Lawrence, Allan, Timms and Huston
Flowering of kiwifruit (Actinidia deliciosa) is reduced by long photoperiods
Mature kiwifruit (Actinidia deliciosa âHaywardâ) vines grown under standard orchard management were exposed to 16-h photoperiods from the longest day in summer until after leaf fall in autumn. Photoperiod extension was achieved with tungsten halogen lamps that produced 2â8 ”mols mâ2 sâ1 photosynthetically active radiation. Long day treatments did not affect fruit dry matter or fruit weight at harvest during the growing season that the treatments were applied or during the following growing season. However, flowering was reduced by 22% during the spring following treatment application. As this reduction in flowering was not accompanied by a decrease in budbreak, the long day effect is not consistent with a delay in the onset of winter chilling. It is suggested therefore, that the observed reduction in flowering may be because of a diminution of floral evocation
Exploring the impact of an evolving war and terror blogosphere on traditional media coverage of conflict
This article analyses the evolution of a war and terror blogosphere between 2001 and 2011. It identifies seven areas where blogs and related online genres could provide
âalternativeâ accounts to traditional media narratives of conflict. The article also assesses the challenges and opportunities of blogs in each area from the perspective of the working journalist in order to deepen our understanding of the changing influence of blogs on traditional media narratives of conflict. Parallel accounts and interpretations of conflict will collaborate and compete in a war and terror blogosphere in the future, but it has been significantly influenced by the adoption of blogging by military actors since 2008. The war and terror blogosphere is no longer a relatively unmonitored online space which is having an impact on both the production of âalternativeâ accounts of conflict and the incorporation of these accounts into traditional journalism
Design and feasibility testing of a novel group intervention for young women who binge drink in groups
BackgroundYoung women frequently drink alcohol in groups and binge drinking within these natural drinking groups is common. This study describes the design of a theoretically and empirically based group intervention to reduce binge drinking among young women. It also evaluates their engagement with the intervention and the acceptability of the study methods.MethodsFriendship groups of women aged 18â35 years, who had two or more episodes of binge drinking (>6 UK units on one occasion; 48g of alcohol) in the previous 30 days, were recruited from the community. A face-to-face group intervention, based on the Health Action Process Approach, was delivered over three sessions. Components of the intervention were woven around fun activities, such as making alcohol free cocktails. Women were followed up four months after the intervention was delivered. Results The target of 24 groups (comprising 97 women) was recruited. The common pattern of drinking was infrequent, heavy drinking (mean consumption on the heaviest drinking day was UK 18.1 units). Process evaluation revealed that the intervention was delivered with high fidelity and acceptability of the study methods was high. The women engaged positively with intervention components and made group decisions about cutting down. Twenty two groups set goals to reduce their drinking, and these were translated into action plans. Retention of individuals at follow up was 87%.ConclusionsThis study successfully recruited groups of young women whose patterns of drinking place them at high risk of acute harm. This novel approach to delivering an alcohol intervention has potential to reduce binge drinking among young women. The high levels of engagement with key steps in the behavior change process suggests that the group intervention should be tested in a full randomised controlled trial
Perturbations of nuclear C*-algebras
Kadison and Kastler introduced a natural metric on the collection of all
C*-subalgebras of the bounded operators on a separable Hilbert space. They
conjectured that sufficiently close algebras are unitarily conjugate. We
establish this conjecture when one algebra is separable and nuclear. We also
consider one-sided versions of these notions, and we obtain embeddings from
certain near inclusions involving separable nuclear C*-algebras. At the end of
the paper we demonstrate how our methods lead to improved characterisations of
some of the types of algebras that are of current interest in the

Universality of pseudogap and emergent order in lightly doped Mott insulators
It is widely believed that high-temperature superconductivity in the cuprates
emerges from doped Mott insulators. The physics of the parent state seems
deceivingly simple: The hopping of the electrons from site to site is
prohibited because their on-site Coulomb repulsion U is larger than the kinetic
energy gain t. When doping these materials by inserting a small percentage of
extra carriers, the electrons become mobile but the strong correlations from
the Mott state are thought to survive; inhomogeneous electronic order, a
mysterious pseudogap and, eventually, superconductivity appear. How the
insertion of dopant atoms drives this evolution is not known, nor whether these
phenomena are mere distractions specific to hole-doped cuprates or represent
the genuine physics of doped Mott insulators. Here, we visualize the evolution
of the electronic states of (Sr1-xLax)2IrO4, which is an effective spin-1/2
Mott insulator like the cuprates, but is chemically radically different. Using
spectroscopic-imaging STM, we find that for doping concentration of x=5%, an
inhomogeneous, phase separated state emerges, with the nucleation of pseudogap
puddles around clusters of dopant atoms. Within these puddles, we observe the
same glassy electronic order that is so iconic for the underdoped cuprates.
Further, we illuminate the genesis of this state using the unique possibility
to localize dopant atoms on topographs in these samples. At low doping, we find
evidence for much deeper trapping of carriers compared to the cuprates. This
leads to fully gapped spectra with the chemical potential at mid-gap, which
abruptly collapse at a threshold of around 4%. Our results clarify the melting
of the Mott state, and establish phase separation and electronic order as

Elimination of Schistosomiasis Transmission in Zanzibar: Baseline Findings before the Onset of a Randomized Intervention Trial.
Gaining and sustaining control of schistosomiasis and, whenever feasible, achieving local elimination are the year 2020 targets set by the World Health Organization. In Zanzibar, various institutions and stakeholders have joined forces to eliminate urogenital schistosomiasis within 5 years. We report baseline findings before the onset of a randomized intervention trial designed to assess the differential impact of community-based praziquantel administration, snail control, and behavior change interventions. In early 2012, a baseline parasitological survey was conducted in âŒ20,000 people from 90 communities in Unguja and Pemba. Risk factors for schistosomiasis were assessed by administering a questionnaire to adults. In selected communities, local knowledge about schistosomiasis transmission and prevention was determined in focus group discussions and in-depths interviews. Intermediate host snails were collected and examined for shedding of cercariae. The baseline Schistosoma haematobium prevalence in school children and adults was 4.3% (range: 0-19.7%) and 2.7% (range: 0-26.5%) in Unguja, and 8.9% (range: 0-31.8%) and 5.5% (range: 0-23.4%) in Pemba, respectively. Heavy infections were detected in 15.1% and 35.6% of the positive school children in Unguja and Pemba, respectively. Males were at higher risk than females (odds ratio (OR): 1.45; 95% confidence interval (CI): 1.03-2.03). Decreasing adult age (OR: 1.04; CI: 1.02-1.06), being born in Pemba (OR: 1.48; CI: 1.02-2.13) or Tanzania (OR: 2.36; CI: 1.16-4.78), and use of freshwater (OR: 2.15; CI: 1.53-3.03) showed higher odds of infection. Community knowledge about schistosomiasis was low. Only few infected Bulinus snails were found. The relatively low S. haematobium prevalence in Zanzibar is a promising starting point for elimination. However, there is a need to improve community knowledge about disease transmission and prevention. Control measures tailored to the local context, placing particular attention to hot-spot areas, high-risk groups, and individuals, will be necessary if elimination is to be achieved
Robust observational constraint of uncertain aerosol processes and emissions in a climate model and the effect on aerosol radiative forcing
The effect of observational constraint on the ranges of uncertain physical and chemical process parameters was explored in a global aerosolâclimate model. The study uses 1 million variants of the Hadley Centre General Environment Model version 3 (HadGEM3) that sample 26 sources of uncertainty, together with over 9000 monthly aggregated grid-box measurements of aerosol optical depth, PM2.5, particle number concentrations, sulfate and organic mass concentrations. Despite many compensating effects in the model, the procedure constrains the probability distributions of parameters related to secondary organic aerosol, anthropogenic SO2 emissions, residential emissions, sea spray emissions, dry deposition rates of SO2 and aerosols, new particle formation, cloud droplet pH and the diameter of primary combustion particles. Observational constraint rules out nearly 98â% of the model variants. On constraint, the ±1Ï (standard deviation) range of global annual mean direct radiative forcing (RFari) is reduced by 33â% to â0.14 to â0.26âWâmâ2, and the 95â% credible interval (CI) is reduced by 34â% to â0.1 to â0.32âWâmâ2. For the global annual mean aerosolâcloud radiative forcing, RFaci, the ±1Ï range is reduced by 7â% to â1.66 to â2.48âWâmâ2, and the 95â% CI by 6â% to â1.28 to â2.88âWâmâ2. The tightness of the constraint is limited by parameter cancellation effects (model equifinality) as well as the large and poorly defined ârepresentativeness errorâ associated with comparing point measurements with a global model. The constraint could also be narrowed if model structural errors that prevent simultaneous agreement with different measurement types in multiple locations and seasons could be improved. For example, constraints using either sulfate or PM2.5 measurements individually result in RFari±1Ï ranges that only just overlap, which shows that emergent constraints based on one measurement type may be overconfident
Taenia solium Infections in a Rural Area of Eastern Zambia-A Community Based Study
Taenia solium taeniosis/cysticercosis is a zoonotic infection endemic in many developing countries, with humans as the definitive host (taeniosis) and pigs and humans as the intermediate hosts (cysticercosis). When humans act as the intermediate host, the result can be neurocysticercosis, which is associated with acquired epilepsy, considerable morbidity and even mortality. In Africa, most studies have been carried out in pigs with little or no data in humans available. In this human study, conducted in a rural community in Eastern Zambia, prevalences for taeniosis and cysticercosis were determined at 6.3% and 5.8% respectively, indicating the hyperendemicity of the area. Cysticercosis infection was strongly related with age, with a significant increase in prevalence occurring in individuals from the age of 30 onward. A collected tapeworm was confirmed to be T. solium. Risk factors associated with the transmission and maintenance of the parasite such as free roaming pigs, households without latrines, backyard slaughter of pigs without inspection and consumption of undercooked pork were also present. The findings of this work have identified the need for further research in the transmission dynamics and the burden that this infection has on the resources of poor local people
