    Examining Users’ Information Disclosure and Audience Support Response Dynamics in Online Health Communities: An Empirical Study

    Online healthcare communities (OHCs) facilitate two-way interaction. Examining users’ information disclosure-audience support response dynamics can reveal insights for fostering a supportive environment, community engagement, bond formation, knowledge sharing, and sustained participation in OHCs. We propose a structural vector autoregression (SVAR) model of user disclosure and response dynamics in OHCs. Based on the health disclosure decision-making model and daily time series data, we examine the two-way interaction of two dimensions of disclosure efficacy with audience support response acceptance. Findings of the impulse response functions reveal that user information density leads to positive support response acceptance, whereas support response acceptance reduces the information density of a user post over time. Further, higher information efficacy leads to more support response acceptance with long run improved information efficacy. Theoretically, findings extend the disclosure decision-making model in OHCs. Practically, the results provide insights for OHC management to facilitate two-way dynamic users’ interactions

    Community acceptance of COVID-19 and demystifying stigma in a severely affected population in Ghana

    Objective: We assessed the level of community acceptance of COVID-19, identified and implemented strategies to demystifying stigma in a severely affected population in Tema.Design and Setting: We conducted a cross-sectional study to assess stigma among the Tema community, then identified and implemented interventions to demystify COVID-19 stigma. We interviewed positive cases, their contacts, contact tracers, case management team members, and community members who shared their first hand experiences and knowledge on the current pandemic.Intervention: Based on the information received, we came up with ways of reducing stigma and implemented them in their community.Main Outcome: Stigma demystifiedResults: Cases and contacts reported being avoided, discriminated against, insulted or had derogatory words used on them by family, friends, work colleagues or the community. Cases and their contacts stated that stigmatisation was fueled by the presence of COVID -19 branded vehicles and security officials at their homes or workplaces. Stakeholder engagement, education and extensive sensitisation of community members were implemented to reduce stigma.Conclusion: We observed deeply entrenched stigma to COVID - 19 positive patients and their contacts in the community. Health care response mechanisms such as the presence of security personnel with contact tracers and case managers and the use of COVID -19 branded vehicles fueled stigma. A multifaceted approach through the engagement of key stakeholders, training of health workers and extensive education and community sensitisation was essential in reducing stigma

    Unique synergistic formulation of curcumin, epicatechin gallate and resveratrol, tricurin, suppresses HPV E6, eliminates HPV+ cancer cells, and inhibits tumor progression

    Curcumin (from curry) (C) is highly potent against cervical cancer cells (CCC), but poor bioavailability has limited its clinical use. Similar natural polyphenols resveratrol (from grapes) (R), and epicatechin gallate (from green tea) (E) also display activity against CCC. By treating CCC (HeLa) with C, E, or R, or combinations of these compounds, we computed combination indices and observed a strong synergism among C, E, and R at the unique molar ratio 4:1:12.5. This combination, named as TriCurin, rapidly down regulated HPV18 E6 and NF-kB expression while concomitantly inducing the tumor suppressor protein p53 in HeLa cells. In the mouse c-Ha-ras and HPV16 E6, E7-expressing TC-1 CCC, both C and TriCurin elicited suppression of E6, induction of both p53 and acetyl-p53 (activated p53), and activation of caspase-3, but the TriCurinevoked changes were several-fold greater than that produced by curcumin (4.7-fold for E6 inhibition, and 2-fold, 6-fold, and 1.7-fold for the induction of p53, acetyl-p53, and active caspase-3, respectively). Consequently, TriCurin was more potent in killing TC-1 and HeLa cells. Intralesional TriCurin treatment of tumors generated in mice by subcutaneously implanting the TC-1 CCC caused an 80–90% decrease in tumor growth. The ability of C to eliminate HeLa cells was significantly stabilized when delivered as TriCurin than when delivered alone. Topical application of TriCurin dispersed in a cream base afforded efficient transfer of C across the skin. Subcutaneous TriCurin injection yielded no adverse effect in tumor-naïve healthy mice. Thus, TriCurin is a safe and promising therapeutic agent against HPV-associated disease

    Geophagy Practices and the Content of Chemical Elements in the Soil Eaten by Pregnant Women in Artisanal and Small Scale Gold Mining Communities in Tanzania.

    Geophagy, a form of pica, is the deliberate consumption of soil and is relatively common across Sub-Saharan Africa. In Tanzania, pregnant women commonly eat soil sticks sold in the market (pemba), soil from walls of houses, termite mounds, and ground soil (kichuguu). The present study examined geophagy practices of pregnant women in a gold mining area of Geita District in northwestern Tanzania, and also examined the potential for exposure to chemical elements by testing soil samples. We conducted a cross sectional study using a convenience sample of 340 pregnant women, ranging in age from 15-49 years, who attended six government antenatal clinics in the Geita District, Tanzania. Structured interviews were conducted in June-August, 2012, to understand geophagy practices. In addition, soil samples taken from sources identified by pregnant women practicing geophagy were analysed for mineral element content. Geophagy was reported by 155 (45.6%) pregnant women with 85 (54.8%) initiating the practice in the first trimester. A total of 101 (65%) pregnant women reported eating soil 2 to 3 times per day while 20 (13%) ate soil more than 3 times per day. Of 155 pregnant women 107 (69%) bought pemba from local shops, while 48 (31%) consumed ground soil kichuguu. The estimated mean quantity of soil consumed from pemba was 62.5 grams/day. Arsenic, chromium, copper, iron, manganese, nickel and zinc levels were found in both pemba and kichuguu samples. Cadmium and mercury were found only in the kichuguu samples. Based on daily intake estimates, arsenic, copper and manganese for kichuguu and copper and manganese for pemba samples exceed the oral Minimum Risk Levels designated by the U.S. Agency for Toxic Substance and Disease Registry. Almost 50% of participants practiced geophagy in Geita District consistent with other reports from Africa. Both pemba and kichuguu contained chemical elements at varying concentration, mostly above MRLs. As such, pregnant women who eat soil in Geita District are exposed to potentially high levels of chemical elements, depending upon frequency of consumption, daily amount consumed and the source location of soil eaten

    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries

    Grade 10 mathematics learners’ errors and misconceptions when simplifying algebraic fraction

    M.Ed.Abstract: This study presents findings on algebraic fractions that was conducted among grade 10 learners in the Eastern Cape Province, South Africa. The study analysed the learners’ performance in algebraic fractions and explored the mistakes made by the learners as they engaged in the tasks of simplifying algebraic fractions. The study required learners to respond to algebraic fraction tasks. A group of 136 grade 10 high school learners were purposefully sampled. They responded to twelve grade 10 mathematics questions that consisted of two knowledge questions, four routine procedural questions, four complex procedures questions and two problem solving questions which were the cognitive levels guide suggested in the TIMSS 1999 study. Descriptor code keys were used to code, categorise and thematised the findings. When the learners’ overall performances were analysed for all twelve questions, the results showed that the majority of the learners did not comprehend what simplify algebraic fractions meant, and their conceptual knowledge of basic algebraic expression and simplification of such expressions was quite weak. The study revealed learners’ conceptual errors as well as procedural errors as being most prominent. These errors hinder learners’ ability to solve other mathematical problems that require prior knowledge to simplifying algebraic fractions

    Examining the teaching strategies of high-performing mathematics teachers in solving challenging 3-dimensional (3D) trigonometry

     In this article, we discuss teaching strategies that high-performing mathematics teachers use to assist learners in solving challenging 3-dimensional (3D) trigonometry concepts more effectively. High school learners often find 3D trigonometry challenging. For this study, a purposively qualitative research design was chosen. Five mathematics teachers were interviewed, and 381 scripts of the learners were analysed with the mathematical language error, procedural error, conceptual error, and application error (MPCA error) analysis protocol. Results from this study showed that using concrete models to illustrate the dimensions of 3D concepts was helpful for high-performing schoolteachers. The mathematical knowledge and the quality of mathematical instructions used by the teacher to unpack the 3D trigonometry concepts were found to play a pivotal role in the learners’ understanding. The teachers’ in-depth knowledge of error analysis was valuable, while the teachers’ ability to implement blended learning was crucial for learners. When a misconception occurs amongst learners, it should be challenged or contrasted with the correct conception. An understanding of learners’ errors in 3D trigonometry helps teachers minimise learners’ errors

    Is First Impression Relevant in Online Health Support Communities? Preliminary Investigation of the Effects of Social Presence

    Patients’ initial impression can influence the kind of reactions they receive and their subsequent participation. Prior studies use inference models to examine participation as a continuum phenomenon. In the online health supporting communities (OHSCs), distinguishing giving participation from receiving participation provide interesting insights at the granular level. Using social presence theory, this study identifies and uses social presence cues in the initial post of 168 patients to predict patients’ giving and receiving participation in a prominent OHSC. Findings reveal that the social presence cues affected the two participation dimensions differently. Specifically, while intimacy is the most important predictor of giving participation, nonverbal communication is the most important predictor for receiving participation. The study offers important contributions to research and practice
