    The church of Santos Juanes as a cohesive element of the market district of Valencia-Spain

    [EN] The study of a factory full of history, its peculiarities and the evidence of a building with characteristics that make it unique, encourage a constructive and urban analysis of the Church of Santos Juanes in Valencia (Spain). It has been the scene of numerous events due to its important commercial position. Therefore, without leaving aside the rest of the particularities that have happened to the building and that largely define its personality and its current configuration, this work intends to attend to the rest of the features that fix its character and that allow it to be framed in a specific place inside the city. This article aims to discern and analyze the constructive evolution that the temple has had from its origins to the present, specifying each one of the interventions and relating them to the responsible historical premises, as well as the consequences that they have had in the urban environment that surrounds.[ES] El estudio de una fábrica cargada de historia, sus peculiaridades y la evidencia de un edificio con unas características que lo hacen único, impulsan a realizar un análisis constructivo y urbano de la Iglesia de los Santos Juanes de Valencia (España). Ha sido escenario de numerosos acontecimientos debido a su importante posición comercial. Por ello, sin dejar de lado el resto de particularidades que han acontecido al edificio y que definen en gran medida su personalidad y su configuración actual, se pretende atender al resto de rasgos que fijan su carácter y que permiten encuadrarlo en un lugar en concreto dentro de la ciudad.     Role of age and comorbidities in mortality of patients with infective endocarditis

    Purpose: The aim of this study was to analyse the characteristics of patients with IE in three groups of age and to assess the ability of age and the Charlson Comorbidity Index (CCI) to predict mortality. Methods: Prospective cohort study of all patients with IE included in the GAMES Spanish database between 2008 and 2015. Patients were stratified into three age groups:<65 years, 65 to 80 years, and = 80 years.The area under the receiver-operating characteristic (AUROC) curve was calculated to quantify the diagnostic accuracy of the CCI to predict mortality risk. Results: A total of 3120 patients with IE (1327 < 65 years;1291 65-80 years;502 = 80 years) were enrolled.Fever and heart failure were the most common presentations of IE, with no differences among age groups.Patients =80 years who underwent surgery were significantly lower compared with other age groups (14.3%, 65 years; 20.5%, 65-79 years; 31.3%, =80 years). In-hospital mortality was lower in the <65-year group (20.3%, <65 years;30.1%, 65-79 years;34.7%, =80 years;p < 0.001) as well as 1-year mortality (3.2%, <65 years; 5.5%, 65-80 years;7.6%, =80 years; p = 0.003).Independent predictors of mortality were age = 80 years (hazard ratio [HR]:2.78;95% confidence interval [CI]:2.32–3.34), CCI = 3 (HR:1.62; 95% CI:1.39–1.88), and non-performed surgery (HR:1.64;95% CI:11.16–1.58).When the three age groups were compared, the AUROC curve for CCI was significantly larger for patients aged <65 years(p < 0.001) for both in-hospital and 1-year mortality. Conclusion: There were no differences in the clinical presentation of IE between the groups. Age = 80 years, high comorbidity (measured by CCI), and non-performance of surgery were independent predictors of mortality in patients with IE.CCI could help to identify those patients with IE and surgical indication who present a lower risk of in-hospital and 1-year mortality after surgery, especially in the <65-year group

Para recuperar el índice de reducción sonora original de la partición, se han sellado las grietas utilizando dos técnicas: el sellado con silicona y el sellado con lana de roca embutida y masilla de yeso superficial. Los resultados muestran que con el sellado de lana de roca embutida y masilla de yeso se consigue la recuperación completa del índice de reducción sonora

    Cost-effectiveness analysis of gemtuzumab ozogamicin for first-line treatment of patients with cd-33 positive acute myeloid leukaemia in spain

    Altres ajuts: Pfizer Spain.Objective: To assess the incremental cost-utility ratio (ICUR) of gemtuzumab ozogamicin (GO) + standard of care (SOC) vs SOC alone for treatment of patients with de novo AML from a Spanish Health Service perspective. Methods: A cohort state-transition model, with 12 health-states, was used to estimate the lifetime accumulated cost and benefits in terms of quality-adjusted-life-years (QALYs) in AML patients with favourable, intermediate, and unknown cytogenetic profiles. Patient profile was defined based on the ALFA-0701 trial. Therapeutic regimens were defined by 5 haematologists. SOC was assumed to be idarubicin and cytarabine, the combination most used in Spain. QALYs were estimated by applying utilities for the time spent by the cohort in each health-state and utility decrements associated with adverse events (AE). Total cost (€,2020) included drug-acquisition, hematologic stem-cell transplantation, disease manage-ment, AE management and end-of-life costs. Unit costs were derived from local databases. All parameters were validated by haematologist. Costs and outcomes were discounted (3%/ year). Results: Higher cost/patient (€177,618 vs €151,434) and greater QALYs (5,70 vs 4,62) were obtained with GO+SOC vs SOC. The ICUR was €24,203/QALY gained. Conclusion: This simulation suggests that GO + SOC could be a cost-effective option for treatment of patients with de novo AML in first line

    PURPOSE: Because the currently available questionnaires to evaluate sexual changes on breast cancer women only address the sexual sphere with a few questions our purpose was to develop a questionnaire that assesses changes in sexual dysfunction and satisfaction in women treated for breast cancer. METHODS: A sample was selected of women aged between 18 and 65 who had had surgery for breast cancer, completed neoadjuvant/adjuvant chemotherapy treatment and could be receiving adjuvant hormonal treatment, with an active sex life at least 3 months before starting treatment. Metastatic disease was excluded. A questionnaire structured in 4 dimensions was developed. The MOS SF-12 and QLQ-BR23 questionnaires were also provided. The following metric properties were evaluated: item analysis; internal consistency; temporal stability; construct validity; concurrent, convergent and divergent validity; and feasibility. RESULTS: Three samples were recruited: a pilot sample of 20; a reduction sample of 152; and a validation sample of 148. The presence of 6 dimensions was confirmed: 1) Loss of sex drive; 2) worsening of body image; 3) psychological coping; 4) discomfort during intercourse; 5) satisfaction with sexual relations; and 6) satisfaction with breast reconstruction. Good goodness-of-fit statistics were obtained (chi(2)/df = 1.5, GFI = 0.9, AGFI = 0.84, CFI = 0.959, RMSEA = 0.062). Reliability was good (alpha = 0.855), as was test-retest stability (r = 0.838). The correlation with the convergent questionnaires proved to be higher than that obtained with generic measurements. CONCLUSIONS: We were able to develop a short questionnaire (17 items) capable of measuring sexual satisfaction in women with breast cancer with good metric properties

    Correlation between objective and subjective assessment of noise barriers

    [EN] There are several international standards that define the way to evaluate the attenuation capacity of noise reducing devices, by single-number quantities representing airborne sound insulation and insertion loss. These two single-value ratings define the quality and performance of acoustic barriers, the former being related to intrinsic and the latter to both intrinsic and extrinsic acoustic characteristics of the devices. However, not many studies can be found on whether these objective parameters correlate to the perception of annoyance reduction. The aim of the present work is to analyze the adequacy of these objective ratings to indicate the performance of noise barriers, by comparing their values with the perception of annoyance reduction. For this purpose, ninety individuals of two different nationalities (Spanish and Portuguese) were asked to rate the perceived annoyance reduction in a listening experimental test, in which they were exposed, under controlled conditions, to several environmental noises and acoustic screened stimuli simulated by audio filters. The obtained results show a high correlation between objective ratings and subjective annoyance perception, with a better correlation being observed for insertion loss single-number parameter than for the airborne sound insulation single-number rating. Furthermore, significant differences were found depending on the gender and nationality of the respondents.
