Comparative effectiveness trial comparing MyPlate to calorie counting for mostly low-income Latino primary care patients of a federally qualified community health center: study design, baseline characteristics.
BackgroundPrimary care-based behavior change obesity treatment has long featured the Calorie restriction (CC), portion control approach. By contrast, the MyPlate-based obesity treatment approach encourages eating more high-satiety/high-satiation foods and requires no calorie-counting. This report describes study methods of a comparative effectiveness trial of CC versus MyPlate. It also describes baseline findings involving demographic characteristics and their associations with primary outcome measures and covariates, including satiety/satiation, dietary quality and acculturation.MethodsA comparative effectiveness trial was designed to compare the CC approach (n = 130) versus a MyPlate-based approach (n = 131) to treating patient overweight. Intervenors were trained community health workers. The 11 intervention sessions included two in-home health education sessions, two group education sessions, and seven telephone coaching sessions. Questionnaire and anthropometric assessments occurred at baseline, 6- and 12 months; food frequency questionnaires were administered at baseline and 12 months. Participants were overweight adult primary care patients of a federally qualified health center in Long Beach, California. Two measures of satiety/satiation and one measure of post-meal hunger comprised the primary outcome measures. Secondary outcomes included weight, waist circumference, blood pressure, dietary quality, sugary beverage intake, water intake, fruit and vegetable fiber intake, mental health and health-related quality of life. Covariates included age, gender, nativity status (U.S.-born, not U.S.-born), race/ethnicity, education, and acculturation.AnalysisBaseline characteristics were compared using chi square tests. Associations between covariates and outcome measures were evaluated using multiple regression and logistic regression.ResultsTwo thousand eighty-six adult patients were screened, yielding 261 enrollees who were 86% Latino, 8% African American, 4% White and 2% Other. Women predominated (95%). Mean age was 42 years. Most (82%) were foreign-born; 74% chose the Spanish language option. Mean BMI was 33.3 kg/m2; mean weight was 82 kg; mean waist circumference was 102 cm. Mean blood pressure was 122/77 mm. Study arms on key baseline measures did not differ except on dietary quality and sugary beverage intake. Nativity status was significantly associated with dietary quality.ConclusionsThe two treatment arms were well-balanced demographically at baseline. Considering rain gauge uncertainty using Kriging for Uncertain Data
In urban hydrological models, rainfall is the main input and one of the main sources of uncertainty. To reach sufficient spatial coverage and resolution, the integration of several rainfall data sources, including rain gauges and weather radars, is often necessary. The uncertainty associated with rain gauge measurements is dependent on rainfall intensity and on the characteristics of the devices. Common spatial interpolation methods do not account for rain gauge uncertainty variability. Kriging for Uncertain Data (KUD) allows the handling of the uncertainty of each rain gauge independently, modelling space- and time-variant errors. The applications of KUD to rain gauge interpolation and radar-gauge rainfall merging are studied and compared. First, the methodology is studied with synthetic experiments, to evaluate its performance varying rain gauge density, accuracy and rainfall field characteristics. Subsequently, the method is applied to a case study in the Dommel catchment, the Netherlands, where high-quality automatic gauges are complemented by lower-quality tipping-bucket gauges and radar composites. The case study and the synthetic experiments show that considering measurement uncertainty in rain gauge interpolation usually improves rainfall estimations, given a sufficient rain gauge density. Considering measurement uncertainty in radar-gauge merging consistently improved the estimates in the tested cases, thanks to the additional spatial information of radar rainfall data but should still be used cautiously for convective events and low-density rain gauge networks.</p
Weekday and weekend patterns of physical activity and sedentary time among Liverpool and Madrid youth
Levels of physical inactivity and sedentary behaviour among English and Spanish youth are high and vary within different regions of each country. Little though is known about these during specific periods of the day. The purpose of this study was to describe physical activity (PA) and sedentary time during segments of the day and week, and compare these critical contexts between youth in the Liverpool and Madrid areas of England and Spain, respectively. PA was objectively assessed in 235 Liverpool- and 241 Madrid youth (aged 10-14 years) who wore accelerometers for seven consecutive days. Minutes of sedentary time, moderate PA, vigorous PA and moderate-to-vigorous physical activity (MVPA) were calculated for weekdays, weekend days, school time, non-school time and after-school. Between-country differences were analysed using analysis of covariance (ANCOVA). Madrid youth spent significantly more time in sedentary activities than their Liverpool counterparts. Madrid youth engaged in more minutes of moderate intensity physical activity (MPA) than Liverpool youth during weekdays, school time and non-school time (P &lt; 0.01). Liverpool children recorded more time in vigorous physical activity (VPA) than Madrid peers during week days and weekend days (P &lt; 0.01) and during school time and after-school periods (P &lt; 0.01). The MVPA was significantly higher among Madrid youth during non-school time (P &lt; 0.01). Around 25% of all youth achieved recommended levels of MVPA. Low levels of MVPA and systematic differences in sedentary time, MPA and VPA exist between Liverpool and Madrid youth. Interventions targeted at the least-active children during weekends, after-school and non-school periods within the cultural contexts common to each city are required. © 2013 © 2013 European College of Sport Science
Recent insights on uncertainties present in integrated catchment water quality modelling
This paper aims to stimulate discussion based on the experiences derived from the QUICS project (Quantifying Uncertainty in Integrated Catchment Studies). First it briefly discusses the current state of knowledge on uncertainties in sub-models of integrated catchment models and the existing frameworks for analysing uncertainty. Furthermore, it compares the relative approaches of both building and calibrating fully integrated models or linking separate sub-models. It also discusses the implications of model linkage on overall uncertainty and how to define an acceptable level of model complexity. This discussion includes, whether we should shift our attention from uncertainties due to linkage, when using linked models, to uncertainties in model structure by necessary simplification or by using more parameters. This discussion attempts to address the question as to whether there is an increase in uncertainty by linking these models or if a compensation effect could take place and that overall uncertainty in key water quality parameters actually decreases. Finally, challenges in the application of uncertainty analysis in integrated catchment water quality modelling, as encountered in this project, are discussed and recommendations for future research areas are highlighted

Host adaptive immunity deficiency in severe pandemic influenza
INTRODUCTION:
Pandemic A/H1N1/2009 influenza causes severe lower respiratory complications in rare cases. The association between host immune responses and clinical outcome in severe cases is unknown.
METHODS:
We utilized gene expression, cytokine profiles and generation of antibody responses following hospitalization in 19 critically ill patients with primary pandemic A/H1N1/2009 influenza pneumonia for identifying host immune responses associated with clinical outcome. Ingenuity pathway analysis 8.5 (IPA) (Ingenuity Systems, Redwood City, CA) was used to select, annotate and visualize genes by function and pathway (gene ontology). IPA analysis identified those canonical pathways differentially expressed (P < 0.05) between comparison groups. Hierarchical clustering of those genes differentially expressed between groups by IPA analysis was performed using BRB-Array Tools v.3.8.1.
RESULTS:
The majority of patients were characterized by the presence of comorbidities and the absence of immunosuppressive conditions. pH1N1 specific antibody production was observed around day 9 from disease onset and defined an early period of innate immune response and a late period of adaptive immune response to the virus. The most severe patients (n = 12) showed persistence of viral secretion. Seven of the most severe patients died. During the late phase, the most severe patient group had impaired expression of a number of genes participating in adaptive immune responses when compared to less severe patients. These genes were involved in antigen presentation, B-cell development, T-helper cell differentiation, CD28, granzyme B signaling, apoptosis and protein ubiquitination. Patients with the poorest outcomes were characterized by proinflammatory hypercytokinemia, along with elevated levels of immunosuppressory cytokines (interleukin (IL)-10 and IL-1ra) in serum.
CONCLUSIONS:
Our findings suggest an impaired development of adaptive immunity in the most severe cases of pandemic influenza, leading to an unremitting cycle of viral replication and innate cytokine-chemokine release. Overexpression of S100A4 in human cancer cell lines resistant to methotrexate
Methotrexate is a chemotherapeutic drug that is used in therapy of a wide variety of cancers. The
efficiency of treatment with this drug is compromised by the appearance of resistance. Combination treatments of
MTX with other drugs that could modulate the expression of genes involved in MTX resistance would be an adequate
strategy to prevent the development of this resistance.
Methods: The differential expression pattern between sensitive and MTX-resistant cells was determined by whole
human genome microarrays and analyzed with the GeneSpring GX software package. A global comparison of all the
studied cell lines was performed in order to find out differentially expressed genes in the majority of the MTX-resistant
cells. S100A4 mRNA and protein levels were determined by RT-Real-Time PCR and Western blot, respectively.
Functional validations of S100A4 were performed either by transfection of an expression vector for S100A4 or a siRNA
against S100A4. Transfection of an expression vector encoding for β-catenin was used to inquire for the possible
transcriptional regulation of S100A4 through the Wnt pathway.
Results: S100A4 is overexpressed in five out of the seven MTX-resistant cell lines studied. Ectopic overexpression of this
gene in HT29 sensitive cells augmented both the intracellular and extracellular S100A4 protein levels and caused
desensitization toward MTX. siRNA against S100A4 decreased the levels of this protein and caused a
chemosensitization in combined treatments with MTX. β-catenin overexpression experiments support a possible
involvement of the Wnt signaling pathway in S100A4 transcriptional regulation in HT29 cells.
Conclusions: S100A4 is overexpressed in many MTX-resistant cells. S100A4 overexpression decreases the sensitivity of
HT29 colon cancer human cells to MTX, whereas its knockdown causes chemosensitization toward MTX. Both

Sarcopenia: etiology, clinical consequences, intervention, and assessment
The aging process is associated with loss of muscle mass and strength and decline in physical functioning. The term sarcopenia is primarily defined as low level of muscle mass resulting from age-related muscle loss, but its definition is often broadened to include the underlying cellular processes involved in skeletal muscle loss as well as their clinical manifestations. The underlying cellular changes involve weakening of factors promoting muscle anabolism and increased expression of inflammatory factors and other agents which contribute to skeletal muscle catabolism. At the cellular level, these molecular processes are manifested in a loss of muscle fiber cross-sectional area, loss of innervation, and adaptive changes in the proportions of slow and fast motor units in muscle tissue. Ultimately, these alterations translate to bulk changes in muscle mass, strength, and function which lead to reduced physical performance, disability, increased risk of fall-related injury, and, often, frailty. In this review, we summarize current understanding of the mechanisms underlying sarcopenia and age-related changes in muscle tissue morphology and function. We also discuss the resulting long-term outcomes in terms of loss of function, which causes increased risk of musculoskeletal injuries and other morbidities, leading to frailty and loss of independence
