A formacao do neonatologista e os paradigmas implicados na relacao com os pais dos bebes na Unidade de Terapia Intensiva Neonatal
Objective:To analyze and to interpret the psychological repercussions generated by the presence of parents in the Neonatal Intensive Care Unit for residents in Neonatology.Methods:Study based on the psychoanalytic theory, involving a methodological interface with qualitative surveys in Health Sciences. Twenty resident physicians in Neonatology, from five public institutions of São Paulo state, responded to a single semi-structured interview. Based on several readings of the material, achieving the core of emergent meanings that would be significant to the object of the survey, six categories were elected for analysis and interpretation: parents' staying at the Neonatal Intensive Care Unit and its effects on the neonatologists' professional practice; communication of the diagnosis and what parents should know; impasses between parents and doctors when the diagnosis is being communicated; doctor's identification with parents; communication of the child's death and their participation in the interview.Results:The interpretation of the categories provided an understanding of the psychic mechanisms mobilized in doctors in their relationships with the children's parents, showing that the residents experience anguish and suffering when they provide medical care and during their training process, and also that they lack psychological support to handle these feelings.Conclusions:There is a need of intervention in neonatologists training and education, which may favor the elaboration of daily experiences in the Unit, providing a less anguishing and defensive way out for young doctors, especially in their relationship with patients and parents.Objetivo:Analizar e interpretar las repercusiones psicológicas suscitadas en el médico neonatologista en formación debido a la entrada y a la permanencia de los padres en la Unidad de Terapia Intensiva Neonatal. Métodos:Estudio sustentado en la teoría psicoanalítica, manteniéndose una interlocución metodológica con la investigación cualitativa en salud. Veinte residentes en Neonatología, de cinco instituciones públicas de la provincia de São Paulo (Brasil), participaron de una entrevista semiestructurada individual. A partir de innúmeras lecturas del material, objetivando los núcleos de sentido emergentes, significativos para el objeto de investigación, se eligieron categorías para análisis e interpretación: permanencia de los padres en la Unidad de Terapia Intensiva Neonatal y su repercusión en la actuación del neonatologista; comunicación del diagnóstico y qué los padres deben saber; impases entre los médicos y los padres de los bebés en la comunicación del diagnóstico; situaciones de identificación con los padres; comunicación de la muerte y participación en la entrevista. Resultados:La interpretación de las categorías propició la comprensión de los mecanismos psíquicos movilizados en los médicos en la relación con los padres de los bebés y expuso la angustia y el sufrimiento suscitados en los residentes en la atención asistencial y en el proceso de formación, para los que están desprovistos de anclajes psíquicos.Conclusiones:A partir del contenido que se explicitó en el análisis de las entrevistas, se verificó la necesidad de una propuesta de intervención en la formación de esos profesionales que favorezca la elaboración de las experiencias vividas en el cotidiano de la Unidad, a fin de propiciar una salida menos angustiante y defensiva para el médico, incluso en la relación con el paciente y con los padres.Objetivo:Analisar e interpretar as repercussões psicológicas suscitadas no médico neonatologista em formação devido à entrada e à permanência dos pais na Unidade de Terapia Intensiva Neonatal.Métodos:Estudo sustentado na teoria psicanalítica, mantendo-se uma interlocução metodológica com a pesquisa qualitativa em saúde. INVESTIGATING CATALYST DESIGN STRATEGIES FOR SELECTIVE REACTION OF CYCLIC C4 OXYGENATES FROM BIOMASS
Numerous studies have shown that the properties of metal catalysts can in principle be fine-tuned by controlling the composition of the metal surface with high precision. The ability to design catalysts capable of high selectivity towards the conversion of a single functional group in a multifunctional molecule is a major objective for heterogeneous catalysis research. This need for high selectivity toward a single functional group is of growing importance in efforts to improve biorefining operations, where biomass-derived multifunctional carbohydrates are key building block intermediates that must be converted to a vast range of commodity chemical products such as fuels, pharmaceuticals, food products, and more. This work focuses on results from high resolution electron energy loss spectroscopy (HREELS) and temperature programmed desorption (TPD) experiments combined with selective use of density functional theory (DFT) calculations on single-crystal surfaces under ultrahigh vacuum conditions to study structure-property relations for a series of C4 cyclic oxygenates on catalytic metal surfaces. The objective of this work is to identify methods to tailor surfaces that are able to selectively catalyze conversions of one functional group in the multifunctional molecule.
Two types of cyclic probe molecules have been studied in particular: 3-membered epoxide rings (in which ring-strain is high and the character of the oxygenate function is therefore more reactive) and 5-membered furanone rings (in which the ring is relatively stable). Both the epoxides and furanones contain an unsaturated C=C bond; for many biorefining applications it is desirable to selectively hydrogenate the olefin while keeping the oxygenate functionality intact. In this contribution, we explore the role of surface structure and composition in dictating the reaction pathways for multifunctional C4 cyclic oxygenates on key transition metal and bimetallic surfaces.
Results for the epoxide probe molecule studies indicate differing modes of interaction with different metal surfaces. On a platinum or palladium surface, the epoxide ring opens irreversibly while the C=C functional group has a strong interaction with the surface. However, on a silver surface, the epoxide ring also opens, but can be made to close reversibly. An effective catalyst design strategy, then, is to combine silver on a predominantly platinum or palladium surface in order to create a bimetallic catalyst with high selectivity toward reaction of the olefin while keeping the epoxide ring intact. Recent studies of the chemistry of furanone species indicate that the olefin group interacts strongly with a platinum or palladium metal surface, and therefore is very likely to also determine how the molecule reacts. In this presentation, relationships between catalyst design strategies for epoxides versus furanones will be discussed, as will the likely biorefining reactions that such strategies can impact
Profile of opioid prescriptions for intubated and mechanically ventilated neonates
Objective: to identify factors that affect the decision of prescribing opioids for intubated and ventilated neonates Method: retrospective study of intubated and ventilated newborn infants for periods longer than one hour, admitted to the NICU from January 1995 to June 1997. During this period, 203 patients fulfilled inclusion criteria and data of 176 charts were reviewed. Charts were analyzed regarding demographic data, characteristics of analgesia and respiratory support, invasive procedures performed and clinical entities diagnosed during the period of mechanical ventilation. Discriminative analysis was used to understand factors that lead to opioid use by some of these patients. Results: Ninety-seven neonates received at least one dose of opioids during the period of mechanical ventilation. None of these patients was evaluated with pain scales, and in 63% of them we could not retrieve any reason for opioid prescription in their charts. Discriminative analysis showed that the main differences between groups were birthweight, gestational age, oxygenation index at intubation, and number of arterial sticks during the first 72 hours of mechanical ventilation. The most mature and heaviest neonates with a more severe respiratory insufficiency received opioid analgesia during ventilation. Conclusion: the decision to use opioids in intubated and ventilated neonates was based on the infants' aspect and respiratory status. It did not consider the pain these patients might be suffering and it was not based on the evaluation of pain.Objetivo: identificar fatores que levaram os médicos a prescreverem opióides a recém-nascidos em ventilação mecânica. Método: estudo retrospectivo de pacientes em ventilação mecânica por cânula traqueal, por mais de 1 hora, internados em UTI neonatal entre janeiro de 1995 a junho de 1997. Nesse período, 203 recém-nascidos preencheram o critério de inclusão, recuperando-se 176 prontuários. Os prontuários foram analisados quanto a dados demográficos, características da analgesia e do suporte ventilatório, procedimentos invasivos realizados e entidades mórbidas diagnosticadas durante o período de ventilação. Para entender os fatores que determinaram o uso da analgesia em parte dessa população, utilizou-se a análise discriminante. Resultados: Nos 97 pacientes que receberam > 1 dose de opióides durante a ventilação, a analgesia foi iniciada, em média, até 24 horas após o início da ventilação. As escalas de avaliação da dor não foram usadas em nenhum paciente e, em 63%, não havia relato do motivo para a analgesia. A análise discriminante mostrou que as variáveis que diferenciaram os grupos submetidos ou não à analgesia foram: peso ao nascer, idade gestacional, índice de oxigenação e número de punções arteriais. Os neonatos com maior chance de receberem alguma dose de opióide durante a ventilação foram os de peso mais elevado, idade gestacional mais avançada, índice de oxigenação mais acentuado no início da ventilação e maior necessidade de gasometrias, ou seja, os bebês mais maduros e com doença respiratória mais grave. A study of the neglected Galactic HII region NGC 2579 and its companion ESO 370-9
The Galactic HII region NGC 2579 has stayed undeservedly unexplored due to
identification problems which persisted until recently. Both NGC 2579 and its
companion ESO 370-9 have been misclassified as planetary or reflection nebula,
confused with each other and with other objects. Due to its high surface
brightness, high excitation, angular size of few arcminutes and relatively low
interstellar extinction, NGC 2579 is an ideal object for investigations in the
optical range. Located in the outer Galaxy, NGC 2579 is an excellent object for
studying the Galactic chemical abundance gradients. In this paper we present
the first comprehensive observational study on the nebular and stellar
properties of NGC 2579 and ESO 370-9, including the determination of electron
temperature, density structure, chemical composition, kinematics, distance, and
the identification and spectral classification of the ionizing stars, and
discuss the nature of ESO 370-9. Long slit spectrophotometric data in the
optical range were used to derive the nebular electron temperature, density and
chemical abundances and for the spectral classification of the ionizing star
candidates. Halpha and UBV CCD photometry was carried out to derive stellar
distances from spectroscopic parallax and to measure the ionizing photon flux.Comment: To be published in Astronomy & Astrophysic
The Nature of Starbursts: I. The Star Formation Histories of Eighteen Nearby Starburst Dwarf Galaxies
We use archival HST observations of resolved stellar populations to derive
the star formation histories (SFHs) of 18 nearby starburst dwarf galaxies. In
this first paper we present the observations, color-magnitude diagrams, and the
SFHs of the 18 starburst galaxies, based on a homogeneous approach to the data
reduction, differential extinction, and treatment of photometric completeness.
We adopt a star formation rate (SFR) threshold normalized to the average SFR of
the individual system as a metric for classifying starbursts in SFHs derived
from resolved stellar populations. This choice facilitates finding not only
currently bursting galaxies but also "fossil" bursts increasing the sample size
of starburst galaxies in the nearby (D<8 Mpc) universe. Thirteen of the
eighteen galaxies are experiencing ongoing bursts and five galaxies show fossil
bursts. From our reconstructed SFHs, it is evident that the elevated SFRs of a
burst are sustained for hundreds of Myr with variations on small timescales. A
long >100 Myr temporal baseline is thus fundamental to any starburst definition
or identification method. The longer lived bursts rule out rapid
"self-quenching" of starbursts on global scales. The bursting galaxies' gas
consumption timescales are shorter than the Hubble time for all but one galaxy
confirming the short-lived nature of starbursts based on fuel limitations.
Additionally, we find the strength of the H{\alpha} emission usually correlates
with the CMD based SFR during the last 4-10 Myr. However, in four cases, the
H{\alpha} emission is significantly less than what is expected for models of
starbursts; the discrepancy is due to the SFR changing on timescales of a few
Myr. The inherently short timescale of the H{\alpha} emission limits
identifying galaxies as starbursts based on the current characteristics which
may or may not be representative of the recent SFH of a galaxy.Comment: 53 pages, 11 figure
HSD3B1 genotype identifies glucocorticoid responsiveness in severe asthma
Asthma resistance to glucocorticoid treatment is a major health problem with unclear etiology. Glucocorticoids inhibit adrenal androgen production. However, androgens have potential benefits in asthma. HSD3B1 encodes for 3β-hydroxysteroid dehydrogenase-1 (3β-HSD1), which catalyzes peripheral conversion from adrenal dehydroepiandrosterone (DHEA) to potent androgens and has a germline missense-encoding polymorphism. The adrenal restrictive HSD3B1(1245A) allele limits conversion, whereas the adrenal permissive HSD3B1(1245C) allele increases DHEA metabolism to potent androgens. In the Severe Asthma Research Program (SARP) III cohort, we determined the association between DHEA-sulfate and percentage predicted forced expiratory volume in 1 s (FEV1PP). HSD3B1(1245) genotypes were assessed, and association between adrenal restrictive and adrenal permissive alleles and FEV1PP in patients with (GC) and without (noGC) daily oral glucocorticoid treatment was determined (n = 318). Validation was performed in a second cohort (SARP I&II; n = 184). DHEA-sulfate is associated with FEV1PP and is suppressed with GC treatment. GC patients homozygous for the adrenal restrictive genotype have lower FEV1PP compared with noGC patients (54.3% vs. 75.1%; P < 0.001). In patients with the homozygous adrenal permissive genotype, there was no FEV1PP difference in GC vs. noGC patients (73.4% vs. 78.9%; P = 0.39). Results were independently confirmed: FEV1PP for homozygous adrenal restrictive genotype in GC vs. noGC is 49.8 vs. 63.4 (P < 0.001), and for homozygous adrenal permissive genotype, it is 66.7 vs. 67.7 (P = 0.92). The adrenal restrictive HSD3B1(1245) genotype is associated with GC resistance. This effect appears to be driven by GC suppression of 3β-HSD1 substrate. Our results suggest opportunities for prediction of GC resistance and pharmacologic intervention
Comprehensive Gene and microRNA Expression Profiling Reveals a Role for microRNAs in Human Liver Development
BACKGROUND AND AIMS: microRNAs (miRNAs) are small noncoding RNAs that regulate cognate mRNAs post-transcriptionally. miRNAs have been implicated in regulating gene expression in embryonic developmental processes, including proliferation and differentiation. The liver is a multifunctional organ, which undergoes rapid changes during the developmental period and relies on tightly-regulated gene expression. Little is known regarding the complex expression patterns of both mRNAs and miRNAs during the early stages of human liver development, and the role of miRNAs in the regulation of this process has not been studied. The aim of this work was to study the impact of miRNAs on gene expression during early human liver development. METHODS: Global gene and miRNA expression were profiled in adult and in 9-12w human embryonic livers, using high-density microarrays and quantitative RT-PCR. RESULTS: Embryonic liver samples exhibited a gene expression profile that differentiated upon progression in the developmental process, and revealed multiple regulated genes. miRNA expression profiling revealed four major expression patterns that correlated with the known function of regulated miRNAs. Comparison of the expression of the most regulated miRNAs to that of their putative targets using a novel algorithm revealed a significant anti-correlation for several miRNAs, and identified the most active miRNAs in embryonic and in adult liver. Furthermore, our algorithm facilitated the identification of TGFbeta-R1 as a novel target gene of let-7. CONCLUSIONS: Our results uncover multiple regulated miRNAs and genes throughout human liver development, and our algorithm assists in identification of novel miRNA targets with potential roles in liver development
Long-term results of diaphragmatic plication in adults with unilateral diaphragm paralysis
<p>Abstract</p> <p>Background</p> <p>In this study we aimed to evaluate the long-term outcome of diaphragmatic plication for symptomatic unilateral diaphragm paralysis.</p> <p>Methods</p> <p>Thirteen patients who underwent unilateral diaphragmatic plication (2 patients had right, 11 left plication) between January 2003 and December 2006 were evaluated. One patient died postoperatively due to sepsis. The remaining 12 patients [9 males, 3 females; mean age 60 (36-66) years] were reevaluated with chest radiography, flouroscopy or ultrasonography, pulmonary function tests, computed tomography (CT) or magnetic resonance imaging (MRI), and the MRC/ATS dyspnea score at an average of 5.4 (4-7) years after diaphragmatic plication.</p> <p>Results</p> <p>The etiology of paralysis was trauma (9 patients), cardiac by pass surgery (3 patients), and idiopathic (1 patient). The principle symptom was progressive dyspnea with a mean duration of 32.9 (22-60) months before surgery. All patients had an elevated hemidiaphragm and paradoxical movement radiologically prior to surgery. There were partial atelectasis and reccurent infection of the lower lobe in the affected side on CT in 9 patients. Atelectasis was completely improved in 9 patients after plication. Preoperative spirometry showed a clear restrictive pattern. Mean preoperative FVC was 56.7 ± 11.6% and FEV1 65.3 ± 8.7%. FVC and FEV1 improved by 43.6 ± 30.6% (p < 0.001) and 27.3 ± 10.9% (p < 0.001) at late follow-up. MRC/ATS dyspnea scores improved 3 points in 11 patients and 1 point in 1 patient at long-term (p < 0.0001). Eight patients had returned to work at 3 months after surgery.</p> <p>Conclusions</p> <p>Diaphragmatic plication for unilateral diaphragm paralysis decreases lung compression, ensures remission of symptoms, and improves quality of life in long-term period.</p
ITGB5 and AGFG1 variants are associated with severity of airway responsiveness
Background: Airway hyperresponsiveness (AHR), a primary characteristic of asthma, involves increased airway smooth muscle contractility in response to certain exposures. We sought to determine whether common genetic variants were associated with AHR severity. Methods: A genome-wide association study (GWAS) of AHR, quantified as the natural log of the dosage of methacholine causing a 20% drop in FEV1, was performed with 994 non-Hispanic white asthmatic subjects from three drug clinical trials: CAMP, CARE, and ACRN. Genotyping was performed on Affymetrix 6.0 arrays, and imputed data based on HapMap Phase 2, was used to measure the association of SNPs with AHR using a linear regression model. Replication of primary findings was attempted in 650 white subjects from DAG, and 3,354 white subjects from LHS. Evidence that the top SNPs were eQTL of their respective genes was sought using expression data available for 419 white CAMP subjects. Results: The top primary GWAS associations were in rs848788 (P-value 7.2E-07) and rs6731443 (P-value 2.5E-06), located within the ITGB5 and AGFG1 genes, respectively. The AGFG1 result replicated at a nominally significant level in one independent population (LHS P-value 0.012), and the SNP had a nominally significant unadjusted P-value (0.0067) for being an eQTL of AGFG1. Conclusions: Based on current knowledge of ITGB5 and AGFG1, our results suggest that variants within these genes may be involved in modulating AHR. Future functional studies are required to confirm that our associations represent true biologically significant findings
Detrimental Effects of Environmental Tobacco Smoke in Relation to Asthma Severity
Background: Environmental tobacco smoke (ETS) has adverse effects on the health of asthmatics, however the harmful consequences of ETS in relation to asthma severity are unknown. Methods: In a multicenter study of severe asthma, we assessed the impact of ETS exposure on morbidity, health care utilization and lung functions; and activity of systemic superoxide dismutase (SOD), a potential oxidative target of ETS that is negatively associated with asthma severity. Findings: From 2002-2006, 654 asthmatics (non-severe 366, severe 288) were enrolled, among whom 109 non-severe and 67 severe asthmatics were routinely exposed to ETS as ascertained by history and validated by urine cotinine levels. ETS-exposure was associated with lower quality of life scores; greater rescue inhaler use; lower lung function; greater bronchodilator responsiveness; and greater risk for emergency room visits, hospitalization and intensive care unit admission. ETS-exposure was associated with lower levels of serum SOD activity, particularly in asthmatic women of African heritage. Interpretation: ETS-exposure of asthmatic individuals is associated with worse lung function, higher acuity of exacerbations, more health care utilization, and greater bronchial hyperreactivity. The association of diminished systemic SOD activity to ETS exposure provides for the first time a specific oxidant mechanism by which ETS may adversely affect patients with asthma. © 2011 Comhair et al
