Correlation of mechanical factors and gallbladder pain
Acalculous biliary pain occurs in patients with no gallstones, but is similar to that experienced by patients with gallstones. Surgical removal of the gallbladder (GB) in these patients is only successful in providing relief of symptoms to about half of those operated on, so a reliable pain-prediction model is needed. In this paper, a mechanical model is developed for the human biliary system during the emptying phase, based on a clinical test in which GB volume changes are measured in response to a standard stimulus and a recorded pain profile. The model can describe the bile emptying behaviour, the flow resistance in the biliary ducts, the peak total stress, including the passive and active stresses experienced by the GB during emptying. This model is used to explore the potential link between GB pain and mechanical factors. It is found that the peak total normal stress may be used as an effective pain indicator for GB pain. When this model is applied to clinical data of volume changes due to Cholecystokinin stimulation and pain from 37 patients, it shows a promising success rate of 88.2% in positive pain prediction
Estimation of genetic variation in residual variance in female and male broiler chickens
In breeding programs, robustness of animals and uniformity of end product can be improved by exploiting genetic variation in residual variance. Residual variance can be defined as environmental variance after accounting for all identifiable effects. The aims of this study were to estimate genetic variance in residual variance of body weight, and to estimate genetic correlations between body weight itself and its residual variance and between female and male residual variance for broilers. The data sets comprised 26 972 female and 24 407 male body weight records. Variance components were estimated with ASREML. Estimates of the heritability of residual variance were in the range 0.029 (s.e.50.003) to 0.047 (s.e.50.004). The genetic coefficients of variation were high, between 0.35 and 0.57. Heritabilities were higher in females than in males. Accounting for heterogeneous residual variance increased the heritabilities for body weight as well. Genetic correlations between body weight and its residual variance were 20.41 (s.e.50.032) and 20.45 (s.e.50.040), respectively, in females and males. The genetic correlation between female and male residual variance was 0.11 (s.e.50.089), indicating that female and male residual variance are different traits. Novel Plasmodium falciparum clones and rising clone multiplicities are associated with the increase in malaria morbidity in Ghanaian children during the transition into the high transmission season
A survey of Plasmodium falciparum infection and clone multiplicity in Ghanaian children was carried out to study the effect of the onset of the malaria transmission season on disease incidence. Fortnightly blood samples were collected from 40 children living in the rural town of Dodowa, between February and August 1998. P. falciparum parasite densities were calculated and PCR genotyping was carried out using the polymorphic MSP-1 and MSP-2 genes as target loci for the estimation of the number of parasite clones in each sample. The average clone number was estimated using maximum likelihood techniques and the minimum number of clones per patient was analysed for the effects of age, sex, season, minimum number of clones per child, level of parasitaemia and parasite genotype. The statistical analysis indicated that the more clones a child carried, the more likely they were to have a clinical malaria episode. This was true after adjusting for age and season effects and for the measured circulating parasitaemia. The probability of clinical disease also increased if the MSP-1 MAD 20 and the MSP-2 FC 27 alleles were present. This longitudinal analysis thus indicates that the probability of a Ghanaian child having a symptomatic malaria episode is positively associated with both increasing numbers and novel types of P. falciparum clones

Multispecies virial expansions
We study the virial expansion of mixtures of countably many different types of particles. The main tool is the Lagrange–Good inversion formula, which has other applications such as counting coloured trees or studying probability generating functions in multi-type branching processes. We prove that the virial expansion converges absolutely in a domain of small densities. In addition, we establish that the virial coefficients can be expressed in terms of two-connected graphs
Randomized multicentre pilot study of sacubitril/valsartan versus irbesartan in patients with chronic kidney disease: United Kingdom Heart and Renal Protection (HARP)- III—rationale, trial design and baseline data
BACKGROUND:
Patients with chronic kidney disease (CKD) are at risk of progression to end-stage renal disease and cardiovascular disease. Data from other populations and animal experiments suggest that neprilysin inhibition (which augments the natriuretic peptide system) may reduce these risks, but clinical trials among patients with CKD are required to test this hypothesis.
METHODS:
UK Heart and Renal Protection III (HARP-III) is a multicentre, double-blind, randomized controlled trial comparing sacubitril/valsartan 97/103 mg two times daily (an angiotensin receptor-neprilysin inhibitor) with irbesartan 300 mg one time daily among 414 patients with CKD. Patients ≥18 years of age with an estimated glomerular filtration rate (eGFR) of ≥45 but <60 mL/min/1.73 m2 and urine albumin:creatinine ratio (uACR) >20 mg/mmol or eGFR ≥20 but <45 mL/min/1.73 m2 (regardless of uACR) were invited to be screened. Following a 4- to 7-week pre-randomization single-blind placebo run-in phase (during which any current renin-angiotensin system inhibitors were stopped), willing and eligible participants were randomly assigned either sacubitril/valsartan or irbesartan and followed-up for 12 months. The primary aim was to compare the effects of sacubitril/valsartan and irbesartan on measured GFR after 12 months of therapy. Important secondary outcomes include effects on albuminuria, change in eGFR over time and the safety and tolerability of sacubitril/valsartan in CKD.
RESULTS:
Between November 2014 and January 2016, 620 patients attended a screening visit and 566 (91%) entered the pre-randomization run-in phase. Of these, 414 (73%) participants were randomized (mean age 63 years; 72% male). The mean eGFR was 34.0 mL/min/1.73 m2 and the median uACR was 58.5 mg/mmol.
CONCLUSIONS:
UK HARP-III will provide important information on the short-term effects of sacubitril/valsartan on renal function, tolerability and safety among patients with CKD
Anisotropic behaviour of human gallbladder walls
Inverse estimation of biomechanical parameters of soft tissues from non-invasive measurements has clinical significance in patient-specific modelling and disease diagnosis. In this paper, we propose a fully nonlinear approach to estimate the mechanical properties of the human gallbladder wall muscles from in vivo ultrasound images. The iteration method consists of a forward approach, in which the constitutive equation is based on a modified Hozapfel–Gasser–Ogden law initially developed for arteries. Five constitutive parameters describing the two orthogonal families of fibres and the matrix material are determined by comparing the computed displacements with medical images. The optimisation process is carried out using the MATLAB toolbox, a Python code, and the ABAQUS solver. The proposed method is validated with published artery data and subsequently applied to ten human gallbladder samples. Results show that the human gallbladder wall is anisotropic during the passive refilling phase, and that the peak stress is 1.6 times greater than that calculated using linear mechanics. This discrepancy arises because the wall thickness reduces by 1.6 times during the deformation, which is not predicted by conventional linear elasticity. If the change of wall thickness is accounted for, then the linear model can used to predict the gallbladder stress and its correlation with pain. Multiple tier fuel cycle studies for waste transmutation.
As part of the U.S. Department of Energy Advanced Accelerator Applications Program, a systems study was conducted to evaluate the transmutation performance of advanced fuel cycle strategies. Three primary fuel cycle strategies were evaluated: dual-tier systems with plutonium separation, dual-tier systems without plutonium separation, and single-tier systems without plutonium separation. For each case, the system mass flow and TRU consumption were evaluated in detail. Furthermore, the loss of materials in fuel processing was tracked including the generation of new waste streams. Based on these results, the system performance was evaluated with respect to several key transmutation parameters including TRU inventory reduction, radiotoxicity, and support ratio. The importance of clean fuel processing ({approx}0.1% losses) and inclusion of a final tier fast spectrum system are demonstrated. With these two features, all scenarios capably reduce the TRU and plutonium waste content, significantly reducing the radiotoxicity; however, a significant infrastructure (at least 1/10 the total nuclear capacity) is required for the dedicated transmutation system
Theory of asymmetric non-additive binary hard-sphere mixtures
We show that the formal procedure of integrating out the degrees of freedom
of the small spheres in a binary hard-sphere mixture works equally well for
non-additive as it does for additive mixtures. For highly asymmetric mixtures
(small size ratios) the resulting effective Hamiltonian of the one-component
fluid of big spheres, which consists of an infinite number of many-body
interactions, should be accurately approximated by truncating after the term
describing the effective pair interaction. Using a density functional treatment
developed originally for additive hard-sphere mixtures we determine the zero,
one, and two-body contribution to the effective Hamiltonian. We demonstrate
that even small degrees of positive or negative non-additivity have significant
effect on the shape of the depletion potential. The second virial coefficient
, corresponding to the effective pair interaction between two big spheres,
is found to be a sensitive measure of the effects of non-additivity. The
variation of with the density of the small spheres shows significantly
different behavior for additive, slightly positive and slightly negative
non-additive mixtures. We discuss the possible repercussions of these results
for the phase behavior of binary hard-sphere mixtures and suggest that
measurements of might provide a means of determining the degree of
non-additivity in real colloidal mixtures
