    The effect of health insurance reform: Evidence from China

    This paper estimates the impact of a health insurance reform on health outcomes in urban China. Using the China Health and Nutrition Survey we find that this reform increases the rate of health insurance coverage significantly among workers in Non-State Owned Enterprises. The double difference (DD) estimations show that the reform also leads to better health outcomes: workers are less likely to get sick and more likely to use preventive care. Using an instrumental variable (IV) approach to look at the causal effect of health insurance, we find those with health insurance use more preventive care but do not report significantly better health outcomes, an increase in health care utilisation, or an increase in out-of-pocket medical expenditure

    Randomized controlled trial: neostigmine for intra-abdominal hypertension in acute pancreatitis

    BACKGROUND: Intra-abdominal hypertension (IAH) in acute pancreatitis (AP) is associated with deterioration in organ function. This trial aimed to assess the efficacy of neostigmine for IAH in patients with AP. METHODS: In this single-center, randomized trial, consenting patients with IAH within 2 weeks of AP onset received conventional treatment for 24 h. Patients with sustained intra-abdominal pressure (IAP) ≥ 12 mmHg were randomized to receive intramuscular neostigmine (1 mg every 12 h increased to every 8 h or every 6 h, depending on response) or continue conventional treatment for 7 days. The primary outcome was the percent change of IAP at 24 h after randomization. RESULTS: A total of 80 patients were recruited to neostigmine (n = 40) or conventional treatment (n = 40). There was no significant difference in baseline parameters. The rate of decrease in IAP was significantly faster in the neostigmine group compared to the conventional group by 24 h (median with 25th–75th percentile: −18.7% [− 28.4 to − 4.7%] vs. − 5.4% [− 18.0% to 0], P = 0.017). This effect was more pronounced in patients with baseline IAP ≥ 15 mmHg (P = 0.018). Per-protocol analysis confirmed these results (P = 0.03). Stool volume was consistently higher in the neostigmine group during the 7-day observational period (all P < 0.05). Other secondary outcomes were not significantly different between neostigmine and conventional treatment groups. CONCLUSION: Neostigmine reduced IAP and promoted defecation in patients with AP and IAH. These results warrant a larger, placebo-controlled, double-blind phase III trial.     Child labour and academic achievement: Evidence from Gansu Province in China

    This paper considers the relationship between child labour and a child\u27s academic achievement in rural China. Using a unique longitudinal, multi-level survey, the Gansu Survey of Children and Families (GSCF) which was enrolled in the Gansu province, I use a quasi-maximum likelihood estimation (QMLE) and find that more than 1 h of child labour in the previous time period has a negative effect on a child\u27s academic achievement in the subsequent period after controlling for child talent. I also show that previous academic achievement has no strong significant effect on current child labour by applying a logistic model. Based on the data, the fact that those effects are not very big or not significant suggests that child labour in China is not a big problem when compared with other developing countries (Bacolod & Ranjan 2008)

    Extracellular electron transfer for aerobic denitrification mediated by the bioelectric catalytic system with zero-carbon source

    The slow rate of electron transfer and the large consumption of carbon sources are technical bottlenecks in the biological treatment of wastewater. Here, we first proposed to domesticate aerobic denitrifying bacteria (ADB) from heterotrophic to autotrophic by electricity (0.6 V) under zero organic carbon source conditions, to accelerate electron transfer and shorten hydraulic retention time (HRT) while increasing the biodegradation rate. Then we investigated the extracellular electron transfer (EET) mechanism mediated by this process, and additionally examined the integrated nitrogen removal efficiency of this system with composite pollution. It was demonstrated that compared with the traditional membrane bioreactor (MBR), the BEC displayed higher nitrogen removal efficiency. Especially at C/N = 0, the BEC exhibited a NO3--N removal rate of 95.42 ± 2.71 % for 4 h, which was about 6.5 times higher than that of the MBR. Under the compound pollution condition, the BEC still maintained high NO3--N and tetracycline removal (94.52 ± 2.01 % and 91.50 ± 0.001 %), greatly superior to the MBR (10.64 ± 2.01 % and 12.00 ± 0.019 %). In addition, in-situ electrochemical tests showed that the nitrate in the BEC could be directly converted to N2 by reduction using electrons from the cathode, which was successfully demonstrated as a terminal electron acceptor

    Piezoelectric Property of a Tetragonal (Ba,Ca)(Zr,Ti)O<sub>3</sub> Single Crystal and Its Fine-Domain Structure

    A tetragonal (Ba,Ca)­(Zr,Ti)­O<sub>3</sub> (BCZT) single crystal was grown by a flux method, and the piezoelectric coefficient (<i>d</i><sub>33</sub>) was characterized. The piezoelectric response was proved to be associated with polarization extension, which was successfully used to explain the variation in <i>d</i><sub>33</sub><sup>*</sup>. From the intrinsic aspect, the compositional effect on Landau free-energy profiles was discussed, showing an “extender” nature of the as-grown crystal and the increasing tendency of structural instability toward the morphotropic phase boundary. From the extrinsic aspect, the evolution of domain structure under various external fields (electric and temperature) was studied, revealing that the fine-domain structure of the as-grown BCZT single crystal was stable to E-field and temperature. The results manifest possibilities of further improving the piezoelectric property of the BCZT single crystal, which requires optimization of the crystal growth technique in future work
