    An Exploratory Study of Extended Health Care Practitioner Roles in Medication Supply and Management in a Rural Community

    Australia has a recognised shortage of qualified health professionals, particularly in rural and regional areas. Research has shown that rural patients indeed have poorer health status compared to metropolitan communities, with rates of mortality and morbidity constantly higher in rural regions. A number of ‘novel’ and ‘extended’ roles have been developed for existing health care professionals. Some of these roles have application to rural areas, such as additional functions in terms of provision of medicines. In Queensland in particular, the Health (Drugs and Poisons) Regulation 1996 (the Regulation), which provides the regulatory framework for the handling of medicines, has been amended to include a range of endorsements, including Indigenous Health Workers (IHW) and Rural and Isolated Practice-endorsed Nurses (RIPRN). Another development is the amendment of the Regulation to allow for a range of health professionals, namely optometrists, physicians’ assistants and nurse practitioners, to prescribe medicines. Whether such developments have addressed the needs of rural communities remains unknown. Inherently, rural communities will suffer from limited access to health care services, and the services of the existing health care providers may be stretched to, or beyond, the scope of their recognised practice in order to meet the needs of the community. Little is known about the extent and nature of these ‘extended’ practices, the perceived need for (or obligation on) health professionals to adopt these roles, and the ethical, professional and legal considerations if/when they extend their services into non-traditional territories. This research specifically focussed on the involvement of various health care providers in patients’ medication management in a defined community or region.     Electronic communication in the construction industry

    PURPOSE : The study aims to investigate how modern methods of communication within the construction industry have brought forth a new cognitive process that participants in this industry should undertake when communicating. DESIGN / METHODOLOGY / APPROACH : To achieve the objectives of the study, a literature review was compiled on the legal status of electronic communication and what the impact of the Electronic Communications and Transactions (ECT) Act, Act 25 of 2002, has had on electronic communication. A questionnaire was also distributed to quantity surveyors to ascertain the level of knowledge with regard to the application of the ECT Act. FINDINGS : The study found that participants are not familiar with the ECT Act and that it is highlyadvisable that parties to the agreement be made fully aware of how communications should be dealt with during the duration of the contract. Research limitations/implications – The study is restricted to the South African construction industry and construction contracts and a small target population of professional quantity surveyors practicing in the Gauteng Province. The results of the research will be taken as representative of the entire country. PRACTICAL IMPLICATIONS : In the modern era, notifications are increasingly being communicated electronically, e.g. by electronic mail, linked computer networks, the Internet and cellular phones with appropriate media capabilities. Participants in the built environment must know how to correctly, effectively and legally, deal with this information revolution. ORIGINALITY / VALUE : Modern means of communication, including in particular electronic emailing, demand that users properly appreciate whether the chosen method of communication has a contractually binding and legally enforceable effect. Thus, in an ever-changing built environment, participants should not only dedicate more time to ensure that information conveyed does not have legal implications, except if so intended, but that the information conveyed is unambiguous, grammatically correct and formulated professionally.     Adjudication in South African construction industry practice : towards legislative intervention

    Adjudication in South African construction practice has, through various initiatives of the South African government and Construction Industry Development Board, the increased use of international standard form construction contracts, and the South African High Court’s robust approach in enforcing adjudicators’ decisions, become relatively commonplace in both the public and private sectors as the first tier in dispute resolution procedures on construction projects across the South African construction industry.     The potential distribution of Bactrocera dorsalis: Considering phenology and irrigation patterns

    A species in the Bactrocera dorsalis (Hendel) complex was detected in Kenya during 2003 and classified as Bactrocera invadens Drew, Tsuruta & White. Having spread rapidly throughout Africa, it threatens agriculture due to crop damage and loss of market access. In a recent revision of the B. dorsalis complex, B. invadens was incorporated into the species B. dorsalis. The potential distribution of B. dorsalis has been previously modelled. However, previous models were based on presence data and did not incorporate information on the seasonal phenology of B. dorsalis, nor on the possible influence that irrigation may have on its distribution. Methyl eugenol-baited traps were used to collect B. dorsalis in Africa. Seasonal phenology data, measured as fly abundance throughout the year, was related to each location's climate to infer climatic growth response parameters. These functions were used along with African distribution records and development studies to fit the niche model for B. dorsalis, using independent global distribution records outside Africa for model validation. Areas at greatest risk of invasion by B. dorsalis are South and Central America, Mexico, southernmost USA, parts of the Mediterranean coast, parts of Southern and Eastern Australia and New Zealand's North Island.     Cross-sector, sessional employment of pharmacists in rural hospitals in Australia and New Zealand: a qualitative study exploring pharmacists’ perceptions and experiences

    Background: Many rural hospitals in Australia and New Zealand do not have an on-site pharmacist. Sessional employment of a local pharmacist offers a potential solution to address the clinical service needs of non-pharmacist rural hospitals. This study explored sessional service models involving pharmacists and factors (enablers and challenges) impacting on these models, with a view to informing future sessional employment. Methods: A series of semi-structured one-on-one interviews was conducted with rural pharmacists with experience, or intention to practise, in a sessional employment role in Australia and New Zealand. Participants were identified via relevant newsletters, discussion forums and referrals from contacts. Interviews were conducted during August 2012-January 2013 via telephone or Skype™, for approximately 40–55 minutes each, and recorded.Results: Seventeen pharmacists were interviewed: eight with ongoing sessional roles, five with sessional experience, and four working towards sessional employment. Most participants provided sessional hospital services on a weekly basis, mainly focusing on inpatient medication review and consultation. Recognition of the value of pharmacists’ involvement and engagement with other healthcare providers facilitated establishment and continuity of sessional services. Funds pooled from various sources supplemented some pharmacists’ remuneration in the absence of designated government funding. Enhanced employment opportunities, district support and flexibility in services facilitated the continuous operation of the sessional service. Conclusions: There is potential to address clinical pharmacy service needs in rural hospitals by cross-sector employment of pharmacists. The reported sessional model arrangements, factors impacting on sessional employment of pharmacists and learnings shared by the participants should assist development of similar models in other rural communities

    Non-host status of commercial export grade lemon fruit (Citrus limon (L.) Burman f. cv. Eureka) for ceratitis capitata, ceratitis rosa, ceratitis quilicii and bactrocera dorsalis (Diptera: Tephritidae) in South Africa

    Markets importing citrus fruit including lemons, Citrus limon (L.) Burman f., from South Africa require that the fruit be free of fruit fly pests (Diptera: Tephritidae). Historically there has been no fruit fly infestation recorded on lemons destined for export from SouthAfrica. In this study, we assessed the host status of commercial export grade Eureka lemons, Citrus limon (L.) Burmanf. cv. Eureka, for four fruit fly pest species of economic importance in South Africa: Ceratitis capitata (Wiedemann), Ceratitis rosa Karsch, Ceratitis quilicii De Meyer, Mwatawala & Virgilio, and Bactrocera dorsalis (Hendel). Trapping was conducted in 10 Eureka lemon orchards in two major citrus production regions over two citrus seasons between 2016 and 2017 to determine the level of fruit fly abundance in the sampled orchards. Lemons were collected at harvest over the two seasons in the same orchards where trapping was conducted. Fruit fly infestation of the sampled lemons was determined by dissection. Additionally, infestation of lemons was determined under forced exposure to mature mated females of C. capitata and B. dorsalis. Trapping data showed the presence of adults of all four fruit fly species in the sampled lemon orchards.Nofruit fly infestation was detected in 43 222 Eureka lemons sampled at harvest. There was also no infestation of lemons under forced exposure conditions. The results of this study provide evidence with 99.99 % efficacy and a 99%confidence level that SouthAfrican commercial export grade Eureka lemon fruit is not a host for C. capitata, C. rosa, C. quilicii or B. dorsalis.http://www.journals.co.za/content/journal/entohttp://www.bioone.org/loi/afen2020-03-01am2018Zoology and Entomolog

    Evaluation of pseudoephedrine pharmacy sales before and after mandatory recording requirements in Western Australia: a case study

    Background: A community pharmacy real-time electronic recording program, ProjectSTOP, enables Australian community pharmacists to verify pseudoephedrine requests. In Western Australia the program was available for voluntary use from April 2007 and became mandatory November 2010. This case study explores the effectiveness of the program by reviewing the total requests for pseudoephedrine products, and the proportion of requests which were classified as ‘denied sales’ before and after mandatory implementation. Seasonal and annual trends in these measures are also evaluated. Methods: ProjectSTOP data recordings for Western Australia pharmacies between 1 December 2007 and 28 February 2014 were analysed. Data included a de-identified pharmacy number and date of each pseudoephedrine product request. The total number of requests and sale classification (allowed, denied, safety, or not recorded) were calculated for each month/pharmacy. The potential influence of mandatory reporting using ProjectSTOP was investigated using a Regression Discontinuity Design. Correlations between sales from the same pharmacy were taken into account by classifying the pharmacy number as a random effect. The main effects of year (continuous variable), and season (categorical variable) were also included in the model. Results: There was a small but steady decline in the total requests for pseudoephedrine per month per 100,000 population (per pharmacy) from the time of mandatory reporting. The number of denied sales showed a steady increase up until mandatory reporting, after which it showed a significant decline over time. Total sales were heavily influenced by season, as expected (highest in winter, least in summer). The seasonal pattern was less pronounced for denied sales, which were highest in winter and similar across other seasons. The pattern over time for safety sales was similar to that for denied sales, with a clear change occurring around the time of mandatory reporting. Conclusion: Results indicate a decrease in pseudoephedrine product requests in Western Australia community pharmacies. Findings suggest ProjectSTOP has been successful in addressing suspicious sales and potential diversion however ongoing data review is recommended

    TRY plant trait database - enhanced coverage and open access

    Plant traits-the morphological, anatomical, physiological, biochemical and phenological characteristics of plants-determine how plants respond to environmental factors, affect other trophic levels, and influence ecosystem properties and their benefits and detriments to people. Plant trait data thus represent the basis for a vast area of research spanning from evolutionary biology, community and functional ecology, to biodiversity conservation, ecosystem and landscape management, restoration, biogeography and earth system modelling. Since its foundation in 2007, the TRY database of plant traits has grown continuously. It now provides unprecedented data coverage under an open access data policy and is the main plant trait database used by the research community worldwide. Increasingly, the TRY database also supports new frontiers of trait-based plant research, including the identification of data gaps and the subsequent mobilization or measurement of new data. To support this development, in this article we evaluate the extent of the trait data compiled in TRY and analyse emerging patterns of data coverage and representativeness. Best species coverage is achieved for categorical traits-almost complete coverage for 'plant growth form'. However, most traits relevant for ecology and vegetation modelling are characterized by continuous intraspecific variation and trait-environmental relationships. These traits have to be measured on individual plants in their respective environment. Despite unprecedented data coverage, we observe a humbling lack of completeness and representativeness of these continuous traits in many aspects. We, therefore, conclude that reducing data gaps and biases in the TRY database remains a key challenge and requires a coordinated approach to data mobilization and trait measurements.     Climatic and soil factors explain the two-dimensional spectrum of global plant trait variation

    Plant functional traits can predict community assembly and ecosystem functioning and are thus widely used in global models of vegetation dynamics and land–climate feedbacks. Still, we lack a global understanding of how land and climate affect plant traits. A previous global analysis of six traits observed two main axes of variation: (1) size variation at the organ and plant level and (2) leaf economics balancing leaf persistence against plant growth potential. The orthogonality of these two axes suggests they are differently influenced by environmental drivers. We find that these axes persist in a global dataset of 17 traits across more than 20,000 species. We find a dominant joint effect of climate and soil on trait variation. Additional independent climate effects are also observed across most traits, whereas independent soil effects are almost exclusively observed for economics traits. Variation in size traits correlates well with a latitudinal gradient related to water or energy limitation. In contrast, variation in economics traits is better explained by interactions of climate with soil fertility. These findings have the potential to improve our understanding of biodiversity patterns and our predictions of climate change impacts on biogeochemical cycles
