Literature Review: Diagnosing Types of Dementia Using Biomarkers
Purpose
This literature review attempts to determine the most effective biomarkers to diagnose the most common types of dementia.
Methods
Information from articles in various databases (PubMed, ProQuest, NIH, etc.) were compiled to form a list of biomarkers for the most common types of dementia.
Results
Biomarkers specific to Alzheimerâs disease (AD) include increased p-tau & t-tau, decreased AÎČ42, increased GFAP, CHIT1, & YKL-40, and increased glutamic acid, hypoxanthine, & anthranilic acid. Frontotemporal dementia (FTD) biomarkers include decreased p-tau & t-tau, increased AÎČ42/40, increased GFAP, CHIT1 & YKL-40, fastest change in NPTX2 & neurofilament light chain, and MRI anterior vs. posterior index. Biomarkers for vascular dementia (VaD) include decreased brevican & neurocan peptides (compared to AD) and increased MR-proANP & CT-proET-1. Finally, Lewy body dementia (LBD) biomarkers include decreased CSF âș-synuclein (compared to AD) and increased GFAP.
Conclusion
Understanding the specific biomarkers for each type of dementia is crucial in establishing an early and definitive diagnosis that can determine the appropriate course of treatment. Each of the biomarkers outlined in this literature review vary in their clinical applicability. Although some of them have already been incorporated into clinical practice alongside manifestations and symptoms characteristic to each type of dementia, other biomarkers (REPS1, etc.) still require further research and studies before they are put to use
Parameterized Model-Checking for Timed-Systems with Conjunctive Guards (Extended Version)
In this work we extend the Emerson and Kahlon's cutoff theorems for process
skeletons with conjunctive guards to Parameterized Networks of Timed Automata,
i.e. systems obtained by an \emph{apriori} unknown number of Timed Automata
instantiated from a finite set of Timed Automata templates.
In this way we aim at giving a tool to universally verify software systems
where an unknown number of software components (i.e. processes) interact with
continuous time temporal constraints. It is often the case, indeed, that
distributed algorithms show an heterogeneous nature, combining dynamic aspects
with real-time aspects. In the paper we will also show how to model check a
protocol that uses special variables storing identifiers of the participating
processes (i.e. PIDs) in Timed Automata with conjunctive guards. This is
non-trivial, since solutions to the parameterized verification problem often
relies on the processes to be symmetric, i.e. indistinguishable. On the other
side, many popular distributed algorithms make use of PIDs and thus cannot
directly apply those solutions
Late widespread skeletal metastases from myxoid liposarcoma detected by MRI only
Background
Myxoid liposarcoma is the second most commonly occurring sub-type of liposarcomas. In contrast to other soft tissue sarcomas, it is known to have a tendency to spread toward extrapulmonary sites, such as soft tissues, retroperitoneum, and the peritoneal surface. Bony spread, however, is not as common.
Case presentation
We report an unusual case of diffuse skeletal metastases from myxoid liposarcoma occurring 13 years after treatment of the primary tumour in the left lower limb. The skeletal spread of the disease was demonstrated on MRI only after other imaging modalities (plain radiography, CT and TC99 bone scans) had failed to detect these metastases.
Conclusion
MRI is an extremely sensitive and specific screening tool in the detection of skeletal involvement in these types of sarcomas, and therefore, should be a part of the staging proces
The impact of induction chemotherapy on the outcome of second-line therapy with pemetrexed or docetaxel in patients with advanced non-small-cell lung cancer
Background: Using data from a large phase III study of previously treated advanced non-small-cell lung cancer (NSCLC) that showed similar efficacy for pemetrexed and docetaxel, this retrospective analysis evaluates the impact of first-line chemotherapy on the outcome of second-line chemotherapy. Patients and methods: In all, 571 patients with advanced NSCLC were randomly assigned to receive pemetrexed 500 mg/m2 or docetaxel 75 mg/m2 on day 1 of a 21-day cycle. Comparisons were made based on type of first-line therapy [gemcitabine + platinum (GP), taxane + platinum (TP), or other therapies (OT)], response to initial therapy, time since initial therapy, and clinical characteristics. The two second-line treatment groups were pooled for this analysis due to similar efficacy and were assumed to have no interaction with the first-line therapies. Results: Baseline characteristics were generally balanced. By multivariate analysis, gender, stage at diagnosis, performance status (PS), and best response to first-line therapy significantly influenced overall survival (OS). Additional factors by univariate analysis, histology, and time elapsed from first- to second-line therapy significantly influenced OS. Conclusions: Future trials in the second-line setting should stratify patients by gender, stage at diagnosis, PS, and best response to first-line therap

The PALM Technique: histological findings of masked phototherapeutic keratectomy on rabbit corneas
BACKGROUND: To compare the corneal healing response between conventional and phototherapeutic keratectomy through a masking agent, in rabbit corneas. METHODS: 24 adult rabbits underwent phototherapeutic keratectomy. Animals were divided in two groups: 12 received photoablation through a masking agent (PALM gel) and the remaining 12 received conventional phototherapeutic keratectomy of equal depth and served as control. Light and transmission electron microscopy was performed in specimens of both groups obtained: immediately after, four hours, one week, one, three and six months after treatment. RESULTS: Reepitheliazation was complete within five days in all eyes. Light and transmission electron microscopy did not reveal any differences of the healing process in the experimental eyes compared to the controls. CONCLUSION: Photoablation through the PALM technique did not result any evident alterations of the reepithelisation and stromal healing process
Prospective randomized study comparing the Teleflex Medical SaphLITE Retractor to the Ethicon CardioVations Clearglide Endoscopic System
BACKGROUND: Several minimally invasive saphenous vein harvesting techniques have been developed to reduce morbidities associated with coronary artery bypass grafting. This prospective, randomized study was designed to compare two commonly used minimally invasive saphenous vein harvesting techniques, the SaphLITE Retractor System (Teleflex Medical) and the Clearglide Endoscopic Vessel Harvesting System (Ethicon CardioVations, Inc.). METHODS: Between January 2003 and March 2004, a total of 200 patients scheduled for primary, nonemergent coronary artery bypass grafting, with or without concomitant procedures were randomized into two groups: SaphLITE (n = 100) and Clearglide (n = 100). Pre-, intra- and postoperative data was collected and subjected to statistical analysis. Randomization provided homogenous groups with respect to preoperative risk factors. RESULTS: Harvest location for the SaphLITE group was thigh (n = 40), lower leg (n = 5) and both lower leg and thigh (n = 55). The location of harvest for the Clearglide group was thigh (n = 3), lower leg (n = 16) and both lower leg and thigh (n = 81). The mean incision length was 3.6 cm (range, 2â6) in the SaphLITE group versus 2.1 cm (range, 1â4) in the Clearglide group (p < 0.05). The total incision length was 12.9 cm versus 8.9 (p < 0.05) in the SaphLITE and Clearglide groups. Conversion to the open technique occurred in 5 SaphLITE patients and 7 Clearglide patients. Intraoperative leg exploration for bleeding occurred in two of the Clearglide patients and none of the SaphLITE patients. Post-operative complications specifically related to minimally invasive harvesting technique, including a two-week post-discharge visit, were not statistically different between the groups. CONCLUSION: The saphenous vein can be safely harvested utilizing the SaphLITE and Clearglide systems. While the Clearglide system allows for fewer incisions (number and length) and less harvest time, these benefits may be outweighed by the increased cost of the Clearglide system compared to the SaphLITE retractor
Genetic association study of QT interval highlights role for calcium signaling pathways in myocardial repolarization.
The QT interval, an electrocardiographic measure reflecting myocardial repolarization, is a heritable trait. QT prolongation is a risk factor for ventricular arrhythmias and sudden cardiac death (SCD) and could indicate the presence of the potentially lethal mendelian long-QT syndrome (LQTS). Using a genome-wide association and replication study in up to 100,000 individuals, we identified 35 common variant loci associated with QT interval that collectively explain âŒ8-10% of QT-interval variation and highlight the importance of calcium regulation in myocardial repolarization. Rare variant analysis of 6 new QT interval-associated loci in 298 unrelated probands with LQTS identified coding variants not found in controls but of uncertain causality and therefore requiring validation. Several newly identified loci encode proteins that physically interact with other recognized repolarization proteins. Our integration of common variant association, expression and orthogonal protein-protein interaction screens provides new insights into cardiac electrophysiology and identifies new candidate genes for ventricular arrhythmias, LQTS and SCD
Perspectives on the chemical etiology of breast cancer.
Multiple factors, known and unknown, contribute to human breast cancer. Hereditary, hormonal, and reproductive factors are associated with risk of breast cancer. Environmental agents, including chemical carcinogens, are modifiable risk factors to which over 70% of breast cancers have been attributed. Polymorphisms of drug-metabolizing enzymes may influence risk of breast cancer from environmental chemicals, dietary agents, and endogenous steroids. The environmental factors discussed in this review include pollutants, occupational exposures, tobacco smoke, alcohol, and diet. Aromatic amines are discussed as potential mammary carcinogens, with a focus on heterocyclic amine food pyrolysis products. These compounds are excreted into the urine after consumption of meals containing cooked meats and have recently been detected in the breast milk of lactating women
