    Liquid phase migration in cemented carbides – Experiments and modelling

    Liquid phase migration (LPM) and its potential impact on the phenomenon known as cobalt capping in cemented carbides have been investigated through experiments on fully densified, pre-sintered samples and simulations. A model for LPM was developed based on interface energies, grain size, and contiguity, and was implemented and integrated with Thermo-Calc software to make predictions about the final microstructure. The model\u27s predictions, when applied to cases with gradients in grain size and volume fraction of the binder phase, were found to somewhat match experimental observations. Additionally, it was obsereved that LPM can cause a phenomenon resembling cobalt capping due to gradients in carbon activity

    BISMICS consensus statement: implementing a safe minimally invasive mitral programme in the UK healthcare setting

    Disseminating the practice of minimally invasive mitral surgery (mini-MVS) can be challenging, despite its original case reports a few decades ago. The penetration of this technology into clinical practice has been limited to centres of excellence and mitral surgery in most general cardiothoracic centres remains to be conducted via sternotomy access as a first line. The process for the uptake of mini-MVS requires clearer guidance and standardisation for the processes involved in its implementation. In this statement, a consensus agreement is outlined that describes the benefits of mini-MVS, including reduced post-operative bleeding, reduced wound infection, enhanced recovery and patient satisfaction. Technical considerations require specific attention and can introduced through simulation and/or use in conventional cases. Either endoballoon or aortic cross clamping are both recommended as well as femoral or central aortic cannulation, with the use of appropriate adjuncts and instruments. A coordinated team-based approach that encourages ownership of the programme by the team members is critical. A designated proctor is also recommended. The organisation of structured training and simulation, as well as planning the initial cases are important steps to consider. The importance of pre-empting complications and dealing with adverse events are described, including re-exploration, conversion to sternotomy, uni-lateral pulmonary oedema and phrenic nerve injury. Accounting for both institutional and team considerations can effectively facilitate the introduction of a mini-MVS service. This involves simulation, team-based training, visits to specialist centres and involvement of a designated proctor to oversee the initial cases

    Mobilities of Ti and Fe in disordered TiFe-BCC assessed from new experimental data

    Pure titanium has an HCP structure and lacks mechanical properties for many industrial purposes. The BCC phase of Ti is required to make alloys with increased strength compared to pure Ti. Iron is the most potent element for stabilising the BCC phase. However, the addition of Fe to Ti causes segregation issues during solidification, which can be avoided by diffusion-driven solid-state alloying. To predict the diffusion kinetics, the interaction mobility parameters of Ti and Fe in the disordered BCC phase of Ti are necessary. In this work, these parameters are optimised based on new experimental data from Ti-Fe diffusion couples produced by the Field Assisted Sintering Technology (FAST). Diffusion couples were held at 1173K and 1273K for one hour. High-resolution Fe concentration profiles are obtained from Electron Probe Micro Analyser (EPMA). Ternary mobility interaction parameters are assessed based on binary endmembers with a DICTRA sub-module, and results are compared to earlier assessments of mobilities of the disordered BCC TiFe system

    Analysis of the impact of length of stay on the quality of service experience, satisfaction and loyalty

    Although length of stay is a relevant variable in destination management, little research has been produced connecting it with tourists' post-consumption behaviour. This research compares the post-consumption behaviour of same-day visitors with overnight tourists in a sample of 398 domestic vacationers at two Mediterranean heritage-and-beach destinations. Although economic research on length of stay posits that there are destination benefits in longer stays, same-day visitors score higher in most of the post-consumption variables under study. Significant differences arise in hedonic aspects of the tourist experience and destination loyalty. Thus, we propose that length of stay can be used as a segmentation variable. Furthermore, destination management organisations need to consider length of stay when designing tourism policies. The tourist product and communication strategies might be adapted to different vacation durations

    Postoperative acute kidney injury in adult non-cardiac surgery:joint consensus report of the Acute Disease Quality Initiative and PeriOperative Quality Initiative

    Postoperative acute kidney injury (PO-AKI) is a common complication of major surgery that is strongly associated with short-term surgical complications and long-term adverse outcomes, including increased risk of chronic kidney disease, cardiovascular events and death. Risk factors for PO-AKI include older age and comorbid diseases such as chronic kidney disease and diabetes mellitus. PO-AKI is best defined as AKI occurring within 7 days of an operative intervention using the Kidney Disease Improving Global Outcomes (KDIGO) definition of AKI; however, additional prognostic information may be gained from detailed clinical assessment and other diagnostic investigations in the form of a focused kidney health assessment (KHA). Prevention of PO-AKI is largely based on identification of high baseline risk, monitoring and reduction of nephrotoxic insults, whereas treatment involves the application of a bundle of interventions to avoid secondary kidney injury and mitigate the severity of AKI. As PO-AKI is strongly associated with long-term adverse outcomes, some form of follow-up KHA is essential; however, the form and location of this will be dictated by the nature and severity of the AKI. In this Consensus Statement, we provide graded recommendations for AKI after non-cardiac surgery and highlight priorities for future research

    Impact of smoke-free legislation on perinatal and infant mortality:a national quasi-experimental study

    Smoke-free legislation is associated with improved early-life outcomes; however its impact on perinatal survival is unclear. We linked individual-level data with death certificates for all registered singletons births in England (1995-2011). We used interrupted time series logistic regression analysis to study changes in key adverse perinatal events following the July 2007 national, comprehensive smoke-free legislation. We studied 52,163 stillbirths and 10,238,950 live-births. Smoke-free legislation was associated with an immediate 7.8% (95%CI 3.5-11.8; p < 0.001) reduction in stillbirth, a 3.9% (95%CI 2.6-5.1; p < 0.001) reduction in low birth weight, and a 7.6% (95%CI 3.4-11.7; p = 0.001) reduction in neonatal mortality. No significant impact on SIDS was observed. Using a counterfactual scenario, we estimated that in the first four years following smoke-free legislation, 991 stillbirths, 5,470 cases of low birth weight, and 430 neonatal deaths were prevented. In conclusion, smoke-free legislation in England was associated with clinically important reductions in severe adverse perinatal outcomes
