    Benchmarking and viability assessment of optical packet switching for metro networks

    Optical packet switching (OPS) has been proposed as a strong candidate for future metro networks. This paper assesses the viability of an OPS-based ring architecture as proposed within the research project DAVID (Data And Voice Integration on DWDM), funded by the European Commission through the Information Society Technologies (IST) framework. Its feasibility is discussed from a physical-layer point of view, and its limitations in size are explored. Through dimensioning studies, we show that the proposed OPS architecture is competitive with respect to alternative metropolitan area network (MAN) approaches, including synchronous digital hierarchy, resilient packet rings (RPR), and star-based Ethernet. Finally, the proposed OPS architectures are discussed from a logical performance point of view, and a high-quality scheduling algorithm to control the packet-switching operations in the rings is explained

    Living with chronic migraine: a qualitative study on female patients' perspectives from a specialised headache clinic in Spain

    OBJECTIVES: The aim of this study was to explore the views and experiences of a group of Spanish women suffering from chronic migraine (CM). SETTING: Headache clinic at a university hospital in Madrid (Spain). PARTICIPANTS: Purposeful sampling of patients that attended a specialised headache clinic for the first time between June 2016 and February 2017 was performed. The patients included were females aged 18–65 and with positive diagnoses of CM according to the International Classification of Headache disorders (third edition, beta version), with or without medication overuse. Accordingly, 20 patients participated in the study with a mean age of 38.65 years (SD 13.85). DESIGN: Qualitative phenomenological study. METHODS: Data were collected through in-depth interviews, researchers’ field notes and patients’ drawings. A thematic analysis was performed following appropriate guidelines for qualitative research. RESULTS: Five main themes describing the significance of suffering emerged: (a) the shame of suffering from an invisible condition; (b) treatment: between need, scepticism and fear; (c) looking for physicians’ support and sincerity and fighting misconceptions; (d) limiting the impact on daily life through self-control; and (e) family and work: between understanding and disbelief. The disease is experienced as an invisible process, and the journey to diagnosis can be a long and tortuous one. Drug prescription by the physician is greeted with distrust and scepticism. Patients expect sincerity, support and the involvement of their doctors in relation to their disease. Pain becomes the main focus of the patient’s life, and it requires considerable self-control. The disease has a strong impact in the work and family environment, where the patient may feel misunderstood. CONCLUSIONS: Qualitative research offers insight into the way patients with CM experience their disease and it may be helpful in establishing a more fruitful relationship with these patients

    Pressure Relieving Support Surfaces for Pressure Ulcer Prevention (PRESSURE 2): Clinical and Health Economic Results of a Randomised Controlled Trial

    Background Pressure ulcers (PUs) are complications of serious acute/chronic illness. Specialist mattresses used for prevention lack high quality effectiveness evidence. We aimed to compare clinical and cost effectiveness of 2 mattress types. Methods Multicentre, Phase III, open, prospective, parallel group, randomised controlled trial in 42 UK secondary/community in-patient facilities. 2029 high risk (acutely ill, bedfast/chairfast and/or Category 1 PU/pain at PU site) adult in-patients were randomised (1:1, allocation concealment, minimisation with random element) factors including: centre, PU status, facility and consent type. Interventions were alternating pressure mattresses (APMs) or high specification foam (HSF) for maximum treatment phase 60 days. Primary outcome was time to development of new PU Category ≥ 2 from randomisation to 30 day post-treatment follow-up in intention-to treat population. Trial registration: ISRCTN 01151335. Findings Between August 2013 and November 2016, we randomised 2029 patients (1016 APMs: 1013 HSF) who developed 160(7.9%) PUs. There was insufficient evidence of a difference between groups for time to new PU Category ≥ 2 Fine and Gray Model Hazard Ratio HR = 0.76, 95%CI0.56–1.04); exact P = 0.0890; absolute difference 2%). There was a statistically significant difference in the treatment phase time to event sensitivity analysis, Fine and Gray model HR = 0.66, 95%CI, 0.46–0.93; exact P = 0.0176); 2.6% absolute difference). Economic analyses indicate that APM are cost-effective. There were no safety concerns. Interpretation In high risk (acutely ill, bedfast/chairfast/Category 1 PU/ pain on a PU site) in-patients, we found insufficient evidence of a difference in time to PU development at 30-day final follow-up, which may be related to a low event rate affecting trial power. APMs conferred a small treatment phase benefit. Patient preference, low PU incidence and small group differences suggests the need for improved targeting of APMs with decision making informed by patient preference/comfort/rehabilitation needs and the presence of potentially modifiable risk factors such as being completely immobile, nutritional deficits, lacking capacity and/or altered skin/Category1 PU

    Advancing measurements and representations of subsurface heterogeneity and dynamic processes: towards 4D hydrogeology

    Essentially all hydrogeological processes are strongly influenced by the subsurface spatial heterogeneity and the temporal variation of environmental conditions, hydraulic properties, and solute concentrations. This spatial and temporal variability generally leads to effective behaviors and emerging phenomena that cannot be predicted from conventional approaches based on homogeneous assumptions and models. However, it is not always clear when, why, how, and at what scale the 4D (3D + time) nature of the subsurface needs to be considered in hydrogeological monitoring, modeling, and applications. In this paper, we discuss the interest and potential for the monitoring and characterization of spatial and temporal variability, including 4D imaging, in a series of hydrogeological processes: (1) groundwater fluxes, (2) solute transport and reaction, (3) vadose zone dynamics, and (4) surface–subsurface water interactions. We first identify the main challenges related to the coupling of spatial and temporal fluctuations for these processes. We then highlight recent innovations that have led to significant breakthroughs in high-resolution space–time imaging and modeling the characterization, monitoring, and modeling of these spatial and temporal fluctuations. We finally propose a classification of processes and applications at different scales according to their need and potential for high-resolution space–time imaging. We thus advocate a more systematic characterization of the dynamic and 3D nature of the subsurface for a series of critical processes and emerging applications. This calls for the validation of 4D imaging techniques at highly instrumented observatories and the harmonization of open databases to share hydrogeological data sets in their 4D components

    Scientific floras can be reliable sources for some trait data in a system with poor coverage in global trait databases

    Aim: Trait‐based approaches are increasingly important in ecology and biogeography, but progress is often hampered by the availability of high‐quality quantitative trait data collected in the field. Alternative sources of trait information include scientific floras and taxonomic monographs. Here we test the reliability and usefulness of trait data acquired from scientific floras against trait values measured in the field, and those in TRY, the most comprehensive global plant trait database. Location: Tenerife and La Palma, Canary Islands, Spain. Methods: We measured leaf area and specific leaf area (SLA) in the field for 451 native vascular plant species and compared them with equivalent trait data digitised from the most recent and comprehensive guide of the Canarian flora, and data sourced from TRY. We regressed the field‐measured traits against their equivalents estimated from the literature and used the regression models from one island to predict the trait values on the other island. Results: For leaf area, linear models showed good agreement between values from the scientific flora and those measured in the field (r2 = 0.86). These models were spatially transferable across islands. In contrast, for SLA we found a weak relationship between field‐measured values and the best estimates from the scientific flora (r2 = 0.11). Insufficient data were available in the TRY database for our study area to calculate trait correlations with other data sources. Conclusions: Scientific floras can act as useful data sources for quantitative plant trait data for some traits but not others, whilst the TRY database contains many traits, but is incomplete in species coverage for our study region, and oceanic islands in general

    Determination of the b quark mass at the M_Z scale with the DELPHI detector at LEP

    An experimental study of the normalized three-jet rate of b quark events with respect to light quarks events (light= \ell \equiv u,d,s) has been performed using the CAMBRIDGE and DURHAM jet algorithms. The data used were collected by the DELPHI experiment at LEP on the Z peak from 1994 to 2000. The results are found to agree with theoretical predictions treating mass corrections at next-to-leading order. Measurements of the b quark mass have also been performed for both the b pole mass: M_b and the b running mass: m_b(M_Z). Data are found to be better described when using the running mass. The measurement yields: m_b(M_Z) = 2.85 +/- 0.18 (stat) +/- 0.13 (exp) +/- 0.19 (had) +/- 0.12 (theo) GeV/c^2 for the CAMBRIDGE algorithm. This result is the most precise measurement of the b mass derived from a high energy process. When compared to other b mass determinations by experiments at lower energy scales, this value agrees with the prediction of Quantum Chromodynamics for the energy evolution of the running mass. The mass measurement is equivalent to a test of the flavour independence of the strong coupling constant with an accuracy of 7 permil.Comment: 24 pages, 10 figures, Accepted by Eur. Phys. J.

    Measurement and Interpretation of Fermion-Pair Production at LEP energies above the Z Resonance

    This paper presents DELPHI measurements and interpretations of cross-sections, forward-backward asymmetries, and angular distributions, for the e+e- -> ffbar process for centre-of-mass energies above the Z resonance, from sqrt(s) ~ 130 - 207 GeV at the LEP collider. The measurements are consistent with the predictions of the Standard Model and are used to study a variety of models including the S-Matrix ansatz for e+e- -> ffbar scattering and several models which include physics beyond the Standard Model: the exchange of Z' bosons, contact interactions between fermions, the exchange of gravitons in large extra dimensions and the exchange of sneutrino in R-parity violating supersymmetry.Comment: 79 pages, 16 figures, Accepted by Eur.
