    Se presenta un caso de celulitis por Aeromonas hydrophila, en un paciente de 23 años, tras una herida antebraquial producida en el medio acuático. La celulitis se resolvió mediante desbridamiento agresivo y quimioterapia. En los controles posteriores no se han apreciado déficits funcionales tras la infección.A case of Aeromonas hidrophila cellulitis developped in a 23-year-old patient, after aquatic environment forearm wound is presented. His cellulitis was effectively resolved with aggressive debridement and chemotherapy. Follow-up examinations have shown no residual functional deficits from the infection

    La bolsa serosa del psoas-ilíaco es una estructura anatómica bien definida, habitualmente presente en el adulto. La comunicación entre dicha bolsa y la articulación de la cadera se asocia frecuentemente a la coxartria. Presentamos un caso de agrandamiento de la bolsa serosa del psoas ilíaco en una paciente de 69 años. El diagnóstico de sospecha se estableció por la presencia de una masa inguinal y dolor de cadera. La ecografía y la T.A.C. confirmaron el diagnóstico. Tras la punción de la bolsa y la instilación de corticoides hemos obtenido la remisión de los síntomas.The iliopsoas bursa is a well-defined anatomic structure rarely absent in adults. Communication existing between the iliopsoas bursa and the hip joint is frequently associated with preexisting hip arthrosis. We present a case of iliopsoas bursa enlargement in a 69-yearold woman. The diagnosis was suspected by the presence of inguinal mass and hip pain. Echography, and later computed tomography confirmed it. The patient was successfully treated with aspiration of cyst content and steroid instillation

    Association of retinol binding protein4 (RBP4) and ghrelin plasma levels with insulin resistance and disease severity in non-diabetic patients with hidradenitis suppurativa

    Hidradenitis suppurativa (HS) is a chronic inflammatory disease associated with insulin resistance (IR). Retinol binding protein 4 (RBP4) and ghrelin are two bioactive proteins that have been involved in glucose metabolism and IR, but also in the regulation of immune and inflammatory processes. The aim of this study was to determine the serum levels of RBP4 and ghrelin in patients with HS, and to assess the possible relationship between these levels and IR, disease severity and HS risk. A total of 137 subjects (77 HS patients and 60 controls) without diabetes mellitus were enrolled in this cross-sectional study. Patients with HS had significantly higher RBP4 but lower ghrelin plasma levels than controls, independently of body mass index (BMI). Serum RBP4 levels were positively correlated to disease severity and IR in HS patients. However, we found no association between ghrelin levels and any clinical or laboratory parameters. Moreover, high serum RBP4 and low ghrelin levels were associated with an increased risk for HS. Our results suggest that high RBP4 levels may be a surrogate biomarker for IR in patients with HS. Moreover, increased RBP4 and decreased ghrelin levels could also be independent risk factors for the development of HS

    The development of the NEMO3 detector, which is now running in the Frejus Underground Laboratory (L.S.M. Laboratoire Souterrain de Modane), was begun more than ten years ago. The NEMO3 detector uses a tracking-calorimeter technique in order to investigate double beta decay processes for several isotopes.     Epidemiology and molecular characterization of multidrug-resistant Escherichia coli isolates harboring blaCTX-M group 1 extended-spectrum β-lactamases causing bacteremia and urinary tract infection in Manhiça, Mozambique

    Background: The emergence and spread of extended-spectrum β-lactamases (ESBLs), especially CTX-M, is an important public health problem with serious implications for low-income countries where second-line treatment is often unavailable. Knowledge of the local prevalence of ESBL is critical to define appropriate empirical therapeutic strategies for multidrug-resistant (MDR) organisms. This study aimed to assess and characterize the presence of ESBL and especially CTX-M-producing Escherichia coli MDR isolates from patients with urinary tract infections (UTIs) and bacteremia in a rural hospital in Mozambique. Materials and methods: One hundred and fifty-one E. coli isolates from bacteremia and UTI in children were screened for CTX-M, TEM, SHV and OXA β-lactamases by polymerase chain reaction and sequencing. Isolates carrying CTX-M group 1 β-lactamases were further studied. The resistance to other antibiotic families was determined by phenotypic and genotypic methods, the location of the blaCTX-M gene and the epidemiology of the isolates were studied, and extensive plasmid characterization was performed. Results: Approximately 11% (17/151) of E. coli isolates causing bacteremia and UTI were ESBL producers. CTX-M-15 was the most frequently detected ESBL, accounting for 75% of the total isolates characterized. The blaCTX-M gene is located in different plasmids belonging to different incompatibility groups and can be found in non-epidemiologically related isolates, indicating the high capacity of this resistance determinant to spread widely. Conclusion: Our data suggest the presence of a co-selection of third-generation cephalosporin-resistant determinants in the study area despite limited access to these antibiotics. This highlights the importance of continuous surveillance of antimicrobial resistance of both genetic elements of resistance and resistant isolates in order to monitor the emergence and trends of ESBL-producing isolates to promote adequate therapeutic strategies for the management of MDR bacterial infections

    The works carried out during the 1989/1990 Campaings have tried to achieve: the thorough documentation of the building using the diaries of the dig: the study of the architecture, paintign, pavements and material culture as well as a diagnosis of how well-preserved the aforementioned remains are. The Home/Caupona (I.8.8-9) belonging to L. Vetutius Placidu, in its initial stages, dates back to the 2nd Century B.C., and three stages have been detected in gradual progression between that date and the year 79 A.D. These reforms can also be seen in the pictorial decoration from the II, III and IV styles and in the opus signinum and «cocciopesto» pavements that have been conserved. The material culture, represented by ceramics (t.s.i, amphoras, dolia, and lamps), metals and objects made out of bone, make up the furnishings of a middle-class home in Pompeii. Finally, the state of conservation of the materials under study has been diagnosed and posible repairs have been suggested.Los trabajos llevados a cabo durante las Campañas de 1989-1990 han pretendido: la documentación exhaustiva del edilicio a través de los diarios de excavación; el estudio de la arquitectura, pintura, pavimento y cultura material y el diagnóstico del estado de conservación de los restos mencionados. La Casa/Caupona (I. 8.8-9) de L. Vetutius Placidus, en su etapa inicial, se remonta al s/II a.C., detectándose tres fases que se escalonan desde esa fecha hasta el año 79 d.C. Estas reformas se aprecian igualmente en las decoracciones pictóricas del II, III y IV estilos y en los pavimentos de opus signinum y «cocciopesto> conservados. La cultura material representada por cerámicas (t.s.i., ánforas, dolia y lucernas), metales y objetos de hueso resume el ajuar de una casa de tipo medio en Pompeya.     Characterisation of extended-spectrum b-lactamases among Klebsiella pneumoniae isolates causing bacteraemia and urinary tract infection in Mozambique

    The aim of this study was to determine the prevalence of extended-spectrum β-lactamase (ESBL)-producing Klebsiella pneumoniae isolated from urinary tract and bloodstream infections in a rural hospital in Manhiça, Mozambique. ESBLs were investigated among ceftriaxone-non-susceptible K. pneumoniae clinical isolates recovered between 2004 and 2009. Characterisation of blaCTX-M, blaSHV, blaOXA and blaTEM genes was performed by PCR and sequencing. Epidemiological relationships were established by phylogenetic analysis, repetitive extragenic palindromic PCR (REP-PCR), pulsed-field gel electrophoresis (PFGE) and multilocus sequence typing (MLST), whilst plasmid transferability was evaluated by conjugation. In addition, the presence of class 1 and 2 integrons was studied. A total of 19 K. pneumoniae were analysed. The blaCTX-M-15 gene was found in all strains. Other ESBL genes were found concomitantly, including blaSHV-5, blaSHV-2, blaSHV-2A, blaSHV-12 and blaSHV-38. In addition, other β-lactamases such as blaTEM-1 and blaOXA-30 were also detected. REP-PCR identified 15 different epidemiological profiles. MLST analysis also showed great variability of sequence types. The blaCTX-M-15 gene showed a high transfer capacity. The presence of class 1 integrons was high. High levels of multidrug resistance were also found. In conclusion, these data show the dominance of the CTX-M-type ESBL, particularly CTX-M-15, supporting its worldwide dissemination, including in areas with limited access to third-generation cephalosporins. This finding is a matter of concern for clinical management as third-generation cephalosporins are an alternative for treating severe cases of multidrug-resistant infections in this community
