    ¿La cirugia bariatrica mejora sensiblemente la calidad de vida del paciente?

    Objective: The aim of this investigation was to assess the effect of malabsorptive bariatric surgery (BS) on the quality of life (QoL), applying the Nottingham Health Profile (NHP) and the bariatric analysis and reporting outcome system (BAROS). Design: A prospective cohort study was performed in 100 adult patients (> 18 years) undergoing bariatric surgery by malabsorptive technique for one year. Research methods and procedures: Patients were monitored from the beginning of the BS program until a year after the intervention, applying the NHP and the BAROS test. At baseline, the mean weight of the women was 132 ± 22 kg and the Body Mass Index (BMI) was 50.7 kg/m2. Results: The values obtained from different areas applying the NHP questionnaire showed statistical significant differences (p < 0.001) with respect to baseline values. According to the BAROS test, 48% of patients lost 25-49% of weight excess and 80.8% had resolved major comorbidities at 1 yr. According to the Moorehead-Ardelt QoL score, there were major improvements in employment and self-esteem in 89% and 87% of patients, respectively, and improvements in physical activity, sexual and social relationships. According to the total mean BAROS score, the outcome was considered “very good”. Conclusion: NHP and BAROS questionnaires appear to be useful and easily applicable tools to assess the QoL of obese patients.Introducción: La obesidad mórbida suele acompañarse de enfermedades graves asociadas que provocan una menor expectativa y peor calidad de vida (CV). Objetivos: evaluar el efecto de la cirugía bariátrica (CB) por técnicas malabsortivas sobre la CV, utilizado (Perfil de Salud del Nottingham (PSN) y el Bariatric analysis and reporting outcome system (BAROS). Material y métodos: Estudio prospectivo, descriptivo, desde Octubre del 2002 hasta Mayo de 2006, seguimiento a los pacientes desde el inicio al protocolo de CB hasta el año post-intervención., donde se incluyeron 100 pacientes. El 86% mujeres, el peso inicial medio 132±22 kg y IMC de 50,7 kg/m2. Se aplicaron 2 cuestionarios: PSN y el BAROS. Resultado: Los valores obtenidos de las diferentes áreas aplicando el cuestionario PSN al año de la intervención muestran diferencias estadísticamente significativas (p < 0,001) con los valores iniciales. Según BAROS: Porcentaje de sobrepeso perdido (%SP). El 48% de los pacientes perdió entre el 25 y el 49% de su exceso de peso. Condiciones médicas. El 80,8% habían resuelto todas las comorbilidades mayores. CV de Moorehead Ardelt. El aspecto más mejorado fue el empleo el 89% , autoestima solo el 13% no presentó cambios, las áreas de la actividad física, relaciones sexuales y sociales fueron consideradas como mucho mejor. Basándonos en esta clasificación nuestros resultados se pueden calificar de “muy buenos”. Conclusión: Los cuestionarios PSN y BAROS parecen ser herramientas útiles y de fácil aplicación para evaluar la calidad de vida de los pacientes obesos

    CEERS Key Paper V: A triality on the nature of HST-dark galaxies

    The new capabilities that JWST offers in the near- and mid-infrared (IR) are used to investigate in unprecedented detail the nature of optical/near-IR faint, mid-IR bright sources, HST-dark galaxies among them. We gather JWST data from the CEERS survey in the EGS, jointly with HST data, and analyze spatially resolved optical-to-mid-IR spectral energy distributions (SEDs) to estimate both photometric redshifts in 2 dimensions and stellar populations properties in a pixel-by-pixel basis. We select 138 galaxies with F150W-F356W>1.5 mag, F356W<27.5 mag. The nature of these sources is threefold: (1) 71% are dusty star-forming galaxies at 2<z<6 with masses 9<log M/M_sun<11 and a variety of specific SFRs (100 Gyr^-1); (2) 18% are quiescent/dormant (i.e., subject to reignition and rejuvenation) galaxies at 3<z<5, masses log M/M_sun~10 and post-starburst stellar mass-weighted ages (0.5-1 Gyr); and (3) 11% are strong young starbursts with indications of high-EW emission lines (typically, [OIII]+Hbeta) at 6<z<7 and log M/M_sun~9.5. The sample is dominated by disk-like galaxies with a remarkable compactness for XELG-z6 (effective radii smaller than 0.4 kpc). Large attenuations in SFGs, 2<A(V)<5 mag, are found within 1.5 times the effective radius, approximately 2 kpc, while QGs present A(V)~0.2 mag. Our SED-fitting technique reproduces the expected dust emission luminosities of IR-bright and sub-millimeter galaxies. This study implies high levels of star formation activity between z~20 and z~10, where virtually 100% of our galaxies had already formed 10^8 M_sun of their stellar content, 60% of them had assembled 10^9 M_sun, and 10% up to 10^10 M_sun (in situ or ex situ).     La secretaría de Extensión de la Universidad Nacional de La Plata en el marco de la convocatoria de proyectos 2019, acreditó el proyecto multidisciplinario Conoces tu color 2, en el cual participan diferentes Unidades Académicas: Facultad de Odontología, Bellas Artes, Ciencias Exactas y Ciencias Naturales, dependientes de la Universidad Nacional de La Plata. Docentes y alumnos de dichas facultades intervinimos con distintas acciones en el proyecto, el cuál tuvimos que adaptar, teniendo en cuenta el contexto de pandemia, contemplando que el cambio de rutinas y comportamientos diarios afecta la salud de la población, consideramos como herramienta eficaz promover la educación para la salud, priorizando incorporar hábitos saludables en el cuidado bucodental; en momentos de pandemia cuidar la salud oral es una prioridad ya que una de las principales vías de entrada del SARS –COV2 es la vía oral. Las familias pertenecientes a los sectores más vulnerables de la sociedad presentan gran cantidad de lesiones bucodentales producidas por una dieta alta en hidratos de carbono, ausencia de higiene oral, inicio tardío del cepillado dental o escasa frecuencia del mismo.Facultad de Odontologí

    Natural History of MYH7-Related Dilated Cardiomyopathy

    BACKGROUND Variants in myosin heavy chain 7 (MYH7) are responsible for disease in 1% to 5% of patients with dilated cardiomyopathy (DCM); however, the clinical characteristics and natural history of MYH7-related DCM are poorly described. OBJECTIVES We sought to determine the phenotype and prognosis of MYH7-related DCM. We also evaluated the influence of variant location on phenotypic expression. METHODS We studied clinical data from 147 individuals with DCM-causing MYH7 variants (47.6% female; 35.6 +/- 19.2 years) recruited from 29 international centers. RESULTS At initial evaluation, 106 (72.1%) patients had DCM (left ventricular ejection fraction: 34.5% +/- 11.7%). Median follow-up was 4.5 years (IQR: 1.7-8.0 years), and 23.7% of carriers who were initially phenotype-negative developed DCM. Phenotypic expression by 40 and 60 years was 46% and 88%, respectively, with 18 patients (16%) first diagnosed at <18 years of age. Thirty-six percent of patients with DCM met imaging criteria for LV noncompaction. During follow-up, 28% showed left ventricular reverse remodeling. Incidence of adverse cardiac events among patients with DCM at 5 years was 11.6%, with 5 (4.6%) deaths caused by end-stage heart failure (ESHF) and 5 patients (4.6%) requiring heart transplantation. The major ventricular arrhythmia rate was low (1.0% and 2.1% at 5 years in patients with DCM and in those with LVEF of <= 35%, respectively). ESHF and major ventricular arrhythmia were significantly lower compared with LMNA-related DCM and similar to DCM caused by TTN truncating variants. CONCLUSIONS MYH7-related DCM is characterized by early age of onset, high phenotypic expression, low left ventricular reverse remodeling, and frequent progression to ESHF.     Genomic investigations of unexplained acute hepatitis in children

    Since its first identification in Scotland, over 1,000 cases of unexplained paediatric hepatitis in children have been reported worldwide, including 278 cases in the UK1. Here we report an investigation of 38 cases, 66 age-matched immunocompetent controls and 21 immunocompromised comparator participants, using a combination of genomic, transcriptomic, proteomic and immunohistochemical methods. We detected high levels of adeno-associated virus 2 (AAV2) DNA in the liver, blood, plasma or stool from 27 of 28 cases. We found low levels of adenovirus (HAdV) and human herpesvirus 6B (HHV-6B) in 23 of 31 and 16 of 23, respectively, of the cases tested. By contrast, AAV2 was infrequently detected and at low titre in the blood or the liver from control children with HAdV, even when profoundly immunosuppressed. AAV2, HAdV and HHV-6 phylogeny excluded the emergence of novel strains in cases. Histological analyses of explanted livers showed enrichment for T cells and B lineage cells. Proteomic comparison of liver tissue from cases and healthy controls identified increased expression of HLA class 2, immunoglobulin variable regions and complement proteins. HAdV and AAV2 proteins were not detected in the livers. Instead, we identified AAV2 DNA complexes reflecting both HAdV-mediated and HHV-6B-mediated replication. We hypothesize that high levels of abnormal AAV2 replication products aided by HAdV and, in severe cases, HHV-6B may have triggered immune-mediated hepatic disease in genetically and immunologically predisposed children
