    A Study to Assess Career Satisfaction among Dental Practitioners in Kanpur City, U.P., India

    BACKGROUND: For almost every health worker, job satisfaction is an important determinant for motivation, retention and performance, and all of these are primarily important to improve the functioning mechanism of health systems in countries of low- and middle income categories.  AIM: This study was conducted to estimate the level of job satisfaction among registered practicing clinical dentists in Kanpur city, U.P., India MATERIALS & METHOD:  A cross sectional study was conducted among 204 practicing registered dentists. A questionnaire was distributed manually to the participating dentists for measuring dimensions of job satisfaction. A 5 point Likert format with a score range from 1(described as strongly dissatisfied) to 4(described as strongly satisfied) was used to describe the items.  Analysis of data was analyzed using SPSS software 21.0 and student t test was applied for measuring the difference of means between the subgroups for each dimension. RESULTS: A significant difference (p<0.005) in levels of satisfaction for various dimensions of job satisfaction within gender, educational qualification and work status was found on analysis. The postgraduates were found to be more satisfied as compared to graduates with a difference in job satisfaction level between genders which was related to the personal time dimension. It was also derived from the study that dentists working full time expressed dissatisfaction in terms of family time, thus to issues in their personal relationships. CONCLUSION:  Overall, it was found that the dentists have a high level of job satisfaction and the socio demographic factors deeply influence this domain of career

    Who is in Your Top Three? Optimizing Learning in Elections with Many Candidates

    Elections and opinion polls often have many candidates, with the aim to either rank the candidates or identify a small set of winners according to voters' preferences. In practice, voters do not provide a full ranking; instead, each voter provides their favorite K candidates, potentially in ranked order. The election organizer must choose K and an aggregation rule. We provide a theoretical framework to make these choices. Each K-Approval or K-partial ranking mechanism (with a corresponding positional scoring rule) induces a learning rate for the speed at which the election correctly recovers the asymptotic outcome. Given the voter choice distribution, the election planner can thus identify the rate optimal mechanism. Earlier work in this area provides coarse order-of-magnitude guaranties which are not sufficient to make such choices. Our framework further resolves questions of when randomizing between multiple mechanisms may improve learning, for arbitrary voter noise models. Finally, we use data from 5 large participatory budgeting elections that we organized across several US cities, along with other ranking data, to demonstrate the utility of our methods. In particular, we find that historically such elections have set K too low and that picking the right mechanism can be the difference between identifying the ultimate winner with only a 80% probability or a 99.9% probability after 400 voters.Comment: To appear in HCOMP 201

    Orthokeratology in Moderate Myopia: A Study of Predictability and Safety

    Purpose: Literature is relatively silent on safety profile and predictability of orthokeratology lenses in terms of myopia correction and prevention of further progression, especially in semi-tropical countries; this study was designed to fill this gap. Methods: This prospective, intervention case series enrolled 30 eyes of 30 patients with myopia up to –5.5 diopters (D). Patients were randomized into two groups of 15 each; the study group was prescribed overnight orthokeratology (OK) lenses, while the control group used daily wear conventional soft contact lenses. Follow-up examinations were performed after 1 h and 6 h, and then at 1, 7, 15, 30 days, and 4 months post lens wear. Uncorrected visual acuity (UCVA), contrast sensitivity, keratometry, central corneal thickness (CCT), and tear film break up time (TBUT) were evaluated at each follow-up examination. Results: All patients attained a visual acuity of 0.00 Logarithm of the Minimum Angle of Resolution (logMAR) after one week of lens use, which was maintained throughout the study period. While patients allotted to the study group had a gain of 8.1 Snellen lines (UCVA), those in the control group gained 8.9 lines (BCVA) at the end of follow-up period. In the OK group, cornea showed a flattening of 0.8 D (mean keratometry) after single overnight usage of OK lens and overall flattening of 1.2 D compared to baseline, at the end of four months. The change in contrast sensitivity, corneal endothelial specular count, axial length and tear film status was not significant in either group. Conclusion: Orthokeratology is an effective and safe modality to correct moderate myopia in motivated young adults. No side effects were encountered after a short-term follow-up in participants who resided in semi-tropical environments

    Effect of water and mercury quenching on microstructure and mechanical behavior of room temperature rolled Zircaloy-2

    The present study investigates the effect of water and mercury quenching on the microstructural and mechanical behavior of room temperature rolled Zircaloy-2. Solution treatment of zircaloy-2 at 1073 K followed by quenching in mercury and water has been performed prior to rolling. Different reduction from 25% to 85% of the quenched alloy and further characterization has been performed by tensile testing, Electron back scattered diffraction (EBSD) and Transmission Electron Microscopy (TEM). Enhanced tensile strength (745 MPa) after 85% rolling reduction was obtained compared to 389 MPa after water quenching. Rolling reduction results an increase in the dislocation density, thereby enhancing the mechanical strength. Initial deformation has been observed by the activation of extension twinning from EBSD microstructure. Twinning results the inclination of ‘c’ axis towards the normal direction which makes near basal grains orientation along the deformation direction. Due to hard orientation i.e. ‘c’ axis aligned along the loading direction, the grain fragmentation is heterogeneous. Grain fragmentation leads to improvement in the ductility with minimal loss in strength owing to rearrangement of dislocations after annealing at 400o C for 30 minutes.     Unconventional Breathing Currents Far beyond the Quantum Tunneling Distances in Large-Gapped Nanoplasmonic Systems

    Localized surface plasmon resonance (LSPR) in plasmonic nanoparticles propels the field of plasmo-electronics, holding promise for transformative optoelectronic devices through efficient light-to-current conversion. Plasmonic excitations strongly influence the charge distribution within nanoparticles, giving rise to electromagnetic fields that can significantly impact the macroscopic charge flows within the nanoparticle housing material. In this study, we present evidence of ultralow, unconventional breathing currents resulting from dynamic irradiance interactions between widely separated nanoparticles, extending far beyond conventional electron (quantum) tunneling distances. We develop an electric analogue model and derive an empirical expression to elucidate the generation of these unconventional breathing currents in cascaded nanoplasmonic systems under irradiance modulation. This technique and theoretical model have significant potential for applications requiring a deeper understanding of current dynamics, particularly on large nanostructured surfaces relevant to photocatalysis, energy harvesting, sensing, imaging, and the development of future photonic devices

    Meta-Analysis of the Relation of Baseline Right Ventricular Function to Response to Cardiac Resynchronization Therapy

    Right ventricular (RV) dysfunction has been associated with adverse clinical outcomes in patients with heart failure (HF). Cardiac resynchronization therapy (CRT) improves left ventricular (LV) size and function in patients with markedly abnormal electrocardiogram QRS duration. However, relation of baseline RV function with response to CRT has not been well described. In this study, we aim to investigate the relation of baseline RV function with response to CRT as assessed by change in LV ejection fraction (EF). A systematic search of studies published from 1966 to May 31, 2015 was conducted using PubMed, CINAHL, Cochrane CENTRAL, and the Web of Science databases. Studies were included if they have reported (1) parameters of baseline RV function (tricuspid annular plane systolic excursion [TAPSE] or RVEF or RV basal strain or RV fractional area change [FAC]) and (2) LVEF before and after CRT. Random-effects metaregression was used to evaluate the effect of baseline RV function parameters and change in LVEF. Sixteen studies (n = 1,764) were selected for final analysis. Random-effects metaregression analysis showed no significant association between the magnitude of the difference in EF before and after CRT with baseline TAPSE (β = 0.005, p = 0.989); baseline RVEF (β = 0.270, p = 0.493); baseline RVFAC (β = -0.367, p = 0.06); baseline basal strain (β = -0.342, p = 0.462) after a mean follow-up period of 10.5 months. In conclusion, baseline RV function as assessed by TAPSE, FAC, basal strain, or RVEF does not determine response to CRT as assessed by change in LVEF

    Tribological behavior and biocompatibility of novel Nickel-Free stainless steel manufactured via laser powder bed fusion for biomedical applications

    Due to the risk of releasing carcinogenic nickel ions from conventional 316L stainless steel under a corrosive human body environment, a new variant of steel termed nickel-free stainless steel (NiFSS) has been investigated. The present study investigates the tribological properties and biocompatibility of NiFSS manufactured via laser powder bed fusion (PBF-LB/M). The ferritic NiFSS exhibited significantly lower coefficient of friction (0.08 to 0.28) and wear rate (1.60 × 10-6 mm3/Nm to 6.60 × 10-6 mm3/Nm) compared to reported values for austenitic 316L SS, under both dry and simulated body fluid (SBF) conditions and various sliding geometries. This improvement is attributed to the superior hardness (3.394 ± 0.1340 GPa) and elastic modulus (238 ± 9.0797 GPa) of NiFSS. To assess the biocompatibility, the viability of mouse pre-osteoblastic MC3T3-E1 cells was evaluated with an Alamar Blue assay when the cells were cultured on top of PBF-LB/M built NiFSS and 316L SS samples. The results indicated that even though cell growth was most optimal on regular cell culture plastic, cell viability was better maintained on PBF-LB/M built NiFSS compared to 316L SS. Therefore, the results of the present study propose that PBF-LB/M fabricated NiFSS holds promise for application in biomedical devices for joint arthroplasty.Peer reviewe
