    Stress-induced changes in immune response of dairy ewes

    two groups of 16 Comisana ewes were selected from a group of 30, and divided, according to their cortisol secretion after isolation in a novel environment, into HC ewes, having a cortisol secretion >90 ng/mL, and LC ewes having a cortisol secretion <80 ng/mL. Blood samples were collected immediately before and immediately after isolation, and 60, 120, 300min and then 24h and 48h after isolation, to evaluate the percentage of lymphocyte T-helper (CD4+) and lymphocyte T-cytotoxic (CD8+) by flow cytometry analysis, and IL-1β and IL-6 concentrations by ELISA. The day before the isolation test, the ewes were milked in the afternoon at 1500h (-19h from isolation), and then milked for the three days starting from the day of the isolation test (at 0, 5, 24, 29, 48 and 53h from isolation) to evaluate milk cortisol secretion, and IL-1β and IL-6 concentrations. Results suggest that the levels of cortisol secretions can influence the immune competence of dairy ewes and cytokines concentrations. Milk cytokine concentrations, and not milk cortisol concentrations, can be considered indicators of the magnitude of the hypothalamic-pituitary-adrenal axis activation

    Influence of ventilation regimen on micro-environment and on ewe welfare and milk yield in summer

    The effects of ventilation regimen on air quality, and on the welfare and production performance of thirty-six Comisanaewes were assessed in a 6-week trial conducted during the summer of 2002. Animals were divided into three groups of12, and subjected to the following treatments: low ventilation regimen providing a mean ventilation rate (VR) of 35 m3/hper ewe, split in 30 min ventilation cycles at an air speed of 2 m/s (LOV-30); moderate ventilation regimen (VR = 70m3/h per ewe) split in 30 min ventilation cycles at an air speed of 4 m/s (MOV-30); moderate ventilation regimen (VR =70 m3/h per ewe) split in 60 min ventilation cycles at an air speed of 2 m/s (MOV-60). Air concentrations of microorganisms,dust, and gaseous pollutants were measured twice weekly. Respiration rate (RR) and rectal temperature (RT)were monitored throughout the trial at 0830 and at 1400. Behavioral traits of ewes were recorded twice per week from0900 to 1200 and from 1500 to 1800. Cell-mediated immune response to phytohemagglutinin (PHA) and humoralimmune response to chicken egg albumin were determined. At d 37 ewes were injected with porcine ACTH, and subjectedto blood sampling for evaluation of cortisol concentrations immediately before and 1, 2 and 4 h after ACTH injection.Milk yield was recorded daily. Individual milk samples were analyzed for composition, renneting parameters, somaticcell count (SCC), and bacteriological characteristics. Averages of maximum THI were about 3 points higher in the LOV-30 and the MOV-30 than in the MOV-60 room, whereas no differences emerged in the air concentrations of dust, gaseouspollutants and microorganisms. Significant interactions of treatment x time (P < 0.05) were found for respiration rate,and for the time the ewes spent lying, idling and eating in the afternoon during weeks 2 and 3 of the study period.Significant effects of ventilation regimen x time (P < 0.05) were also observed for milk yield and milk renneting parameters,the LOV-30 ewes giving smaller volumes of milk with a deteriorated coagulating behavior than those of the MOV-60 group during the second half of the trial. No significant differences emerged in ewe immune and endocrine responses.Results show that ventilation regimen had a moderate impact on ewe behavior, physiology and production performance.This experiment suggests that the length of ventilation cycles and air speed, together with ventilation rate, arecritical for efficient ventilation regimens

    Overground walking training with the i-Walker, a robotic servo-assistive device, enhances balance in patients with subacute stroke: a randomized controlled trial

    Background: Patients affected by mild stroke benefit more from physiological overground walking training than walking-like training performed in place using specific devices. The aim of the study was to evaluate the effects of overground robotic walking training performed with the servo-assistive robotic rollator (i-Walker) on walking, balance, gait stability and falls in a community setting in patients with mild subacute stroke. Methods: Forty-four patients were randomly assigned to two different groups that received the same therapy in two daily 40-min sessions 5 days a week for 4 weeks. Twenty sessions of standard therapy were performed by both groups. In the other 20 sessions the subjects enrolled in the i-Walker-Group (iWG) performed with the i-Walker and the Control-Group patients (CG) performed the same amount of conventional walking oriented therapy. Clinical and instrumented gait assessments were made pre- and post-treatment. The follow-up observation consisted of recording the number of fallers in the community setting after 6 months. Results: Treatment effectiveness was higher in the iWG group in terms of balance improvement (Tinetti: 68.4 ± 27.6 % vs. 48.1 ± 33.9 %, p= 0.033) and 10-m and 6-min timed walking tests (significant interaction between group and time: F(1,40) = 14.252, p = 0.001; and F (1,40) = 7.883, p = 0.008, respectively). When measured, latero-lateral upper body accelerations were reduced in iWG (F= 4.727, p= 0.036), suggesting increased gait stability, which was supported by a reduced number of falls at home. Conclusions: A robotic servo-assisted i-Walker improved walking performance and balance in patients affected by mild/moderate stroke, leading to increased gait stability and reduced falls in the community.Peer ReviewedPostprint (published version

    Negli animali d’interesse zootecnico la difficoltà di adattamento alle condizioni ambientali può portare allo sviluppo di patologie che si ripercuotono anche sulla qualità delle produzioni. La presenza in allevamento di strutture ed impianti che permettono una buona ed agevole gestione degli animali è un aspetto fondamentale nella determinazione del benessere degli ovini da latte. La stabulazione deve garantire ad ogni animale il continuo accesso all’alimento e all’acqua, un’area di riposo confortevole e asciutta, e spazio a sufficienza per muoversi e comportarsi secondo la normale gamma di comportamenti caratteristici della specie. In tema di benessere animale la mungitura rappresenta l’operazione critica nell’azienda zootecnica da latte. Il vuoto, la pulsazione ed il gruppo prendicapezzoli sono i principali elementi che influiscono sul benessere animale. Soltanto attraverso l’equilibrato rapporto di questi tre fattori si possono garantire le migliori prestazioni della mungitrice. Lo scopo del lavoro è quello di valutare con attenzione le strutture e gli elementi climatici ad esse connessi, nonché i principali aspetti tecnologici ed operativi dell’impianto di mungitura che possono influenzare la qualità del latte e il benessere animale. In quest’ottica, si è realizzato un prototipo di impianto che consente la mungitura ad un basso livello di vuoto operativo

    A pilot study on brain plasticity of functional connectivity modulated by cognitive training in mild Alzheimer's disease and mild cognitive impairment

    Alzheimer's disease (AD) alters the functional connectivity of the default mode network (DMN) but also the topological properties of the functional connectome. Cognitive training (CT) is a tool to slow down AD progression and is likely to impact on functional connectivity. In this pilot study, we aimed at investigating brain functional changes after a period of CT and active control (AC) in a group of 26 subjects with mild AD (mAD), 26 with amnestic mild cognitive impairment (aMCI), and a control group of 29 healthy elderly (HE) people. They all underwent a CT and AC in a counterbalanced order following a crossover design. Resting-state functional MRI and neuropsychological testing were acquired before and after each period. We tested post-CT and post-AC changes of cognitive abilities, of the functional connectivity of the DMN, and of topological network properties derived from graph theory and network-based statistics. Only CT produced functional changes, increasing the functional connectivity of the posterior DMN in all three groups. mAD also showed functional changes in the medial temporal lobe and topological changes in the anterior cingulum, whereas aMCI showed more widespread topological changes involving the frontal lobes, the cerebellum and the thalamus. Our results suggest specific functional connectivity changes after CT for aMCI and mAD

    Effect of Manure vs. Fertilizer Inputs on Productivity of Forage Crop Models

    Manure produced by livestock activity is a dangerous product capable of causing serious environmental pollution. Agronomic management practices on the use of manure may transform the target from a waste to a resource product. Experiments performed on comparison of manure with standard chemical fertilizers (CF) were studied under a double cropping per year regime (alfalfa, model I; Italian ryegrass-corn, model II; barley-seed sorghum, model III; and horse-bean-silage sorghum, model IV). The total amount of manure applied in the annual forage crops of the model II, III and IV was 158, 140 and 80 m3 ha−1, respectively. The manure applied to soil by broadcast and injection procedure provides an amount of nitrogen equal to that supplied by CF. The effect of manure applications on animal feeding production and biochemical soil characteristics was related to the models. The weather condition and manures and CF showed small interaction among treatments. The number of MFU ha−1 of biomass crop gross product produced in autumn and spring sowing models under manure applications was 11,769, 20,525, 11,342, 21,397 in models I through IV, respectively. The reduction of MFU ha−1 under CF ranges from 10.7% to 13.2% those of the manure models. The effect of manure on organic carbon and total nitrogen of topsoil, compared to model I, stressed the parameters as CF whose amount was higher in models II and III than model IV. In term of percentage the organic carbon and total nitrogen of model I and treatment with manure was reduced by about 18.5 and 21.9% in model II and model III and 8.8 and 6.3% in model IV, respectively. Manure management may substitute CF without reducing gross production and sustainability of cropping systems, thus allowing the opportunity to recycle the waste product for animal forage feeding

    How future surgery will benefit from SARS-COV-2-related measures: a SPIGC survey conveying the perspective of Italian surgeons

    COVID-19 negatively affected surgical activity, but the potential benefits resulting from adopted measures remain unclear. The aim of this study was to evaluate the change in surgical activity and potential benefit from COVID-19 measures in perspective of Italian surgeons on behalf of SPIGC. A nationwide online survey on surgical practice before, during, and after COVID-19 pandemic was conducted in March-April 2022 (NCT:05323851). Effects of COVID-19 hospital-related measures on surgical patients' management and personal professional development across surgical specialties were explored. Data on demographics, pre-operative/peri-operative/post-operative management, and professional development were collected. Outcomes were matched with the corresponding volume. Four hundred and seventy-three respondents were included in final analysis across 14 surgical specialties. Since SARS-CoV-2 pandemic, application of telematic consultations (4.1% vs. 21.6%; p &lt; 0.0001) and diagnostic evaluations (16.4% vs. 42.2%; p &lt; 0.0001) increased. Elective surgical activities significantly reduced and surgeons opted more frequently for conservative management with a possible indication for elective (26.3% vs. 35.7%; p &lt; 0.0001) or urgent (20.4% vs. 38.5%; p &lt; 0.0001) surgery. All new COVID-related measures are perceived to be maintained in the future. Surgeons' personal education online increased from 12.6% (pre-COVID) to 86.6% (post-COVID; p &lt; 0.0001). Online educational activities are considered a beneficial effect from COVID pandemic (56.4%). COVID-19 had a great impact on surgical specialties, with significant reduction of operation volume. However, some forced changes turned out to be benefits. Isolation measures pushed the use of telemedicine and telemetric devices for outpatient practice and favored communication for educational purposes and surgeon-patient/family communication. From the Italian surgeons' perspective, COVID-related measures will continue to influence future surgical clinical practice

    Management of acute diverticulitis with pericolic free gas (ADIFAS). an international multicenter observational study

    Background: There are no specific recommendations regarding the optimal management of this group of patients. The World Society of Emergency Surgery suggested a nonoperative strategy with antibiotic therapy, but this was a weak recommendation. This study aims to identify the optimal management of patients with acute diverticulitis (AD) presenting with pericolic free air with or without pericolic fluid. Methods: A multicenter, prospective, international study of patients diagnosed with AD and pericolic-free air with or without pericolic free fluid at a computed tomography (CT) scan between May 2020 and June 2021 was included. Patients were excluded if they had intra-abdominal distant free air, an abscess, generalized peritonitis, or less than a 1-year follow-up. The primary outcome was the rate of failure of nonoperative management within the index admission. Secondary outcomes included the rate of failure of nonoperative management within the first year and risk factors for failure. Results: A total of 810 patients were recruited across 69 European and South American centers; 744 patients (92%) were treated nonoperatively, and 66 (8%) underwent immediate surgery. Baseline characteristics were similar between groups. Hinchey II-IV on diagnostic imaging was the only independent risk factor for surgical intervention during index admission (odds ratios: 12.5, 95% CI: 2.4-64, P =0.003). Among patients treated nonoperatively, at index admission, 697 (94%) patients were discharged without any complications, 35 (4.7%) required emergency surgery, and 12 (1.6%) percutaneous drainage. Free pericolic fluid on CT scan was associated with a higher risk of failure of nonoperative management (odds ratios: 4.9, 95% CI: 1.2-19.9, P =0.023), with 88% of success compared to 96% without free fluid ( P &lt;0.001). The rate of treatment failure with nonoperative management during the first year of follow-up was 16.5%. Conclusion: Patients with AD presenting with pericolic free gas can be successfully managed nonoperatively in the vast majority of cases. Patients with both free pericolic gas and free pericolic fluid on a CT scan are at a higher risk of failing nonoperative management and require closer observation

    Reducing the environmental impact of surgery on a global scale: systematic review and co-prioritization with healthcare workers in 132 countries

    Abstract Background Healthcare cannot achieve net-zero carbon without addressing operating theatres. The aim of this study was to prioritize feasible interventions to reduce the environmental impact of operating theatres. Methods This study adopted a four-phase Delphi consensus co-prioritization methodology. In phase 1, a systematic review of published interventions and global consultation of perioperative healthcare professionals were used to longlist interventions. In phase 2, iterative thematic analysis consolidated comparable interventions into a shortlist. In phase 3, the shortlist was co-prioritized based on patient and clinician views on acceptability, feasibility, and safety. In phase 4, ranked lists of interventions were presented by their relevance to high-income countries and low–middle-income countries. Results In phase 1, 43 interventions were identified, which had low uptake in practice according to 3042 professionals globally. In phase 2, a shortlist of 15 intervention domains was generated. In phase 3, interventions were deemed acceptable for more than 90 per cent of patients except for reducing general anaesthesia (84 per cent) and re-sterilization of ‘single-use’ consumables (86 per cent). In phase 4, the top three shortlisted interventions for high-income countries were: introducing recycling; reducing use of anaesthetic gases; and appropriate clinical waste processing. In phase 4, the top three shortlisted interventions for low–middle-income countries were: introducing reusable surgical devices; reducing use of consumables; and reducing the use of general anaesthesia. Conclusion This is a step toward environmentally sustainable operating environments with actionable interventions applicable to both high– and low–middle–income countries
