    Percepción de los escolares sobre los accidentes y las lesiones accidentales

    Antecedentes/Objetivos: Las lesiones no intencionales constituyen la primera causa de muerte en la población infantil española (1-14 años) con una tasa de mortalidad de 6,5 por 100.000 niñas/os. Según datos de la Encuesta Nacional de Salud de 2006, la incidencia acumulada de lesiones accidentales en el último año previo a la encuesta fue de 10,6% de las/os niños entrevistadas/os, más frecuente en niños (51,4%) que en niñas (48,6%). Los accidentes se produjeron con mayor frecuencia en casa (29%), en la calle (23%) y en la escuela (22%). Pocos estudios han investigado la perspectiva de las/os niñas/os sobre las lesiones accidentales que sufren. El objetivo del estudio es explorar, describir y categorizar las percepciones de las/os niñas/os sobre las lesiones accidentales y las explicaciones de los accidentes que hayan podido experimentar. Métodos: Estudio exploratorio mediante análisis cualitativo sobre la percepción de salud de la población infantil. Los datos se obtuvieron de 6 grupos de discusión de entre 8 a 10 niños/as formados en 2006 mediante muestreo teórico intencional de niños/as de edades entre 8 y 12 años de 3º y 6º curso de primaria de colegios públicos y concertados de Alicante con los criterios de inclusión: Centro público/concertado; Zona centro o residencial/zona de clase trabajadora; Niños/niñas/grupo mixto; Tercero y Sexto de primaria. Los discursos fueron grabados, transcritos y posteriormente analizados usando el programa ATLASTI.v.4.1 y aplicando los principios de la grounded theory. Dos investigadores identificaron una estructura temática y un proceso de codificación abierta revisada por un tercero para validar el proceso y determinar las categorías y subcategorías resultantes. Resultados: Los accidentes fue un tema emergente del análisis del discurso. Se identificaron las categorías y subcategorías: Conceptualización de accidente (a. Oposición a salud, “Un niño sano no se rompe nada” b. Asociado a enfermedad “Cuando te rompes la mano o la muñeca” c. Suceso inevitable “Es algo que no se puede prevenir”) Causa y tipo de lesiones (“Por mucho que me caiga sólo me he hecho moraos”) Lugares donde se producen accidentes (a. Escuela “El patio es plano y cuando juegas al futbol algún niño se cae” b. Otros “Me hice una fisura en la calle”) Sugerencias para evitar accidentes (a. Espacios escolares “Arreglar el suelo del patio” b. Conductas personales “Tener más cuidado”). Conclusiones: La población infantil sabe identificar los accidentes como problema de salud. Los percibe como inevitables aunque propone formas de evitarlos. Ayudar a los escolares a identificar las causas de los accidentes puede eliminar su percepción de inevitabilidad y fortalecer su capacidad para afrontarlos.Escuela Valenciana de Estudios en Salud.     Population, activity and unemployment in the second half of the 20th century in Extremadura.

    A lo largo de la segunda mitad del siglo XX se han producido las más profundas transformaciones de la historia regional, impulsadas tanto por factores exógenos como endógenos de toda índole. Se ha pasado de una economía agraria de subsistencia a una economía competitiva de los servicios y del conocimiento, lo que ha supuesto enormes excedentes de mano de obra agraria hasta el final de siglo. Excedentes que fueron engrosando la intensa sangría emigratoria de los sesenta y setenta, que arrastró al 40% de la población regional. Sin embargo, desde comienzos de los ochenta, la población ha tendido a estabilizarse. Pero la emigración ya había provocado un estrangulamiento irreparable en las edades activas y de procreación, un fuerte descenso de la natalidad acentuado por la caída de la fecundidad posteriormente y un profundo envejecimiento. Por otra parte, la retención de los efectivos jóvenes y adultos en sus lugares de origen, junto a otros factores, incrementaron el paro hasta niveles desconocidos, si bien es un fenómeno que se ha conseguido paliar en buena medida en los últimos años del siglo. El futuro se presenta incierto, pues aunque no existe ningún núcleo abandonado, el envejecimiento, la baja natalidad y el paro están hipotecando el desarrollo endógeno de los núcleos rurales de la región.Along the second half of the 20th century the deepest transformations of the regional history have been produced so much by factors exogenous as endogenous of every kind. It has passed of an agrarian economy of subsistence to a competitive economy of the services and of the knowledge, what has supposed enormous excesses of agrarian unemployment to the end of century. Surplus that enlarged the great volume of emigrant population of the sixties and seventy that supposed the 40% of the regional population. Nevertheless, from beginnings of the eighty, the population is being stabilized. But the emigration already had caused an irreparable strangulation in the active ages and of procreation and a strong descent of the birthrate accentuated by the fall of the fertility and a deep aging. On the other hand, the retention of the young and adult population in its places of origin, next to other factors, it has increased the unemployment to unknown levels, though is a phenomenon that has managed to alleviate itself in good measure in recent years of the century. The future is presented uncertain, therefore although no nucleus abandoned exists, the aging, the low birthrate and the unemployment are mortgaging the endogenous development of the rural nuclei of the region.peerReviewe

    A phase 3 trial of azacitidine versus a semi-intensive fludarabine and cytarabine schedule in older patients with untreated acute myeloid leukemia

    PETHEMA Group.[Background] Options to treat elderly patients (≥65 years old) newly diagnosed with acute myeloid leukemia (AML) include intensive and attenuated chemotherapy, hypomethylating agents with or without venetoclax, and supportive care. This multicenter, randomized, open-label, phase 3 trial was designed to assess the efficacy and safety of a fludarabine, cytarabine, and filgrastim (FLUGA) regimen in comparison with azacitidine (AZA).[Methods] Patients (n = 283) were randomized 1:1 to FLUGA (n = 141) or AZA (n = 142). Response was evaluated after cycles 1, 3, 6, and 9. Measurable residual disease (MRD) was assessed after cycle 9. When MRD was ≥0.01%, patients continued with the treatment until relapse or progressive disease. Patients with MRD < 0.01% suspended treatment to enter the follow-up phase. [Results] The complete remission (CR) rate after 3 cycles was significantly better in the FLUGA arm (18% vs 9%; P = .04), but the CR/CR with incomplete recovery rate at 9 months was similar (33% vs 29%; P = .41). There were no significant differences between arms in early mortality at 30 or 60 days. Hematologic toxicities were more frequent with FLUGA, especially during induction. The 1-year overall survival (OS) rate and the median OS were superior with AZA versus FLUGA: 47% versus 27% and 9.8 months (95% confidence interval [CI], 5.6-14 months) versus 4.1 months (95% CI, 2.7-5.5 months; P = .005), respectively. The median event-free survival was 4.9 months (95% CI, 2.8-7 months) with AZA and 3 months (95% CI, 2.5-3.5 months) with FLUGA (P = .001). [Conclusions] FLUGA achieved more remissions after 3 cycles, but the 1-year OS rate was superior with AZA. However, long-term outcomes were disappointing in both arms (3-year OS rate, 10% vs 5%).     A High Dynamic Range ASIC for Time of Flight PET with monolithic crystals

    The HRFlexToT is a 16-channel ASIC for SiPM anode readout designed for Positron Emission Tomography (PET) applications that features high dynamic range (>8 bits), low input impedance, common cathode connection, high speed and low power (~3.5 mW/ch). The ASIC has been manufactured using XFAB 0.18 mm CMOS technology. The main characteristics of the HRFlexToT, compared to its predecessor, are a new energy measurement readout providing a linear Time Over Threshold (ToT) with an extended dynamic range, lower power consumption and better timing response. Initial measurements show a linearity error below 3%. Single Photon Time Resolution (SPTR) measurements performed using a Hamamatsu MPPC S13360-3050CS (3x3 mm2 pixel, 50 umm cell) shows 30% improvement with respect to the previous version of the ASIC, setting this specification in the order of 141 ps FWHM and reducing 3 times power consumption. It is important to highlight that an SPTR of 141 ps FWHM is, according to the best of our knowledge, the best resolution achieved so far for this sensor.     Application of new indicators to assess the quality of antimicrobial use in intensive care units

    This study explored the feasibility of a bundle of indicators aimed at assessing the quality of antimicrobial use in intensive care units (ICUs) through an observational prospective study spanning 12 quarters (January 2019-December 2021) in a 1290-bed teaching hospital in Spain. Members of the antimicrobial stewardship programme team selected the indicators to analyse the quality of antimicrobial use based on consumption data from a list proposed in a previous study. Antimicrobial use in the ICU was measured as defined daily dose (DDD) per 100 occupied bed-days. Trends and points of change were analysed with segmented regression. The intravenous macrolides/intravenous respiratory fluoroquinolones ratio in the ICU increased progressively, although not significantly, by 11.14% per quarter, likely related to prioritization of the use of macrolides in serious community-acquired pneumonia and the coronavirus disease 2019 pandemic. A remarkable upward trend of 2.5% per quarter was detected in the anti-methicillin-susceptible Staphylococcus aureus/anti-methicillin-resistant S. aureus agents ratio in the ICU, which could be explained by the low prevalence of methicillin-resistant S. aureus at the study centre. Patterns of amoxicillin-clavulanic acid/piperacillin-tazobactam ratio and diversification of anti-pseudomonal beta-lactams showed an increment in use over the study. The use of these novel indicators provides additional information for the current analysis of DDD.     The Spanish Infrared Camera onboard the EUSO-BALLOON (CNES) flight on August 24, 2014

    The EUSO-Balloon (CNES) campaign was held during Summer 2014 with a launch on August 24. In the gondola, next to the Photo Detector Module (PDM), a completely isolated Infrared camera was allocated. Also, a helicopter which shooted flashers flew below the balloon. We have retrieved the Cloud Top Height (CTH) with the IR camera, and also the optical depth of the nonclear atmosphere have been inferred with two approaches: The first one is with the comparison of the brightness temperature of the cloud and the real temperature obtained after the pertinent corrections. The second one is by measuring the detected signal from the helicopter flashers by the IR Camera, considering the energy of the flashers and the location of the helicopter

    Empoderamiento y feminismo comunitario en la conservación del maíz en México

    Articulo científico para revista indizada.El objetivo es analizar, desde una perspectiva basada en el feminismo comunitario, el proceso de empoderamiento de las mujeres que conforman un grupo de ocho integrantes matlatzincas de la comunidad de San Francisco Oxtotilpan, México, a través de prácticas productivas, alimentarias y culturales en torno al maíz nativo. Los datos fueron recogidos durante 2014 y 2015 con técnicas etnográficas que incluyen: observación participante, historias de vida, grupos focales, entrevistas semiestructuradas y a profundidad. Son mujeres que muestran cinco dimensiones de poder (social, corporal, material, simbólico y cognitivo) que repercuten en la preservación del maíz nativo, al generar la masa crítica necesaria para incorporar a otras mujeres en acciones favorables para la soberanía alimentaria

    Impact of Aetiological Treatment on Conventional and Multiplex Serology in Chronic Chagas Disease

    The main criterion for treatment effectiveness in Chagas Disease has been the seronegative conversion of previously reactive serology, generally achieved many years post-treatment. The lack of reliable tests to ensure parasite clearance and to examine the effect of treatment is the main difficulty in evaluating treatment for chronic Chagas disease. Decreases of conventional and non-conventional serological titers can be useful tools to monitor the early impact of treatment. We serially measured changes in antibody levels, including seronegative conversion as well as declines in titers in 53 benznidazole-treated and 89 untreated chronically T. cruzi-infected subjects. Seronegative conversion as well as decreases of titers was significantly higher in treated compared with untreated patients. A strong concordance was found between decreases of titers of conventional and non-conventional serologic tests post-treatment, reaffirming the findings. When seronegative conversion plus decreases of titers were considered altogether, the impact of treatment was higher, in a shorter follow-up period than previously considered. New tools for monitoring the effectiveness of treatment of chronic Chagas disease are necessary, and the results showed in this study is a contribution to researchers and physicians who assist patients suffering from this disease

    Variación estacional de la condición nutricional larvaria del boquerón (Engraulis encrasicolus) en el estuario del Guadalquivir

    European anchovy recruitment to the Guadalquivir estuary mainly occurs when the environmental conditions show more stability. Unusual changes in the environmental conditions of this estuary negatively impacted on key preys of anchovy and therefore on its nursery function. In this way, nutritional condition should determine strongly the physiological state of this species in estuaries and thus its survival and recruitment. RNA:DNA ratio has been applied with success as indicator of nutritional condition and growth in marine organisms. We study RNA:DNA ratio in anchovies of the coupled system Guadalquivir estuary-coastal influenced area in the Gulf of Cádiz in order to evaluate the condition seasonal variability of this species along this spatial gradient. Spatial differences in the size distribution of anchovy and high values of RNA:DNA in the Guadalquivir estuary compared to its influence coastal area suggest that anchovies migrate to the estuary and they acquire a very good condition. We conclude that RNA:DNA is an useful tool to understand the ecological mechanisms by which the coupled system Guadalquivir estuary-coastal influenced area in the Gulf of Cádiz control the success or failure of the recruitment of this important fishery resource in the region
