A randomized controlled trial comparing nerve block and mandibular infiltration techniques in posterior mandible implant surgeries
To compare global surgical pain under nerve block and mandibular infiltration anesthesia techniques, and to evaluate pain during drilling and the distance to the mandibular canal in posterior mandible implant surgeries. A prospective, randomized, controlled, double-blind, clinical trial was conducted to compare nerve block (Group A) to mandibular infiltration (Group B) techniques for dental implant placement. Global surgical pain (VAS = visual analogue scale), pain during drilling or implant placement (MPQ = McGill pain questionnaire) and distance to the mandibular canal (Image J) were statically analyzed. Age, gender, anxiety levels, tooth to be replaced, implant size, adjacent teeth and duration of surgery were also analyzed. 172 patients were included and 283 dental implants were analyzed. VAS values were significantly higher in Group B (p<0.05). In Group A, 99% of the surgeries were performed painlessly during drilling and implant placement, but in Group B, 11.6% of implant placements (17 implants) felt pain during these surgical steps. Mean distance to mandibular canal (3.8 mm, range: 0.0 to 7.0) in those 17 implants placed under mandibular infiltration was clinically and statistically similar to the mean distance (3.0 mm, range: 0.0 to 9.0) of 130 implants placed painless (p=0.10). Pain during drilling under mandibular infiltration was significantly associated with the duration of surgery (p<0.05) and to both adjacent teeth being present (p<0.05). Although both techniques are safe and effective for placing implants in the posterior mandible, nerve block provides a more profound analgesia than mandibular infiltration. When placing implants under mandibular infiltration, as getting closer to the canal does not increase the feeling of pain, it is not recommended to use the presence of pain as a preventive resource to avoid inferior alveolar nerve injuries
Rapid on-Chip Assembly of Niosomes: Batch versus Continuous Flow Reactors
The large-scale continuous production of niosomes remains challenging. The inherent drawbacks of batch processes such as large particle polydispersity and reduced batch-to-batch reproducibility are here overcome by using commercially available microfluidic reactors. Compared to the traditional batch-based film hydration method, herein, we demonstrate that it is possible to carry out the homogeneous, large-scale (up to 120 mg/min) production of niosomes using two different synthesis techniques (the thin film hydration method and the emulsification technique). Niosomes particle size can be controlled depending on the need by varying the synthesis temperature. The high cytocompatibility of the resulting niosomes was also demonstrated in this work on three different somatic cell lines. For the first time, the structure of the niosome multilamellar shell was also elucidated using high-resolution transmission electron microscopy (HR-STEM) as well as their colloidal stability over time (6 weeks) under different storage conditions. The morphology of cryo-protected or as-made niosomes was also evaluated by HR-STEM after freeze-drying. Finally, the dual ability of those synthetic, nonionic, surfactant-based vesicles to carry both hydrophilic and hydrophobic molecules was also here demonstrated by using laser scanning confocal microscopy
Angle-dependence of quantum oscillations in YBa2Cu3O6.59 shows free spin behaviour of quasiparticles
Measurements of quantum oscillations in the cuprate superconductors afford a
new opportunity to assess the extent to which the electronic properties of
these materials yield to a description rooted in Fermi liquid theory. However,
such an analysis is hampered by the small number of oscillatory periods
observed. Here we employ a genetic algorithm to globally model the field,
angular, and temperature dependence of the quantum oscillations observed in the
resistivity of YBa2Cu3O6.59. This approach successfully fits an entire data set
to a Fermi surface comprised of two small, quasi-2-dimensional cylinders. A key
feature of the data is the first identification of the effect of Zeeman
splitting, which separates spin-up and spin-down contributions, indicating that
the quasiparticles in the cuprates behave as nearly free spins, constraining
the source of the Fermi surface reconstruction to something other than a
conventional spin density wave with moments parallel to the CuO2 planes.Comment: 8 pages, 4 figure
Magnetic nano-fluctuations in a frustrated magnet
Frustrated systems exhibit remarkable properties due to the high degeneracy
of their ground states. Stabilised by competing interactions, a rich diversity
of typically nanometre-sized phase structures appear in polymer and colloidal
systems, while the surface of ice pre-melts due to geometrically frustrated
interactions. Atomic spin systems where magnetic interactions are frustrated by
lattice geometry provide a fruitful source of emergent phenomena, such as
fractionalised excitations analogous to magnetic monopoles. The degeneracy
inherent in frustrated systems may prevail all the way down to absolute zero
temperature, or it may be lifted by small perturbations or entropic effects. In
the geometrically frustrated Ising--like magnet Ca3Co2O6, we follow the
temporal and spatial evolution of nanoscale magnetic fluctuations firmly
embedded inside the spin--density--wave magnetic structure. These fluctuations
are a signature of a competing ferrimagnetic phase with an incommensurability
that is different from, but determined by the host. As the temperature is
lowered, the fluctuations slow down into a super-paramagnetic regime of stable
spatiotemporal nano-structures

articulada, no ensino-aprendizagem da Matemática e na actividade profissional do
professor. Para isso, é necessário conceber tarefas que possam ser o ponto de partida para
investigações e explorações matemáticas dos alunos e discutir o modo como podem ser
trabalhadas na sala de aula. Recorrendo a exemplos de actividades realizadas por
professores de Matemática portugueses, analiso a actividade de aprendizagem suscitada
por tarefas deste tipo e discuto as respectivas potencialidades. Finalmente, refiro as
condições respeitantes à cultura profissional dos professores que podem favorecer uma
actividade de investigação sobre a sua própria profissional, com relevo para a colaboração

to design tasks that may be the starting point for mathematical investigations and

the activity of learning raised by such tasks and discuss their potencial. Finally, I refer to
conditions relating to the professional culture of teachers who can promote the research

Safety and preliminary activity results of the GATTO study, a phase Ib study combining the anti-TA-MUC1 antibody gatipotuzumab with the anti-EGFR tomuzotuximab in patients with refractory solid tumors

The phase I GATTO study (NCT03360734) explored the feasibility, tolerability and preliminary activity of combining gatipotuzumab, a novel humanized monoclonal antibody binding to the tumor-associated epitope of mucin 1 (TA-MUC1) and an anti-epidermal growth factor receptor (anti-EGFR) antibody in refractory solid tumors.
Patients and methods
Initially the study enrolled primary phase (PP) patients with EGFR-positive metastatic solid tumors, for whom no standard treatment was available. Patients received gatipotuzumab administered at 1400 mg every 2 weeks, 6 weeks after the start of the glyco-optimized anti-EGFR antibody tomuzotuximab at 1200 mg every 2 weeks. As this regimen was proven safe, enrollment continued in an expansion phase (EP) of patients with refractory metastatic colorectal cancer, non-small-cell lung cancer, head and neck cancer and breast cancer. Tomuzotuximab and gatipotuzumab were given at the same doses and gatipotuzumab treatment started 1 week after the first dose of the anti-EGFR antibody. Additionally, investigators could use a commercial anti-EGFR antibody in place of tomuzotuximab.
Results
A total of 52 patients were enrolled, 20 in the PP and 32 in the EP. The combined treatment was well tolerated and no dose-limiting toxicity was observed in the whole study, nor related serious adverse event or death. Preliminary activity of the combination was observed, with one and four RECIST partial responses in the PP and EP, all in colorectal cancer patients. The trial was accompanied by a comprehensive translational research program for identification of biomarkers, including soluble TA-MUC1 (sTA-MUC1) in serum. In the EP, patients with baseline sTA-MUC1 levels above the median appeared to have improved progression-free survival and overall survival.
Conclusions
Combination of a TA-MUC1-targeting antibody and an EGFR-targeting antibody is safe and feasible. Interesting antitumor activity was observed in heavily pretreated patients. Future studies should test this combination together with chemotherapy and explore the potential of sTA-MUC1 as a companion biomarker for further development of the combination.This work was supported by Glycotope GmbH (no grant number)
Complete and incomplete spin transitions in 1D chain iron(II) compounds.
The synthesis and characterisation of two new octahedral iron(II) SCO coordination polymers [FeL1(bimm)] (1) and [FeL2(bppa)](MeOH)0.5 (2) (L1 = [3,30]-[1,2- phenylenebis-(iminomethylidyne)bis(4-phenyl-,4-butanedionato)(2-)-N,N0,O2,O20], L2 = [E,E]-[{diethyl 2,20-1,2- phenylenebis(iminomethylidyne)bis(3-oxo-3-phenylpropanato)}(2-)-N,N0,O3,O30], bimm = bis(1H-imidazol-1-yl)methane and bppa = 1,3-bis(pyridine-4-yl)propane) is presented.
Results from X-ray structure analysis at different temperatures revealed in the case of 1 that the transition from a gradual to a cooperative SCO with a 5 K wide hysteresis is due to an increase of the short intermolecular contacts, which exceed a certain threshold for the cooperative effect. In the case of compound 2 an incomplete spin transition with a 4 K wide hysteresis was observed. The low temperature wMT product remains constant at a value typical for a mixed HS/LS state in stepwise spin transitions. A quantitative correlation between the cooperative effects of 12 monomer and polymer iron(II) SCO complexes and their structural properties derived from X-ray structure analysis, the so-called crystal contact index, CCI, is introduced
Angiography with optical coherence tomography as a biomarker in multiple sclerosis
Purpose
To investigate superficial retinal microvascular plexuses detected by optical coherence tomography angiography (OCT-A) in multiple sclerosis (MS) subjects and compare them with healthy controls.
Methods
A total of 92 eyes from 92 patients with relapsing-remitting MS and 149 control eyes were included in this prospective observational study. OCT-A imaging was performed using Triton Swept-Source OCT (Topcon Corporation, Japan). The vessel density (VD) percentage in the superficial retinal plexus and optic disc area (6 x 6 mm grid) was measured and compared between groups.
Results
MS patients showed a significant decrease VD in the superior (p = 0.005), nasal (p = 0.029) and inferior (p = 0.040) parafoveal retina compared with healthy subjects. Patients with disease durations of more than 5 years presented lower VD in the superior (p = 0.002), nasal (p = 0.017) and inferior (p = 0.022) parafoveal areas compared with healthy subjects. Patients with past optic neuritis episodes did not show retinal microvasculature alterations, but patients with an EDSS score of less than 3 showed a significant decrease in nasal (p = 0.024) and superior (p = 0.006) perifoveal VD when compared with healthy subjects.
Conclusions
MS produces a decrease in retinal vascularization density in the superficial plexus of the parafoveal retina. Alterations in retinal vascularization observed in MS patients are independent of the presence of optic nerve inflammation. OCT-A has the ability to detect subclinical vascular changes and is a potential biomarker for diagnosing the presence and progression of MS
Odderon Exchange from Elastic Scattering Differences between pp and p(p)over-bar Data at 1.96 TeV and from pp Forward Scattering Measurements
We describe an analysis comparing the p (p) over bar elastic cross section as measured by the D0 Collaboration at a center-of-mass energy of 1.96 TeV to that in pp collisions as measured by the TOTEM Collaboration at 2.76, 7, 8, and 13 TeVusing a model-independent approach. The TOTEM cross sections, extrapolated to a center-of-mass energy of root s = 1.96 TeV, are compared with the D0 measurement in the region of the diffractive minimum and the second maximum of the pp cross section. The two data sets disagree at the 3.4s level and thus provide evidence for the t-channel exchange of a colorless, C-odd gluonic compound, also known as the odderon. We combine these results with a TOTEM analysis of the same C-odd exchange based on the total cross section and the ratio of the real to imaginary parts of the forward elastic strong interaction scattering amplitude in pp scattering for which the significance is between 3.4s and 4.6s. The combined significance is larger than 5 sigma and is interpreted as the first observation of the exchange of a colorless, C-odd gluonic compound.Peer reviewe
